We're making Team Player Podcast History AGAIN as we welcome our 1st ever Aquatics coach into the Team Player Studios! He was awarded the 2022 Asst Coach of the Year at Coach Kovo's alma mater, Fort Bend Austin HS, as well as being another in a long line of Austin College Team Player Podcast alums. He is currently the Asst Swim Coach at AHS, along with starting a brand new program this year as Head Water Polo coach, welcome Sean Brogan to the show! - Growing up in the Quail Valley neighborhood in Missouri City, TX and being one of those kids that played in the street until it was time for dinner - Being a child of educators means sometimes you attend school where they teach, which meant Coach had to leave his friends behind in the Elkins zone and make the early drive every morning over to Dulles HS with his dad who was an AP there at the time. Perhaps some of his transgressions as a young man played a factor in the decision as well so Mr. Brogan could keep a watchful eye! - He formed a special relationship with his Head Coach Jim Creech and OL Coach Chris Kahl was also being a member of the varsity swim team at Dulles. In what might be another TPP first AND last, Coach was one of the few that can proudly say they played Right Tackle and was a sprinter on the swim team! - Being treated as "the best thing since sliced bread" at a senior showcase is one way to get a Houstonian to make the 5-hour drive up to Sherman, TX for school! After battling through injuries his first 2 years on the Austin College football team, he rediscovered his love for swimming with a special coach in Bob Filander - Coach has a special talent whereas it seems wherever he goes, change soon follows. Be weary of buying him a jersey of your favorite athlete because 5 or 6 in a row of those examples ended up almost immediately traded away! Despite his propensity for being a harbinger of change coach carved out a nice career in the Football world working on a great staff at Fort Bend Clements under fellow TPP alum Bobby Darnell (ep35). However, it wasn't long before his true love came calling again, and he found himself back in the pool - Finally as the son of native Philadelphian, his Eagles and Phillies love runs true, with a bit of Sixers and Flyers mixed-in. Coach even brought the energy into the studio matching Coach Kovo as he rocked a Joel Embid Sixers jersey (keep your eyes on the headlines this summer for a blockbuster trade!) and he shared with us where the real ones go for a Cheesesteak in the City of Brotherly love, hint: it's not the tourist traps Coach Kovo fell for on his last visit. Wernicke's encephalopathy (WE) is a neurological syndrome secondary to severe, short-term B1 deficiency. In the past, but less frequently nowadays, it was more commonly associated with alcohol use disorder. However, today we know that any condition that decreases dietary intake and increases thiamine use, or its elimination, puts patients at risk of developing this encephalopathy. Causes:Chronic alcoholism is the most important cause of WE. Around 70-80% of the causes are associated with chronic alcohol consumption. Non-alcoholic WE may be caused by Decreased intake: Some types of WE may be caused by a psychiatric illness that decreases the dietary intake of B1, such as anorexia nervosa, schizophrenia, or dementia. Arreaza: Also, prolonged fasting or starvation.Lack of absorption of B1: Other causes might be related to malabsorptive syndromes, bariatric surgeries, or hyperemesis gravidarumIncreased use of B1: Any disease that increases the use of B1 and, therefore, low levels of thiamine, such as cancer, thyrotoxicosis, and systemic illnesses like infections. High-carb diets are associated with high thiamine use. Also, patients who receive IV glucose w/o supplements are at risk of developing Wernicke's encephalopathy.Increased elimination of B1: Other causes are related to increased elimination of B1, such as dialysis.Immunodeficiencies: Immunodeficiency syndromes and transplantation also cause WE.Why is thiamine important?Thiamine is one of the main cofactors in three key enzymes for energy metabolism: alpha-ketoglutarate dehydrogenase, pyruvate dehydrogenase, and transketolase. If we go back to biochemistry in med school, we can remember these enzymes play a significant role in the Krebs cycle and pentose phosphate pathways. Thiamine uses Mg+2 as a cofactor, so a magnesium deficiency can mimic WE.Pathophysiology.B1 deficiency causes lactic acid accumulation due to anaerobic glycolysis, leading to neuronal cytotoxic edema and vasogenic edema with petechial hemorrhages. MRI of the brain shows symmetrical