Curious about the white lines on my nails. I've had them on and off throughout my life. I heard it could be about nutrient deficiencies. I've done the HTMA and completed a protocol to balance my minerals, I eat a clean whole food diet, I take HCL with my meals to help digestion. I also get a lot of random bruises on my legs, unsure if this is related. I've discussed topical steroid withdrawal symptoms and TSW skin concerns extensively on the Healthy Skin Show, but there is still so much about this condition that isn't well understood.Partly because we don't yet have much research on TSW.So when I heard that Dr. Ian Myles was looking into topical steroid withdrawal symptoms, I had to get him on the show to break it down for you in layman's terms (because his paper is complex).Topical Steroid Withdrawal (aka. TSW and also sometimes called red skin syndrome online), is a condition triggered by the use of glucocorticoid steroids often used topically (and typically prescribed for chronic skin conditions like eczema and psoriasis).Because topical steroids are the standard of care for these conditions, many people unfortunately end up using them for extended periods of time (sometimes without any breaks and with increasing potency). And some go on to develop TSW, which is a horribly debilitating condition with symptoms that can drag on for years after stopping all steroid exposure.The research we're discussing in this episode has yet to undergo peer-review, but is in the process. Either way, the ideas are fascinating, albeit complex (especially for those with little to no experience with nutritional biochemistry). So we're hoping that this conversation helps you understand the ideas while also adding context to what's next for TSW research.My guest today is Dr. Ian Myles! There is a woman on Instagram who has recently gone viral. Maybe you have seen her? She is very vulnerable sharing her extreme case of eczema. It's all over her face, arms, hands -it's everywhere. She cannot function in her daily life as a mom because her rash is so severe. Even worse, she went to the doctor and dermatologist and was prescribed steroids. On top of her eczema, she is now experiencing Topical Steroid Withdrawal Syndrome. If you or your child suffer from an inflammatory skin condition like eczema, you are well aware that western medicines go-to solution is steroid creams. The problem with this is that steroid creams and even more natural based topical products is they don't address the root of your eczema rash. The steroids may work. The rash may go away but essentially topical treatment are a bandaid. They work by quieting the body down when really, the body doesn't want to quiet down. It developed a rash in the first place because something is wrong and it's asking you for help. When you bandaid the deeper problem with topical treatments you miss the opportunity to really heal. This is a week when I'd usually share a solo episode but I had a conversation that I just couldn't wait to let out because it's with my friend Michelle. Michelle and I know each other through our boyfriends who happen to live in the same building. A few weeks ago, I was scrolling through Tik Tok and came across a post from my friend Michelle. Michelle usually shares fun TikTok's of her boyfriend Jackson (sometimes without him knowing – like when he's sleeping!) but this one was different. In this one Michelle shared pictures of her life the past 6 months struggling with topical steroid withdrawal. TSW is XYZ I knew Michelle was going through something and that it was affecting her skin, but I had no idea the severity of it. I thought it was so incredibly brave and knew it would help so many people. I saw her a few hours later watching rugby with a group of friends and she told me it had gone viral with over 2 million views and people asking her questions and saying they were struggling too. As we were chatting about it I realized I wanted to sit down with her to learn more and share her story with others. She was so excited (and nervous to share) but also really wanted to raise awareness about topical steroids and the potential impact that so many people are experiencing but we don't know about. When Michelle was 16 she went to the dermatologist to treat her eczema and left with a low potency topical steroid - very common for treating eczema. Over the next 10 years, the dose stopped working and she continued to get increased doses to the point where it was 650x the potency of hydrocortisone. 650! And then, it stopped working entirely and her body went into withdrawal. For 4 months, she was in pain with bruising all over her body, itchy skin, couldn't wash her hair or get into the shower without pain, and was emotionally withdrawn. She couldn't leave her house and if you know Michelle, you know she is so social - always planning with friends and family. During our chat we talk about: What topical steroid withdrawal (TSW) is How she knew that's what she was experiencing and when it happened Her symptoms How it impacted her life Her support system and how critical it has been for her both mentally and physically Advocating for yourself at the doctor and seeking multiple opinions Gratitude for the little things in life that we often take for granted How she is working to heal and tackling the journey ahead Michelle and I both hope that this conversation raises awareness and any one who is using steroids understands the potential risks and advocates for their health.

