In 2008, Barbara Harmon volunteered at an outreach program for individuals with mental illness. Half of the patients attending the program wanted to work but were told by their clinical staff that the stress of working would cause more symptoms and more hospitalization. Barbara began researching organizations that helped individuals with mental illness find employment. She read an article about Fountain House in New York City. Fountain House is the first Clubhouse and is the blueprint for 320 Clubhouses around the world. Barbara opened a Clubhouse in Pompano Beach, Florida in 2010. Since starting Foot Print to Success Clubhouse, the organization has offered people with mental illness hope and opportunities to achieve their full potential. On this episode of Bar Hacks, Barbara shares the Foot Print to Success Clubhouse mission, talks about the benefits to program participants and hospitality operators, addresses the stigma of mental illness, and more. Send us a Text Message.Dr. Ashwin Vasan, MD, PhD is the Commissioner of the New York City Department of Health and Mental Hygiene ( https://www.nyc.gov/site/doh/about/about-doh/the-commissioner.page ).Dr. Vasan is a primary care physician, epidemiologist and public health expert with nearly 20 years of experience working to improve physical and mental health, social welfare and public policy for marginalized populations here in New York City, nationally and globally. Since 2014 he has served on the faculty at the Columbia University Mailman School of Public Health and Vagelos College of Physicians and Surgeons, and he continues to see patients as a primary care internist in the Division of General Medicine at New York-Presbyterian Hospital/Columbia University Irving Medical Center.Dr. Vasan most recently served as the President and CEO of Fountain House, a national nonprofit fighting to improve health, increase opportunity, and end social and economic isolation for people most impacted by mental illness. During his tenure, he grew the organization from a New York-based community mental health organization to a national network across eight markets, and grew the budget by nearly $20 million annually. He helped navigate the organization through COVID-19 by driving new telehealth and digital mental health programs while its physical locations closed, as well as developing new community-based outreach and accompaniment programs. Further, Dr. Vasan led the creation of a national policy office in Washington, D.C., working to change national mental health policy on the issues of crisis response services and funding for and quality of community-based mental health services. From 2016 to 2019, Dr. Vasan served as the founding Executive Director of the Health Access Equity Unit at the NYC Department of Health and Mental Hygiene, which focused on the intersection of clinical and social services for the care of people involved in the justice system and other vulnerable populations — a first-of-its-kind government program in the nation. Under his leadership the team launched the NYC Health Justice Network — an innovative partnership between community-based primary care providers, criminal legal system reentry organizations, the Health Department and the Fund for Public Health — to embed tech-enabled, peer community health workers to promote health and wellness of people reentering the community from incarceration, and their families. Early in his career, Dr. Vasan spent nearly a decade at the intersection of global health, HIV and primary care, working with the nonprofit Partners In Health (PIH) in Rwanda, Lesotho and Boston, and at the World Health Organization (WHO) in Uganda and Geneva. At PIH, he led efforts to improve integrated primary care and HIV care delivery using WHO Integrated Management guidelines and training. Dr. Vasan worked as a Technical Officer under Dr. Jim Yong Kim on the WHO/UNAIDS "3by5 Initiative" to expand HIV treatment access in the developing world, focusing on pricing and access to HIV medications, and on health worker training and education.Dr. Vasan received his BA in Economics from the University of California, Los Angeles; his ScM in Epidemiology from the Harvard School of Public Health; his MD from the University of Michigan; and his PhD in Public Health from the London School of Hygiene and Tropical Medicine. Ordinary Unhappiness
62: Lacan and Psychosis in the City feat. Loren Dent

