Did you know that musculoskeletal care is one of the biggest challenges facing the value transformation of healthcare? Musculoskeletal healthcare spending is rising at an unsustainable rate – having doubled in the last decade! Many of these procedures (such as knee replacements and spinal fusions) contribute to overspending on care that is not even needed, as it has been estimated that only 50% of MSK procedures are evidence-based. The rising costs ofmusculoskeletal care is now a top cost driver for employers and health plans. Yet despite rising costs, MSK outcomes for members haven't gotten better over the last ten years. Studies have shown that, despite a dramatic increase in health care expenditures for patients with back and neck problems over the last decade, there has not been a corresponding improvement in patient clinical outcomes. In short, spending more money has not correlated with better outcomes. We are in dire need of a value-based revolution in the provision of musculoskeletal care. The current system is clearly broken. Studies have demonstrated that common approaches to MSK care (surgery, imaging, injections, opioids) do not effectively resolve back and joint pain. And the challenges of escalating costs associated with such low value are too big to ignore. In this episode, we discuss how you can't separate racism from healthcare with Dr. Uchenna “UC” Ossai, PT, DPT, WCS, CLT. As the only Black pelvic health physical therapist certified in Sexuality Counseling, UC has been empowering people to embrace their sexuality with their whole selves - and in spite of what society may be telling them. Dr. Elizabeth Teisberg is a leading figure in the value-based health care strategy movement and is the executive director of the Value Institute for Health and Care as well as a Professor at the Dell Medical School at the University of Texas at Austin. With her deep background in strategy and innovation, and with special attention to the health care sector, she collaborates closely with Michael Porter, renowned authority on competitive strategy. Together they co-authored “Redefining Health Care: Creating Value-Based Competition on Results” (Harvard Business Review Press, 2006), which received the American College of Healthcare Executives' 2007 James A. Hamilton book of the year award. Teisberg's forthcoming book, “Capability, Comfort and Calm: Designing Health Care Services for Excellence and Empathy” is co-authored with Scott Wallace, who also recently joined the Dell Medical School faculty. While her definition of value in health care, “the measured improvement in a patient's health outcomes for the cost of achieving that improvement,” has largely been adopted by the industry, the VBHC movement continues to move at a glacial pace in juxtaposition with the moral and economic imperative to make it happen. We struggle with how to operationalize it as we often conflate value-based health care with operational programs that focus on cost reduction, quality improvement, or patient satisfaction. Those efforts – while important – are not the same as value, which focuses primarily on improving patient health outcomes. We are not pursuing more treatment, we are pursuing more health and more caring. Dr. UC is assistant professor at the University of Texas Dell Medical School and also serves as pelvic health program manager at UT Health Austin. She is on faculty for the University of Michigan, School of Social Work's Sexual Health Certification Program. Dr. UC is one of few licensed physical therapists in the world with an AASECT certification in sexuality counseling; as well as the only Black physical therapist with this specific designation. Dr. Buckley is the Medical Director and primary surgeon at The Center for Heartburn & Esophageal Disorders at UT Health Austin. Many of us have experienced heartburn or reflux (the movement of contents from the stomach to the esophagus) at some point in our lives- it is practically a household term these days. But did you know that, if left untreated, the consequences can really take a toll on our health? Dr. Buckley shares some of the symptoms of these conditions with us today, which range in severity from regurgitation or choking, to changes in voice and even affecting our lung function.Treatment options for heartburn usually begin with dietary changes and acid-reducing medications (e.g. Zantac, Protonix); however, this will only provide relief for 60-70% of people. For those who do not experience relief from traditional treatments, all hope is not lost!
