Send us a textEndocrine Review Course Learning Objectives:- Discuss the differential diagnosis of a High TSH- Identify physiological mild TSH elevation with advanced age that does not require treatment - Be able to pick up on exam question clues of high TSH scenarios Text: A 20-year-old female presents to her primary care physician for a wellness check. She reports fatigue and sleepiness. Family history includes hypothyroidism in her mother. Her physical exam reveals slightly enlarged thyroid gland. Her vital signs within normal reference ranges. Her TSH was measured and was elevated at 7.5. What's the best next step:· Check FT4· Start levothyroxine· Perform thyroid US· Check Thyroid peroxidase antibody And the answer is Check FT4. This would be needed to confirm and characterize hypothyroidism (overt vs subclinical). Starting levothyroxine at this point is premature. Although this patient has enlarged thyroid, however thyroid ultrasound is not a routine part of hypothyroidism evaluation and would not be the best next step. While measuring TPO may help explain the etiology, it would not be the best next step. Reference range can vary widely depending on the assay measurement. With that caveat for serum TSH, normal reference range is typically between 0.4 – 4.0 mU/l. Above 2mU/l the risk of developing hypothyroidism increases especially in the presence of TPO antibodies. When encountering high TSH, you should immediately have 3 differential categories:o First category is Appropriate pituitary response to low levels of thyroid hormone, in this case, the pituitary is attempting to increase thyroid hormone production§ Such as in primary clinical and subclinical hypothyroidism, from Hashimoto's, thyroid radiation, previous thyroid surgery, and drug induced hypothyroidism like amiodarone, lithium, interferon-alpha, and immune checkpoint inhibitors, TKI, physiological increase with later age, morbid obesity Second category is Inappropriate pituitary response, in this case, thyroid levels are high, but the pituitary continued to provide stimulation to the thyroid due to pituitary pathology or resistance (TSHoma or thyroid hormone resistance) which will be discussed in detail in a future episode. Briefly TSHoma or thyrotropinoma is rare TSH-secreting pituitary tumor. Thyroid hormone resistance is due to mutation in TSH receptor. This can also be seen in the late recovery phase of thyroiditis and in recovery of sick euthyroid syndrome. Third category is lab assay abnormalities such as Macro-TSH. We previously mentioned assay interference with antibodies but also Macro-TSH can cause interference. TSH is elevated usually VERY HIGH like 100 mIU/L and thyroid hormone levels here are typically normal. Macro-TSH is a macromolecule made from the autoimmune anti-TSH immunoglobulin and TSH molecule. It is biologically inactive. The gold standard method to detect macro-TSH is chromatography. It is also important to keep in mind macro-TSH for persistent TSH elevation. Exam Clues- In each Clinical scenario, you should screen the question for specific supportive clues, for example:o Family or personal history of autoimmune disease could support primary hypothyroidism/Hashimoto's diseaseo Strong multigenerational Family history of thyroid disorder (autosomal dominant pattern) could support TSH resistance, genetic testing for THR-B is possibleo Recent critical illness could indicate sick euthyroid in recovery phaseo Unusual diet such as kelp/seaweed/seamoss diet or recent IV contrast may sSupport the show

According to the World Watch List from Open Doors, more than 360 million Christians are persecuted for their faith, which is one in seven believers. The high temperature will settle in at around 43 degrees.Bald eagle surveys in Iowa in 2023 showed a return to normal after 2022, the worst nesting season for many years because of bird flu, the Iowa Department of Natural Resources reported.Volunteers in January 2023 counted more than 2,900 eagles along 1,663.5 miles of Iowa waterways, which works out to almost two eagles per river mile surveyed, the DNR reported. The most eagles were spotted on the Mississippi and Des Moines rivers but the highest density (eagles per mile) was on the Iowa River.The H5N1 strain of the bird flu virus has affected both domestic poultry and wild birds in Iowa, and eagles are no exception.In 2022, the number of young bald eagles fledged per nest in Iowa dropped below the threshold of one per nest for the first time in the survey's history. Only an average of .88 young were fledged per Iowa nest in 2022.The DNR numbers indicated that considering the rate of successful nests, a possible 720 young eagles were fledged by Iowa nests in 2023.If you haven't gotten your COVID-19 vaccine updated, yet, you are not alone.According to reporting from the Iowa Capitol Dispatch, the number of Iowans who have up-to-date COVID-19 vaccinations has plummeted in recent months, after federal officials ended a public