This week's episode is slightly different to the usual topics we cover here in The Labia Lounge, but just as important to holistic health and wellbeing. I got a friend Carla Leske in to chat about eczema and topical steroid withdrawal (Otherwise known as TSW) because it's something that she's been battling with for over 2 years now, and something very relevant to me currently given my crazy eczema journey. Everyone needs to know about this because it's likely that you or someone close to you, including your children perhaps, will experience eczema or some sort of skin condition at some point and if you're not equipped with this information then you could be walking into a shitstorm, as you'll learn from this episode! We discussed the horrifying reality of what can happen if you use topical steroid creams for skin conditions like eczema for too long, how a dependence/addiction develops and the effect this has on your adrenals and nervous system, the symptoms of TSW and Carla's journey with it, the response from the western medical system to this condition and the resources and advice out there for how to avoid or overcome it. This is such a commonly prescribed and recommended product that sooo many of us put on our bodies without realising the severity and the consequences that could entail, so I really wanted to spread awareness about it in the hopes of preventing others from finding out about it the hard way like Carla did. Listen to 32 year old Cannabis farmer Luke Spaulding tell his experience going through Topical Steroid Addiction and Withdrawal. Luke's background: California native. Moved to Oregon and is in the process of building his Cannabis farm. Has a love and appreciation for agriculture and alternative medicines for healing. At one point during his 6 months of TSW, Luke was working on his farm in the heat and had to make a bed of dirt to lay on while giving orders to his farm hands because his legs were so bad that he could not walk. He actually even contemplated ordering a wheelchair since he wouldn't walk due to his legs being so inflamed, swollen and cracked behind the knees. Luke began his Topical Steroid Withdrawal February of 2022 but had been experiencing cycles of it without knowing it before that time. As a child he always questioned if it was ok taking all these steroids, but listened to his doctors and used them for years, putting them everywhere even on his face, eyes and entire body. When asked what he feels was his worst TSW symptom? Luke responded with it being lack of sleep and insomnia (I agree it's one of the worst for sure!) Topical Steroid Withdrawal (TSW) Syndrome is an iatrogenic condition that is caused from the use of topical steroids to treat a skin problem, such as eczema. But someone without prior skin problems can also be affected by Topical Steroid Withdrawal. Cosmetic use of topical steroids used to treat things like acne or hyperpigmentation can also suffer the consequences and experience TSW. And how about mothers who put medicated ointments on their babies? Or Put OTC creams like cortisone on for bug bite relief? Yep. That can get you into Topical Steroid Withdrawal too. Topical Steroid Withdrawal is sometimes also referred to as Topical Steroid Addiction (TSA) or Red Skin Syndrome (RSS), and although these terms are often used interchangeably, they do have slightly different meanings. This article is going to focus on What is Topical Steroid Withdrawal, but for your reference and education, I'll explain what the other two can be defined as. Simply put, Topical Steroid Addiction (TSA) is when you know you're in trouble, as it's when your TSW journey to healing will begin. Your skin builds up a high tolerance to the topical steroid, starts wanting more of the drug to achieve the same results and benefit, and when you deprive the skin of the drug, it rebounds and goes into withdrawal. The “flare up” that occurs once you lessen the dosage or stop the drug all together gives you the symptoms you had experienced before taking the steroid, and often times, as is with mostly all Topical Steroid Withdrawal sufferers, the symptoms have worsened by a lot (and that's a huge understatement). RSS, or Red Skin Syndrome