A Tongaat man has told East Coast Breakfast about the heartbreaking moment he lost his wife when Monday's tornado destroyed their home. On Monday afternoon, large parts of KwaZulu-Natal (KZN) were battered by damaging winds and torrential rain. Some areas, particularly in Tongaat, were devastated by a wedge tornado. The East Coast Breakfast team, in collaboration with The Gift of the Givers, hosted a first-of-its-kind 'WhatsAppathon' on Friday morning. The event was broadcast live from the Vishwaroop Temple in the Tongaat area to raise funds for those affected. One Tongaat resident, Viven Gounden, (pictured below) joined the East Coast Breakfast team to share his story. He told the team how his wife, Rennisha, died after the collapse of their home. According to Gounden, he managed to shield his seven-year-old son, but the strong winds caused the ceiling to collapse. "In one minute, everything stopped; the entire house was completely wrecked." As a nurse, Gounden immediately attempted to resuscitate his wife, but was unable to sustain his efforts due to the severity of her injuries. Webpage

A near-death experience (NDE) is the conscious, semi-conscious or recollected experience of someone who is approaching or has temporarily begun the process of dying. An example is a cardiac arrest that is followed by resuscitation. People who recall near-death experiences have described perceiving a variety of surreal phenomena, such as seeing themselves from above or passing through a tunnel of light. Reports of these experiences sometimes include religious or spiritual interpretations and have fueled debates about whether a person's consciousness can persist after death. Some scientists dispute that NDEs reflect post-mortem consciousness and have sought to explain them in terms of changes in brain function during the process of dying. This show is a dynamic scientific and psychological live in-depth interview with world re-known expert and author on NDE, Dr. Bruce (Charles) Greyson.