health emergency declaration and there was a sense that the risk of catching the virus had lowered.Data from towa Department of Health and Human Services indicated about 10 percent of Iowans were considered immunized against COVID-19. That is calculated using the state's database of immunization records.The HHS also now lists the chance of catching COVID-19 in Iowa as Very High, with the number of flu infections also rising during recent months.More than 60 percent of Iowans were vaccinated during the worst throes of the coronavirus pandemic, but those initial vaccinations have become less effective over time.As we head into a busy travel week, the U.S. Centers for Disease Control and Prevention is now urging health care providers to vaccinate their patients and to recommend antiviral medications to those with flu or COVID. It has also provided a “vaccination conversation guide” intended to address concerns about the vaccine.

Where did you start as an AT? Started at an orthopedic clinic working at a high school right out of college. New school so the first AT they had. I was also the first and only full-time outreach ATC Rockhill had. Loved my school. Very High turnover in management at the clinic How long were you there? Nearly four years What was next (tell more of your story)? So this is kind of two parts. Before COVID I started a cash-based rehab services business. Partnered with local gyms. This was my plan to leave and do full-time. Was about to make the jump then COVID hit and the gyms shut down. This kind of killed my momentum and made me pivot. I tried to go into the online space and still do that some but needed it to be a side gig for now at least. Things came about with the clinic I was at. Did not feel appreciated or supported anymore and had the opportunity to try Sales. Gave me more “regular” hours. Higher pay potential and higher pay to start. Dynasplint for 6 months. I ended up working more hours. Being bored with the work and just did not like my situation. Where are you now? This led me back to “traditional” - I am with a children's hospital here in Kansas City as the ATC of a Class 5 high school. It is the district I live in, and my wife teaches at the school. Just started at this school after being at another one before internally transferring when this came open. Is this where you plan to stay? The answer is maybe. I don't want to leave for another school or hospital system. CMH treats me well and the school situation is about as ideal as they come. But we just had a baby 6 weeks ago. And I can see my priorities already shifting and wanting to be home more. I know I won't be willing to miss his sporting events and other activities. Other options I am keeping an eye on: Industrial, Military, Healthcare Admin/Leadership, Self-Employed What pushed you to change settings? Feeling underappreciated Lack of work-life balance Pay Advancement opportunities Tyler, what held you back from moving on? Fear of failure Lack of opportunity at first Not want to leave my athletes Really enjoyed the school I worked with A favorite resource for someone looking to change settings? Fellow ATCs!! Speak to others in the setting you are thinking about or who are with the company you want to apply for. Former employees at those places can also be very helpful Call to action: Do your homework before jumping into just any opportunity Now is a great time to advocate for your worth in many ways If you want to stay in traditional settings, don't feel bad about it. I do not see that any of her peers have the same problem. I thought giving her DHA may help, I just struggle to give it to her as she dislikes the taste, even the lemon one. I even tried to sneak it in apple juice but I got found out. What are your thoughts on DHA and would you have any other suggestions on what we could do to improve things? No cycle for 2 years. Has had high levels of deep emotional stress for 2 years. Is coming out the other side. Thank goodness!! Suffered chronic fatigue in past from high levels of stress/loss/trauma and over training / under eating scenarios when younger. Anyway been suffering with gut issues bloating gas etc, for long time. Episode 45: Osteoporosis Update. Dr Linares (endocrinologist) explains the basics of screening and treatment of osteoporosis, referring frequently to the updated guidelines of osteoporosis by AACE and ACE (2020). A new group of residents is introduced. We hope you enjoy your time with us.Today is March 22, 2021.Implanted pacemakers and defibrillators are equipped with a switch that responds to magnetic forces to stop them when needed. Magnetic interference between these cardiac implantable electronic devices (CIEDs) and mobile devices have been investigated for years. It has been established that magnetic fields stronger than 10 gauss can deactivate these cardiac devices, causing pacemakers to give asynchronous pacing and ICDs to stop tachyarrhythmia detection.The Heart Rhythm Society journal, published in October 2009 (that was 11 years ago), an association between portable headphones and significant electromagnetic interference (EMI) in patients with implantable cardioverter-defibrillators (ICD) and pacemakers (PM). 