Is there a “right time” to have kids, when you have a career in medicine? Perhaps that's not the right question. Instead, in this episode, two rural doctors speak about the “right way” to think about making decisions about family and career.Guests:Dr Holly Atkinson- General Physician and Director of General Medicine at  West Gippsland Healthcare Group in Warragul. She's a medical oncologist and palliative care specialist.Dr Alex Tai- Infectious Diseases Physician at West Gippsland Hospital and at Latrobe Regional Health in Traralgon. He also works at the Gippsland Region Public Health Unit. Both Holly and Alex discuss how they manage the busy-ness of being parents, with the demands of being a doctor. Holly is the mother of twin 7 year-old girls and she also has a 5 year old son. She recounts how she would walk the hospital wards with babies in a carrier, when she first returned to work. Holly also speaks about what having children at different stages of your career could look like from a work perspective; including the trainee years.Alex is the father of two primary-school aged boys and also cares for his wife, who has chronic fatigue syndrome. He discusses how he overcame feeling uneasy about asking for support from family and friends and how changing the type of medical work he does has given him greater flexibility.Ultimately, it is possible to have children and a great career in medicine. Send us a Text Message.Dr. Ashwin Vasan, MD, PhD is the Commissioner of the New York City Department of Health and Mental Hygiene ( https://www.nyc.gov/site/doh/about/about-doh/the-commissioner.page ).Dr. Vasan is a primary care physician, epidemiologist and public health expert with nearly 20 years of experience working to improve physical and mental health, social welfare and public policy for marginalized populations here in New York City, nationally and globally. Since 2014 he has served on the faculty at the Columbia University Mailman School of Public Health and Vagelos College of Physicians and Surgeons, and he continues to see patients as a primary care internist in the Division of General Medicine at New York-Presbyterian Hospital/Columbia University Irving Medical Center.Dr. Vasan most recently served as the President and CEO of Fountain House, a national nonprofit fighting to improve health, increase opportunity, and end social and economic isolation for people most impacted by mental illness. During his tenure, he grew the organization from a New York-based community mental health organization to a national network across eight markets, and grew the budget by nearly $20 million annually. He helped navigate the organization through COVID-19 by driving new telehealth and digital mental health programs while its physical locations closed, as well as developing new community-based outreach and accompaniment programs. Further, Dr. Vasan led the creation of a national policy office in Washington, D.C., working to change national mental health policy on the issues of crisis response services and funding for and quality of community-based mental health services.From 2016 to 2019, Dr. Vasan served as the founding Executive Director of the Health Access Equity Unit at the NYC Department of Health and Mental Hygiene, which focused on the intersection of clinical and social services for the care of people involved in the justice system and other vulnerable populations — a first-of-its-kind government program in the nation. Under his leadership the team launched the NYC Health Justice Network — an innovative partnership between community-based primary care providers, criminal legal system reentry organizations, the Health Department and the Fund for Public Health — to embed tech-enabled, peer community health workers to promote health and wellness of people reentering the community from incarceration, and their families. Early in his career, Dr. Vasan spent nearly a decade at the intersection of global health, HIV and primary care, working with the nonprofit Partners In Health (PIH) in Rwanda, Lesotho and Boston, and at the World Health Organization (WHO) in Uganda and Geneva. At PIH, he led efforts to improve integrated primary care and HIV care delivery using WHO Integrated Management guidelines and training. Dr. Vasan worked as a Technical Officer under Dr. Jim Yong Kim on the WHO/UNAIDS "3by5 Initiative" to expand HIV treatment access in the developing world, focusing on pricing and access to HIV medications, and on health worker training and education.Dr. Vasan received his BA in Economics from the University of California, Los Angeles; his ScM in Epidemiology from the Harvard School of Public Health; his MD from the University of Michigan; and his PhD in Public Health from the London School of Hygiene and Tropical Medicine. Dr. Vineeth Jaison is currently working as Associate Professor in Neurology at Christian Medical College and Hospital, Ludhiana, India. He did his undergraduate, post graduate and Neurology training in CMC, Ludhiana. He grew up in a family of missionary doctors and would often visit his maternal uncle Laji Verghese who was Surgeon at Lady Willingdon hospital in Manali, Himachal Pradesh. He spent two years after graduation in a mission hospital at Robertsganj in Uttar Pradesh. Where he took care of Pediatrics and General Medicine while helping his wife Dr. Preethi Paul manage Obs and Gyne. Doing Ultrasounds for Obstetrics was a part of his routine work. He also spent three months managing the eye OPD with over 100 patients a day. Often during that time he would consult his seniors for difficult cases in the clinic or admitted in the ward. He also observed the immense capacity for well trained nurses to manage repetitive yet high skilled tasks. During his DM Neurology training he set up a Physician assistant programme for task shifting highly skilled yet repetitive procedures to address the shortage of doctors. He has worked closely with Dr. Jeyaraj Pandian current Vice President of World Stroke Organisation and has developed a keen interest in Public health. These laid the foundation for him to see the value in telemedicine now as qualified Neurologist. Dr. Summers is the Chief Medical Officer of Atrium Health Wake Forest Baptist Medical Center in Winston Salem, NC. In October 2020, Wake Forest Baptist Health formalized a strategic partnership with Charlotte-based Atrium Health to become one of the largest academic health systems in the U.S.Dr. Summers joined Wake Forest Baptist Health in July of 2010 as a Hospitalist in the Section of General Medicine. By 2012, he was named the first Chief in the Section on Hospital Medicine. In 2014, he was appointed Associate Chief Medical Officer, and in 2018 was promoted to Chief Medical Officer at Wake Forest Baptist Medical Center. As CMO, he holds responsibilities including leadership over the Medical Staff, Advanced Practice Providers, Care Coordination, and the Clinical Documentation Excellence Team.Dr. Summers career has been defined by the ability to implement new processes quickly and efficiently to improve patient care. He has succeeded in three major academic medical centers making change (East Carolina Health, Mayo Clinic, Atrium Health Wake Forest).He brings disciplined leadership to bear on complex issues where clinical expertise, strategic thinking, and organizational savvy are essential. With advances in genetics and other human data, researchers and doctors will one day be able to practice precision medicine. However, predicting how a patient will respond to a medicine is challenging in under-represented patients who are often not included in clinical trials. This is due in part to systemic issues that deter people from participating in research, especially those who have been historically excluded due to factors such as race, ethnicity, sex, and age.