hyperintensities, most commonly in the thalamus, mammillary bodies, cerebellum, and the periaqueductal area surrounding the third and fourth ventricles. The diagnosis of WE is made clinically, even though the MRI is a useful complementary tool to the clinical diagnosis. Diagnosis.WE presentation has always been described as the classic triad of ophthalmoplegia (or nystagmus), encephalopathy (confusion or memory impairment), and gait ataxia. However, this presentation is present only in less than 20% of patients, and most of the patients present with a neurologic syndrome that includes 2 out of the classic triad plus nonspecific symptoms such as hallucinations, hypothermia, hypotension, indifference or inattentiveness, seizures, behavioral disturbances, and bilateral lower extremity weakness. In 1997, Caine et al. suggested that a diagnosis of WE can be made if 2 out of 4 of these criteria were present in a patient with ophthalmoplegia/nystagmus + ataxia + memory impairment or confusion and clinical evidence of malnutrition or from laboratory data. Thiamine levels can be normal in patients with WE, so thiamine level is not a requirement for diagnosis.Almost 80% of WE cases are diagnosed on autopsy, which means this disease goes undiagnosed most of the time. The diagnosis is clinical, and MRI can assist in cases that are uncertain. Treatment.Thiamine supplementation is inexpensive, accessible, and easy to administer, so if we have a patient with a suspicious neurologic syndrome that could be WE, B1 must be given as soon as possible. Treatment should not be delayed while waiting for MRI results, The treatment consists of IV thiamine 500mg TID for 2-3 days, followed by 250 IM or IV for additional 5 days, in combination with other B vitamins. Because GI absorption of thiamine is impaired in alcoholics and malnourished patients, oral administration is contraindicated during initial treatment for WE. Thiamine 100 mg PO should be continued after the completion of parenteral treatment and after discharge from the hospital until patients are no longer considered at risk. Magnesium and other vitamins are replaced as well, along with other nutritional deficits if present.B1 blood levels or erythrocyte transketolase activity (which is a way to evaluate thiamine deficiency) are measured before and after thiamine pyrophosphate supplementation. A low transketolase and a more than 25 percent stimulation are positive for thiamine deficiency. This test is often not readily available, especially at the ED. High-quality liquid chromatography can also measure serum thiamine or TPP level in serum or whole blood.For practical purposes, and given high undiagnosed rates and mortality, IV thiamine should be given to all patients with malnutrition admitted to the hospital or seen in clinic. __________________________Conclusion: Now we conclude episode number 117 “Wernicke's Encephalopathy.” Dr. Malave explained that a low thiamine level is not required for the diagnosis of Wernicke's encephalopathy. Start supplementation if you have a high suspicion of thiamine deficiency, especially before giving IV glucose to a malnourished patient. Recovery is never a straight line. Things we do to comfort ourselves can create the most uncomfortable outcomes. But us humans can overcome even the most difficult situations and with good bones we can reanimate the animal within. Welcome to episode one seventy eight of Above Ground Podcast. This week we are joined by Mike Langone. Mike is the singer for Nippertown based band Black Belt Jones, the promoter of Upstate Punk Rock Flea Market and the bone artist behind Odds, Sweet Things and Dead Ends. "You can't focus so much on why, it's more like how I can make things better", says Langone about his struggles with addiction. Langone made his way to Nippertown via the Lower East Side of Manhattan more than twenty years ago. Langone took his experience and turned it into a career. First as a CASAC (Credentialed Alcoholism and Substance Abuse Counselor) and now as a Housing Specialist for an Albany, NY based non-profit who helps the homeless population. Langone expresses how much his life has changed from living the life of an addict. We take a trip down memory lane over a career that has spanned 6 decades of involvement with the game of football, including being named to the All Texas College Team and the 1971 All-Canadian team. He is a true Austin College football legend with his induction into the AC Athletics Hall of Honor in 2018. Please welcome Keith Johnston to the show! - While he was born in Toronto, Ontario it seemed Keith was destined to be linked to rodeo as he moved out