100 patients with implanted devices were tested with different portable headphones. Headphones effectively deactivated implanted devices when held less than 2 cm from skin on the left side of chest. There was not interference when headphones were placed farther than 3 cm. In this study, normal functioning of the devices was restored in 29 out of 30 cases when the headphones were removed from the patient’s chest. The recommendation from that study was to recommend patients to keep their portable headphones at least 3 cm away from their implanted device.More recently, in January 2021, the same journal posted the effect of iPhone 12 on ICDs deactivation. iPhone 12 and MagSafe technology, which allows faster wireless charging, contain strong magnets. iPhone 12 successfully deactivated a Medtronic Inc. ICD when tested by a group of investigators in a patient[2]. The official Apple Support website posted on February 25, 2021, “To avoid any potential interactions with these devices, keep your iPhone and MagSafe accessories a safe distance away from your device (more than 6 inches / 15 cm apart or more than 12 inches / 30 cm apart if wirelessly charging)”[3]. The recommendations from these organizations may be different than the ones given by the American Academy of Family Physicians (AAFP) or the United States Preventive Services Taskforce (USPSTF), which are organizations we are more familiar with as family physicians.The questions analyzed during this conversation includes:When would you consider a DEXA scan to screen a woman younger than 65 for osteoporosis? What to do when the report says Osteopenia (T score -1.0 to -2.5)? Let’s mention the recommended dose of Vitamin D and Calcium. What is the FRAX score? What is an easy work up we can do to rule out a secondary cause of osteoporosis before sending patient to you? The new guidelines divide patients in two categories: “High risk/no risk of fractures” and “VERY High risk/prior fractures”, What’s the difference in management between those two categories? (alendronate in high risk vs abaloparatide in very high risk). How can you tell the patient has a good response after 1 year of treatment (Dexa scan, bone turnover markers)? What is a drug holiday? ___________________________Now we conclude our episode number 45 “Osteoporosis Update”. Dr Linares explained what the FRAX score is and mentioned the different options we have for treatment of osteoporosis. DEXA scan continues to be the gold standard for screening, diagnosis and monitoring of osteoporosis. Risk Assessments, Frameworks, and Approaches Risk Assessments are the topic for this episode of the CISO Dojo Podcast. What is a risk assessment: The identification, evaluation, and estimation of the levels of risks involved in a situation, with comparisons against benchmarks or standards, and determination of an acceptable level of risk. There are two types of risk assessments we discuss in this episode: Quantitative Risk Assessment: This one uses actual data and amounts during the risk assessment. Qualitative Risk Assessment: “Relative measure of risk or asset value based on rankings such as low, medium or high; not important, important very important, or on a scale from 1 to 10.” Risk Assessment Frameworks We are going to discuss two commonly use frameworks often utilized for risk assessments: FAIR (Factor Analysis of Information Risk) Defines value/liability as: Criticality Cost Sensitivity Embarrassment Competitive advantage Legal/regulatory General FAIR also defines six kinds of loss: Productivity Response Replacement Fines and judgments Competitive advantage NIST Special Publication 800 – 30 Risk Assessment Framework: NIST 800-30 is a 9 step approach to risk assessments that includes: Step 1: System Characterization Step 2: Threat Identification Step 3: Vulnerability Identification Step 4: Control Analysis Step 5: Likelihood Determination Step 6: Impact Analysis Step 7: Risk Determination Step 8: Control Recommendations Step 9: Results Documentation Types of Risk Assessments In this episode we briefly cover a few common types of risk assessments: RIA: Risk Impact Assessment This is the initial risk assessment that classifies the risk level of the system (Low, Moderate, High, Very High) and mitigating controls. BIA: Business Impact Assessment This is usually used during BPC/DR planning and determines the impact of losing your business-critical systems. PIA: Privacy Impact Assessment This one identifies PII that is collected; why the information is collected; and how the data will be used, shared, stored, and protected. DRIA: Detailed Risk Impact Assessment This one is more detailed than a regular risk assessment and outlines more robust security controls that are commensurate with