is something that can happen as a steroid withdrawal symptom. As the name states, your skin becomes inflamed and turns red because of the vasodilation of capillaries. The red color of the skin is accompanied by a very painful burning sensation. There are MANY symptoms of TSW, the most common being relentless itching, burning, weeping, flaking, shedding, peeling, spreading, swelling, redness, wrinkling, thin skin, pus-filled bumps, cracking, itching, nodules, swollen lymph nodes, pain, insomnia, hair loss, shivering thermoregulation, fatigue, depression and more. After a while (and sometimes a very short while) the application of steroids begins to work less and less and the original problem comes back and spreads to other areas of the body. For me, when this happened, I got the misdiagnosis (from multiple docs) that what I was experiencing was worsening eczema. Topical Steroid Withdrawal is debilitating physically, mentally and emotionally. It can leave you bedridden, jobless, hopeless and depressed. The good news? You now know the answer to the question, What is Topical Steroid Withdrawal! And you can now begin educating others and getting the help you need to heal. While there are currently no proven treatments to speed up the Topical Steroid Withdrawal recovery, there are things you can do to help comfort yourself and lessen some of the severity of the symptoms. The symptoms of TSW can include burning, weeping, flaking, shedding, peeling, spreading, swelling, redness, wrinkling, thin skin, pus-filled bumps, cracking, itching, nodules, pain, insomnia, hair loss, shivering, fatigue, depression and disability. In this episode, we are going to focus on swollen lymph nodes. Swollen Lymph Nodes are very normal during Topical Steroid Withdrawal, but please note! *If you are worried about anything seeming off or irregular or just want peace of mind, call your doctor to schedule an appointment for an exam. A couple of helpful tips: Dry brush - Grab one here (my go-to for major itch attacks!) Lymphatic drainage massage - go treat yourself! What does dry brushing do? Dry brushing unclogs pores in the exfoliation process and helps detoxify your skin by increasing blood circulation and promoting lymph flow/drainage. What does a Lymphatic drainage massage do? Lymphatic drainage massage relieves swelling that happens when an illness (in this case, TSW) blocks your lymphatic system. Lymphatic drainage massage involves gently manipulating specific areas of your body to help lymph move to an area with working lymph vessels. How do I drain my lymphatic myself? Place your flat hands on either side of your neck, just under your ears. Gently stretch the skin back (away from your face) and down, then release. Marvin Rapaport MD is widely accepted as a leading physician in Topical Steroid Withdrawal. Have you heard that if you're going through Topical Steroid Withdrawal (TSW), that you just have to wait it out? "Just give it time... time is what heals." But when you say that to someone who's years (3 or 5 or even 8 years) into TSW, that statement starts feeling like a bitter pill with no hope in sight. Some practitioners (like myself) are finding that there are things you can do to better support your body as it goes on this journey. There's certainly no silver bullet solution as we still do not have enough research to know the best way forward. But if you could help your system not be as reactive, that may be a win to make things more manageable for you. Some of them are 150 times, so it depends on the strength of the steroids. The surface area that you're using on, how frequently and how long have you used it. And of course, one thing that we forget to mention is that we have higher systemic absorption of these topical steroids on compromised or broken skin.” “We know that that red, inflamed skin is vasodilation. The vessels have been constricted for so long artificially with the topical whatever you're using and now they're also bouncing back. So we get vasodilation, which means that the blood vessels are over-expanded, over dilated. We get this blood influx, which is why they have that red, inflamed skin and moisture leaking out of their skin and they're oozing.”