Ordinary Unhappiness

Play Episode Listen Later Aug 3, 2024 127:14

Abby and Patrick welcome Loren Dent, a clinical psychologist in the Lacanian tradition. The topic is psychosis, both as understood theoretically by Freud and Lacan, and also as experienced and encountered by real people in New York City, where Loren practices and where he has helped establish an innovative program of treatment and care. Starting by tackling a basic question – what is “psychosis?” – the three chart Freud's struggles to grasp psychotic phenomena, his messy efforts to make the notorious case of Judge Daniel Paul Schreber fit his theories about sex, and his late-career notion of “disavowal” as a mechanism of psychosis distinct from neurotic repression. Loren then describes how Jacques Lacan took this last concept, often translated as “foreclosure,” and integrated it with his own accounts of language, desire, and otherness. When taken together with therapeutic innovations by radical psychoanalytic thinkers like Félix Guattari, François Tosquelles, and Jean Oury, Lacan's insights, as Loren explains, lay the groundwork for a robust and efficacious approach to treating psychotic patients in ways that challenge traditional hierarchies in hospitals, group homes, and beyond. After walking Abby and Patrick through what talk therapy looks like with patients with psychosis, Loren outlines his recommendations for treatment and support in the clinic and beyond. As Loren explains, this approach goes against the grain of how psychotic patients have been processed by institutions under contemporary neoliberalism, and has grown only more urgently necessary in New York City under the mayorship of Eric Adams. It also forces us all to confront and manage our anxieties about “madness,” from which Freud himself was hardly immune, which haunt commonplace assumptions about normative behavior and market rationality, and which manifest in day-to-day acts of avoidance, confinement, neglect, and violence that people with psychosis encounter in urban life.Key texts cited in the episode:Gilles Deleuze and Félix Guattari, Anti-OedipusBret Fimiani, Psychosis and Extreme States: An Ethic for TreatmentFreud, Civilization and its DiscontentsFreud, “Psychoanalytic Notes on An Autobiographical Account of a Case of Paranoia (Dementia Paranoides)”Nev Jones & Robyn Lewis Brown, “The absence of psychiatric C/S/X perspectives in academic discourse: Consequences and Implications.” Disability Studies Quarterly, 33(1).Darian Leader, What is Madness?Camille Robcis, Disalienation: Politics, Philosophy, and Radical Psychiatry in Postwar FranceStijn Vanheule, The Subject of Psychosis: A Lacanian PerspectiveFoundation for Community Psychoanalysis: https://www.communitypsychoanalysis.org/Fountain House: https://www.fountainhouse.org/The Greene Clinic: www.greeneclinic.com A podcast about psychoanalysis, politics, pop culture, and the ways we suffer now. Beyond Treatment. How Clubhouses for People Living with Serious Mental Illness Transform Lives and Save Money: A Conversation with Fountain House

Heart Forward Conversations from the Heart

In this episode, we visit with two representatives from Fountain House in New York.  Our primary intent was to provide a platform to share the results of a recent research report issued by Fountain House:  Beyond Treatment: How Clubhouses for People Living with Serious Mental Illness Transform Lives and Save Money.  This first of its kind analysis not only offers a fuller accounting of the fiscal and societal costs of untreated mental illness — looking beyond health care spending to include lost wages and productivity, disability benefits, repeated emergency room visits, and criminal justice impacts — but demonstrates how clubhouses are uniquely positioned to drive down spending across the board.  The report finds that if clubhouses were appropriately resourced and expanded to serve even just five percent of the 15.4 million adults in the U.S. who live with serious mental illness, the net societal benefit would exceed $8.5 billion and offer a dramatic improvement in quality of life for countless individuals, their families, and their communities. The report notes that the U.S. has historically spent most of its mental health care dollars on clinical treatment, such as medication and therapy, with a fraction allocated to fund the community-based social supports people also need to manage their mental illness. These are policy decisions that can and should be changed. Fountain House is knitting together a national network of clubhouses to help amplify voices throughout the country to underscore the importance of the clubhouse model as a compelling mental health intervention that should be more robustly funded. The bios for our two guests are linked on the Fountain House website. Rev. Dr. Phillip Fleming wears many hats, including member, certified peer specialist and member of the Fountain House board of directors. Dr. Joshua Seidman is the Chief Research and Knowledge Officer.   Other resources and reports mentioned in this interview: Community as Therapy: The Theory of Social Practice. America faces a mental health crisis. Problems with mental health care preceded the pandemic, but over the past two years we've seen these problems grow into a crisis as young people were more likely to die from deaths of despair than from COVID-19. This presentation describes the path to solve this mental health crisis. Dr. Tom Insel says part of the solution involves fixing the care system—moving from a crisis-driven sick-care system to a comprehensive, continuous health-care system. Innovative technology will be part of this fix. So will innovative policy. But mental health is about more than mental health care. Insel says the path to mental health requires a focus on recovery defined by the 3 P's: people, place and purpose. Join us as Insel urges that we reframe our approach to mental illness, recognizing that the problem is medical but the solutions are social, environmental and political.