the inherent risks of the system. We aren’t going to get into Risk Analysis, because there’s a larger conversation that needs to be had here. An organization needs understand what their top risks are so they can know here to start the risk assessment process. Top security risks for businesses Let’s take a look at where a lot of organizations are incurring the greatest amount of risk with their security posture, or lack of security posture. Your Organization is a Target Traditionally smaller businesses weren’t an appealing target for threat actors. That changed when ransomware arrived on the scene. Smaller organizations are a more appealing target for ransomeware because they typically have less budget to spend on backing up their data, business continuity, and disaster recovery. When a small business experiences ransomware, more often than not , they are forced to pay the ransom to recover their data and return to normal operations. If it’s not ransomware, the second favorite cyber attack of threat actors is crypto mining malware that runs silently on the systems consuming resources and mining cryptocurrency for the attacker. Cyber Security Budget Many of the organizations aren’t aware if they are over invested or under invested in security. Over investments takes funds away from other strategic business objectives, while under investment incurs too much risk for the organization. Over investment isn’t a difficult problem to solve, but under investment can be challenging to rectify. The best approach to determining where you stand is to map out the maturity of your organization in relation what the industry is doing. I’ll use the NIST Cybersecurity Framework functions to measure the maturity of the security program: Identify Protect Detect Respond Recover Next, map the maturity levels of 0-5 using the Capability Maturity Model. 0 is the least mature and 5 is the most mature. Most organizations should strive for a maturity level of 3 across the five functions of the NIST CSF. If you are not at level 3, you are under invested in that particular function. If you are at a 4-5 maturity level for a particular function, you might be over invested in that function. Patching and Vulnerability Management Risk Assessments An effective cyber security program includes patching and vulnerability management. Unpatched vulnerabilities provide opportunities for threat actors to compromise your systems and networks. Even in the best organizations achieve about a 75% success rate. In an organization that lacks patching and vulnerability management the risk for a breach is considerable. A successful patching and vulnerability management program starts with asset inventory. You need to know what assets you have and then you need a way to identify and monitor your patching and vulnerability exposure and remediation progress. Email Security Risk Assessments Breaches often start with malware, phishing, or spam as the entry point into the organization. This indicates a lack of technical controls at the email server, as well as the administrative control of a security awareness program. If you are hosting email in house with no spam filtering, anti-malware, or other technical controls, now is a good time to consider outsourcing email to Office 365 or Google Apps. The benefits are less maintenance, more security, reduced costs and administration time. Data Backup, Testing, and Recovery A lot of organizations lack a backup plan, back up retention, and testing of backups. The problem is usually a lack of understanding of what their mission critical data is. This goes back to the lack of a mature security program. Organizations that are backing up their data usually fail to test their backups due to a lack of time and lack of staff. This is something that should also be addressed in the over all security program for the organization or perhaps outsourced to a third party for business continuity and disaster recovery purposes. BYOD Cyber Security Risk Assessments Mobile devices are growing in popularity as an entry point for threat actors and careful consideration should be given to BYOD programs. While there is a lot of benefit to BYOD (bring your own device) there are also a lot of risks. The main issues are co-mingling of data, eDiscovery, terminations, data security, and mobile device management. Mobile device manage is critical if you allow employees to utilize their own mobile devices for work purposes. You should also include and mobile device threat prevention solution that detects and prevents malware, phishing over text message (smishing), and rooting or jail breaking of mobile devices. Also consider a VPN for secure connections from the mobile device back to the corporate network. No Cyber Security Program This by far is one of the most common problems I encounter when consulting with small, medium, and even large enterprise level businesses. There should be an overarching policy from the executive level that the organization