Skin conditions and treatments. That is this week's show with naturopathic doctor and herbalist Dr Julie Greenberg, founder of rootcausedermatology.com, helping clinicians get to the root of why skin presentations might be happening for that person and how to treat more holistically. She founded her clinic in LA, USA, The Center for Integrative & Naturopathic Dermatology Inc where she also sees patients to support them. Adult hormonal acne can start to affect every facet of your life. It can cause self-esteem issues, stress, and anxiety. Especially if you have tried the usual conventional treatments (like birth control pills and antibiotics) and alternative options (like an acne diet) — and still, your acne persists (or gets worse over time). That's why digging deeper is so important. Acne (and even hormonal acne) is usually a sign of a bigger problem lurking under the surface pointing towards issues like gut microbiome imbalances, thyroid disease, sex hormone issues, poor sleep, and blood sugar imbalances.Though diet can play a role, acne diet approaches aren't a quick fix, nor are they always successful. While many recommend eliminating dairy consumption, there's a growing number of nutrition professionals pushing back against this.So if you think that figuring out what's going on with your acne is confusing, you're not alone! If you've been disappointed by keratosis pilaris treatment options + natural remedies -- you're not alone! Keratosis Pilaris (aka. Chicken skin)  is a complex, poorly understood skin problem that can make you feel really insecure (because of how it looks). It can also make your skin texture feel unpleasant + rough (potentially triggering skin picking). The most common keratosis pilaris treatment includes scrubbing off the involved areas of rough skin, but beyond that – you're probably already figured out the options are limited. There is no medicine for keratosis pilaris AND there's very little research on keratosis pilaris causes. That's because conventional medicine doesn't recognize it as a significant skin problem, even though the Cleveland Clinic states that "about 50% to 80% of teenagers and 40% of adults will develop" keratosis pilarsis during their lifetime. (ref) I previously discussed keratosis pilaris diet concerns HERE, and wanted to share more up-to-date info that will hopefully help you on your journey to ditching chicken skin! Alopecia has been a hot topic in the media lately, but for others who have been dealing with hair loss for some time, it can be triggering and even devastating. My guest today, Dr. Julie Greenberg, is a licensed ND who specializes in integrative dermatology and works with hair loss in her practice. She is the founder of the Center for Integrative Dermatology, a holistic dermatology clinic that approaches skin problems by finding and treating the root cause. Dr. Greenberg holds degrees from Northwestern University, Stanford University and Bastyr University, and received advanced clinical training at the Dermatology Clinic at the University of Washington Medical School and at the Pediatric Dermatology Center at Seattle Children's Hospital. She is also the Program Chair of the Naturopathic & Integrative Dermatology series on LearnSkin.com, a learning platform for integrative health care professionals. Join us as we discuss what a functional approach to alopecia areata is and learn about the many types of alopecia there are. I'd love to know if you have tried any of the tips that Dr. Greenberg has shared in this episode -- Let me know in the comments! In this episode: Different types of alopecia Patterns in stool testing that Dr. Greenberg tends to see in hair loss cases What vitamins are actually important for hair health (hint - it's not biotin!) Why autoimmunity is more than just gut dysbiosis Are biologics or topical minoxidil worth trying if you want to deal with alopecia holistically? Why it's important to catch hair thinning early rather than waiting it out Quotes “Biotin always comes up and it's another one of those things where, absolutely, if someone is deficient in biotin, it can cause hair loss and we would supplement, but biotin's got a really great PR person and it's out there in every supplement, and every patient who's losing their hair comes to me just taking tons of biotin. And there's no evidence to prove, unfortunately, that if you're not deficient in biotin, that biotin supplementation is going to help, and it can throw off a lot of labs, so a lot of my patients, I have to take off of biotin." [14:51] “When we think about an autoimmune disease, we have to stop for a minute and think like, wow, things have really gone awry in the body when the body can't identify self, right, because that's its number one job, is we have to know, what's self and what's an invader? We don't want to be attacking self. That's a self-defeating purpose. And so, for the body to get that far off, it usually takes both the microbiome gut dysfunction, leaky gut picture, and a toxic element where it's just so overwhelmed, it's under so much attack, it's just so confused that it's just firing at everything." So if you've suffered with skin problems for months, years or even decades, it might just be your body's way of telling you there's a problem inside. Have you heard that if you're going through Topical Steroid Withdrawal (TSW), that you just have to wait it out? "Just give it time... time is what heals." But when you say that to someone who's years (3 or 5 or even 8 years) into TSW, that statement starts feeling like a bitter pill with no hope in sight. Some practitioners (like myself) are finding