West at age 4 to the home of the Stampede in Calgary, Alberta and then down South to Dallas, TX at age 11 - Starting a new program as a player at W.T. White HS in Dallas, TX in the 1960's - He formed a special relationship with his Head Coach Roland Hallmark that continued for almost 50 years in the form of reunions all the way up until Coach Hallmark's passing in 2020 - Keith shares some harrowing tales of big comebacks and fun times as a member of the Austin College football team, as well as continuing his college football career in Canada as a graduate student at McMaster University in Hamilton, Ontario. - He spent decades on the sidelines but not in the same way as most of our TPP alums. He was a Head Linesman as a Varsity football official in the Houston area, but his career took him all over the world including to England where he officiated a rivalry game the likes of Texas and Oklahoma in the form of Cambridge vs Oxford. "Healing doesn't happen when we're muscling through it", says our guest Melissa Spaulding. Melissa is a Clinical Mental Health Counselor and EMDR Practitioner based in Utah, and is our guest for episode one seventy six of Above Ground Podcast. Melissa Spaulding is a CMHC at Guided Wellness Counseling of George, Utah. Melissa's mission is to help women move past their past and to live an adventurous life. Melissa helps women work through trauma, depression and anxiety. One tool in Melissa's arsenal is EMDR or Eye Movement Desensitazation & Reprocessesing. This mode of therapy allows the receiptient to resolve unprocessed traumatic memories in their brains. There are three brains; the head, the heart and the body and as Bessel van der Kolk taught us, the body keeps the score. Melissa looks at our memories like sticky notes. When we think of sticky notes they are usually so you can take a number or name or notation. Something imortant at the time, but may not be necessary to hold on to long term. That's where EMDR comes in. It helps you clear the mental sticky notes and stop holding on to memories that no longer require you to hold on to them. Welcome Zane Brown to the show! - Growing up in Storm Lake, Iowa before making his way down to Houston in the 1st grade - Starting a new program as a player at Fort Bend Travis High School and being a part of the famed LOD defense - It may not be often that you see a Nose Guard who is also all-district on the baseball diamond but he did it as he helped lead the Travis Tigers to a district baseball championship in his senior year - After a storybook career at Travis, Coach was able to deliver an encore performance at Texas A&M Kingsville where he became one of the most decorated players in program history - Coach has had a meteoric rise in his coaching career as he tells the story of how he earned his keep from up in the box while at Stafford to how he helped Willowridge continue the big turnaround under Head Coach Richard Lazarou. Sometimes you're the scariest table in the room other times you're amongst your tribe. But now she is screaming the way out is through. We continue with our suicide prevention series with a bad ass story of how Valery went through and has a book of her poetry “through”. She has a lifetime invitation here at AGP and after listening to her you will understand why. Congratulations to her and Shane on their recent marriage. We wish them both all the best. It's expensive to be crazy, jokes Valery Brosseau. About what the cost is to have a mental illness, and run your own business. Val is a self contained creator of sorts. More humor and levity is necessary when combating stigma and allowing for people to see someone they might view differently. The universe will come together when you represent what the universe wants to reflect in you, expounds Val., In Through, her first poetry book, she opens up her hope chest and displays it in the rawest of view, for the world to see, to judge. But judgement don't matter when you are a black belt in karate and a purple in Brazilian jujitsu. I told you, she kicks ass. If you need one person or five people to pick you back up, there is no shame in any of that, expounds Brosseau. Self compassion she views as a key ingredient in healing and moving forward. Setbacks will happen it is inevitable. The best response is to take a deep breath and explore your body, your mind. We are fallible. We are human doings. Little moments of joy and being able to find those moments she acknowledges is the second key to getting over a setback. The only path we have is to move forward. Val was recently excepted into a Master's of Social Work program and is pursing her dreams.