understands the importance of cyber security and will have a cyber security program. A typical cyber security program should include: Security Awareness Business Continuity and Disaster Recovery Physical Security Acceptable use policies for email, Internet, and mobile devices Password policy Encryption Policy Cloud Storage and provisioning policy Incident response policy Vendor Management Policy Cyber Risk Appetite Statement The above is not a comprehensive list and will differ from organization to organization. Preventing breaches, business impact, and security incidents starts with risk assessments and a cyber security program. Having a formal security program also means having someone in charge of security to drive it forward. This is usually a CISO or VCISO depending on the size of the organization. The world premiere of the ID.4 follows at the end of September. Preparations to roll out the electric SUV at international level are also in full swing. Pre-production of the ID.4 has already started at the Anting plant in China, the Chattanooga site will start the ID.4 production in 2022. The SUV will initially be launched with rear-wheel drive, while an electric all-wheel drive version will be added a later date. PushEVs points out: "The Volkswagen ID.4 made in Europe is using the new NCM 712 battery cells made by LG Chem in Poland. Later on, the made in USA version will get its battery cells from SK Innovation and the made in China version will probably get its battery cells from CATL. Made with very energy dense and affordable NCM 712 batteries – that cost below 100 euros per kWh -, the expensive Volkswagen ID.3 and ID.4 will be extremely profitable electric cars (on average way more than 15.000 euros per unit). EVS OUTPERFORM NEW CAR MARKET IN JULY 2020 "The electric car market has once again significantly outperformed the UK new car market as a whole, with growth of 260% for pure-EVs in July 2020 and 320% for PHEVs, compared to the same period last year." says Next Green Car: "The latest set of Society of Motor Manufacturers and Traders published today (Wednesday 5th August), shows continued success for electric cars. It's a trend that has been seen practically all year, with most months having triple digit growth in the pure-electric market. July's figures see pure-EVs make up 4.7% of all new cars sold in July 2020, with a total of 8,162 units registered. Plug-in Hybrid figures are similar, with 7,447 units, and market share of 4.3%. Combined plug-in vehicle registrations come in at 15,609 registrations - a market share of 8.9%." While the camouflage hides the specific design details, it's easy to get an idea of what this EV looks like when the concealment comes off. BMW is positioning the iNext as being a technological tour-de-force. One of the most interesting decisions is what the company is doing with its iconic kidney grille. As an electric vehicle, the openings to cool the engine are no longer necessary. Instead, the engineers are packing this area with many sensors to serve as the "eye" of the iNext." says Motor1.com: "The iNext will use BMW's fifth-generation EV powertrain. While power figures aren't available yet, the company intends the range to be in excess of 373 miles (600 kilometers). BMW will fully debut the iNext in 2021, and it'll be on sale later that year. Somewhere down the line, it's likely we'll also see a smaller version of the electric pickup, according to a recent tweet from CEO Elon Musk." says Motortrend: "Responding to a question about the possibility of a smaller truck for Europe, Musk said a smaller Tesla Cybertruck is "highly likely down the road." Everyone has their favorite and using geotagged data from Twitter, partcatalog.com built a map showing the most anticipated electric truck in every state. They said about the methodology: “The map is based on geotagged Twitter data in the last month, tracking tweets and hashtags about electric trucks in every state, including the models most mentioned. Over 90,000 tweets were tracked.”" writes Electrek: " Tesla, the automaker claimed over 200,000 reservations within the first week of taking orders and the latest tally put reservations at over 650,000 over the last 9 months. While fans had assumed over 700,000 reservations based on the analysis of reservation numbers, Elon Musk has now named a significantly lower number. In an interview with Automotive News, Musk said that the number of Cybertruck reservations was “over 200,000”. This comes as a surprise since Elon Musk had already announced 250,000 pre-orders just a few days after the presentation of the Cybertruck in November. This presumably points to cancellations." says Electrive. Whilst CNET reports: "Fans keeping track of their order numbers estimated that there have been approximately 727,000 reservations for the electric pickup truck. The answer is not 43: it is approximately 30,000, even if the reservation numbers are now around 56,000. How?" says InsideEVs: "If it starts delivering the pickup truck by July 2021 and producing 20,000 per year, as promised, it will let everyone that order an R1T have their cars by August 2022, according to John King's calculations. That happens because he works with a more precise number of preorders: 24,832. For steep driveways or places where you repeatedly need a higher ride height, tap "Always Auto-Raise at this Location." BP TO CUT OIL PRODUCTION 40% "In some refreshingly good news for the environment, news coming out of London, BP has announced that it is planning to cut oil and gas production by 40% and invest billions of dollars into clean energy. This is part of a major strategic plan that was unveiled today along with a gigantic second-quarter loss and dividend cut." says CleanTechnica: "BP expects that demand for fossil fuels will fall by 75% over the next 30 years if global temperatures continue to increase. Its head of strategy, Giulia Chierchia, explained to investors that if the increase in global temperatures is limited to 1.5 degrees Celsius, then the demand for fossil fuels will fall drastically. Developing a performance car generally requires removing as much weight as possible, but designing an electric car with an acceptable amount of driving range normally means using a bulky battery pack." says Autoblog. Silke Bagschik, Volkswagen's head of electric car marketing and sales, told Autoblog; ""Looking at the weight distribution, this decision was made very early. With the battery adding weight, you have a super low center of gravity. In most front-wheel drive electric cars, you don't have that much fun when you're taking off because the front wheels spin, especially when it's wet. With rear-wheel drive, that never happens. This is the reason we did that; fun. 022920 This is Coronavirus 4-1-1. The latest Coronavirus info for Saturday February 29th, 2020. The U.S. now has at least 67 cases of COVID-19.With four of the most recent cases appearing to have come from “community transmission”, CNN notes. Two of these cases are in California, one in Oregon and one in Washington State. The World Health Organization has upped the risk assessment for COVID-19 to “Very High” on a global level. The CDC has updated travel guidance and has put China, South Korea, Iran and Italy as “Level 3”, avoid Non-essential travel. The CDC also asks “that all travelers reconsider cruise ship voyages into or within Asia at this time”. The United Kingdom appears to have its first case of “community transmission” with a man from Surrey diagnosed with no travel history to an infected area or close contact with an infected person according to the New York Times. The Times also notes, a Japanese man, who tested negative and was released from the Diamond Princess cruise ship, has now tested positive.He is the 5th passenger that has tested positive after being released and allowed to travel freely.CNN reports Osaka and Tokyo have cancelled their Cherry Blossom Festivals due to concerns over spread of COVID-19 as the global total of cases reaches 85,055 with 2,922 deaths.The locations of U.S. and country diagnoses in a moment.There have been 67 confirmed cases of COVID-19 in the U.S.Of those 67, 6 have recovered and those patients removed from the following list. A whole lot of them have MONEY, and lots of it. And all of their kids and grandchildren have spread out all over the country. What are these people doing? They’re buying RV’s - in MASSIVE volumes - and taking their home with them all over the country.So what opportunity does this create for me and you… and what is the ACTUAL opportunity that my friend reached out to me about?RV parks, baby! Imagine the benefits of owning a hotel… where you get a very, very high rental price for a very, very small portion of real estate… but you do NOT have to think about things like laundry or furniture or linens or any of the things that makes a hotel so very expensive to establish, operate and own.Instead, what you’re renting out is, essentially, a piece of concrete. A small piece of concrete, where your customers bring THEIR OWN bed and THEIR OWN furniture and THEIR OWN linens. You see, we know, as a sociological fact, that people who use RV parks are, by and large, very habitual in their behavior. Once they find an RV park that they like, chances are they’ll come back every year or two over and over and over again…...and that means not only are the profits in this business VERY HIGH, but they can also be incredibly CONSISTENT.Both of the deals that we’re buying and closing on in the next two weeks are actually ALREADY very profitable… and the REALLY beautiful thing is this: A little money spent wisely goes a REALLY LONG WAY with RV parks. There are two examples I want to share with you, both of which will ABSOLUTELY blow your mind… you’ll see why we are SO UTTERLY THRILLED with this asset class.But before I share those two examples with you, think about this name: Sam Zell. Sam Zell is a