that there are things you can do to better support your body as it goes on this journey. There's certainly no silver bullet solution as we still do not have enough research to know the best way forward. But if you could help your system not be as reactive, that may be a win to make things more manageable for you. My guest today, Dr. Julie Greenberg, is a licensed ND who specializes in integrative dermatology. Join us as we discuss whether TSW really is just a "waiting game" or if there are other things you can do that can help. I'd love to know if you have tried any of the tips that Dr. Greenberg has shared in this episode -- Let me know in the comments! In this episode: What is happening to someone going through Topical Steroid Withdrawal (TSW)? Testing that could be helpful for TSW Concerns over vastly different potencies of different types of steroid creams Is TSW the ONLY thing you need to worry about at the moment (if you're experiencing it)? Collagen supplementation for TSW Thoughts on adaptogenic herbs -- do they have a place to support your body through TSW? Quotes “You have to think if you put that hydrocortisone on 600 times, that's equivalent to one dose of betamethasone or clobetasol. Some of them are 150 times, so it depends on the strength of the steroids. The surface area that you're using on, how frequently and how long have you used it. And of course, one thing that we forget to mention is that we have higher systemic absorption of these topical steroids on compromised or broken skin.” “We know that that red, inflamed skin is vasodilation. The vessels have been constricted for so long artificially with the topical whatever you're using and now they're also bouncing back. So we get vasodilation, which means that the blood vessels are over-expanded, over dilated. We get this blood influx, which is why they have that red, inflamed skin and moisture leaking out of their skin and they're oozing.”

Skin issues can come from just about anywhere. Frequently, people are given inaccurate or unhelpful advice from conventional medical providers. That’s why it’s so important to understand where these skin issues come from and how we can best treat them.   About Dr. Julie Greenberg Dr. Julie Greenberg is a licensed Naturopathic Doctor who specializes in integrative dermatology. She is the founder of the Center for Integrative Dermatology, a holistic clinic that approaches skin problems by finding and treating the root cause. Dr. Greenberg holds degrees from Northwestern University, Stanford University, and Bastyr University. She lectures at naturopathic medical schools and speaks at conferences across the U.S. on the topics of hair, skin, and nails. Dr. Greenberg is the Program Chair for the Naturopathic & Integrative Dermatology Series at LearnSkin, a 20 course CE program for doctors that discuss evidence-based alternative approaches to treating dermatological conditions.   Getting to the Root Cause of Chronic Skin Conditions No single system of your body exists in isolation. This includes your skin. Problems in your gut often show up as a skin condition. Dr. Greenberg explains how to heal your microbiome by getting diverse inputs from lots of different foods, particularly fibrous ones, to get your gut back on track. By correctly addressing the root causes of skin conditions this way, you can start to see actual changes. Fixing the gut to heal the skin usually means getting your microbiome back in balance. Dr. Greenberg talks about how antibiotics, certain medications, alcohol, and lack of fiber can all lead to issues like leaky gut. Leaky gut is at the root of so many dermatological conditions. When food passes through the mucosal barrier in the intestines, inflammation will naturally follow.   The Connection Between Your Skin and Your Food Fortunately, there is a lot that you can do to address these issues. Dr. Greenberg describes how reducing toxins and taking care of your immune system goes a long way. There are also some foods to avoid if you have certain sensitivities. Dairy and wheat are big ones to stay away from for many people in general since they often cause inflammation. For those with specific diseases, such as rosacea, hot foods and spice are often the culprits. Since food sensitivities come in so many forms, it is important to work with your practitioner to discover what’s best for you. Food can absolutely be a trigger and make your symptoms worse, but it is generally not the root cause, so we still have to go deeper.” [11:30] “You can’t fix these chronic dermatological issues just from the outside in. The body is telling you there’s a problem inside. [17:03] “I find that usually my approach and my protocols as a naturopathic doctor treating skin are much more aligned with nutritionists than dermatologists because steroids are not the way to go.” [47:52]   Review Fed and Fearless on Apple Podcasts! You'll also learn if coal tar shampoo and zinc pyrithione is a good option for seborrheic dermatitis. Why you should not apply undiluted essential oils directly to the skin We also talked about how seborrheic dermatitis is caused by malassezia.  Everyone has malassezia. It is a lipophilic yeast that eats lipids, and it's a normal part of our body and our microflora. There are 14 different species of malassezia on humans, but they're on other mammals like dogs. We actually think that every mammal has malassezia -  but most people haven't heard of it! It's a yeast that everyone has that isn't talked about a lot.    Malassezia causes many different skin conditions, but the most common is seborrheic dermatitis, and we call it Seb Derm.