Did you know that about 10% of people using Dupixent develop awful face and neck dermatitis? This issue (more officially called Dupilumab Facial Redness (DFR)) wasn't flagged during the randomized FDA trials, but has some dermatologists concerned because DFR can be incredibly severe, just as it was for one of my clients. Since Dupixent is a biologic drug used by some with Eczema and Topical Steroid Withdrawal to ease symptoms, this new problem isn't ideal. Rather than just assume that the person has a sensitivity to Dupixent, new research is showing that this may be a different problem — Malasezzia hypersensitivity. If you recall, Malasezzia is a fungal organism that normally lives in your skin's microbiome. It really shouldn't be causing an issue like this, but something about the way that Dupixent interacts with your immune system along with a compromised skin barrier could play a role. Here are the current papers discussing this topic so you have something to bring to your prescribing dermatologist so you can get the help you need if you are experiencing this! In this episode: Facial + neck redness that occurs in about 10% of Dupixent users What current research on what may be driving this weird “side effect” Treatment options listed in currently published articles The blood test marker that could be helpful in getting a clear diagnosis What to do if YOU have face + neck redness from Dupixent Quotes: Approximately 10% of Dupixent users develop red, inflamed, dry, scaly and itchy face + neck rashes requiring antifungal medication treatment. Dupixent Facial Redness was never described or mentioned in the Dupixent clinical trials for the FDA (which is surprising considering that it impacts approximately 10% of patients).

Topical Steroid Withdrawal can make you feel hopeless, because it can be so difficult to control. Plus symptoms that you experience can be incredibly debilitating. I've personally worked with a number of TSW clients who said that the TSW was by far worse than their original skin issue and that some of the symptoms (like hair loss and not being able to sleep) really pushed them to a scary limit mentally. Because I'm not trained to support people in this state, I started to ask colleagues if they knew of anyone who offered counseling to those going through TSW who also had knowledge about what they were going through. The topic of topical steroids is a tricky one… very controversial these days for some who've discovered that their body has become addicted. As you know, I've covered Topical Steroid Withdrawal (TSW) in multiple episodes of the Healthy Skin Show. Today's episode is a focused on my own personal insight as a clinical nutritionist directed specifically to support those who find themselves in TSW. There are some common assumptions that float through the TSW community that I've found to be unfortunately concerning. These assumptions have at times ended up creating an even worse state of health for a client because the advice they were given was not appropriate for them (and at times, inaccurate). While I realize that TSW is not an official medical diagnosis leaving people to essentially self-diagnose, there is a greater risk of misstep because of the complexities not just of the human body, but also because of the lack of medical support. Advocates do and share what they can, but there can be significant gaps in knowledge that many of us who work with individuals in this community are hoping to fill with new research and clinical experience. I'd like to help you avoid some pitfalls that I commonly see by sharing some pearls from my clinical practice where I work with TSW clients in addition to my ongoing research. The intention is to offer a perspective that could open up new avenues for you of working on your skin and ultimately your health. In this episode: Elimination diets + Topical Steroid Withdrawal Nutrient deficiencies in clients with TSW Could something else be making your TSW experience WORSE? Hidden reasons driving intense itchiness associated with TSW Staph infections + Topical Steroid Withdrawal Quotes: Clients with TSW tend to have a crazy amount of nutrient depletions that can be very challenging to correct. Extreme itchiness typical of TSW could be from other issues like a hidden gut infection (like H.pylori or Blastocystisis homonis), mold exposure or zinc deficiency.

Topical Steroid Withdrawal (TSW) can cause chaos in the body. But it can also begin to affect one's emotional health, as well as their relationships. My guest today, Kathryn Tullos is a Registered Nurse from Houston, Texas with a degree in Communications. She is a wife and mom of 4 kids - ages 14, 12, 10, and 9. In mid-2013, her son suffered severely from Topical Steroid Withdrawal Syndrome (TSW) for 18 months, due to steroid creams prescribed for his eczema. Kathryn threw herself into patient advocacy to try to help others navigate this often misunderstood iatrogenic disease. She joined the ITSAN board in 2015, helped write and relaunch the ITSAN website, and eventually became Executive Director. She stepped down briefly in 2018, but she couldn't stay away for long. Dr. Lio is a Clinical Assistant Professor of Dermatology & Pediatrics at Northwestern University Feinberg School of Medicine. He received his medical degree from Harvard Medical School, completed his internship in Pediatrics at Boston Children's Hospital, and his Dermatology training at Harvard where he served as Chief Resident in Dermatology. While at Harvard, he received formal training in acupuncture. Dr. Lio has written a textbook on Integrative Dermatology and has published over 100 papers.