LEGENDARY real estate investor, Bloomberg pegs his net worth at $4.4 BILLION. Zell started and currently owns a very large percentage of several publicly-traded REITs - kind of like a mutual fund for real estate investments - and each of those REITs are focused on different real estate segments like commercial real estate, multi-family and… surprise, surprise… RV parks and mobile home parks. And guess which one is outperforming the others? He was the first person to sail single-handed non-stop westwards around the world (1971), on a 59-foot boat called British Steel. Blyth was born in Hawick, Roxburghshire. He joined the British Army Parachute Regiment when he was 18 and was promoted to Sergeant at the age of 21. In 1966, while in the army, Blyth, together with Captain John Ridgway, rowed across the North Atlantic in a 20 ft open dory called English Rose III. After successfully completing this in 92 days, Blyth was awarded the British Empire Medal (BEM). Rowing the Atlantic first became mainstream when the first Atlantic Rowing Race was launched by Sir Chay Blyth, after reflecting on his own ocean row that propelled him to international fame. This was the Port St. Charles, Barbados Atlantic Rowing Race. Thirty double-handed teams lined up at the start line in a "one design" rowing boat just outside Los Gigantes marina on Sunday 12 October 1997. The race was won by Kiwi Challenge, rowed by Rob Hamill and Phil Stubbs after 41 days at sea. Video of the Week: Talisker Whisky Atlantic Challenge 2017 - Race Promo Don't Fry Bacon Naked: Last week we discussed how to not get burned by getting your altitude acclimatization right. The example we were discussing was Bill had been at sea level for a week, came back to Colorado to do the Triple Bypass and felt a lack of fitness. How much of that was due to a loss of altitude acclimatization? How quickly to make an altitude acclimatization? http://www.bodyresults.com/e2deacc.asp According to Lawrence Armstrong, PhD, in his book, Performing in Extreme Environments (1), the rate of disappearance of the body’s adaptations to high altitude varies widely from person to person; just as it’s difficult to tell who exactly will experience signs of altitude illness, it’s hard to know how long your acclimatized state will last once you descend from high altitude. If you spend less than a day or two at altitude (say, on a moderate climb of a peak like Baker or Rainier, where most people return to sea level within 24 hours of reaching the summit), your body will not have had enough time to permanently adapt to the altitude. The composition of the blood changes after about 2 weeks of altitude exposure by producing more red blood cells and hemoglobin (the iron-protein compound that transports oxygen) (3) but most people climbing peaks in the Pacific Northwest are only exposed to elevation for about 3-4 days at a time. Training acclimatization time needs to be longer as the altitude becomes higher. Training for 14 days at or above 6,500 feet (as at the U.S. Olympic Training Center in Colorado Springs) and 28 days at or above 8,000 feet are currently the best recommendations for athletes wishing to compete at similar elevations, while complete adaptation to the extreme altitude of 13,000 feet is possible after a continuous stay for 14 months (3). Obviously, climbers have a tiny fraction of this time open to them. One study cited by Armstrong indicates that the red blood cell volume of high-altitude natives (people who spend most of their lives above 7000 ft) decreases as quickly as ten days after spending time at sea level. Someone spending several hours to perhaps a day or two at altitude simply won’t have enough time for any long-lasting physiological changes. Those who choose to trek in Nepal, however, or participate in an expedition-type climb of a peak over 15,000 ft will have to spend a substantial amount of time adapting to the altitude in order to prevent altitude sickness. This is why climbers who gradually ascend their first peak in Alaska, Bolivia, Chile, or the Himalayas in order to get properly acclimatized can then speed up subsequent peaks, because the body’s ability to perform physical work at high altitude can persist for a few weeks (2). Through personal discussion with our African guide, cook, and porters on Mt. Kilimanjaro, and in talking with local Northwest guides and rangers who spend a lot of time on the mountains, we learned that they typically spend a week in the mountains above 10,000 ft and a week back home; their acclimatization and improved cardiovascular function may persist for several months after returning from altitude, and allows them to make subsequent trips quite easily without needing extra time to adapt. https://www2.palomar.edu/anthro/adapt/adapt_3.htm How quickly to lose altitude advantage? How does living at altitude affect how quickly you lose or gain altitude adaptations? https://www.princeton.edu/~oa/safety/altitude.html What is High