Intubation is a life-saving intervention, but it can also kill your patient! That's why this follow-up episode to last week's patient story is covering the process of intubation, what can go wrong and how nurses can mitigate risk factors to avoid bad outcomes like cardiac arrest. In part two of this intubation series, host Sarah Lorenzini details proper intubation preparation and the role of nurses during intubation. She walks through the entire process from assembling a team to what supplies and medications you need, and how to use them so that you reduce harm to the patient. You'll hear about key factors to consider when giving patients sedative or paralytic agents, as well as how to prepare for a blood pressure drop after intubation. This episode is full of useful information that will help you during your next intubation, such as pre-oxygenation strategies, RSI (rapid sequence intubation) vs when to opt for DSI (delayed sequence intubation), and how to mitigate some of the negative side effects that go along with these drugs.Plus, find out how Sarah's patient story from last week's episode ends. Contributor: Meghan Hurley MD Educational Pearls: What is ATLS? Advanced Trauma Life Support (ATLS) is a systematic and comprehensive approach to the evaluation and management of trauma patients It was developed by the American College of Surgeons (ACS) The key components include the Primary Survey ("ABCDE"), the Secondary Survey, Definitive Care, and Special Considerations What are the issues with ATLS? ATLS relies on many algorithms and rules-of-thumb, which might be helpful for individuals with basic skills and training but might actually present obstacles for those with higher levels of training. Dr. Hurley cites several examples. Example 1: ABC approach to trauma patients ABC stands for Airway, Breathing, and Circulation but focusing on the airway first is not always the best decision. Immediate attention may need to be applied to massive hemorrhage. Intubating a patient that is hemodynamically unstable may cause cardiac arrest. A more helpful phrase might be “Resuscitate before you intubate.” Example 2: C-spine precautions Cervical collars may impede the likelihood of first-pass success when intubating. The risk of complications from a failed airway may often outweigh the risk of causing a spinal cord injury. Example 3:Cutting clothes off. The E of ABCDE stands for exposure which means fully undressing the patient to look for missing injuries. This often involves cutting their clothes off. This practice might be too broadly applied and leave low-risk trauma patients without any clothes to wear when discharged home. Example 4: Digital rectal exam A rectal exam can be a useful tool in the evaluation of patients with abdominal or pelvic injuries. It can help screen for rectal bleeding, pelvic fractures, and neurological function However, the rectal exam is not a sensitive test. A retrospective study from the Indian Journal of Surgery found that a rectal exam missed 100% of urethra injuries, 92% of spinal cord injuries, 93% of small bowel injuries, 100% of colon injuries, and 67% of rectal injuries in trauma patients. Example 6: Pushing on pelvis for pelvic injuries Pushing on the pelvis to check for instability can cause further damage to an unstable pelvis. Imaging the pelvis is far more important than pressing on it if a pelvic fracture is suspected. Example 7: FAST exam A FAST exam, which stands for "Focused Assessment with Sonography for Trauma," is a rapid ultrasound examination used to assess trauma patients for signs of internal bleeding or organ damage in the abdomen and chest. These can be very useful as an initial test to tell a trauma surgeon where to start looking for internal bleeding in an unstable blunt traumatic injury If a patient is stable and likely going to get a CT scan whether the FAST is positive or negative then the test is unnecessary References ATLS Subcommittee; American College of Surgeons' Committee on Trauma; International ATLS working group. To be “Demoralized” means to be deflated of all that makes you strong – to have too many bad and discouraging things happen to you until you lose your mental clarity, and lose your drive to bring good into the world. It includes disempowerment, but it takes it one terrible step further – to where you lose your awareness of what is GOOD, and what is right. The most important first step of healing is to recover your faith that GOOD is real, and it matters. Full Question: What does the Bible say about death and dying, and how it applies Do Not Resuscitate orders? I am an ICU nurse and I deal with death on a regular basis, and lately I've been having conversations with God about it and wanted to see your thoughts. I was thinking and praying about families who hold on to their loved ones with very poor prognosis and they want aggressive treatment, and how it's almost "un-Christian" to be a DNR, akin to even murder. I was talking to God and I thought about how in the Bible, it talks a lot about dying and how it can happen at any moment, and that it is finishing the race, fighting the good fight, and not to cling to our life on this earth. But it's really heartbreaking for me to see families that profess the faith and go through this dilemma and I wanted some more Godly insight on the subject. In this episode of Yes, Please, Lisa is here to share the story of how she redefined her sexuality after becoming a mother and facing a waning sex drive. She restructured her mindset and resuscitated her pussy and her sexuality so that she would want to have sex and BE an erotic and sexual woman again. What started as a project to rekindle her sexual life, ended up as a personal discovery journey and a way to express herself while helping other women do the same. Lisa is the creator of Please Pinch Me Hard, multilingual, mother of two, wife, and author of erotica that is full of empowering and unspoken truths. If you had told Lisa, having children would leave her with no sex drive and imminent divorce, she wouldn't have believed you. If you had told her that writing a book with naughty stories would save her, she also wouldn't have believed you. But never say never! Lisa completely restructured her lifestyle and redefined her sexuality. In the process, she began writing short, erotic stories. And now? It's what makes her thrive and separates the woman from the mum. Humans are sexual and we crave sex-iness. The first step to embracing our sexual identity is to recognize that it is a normal and natural part of being human and that we all have sexual desires and needs. There is nothing wrong with wanting to explore them, however, it is no secret that our society has complicated our relationship with our bodies and with sex and sexuality. Too often, we are taught to be ashamed of our sexual desires and to keep them hidden away. This is especially true for women, who are expected to be demure. But it doesn't have to be this way. When it comes to pleasure and arousal, the brain is key, but it will also be key to maintaining desire in a long-term relationship. To create a heightened level of arousal, the brain needs to be in “sex mode.” One way to get the brain into this mode is through self-seduction. This can be done through activities such as shopping for lingerie, watching porn, and reading erotica. Now add pregnancy and motherhood on top of that. When couples become parents, it is easy to become overwhelmed with the demands of parenting, but the relationship is still important, it is the foundation of the family so communication and intimacy should be a priority. Hello Lifer, it's therapy Thursday where we answer all your deep dark and burning questions.BUT FIRST! Is the Bachelor doneski for good? It has been announced that the recent series that was shot with three Bachelors is being rested until next year, and well.... we have some feelings. Then we get into your questions and this week we have some goodies! I've just gotten back into the dating game, went on a date and he was truly great! When we got back to my house, the sex was super disappointing. We did it again in the morning (sober) and it was just as bad... My question is can you teach someone to be good at sex and is the effort worth it? Last night while having sex, the guy I've been dating whispered in my ear "my chubby, little whore." I'm not kink shaming him, but I was offended by what he said and I don't know how to talk about it with him I caught a glimpse of a snapchat conversation that my brother in law of 20 years was having with someone that is not my sister. I THINK I saw it come through from the other person and it said "I love you." I'm not sure what to do, as I'm not entirely sure what I saw... If you're a cheeky peach and want to get yourself a copy of our book WE LOVE LOVE - Jump on our website here www.lifeuncutpodcast.com.au it is hitting the shelves on Wednesday 5th and we don't want you to miss it!If you loved the episode, you know the drill - Tell your mum, tell your dad, tell your dog, tell your friends and share the love... because.... Guest presenter Dr. Cheryl Wood shares an empowering message about walking in alignment with God despite any circumstances or trials you face. No more playing it safe. There are divine appointments with your name on them. 24:05 - 1 Peter 4:10. God has uniquely placed a gift inside you that can't be duplicated. There is power in saying yes to God that opens doors in your life. Your gift could also have the answer for others to walk with God wholeheartedly. 28:22 - “Don't excuse yourself out of your divine calling.” Speak positively to yourself every day. There is power or death in the words that we speak! Proverbs 18:21 40:45 - “God can elevate anything when he's in the midst of it.” 43:27 - “God will provide enough grace to walk through what he's called you to.” Recite God's promises in times of need. 1:06:03 - “Honor god with the gift that he has given you.” Takeaways Responsibility comes with saying yes. Remain confident when asking God to use you! The divine assignment has nothing to do with the people next to you. Show up, believe and let God worry about the logistics of how something will occur. Without knowing how much time we have on the earth, we must spend our time creating legacies for people coming after us to follow.

You ever want to intubate someone, but they tell you no? Chris swears a little less (barely) in today's episode that examines DNR's, POLST forms, and inaccurate SaO2 probes. We also get another rant from Chris!