Altitude? Altitude is defined on the following scale High (8,000 - 12,000 feet [2,438 - 3,658 meters]), Very High (12,000 - 18,000 feet [3,658 - 5,487 meters]), and Extremely High (18,000+ feet [5,500+ meters]). Since few people have been to such altitudes, it is hard to know who may be affected. There are no specific factors such as age, sex, or physical condition that correlate with susceptibility to altitude sickness. Some people get it and some people don't, and some people are more susceptible than others. Most people can go up to 8,000 feet (2,438 meters) with minimal effect. If you haven't been to high altitude before, it's important to be cautious. If you have been at that altitude before with no problem, you can probably return to that altitude without problems as long as you are properly acclimatized. What Causes Altitude Illnesses? The concentration of oxygen at sea level is about 21% and the barometric pressure averages 760 mmHg. As altitude increases, the concentration remains the same but the number of oxygen molecules per breath is reduced. At 12,000 feet (3,658 meters) the barometric pressure is only 483 mmHg, so there are roughly 40% fewer oxygen molecules per breath. In order to properly oxygenate the body, your breathing rate (even while at rest) has to increase. This extra ventilation increases the oxygen content in the blood, but not to sea level concentrations. Since the amount of oxygen required for activity is the same, the body must adjust to having less oxygen. In addition, for reasons not entirely understood, high altitude and lower air pressure causes fluid to leak from the capillaries which can cause fluid build-up in both the lungs and the brain. Continuing to higher altitudes without proper acclimatization can lead to potentially serious, even life-threatening illnesses. Acclimatization What is altitude illness? The major cause of altitude illnesses is going too high too fast. Given time, your body can adapt to the decrease in oxygen molecules at a specific altitude. This process is known as acclimatization and generally takes 1-3 days at that altitude. For example, if you hike to 10,000 feet (3,048 meters), and spend several days at that altitude, your body acclimatizes to 10,000 feet (3,048 meters). If you climb to 12,000 feet (3,658 meters), your body has to acclimatize once again. A number of changes take place in the body to allow it to operate with decreased oxygen. The depth of respiration increases. Pressure in pulmonary arteries is increased, "forcing" blood into portions of the lung which are normally not used during sea level breathing. The body produces more red blood cells to carry oxygen, The body produces more of a particular enzyme that facilitates the release of oxygen from hemoglobin to the body tissues. Are there genetic dispositions to altitude? There is considerable variability between individuals and between populations in their ability to adjust to the environmental stresses of high mountain regions. Usually, the populations that are most successful are those whose ancestors have lived at high altitudes for thousands of years. This is the case with some of the indigenous peoples living in the Andes Mountains of Peru and Bolivia as well as the Tibetans and Nepalese in the Himalaya Mountains. The ancestors of many people in each of these populations have lived above 13,000 feet (ca. 4000 meters) for at least 2,700 years. More from The Sports Gene by David Epstein in Chapter 14. In addition, GM vehicles equipped with those two safety features had 64% fewer front-to-rear crashes with injuries. The study also assessed forward collision technology separately. The hotel group, which runs more than 6,700 properties around the world, was informed in September about an attempt to access the database, and an investigation this month revealed that unauthorized access had been made on or before Sept. 10, Marriott said in a statement. The investigation also found that an “unauthorized party had copied and encrypted information, and took steps toward removing it,” the statement said. Dunkin’ Donuts has announced that it discovered a recent digital security breach in which hackers may have accessed the names and email addresses of customers signed up for the chain’s DD Perks mobile app, its primary rewards program. The alleged attack occurred in October. Customers' 16-digit DD Perks account numbers and QR codes for the program may have also been compromised. On Wednesday, the company released a statement explaining how “third-parties” acquired the information: “Although Dunkin’ did not experience a data security breach involving its internal systems, we’ve been informed that third-parties obtained usernames and passwords through other companies’ security breaches and used this information to log into some Dunkin’ DD Perks accounts.” The company added, “These individuals then used the usernames and passwords to try to break into various online accounts across the Internet.
