The glucagon-like peptide 1 (GLP-1)–based diabetes and obesity drug semaglutide, with trade names Ozempic and Wegovy, will become the second best-selling drug in 2024, with estimated worldwide Ozempic sales of US ~$16 billion. Finally, the pharmacological treatment of obesity seems to have a breakthrough after decades of setbacks, with previous weight-loss medications withdrawn from the market because of serious side effects. In this podcast, the co-author of a guest editorial, “Glucagon-Like Peptide 1 Receptor Agonists in Psychiatry,” Hubertus Himmerich, MD, reviews the implications of GLP-1 drugs for psychiatry and healthcare globally. The article is published in the May/June 2024 issue of the Journal of Clinical Psychopharmacology. Dr. Himmerich, who is from the department of psychological medicine, Institute of Psychiatry, Psychology & Neuroscience at King's College in London, authored the editorial with Dr. Susan L. McElroy, of the Lindner Center of HOPE, Mason, OH, and the Department of Psychiatry and Behavioral Neuroscience at the University of Cincinnati College of Medicine.

Two articles in the March-April 2024 issue of the Journal of Clinical Psychopharmacology present research on glucagon-like peptide-1 receptor agonists (GLP-1 RAs),  a class of medications that has drawn considerable media attention in recent months for their ability to promote significant weight loss. The articles in JCP consider their use in specific populations of patients: individuals with bipolar disorder or eating disorders (in this case, atypical anorexia nervosa). This podcast features a discussion of the articles by authors Susan L. McElroy, MD; Anna Ward, ARPN; and Dr. Anna Guerdjikova, PhD, LISW, from the Lindner Center of HOPE in Mason, OH, and the University of Cincinnati. The first article, Liraglutide in Obese or Overweight Individuals With Stable Bipolar Disorder, reports on a placebo-controlled double-blind trial, which showed that compared with placebo, liraglutide was associated with greater reductions in percent change in body weight, and reductions in weight, body mass index, binge eating and hunger. Obesity is common among individuals with bipolar disorder, possibly contributing to their well-documented higher risk of cardiovascular-related mortality. The second article is titled Semaglutide Misuse in Atypical Anorexia Nervosa – A Case Report. The report presents the case of a patient with a history of an eating disorder who intentionally misused the GLP-1 semaglutide and lost more than 40  pounds in 9 months. The case report is designed to alert clinicians of the importance of reviewing patients' medical and psychiatric histories, being attuned to patient histories of weight fluctuations, and screening for eating dysregulation when prescribing weight-loss approved medications.

In honor of World Mental Health Day, this episode explores mental health and breaking the stigma surrounding mental health, especially as it relates to communities of color. This episode provides tools for both patients and physicians to discuss their mental health openly in safe spaces without the fear of judgment or feeling like they are "weak". This episode features Dr. Danielle Johnson, MD, FAPA who is board certified by the American Board of Psychiatry and Neurology. She treats adults in the inpatient and outpatient settings and has expertise in treating women's mental health issues. She is a board member of the International Society of Reproductive Psychiatry. Dr. Johnson is an Assistant Clinical Professor in the University of Cincinnati College of Medicine Department of Psychiatry and Behavioral Neurosciences. She has spent her career at the Lindner Center of HOPE in Mason, OH. Dr. Johnson has spoken locally and nationally about mental health, she is particularly passionate about topics relating to women and minority and underserved communities. On this episode, Dr. Johnson, speaks candidly about eradicating mental health disparities, lack of access, provider bias, shortages of mental health professionals particularly those of color, and ways to break the stigma surrounding mental health.

Scott Robertson joined me on the podcast to discuss mental health in the workplace. Scott is a leader in the business and philanthropic community in Cincinnati as well as nationally.  He sits on the board for many organizations but one in particular as it relates to mental health is the Lindner Center of Hope in Cincinnati. He was invited personally by the Lindner family to help join their cause to address mental health in the community and around the world.  Scott is a rare individual in that he is very successful in business but also extremely compassionate and educated when it comes to behavioral health. He knows how important it is to open the flood gates around mental health and for us all to participate in open, honest dialogue around this topic so we can work to end the stigma that is associated with it.  Mental health in business is rarely talked about but Scott is helping to lead a charge to change that. He wrote an article for the Cincinnati Business Courier in 2019 where he put his flag in the ground as someone who wants to be an advocate for change. This episode is with Dr. Charles Brady, Clinical Director of Outpatient Services and Staff Psychologist at the Lindner Center of HOPE in Ohio. He is also an OCD/CBT Psychotherapist and Associate Professor at the University of Cincinnati’s Department of Psychiatry, where he has been for more than 25 years. Dr. Brady earned his Doctorate in Clinical-Community Psychology from the University of South Carolina and completed his post-doctoral fellowship in the Department of Psychiatry at the University of Cincinnati College of Medicine. He also has completed advanced training in the treatment of OCD and OCD-spectrum disorders through the Behavior Therapy Institute. Dr. Brady is Board Certified in Clinical Psychology by the American Board of Professional Psychology. Dr. Brady is a regional expert in the treatment of obsessive-compulsive disorder and has made numerous presentations regarding the treatment of OCD and anxiety disorders and we are so lucky to have him on the show today. Three main topics with Dr. Brady: In the treatment of OCD, oftentimes a patient will have to experience discomfort as they are exposed to the thing or things that cause them anxiety. Using this as a metaphor, we first discuss Dr. Brady’s experience as a leader and how being a leader often means doing what’s uncomfortable. Secondly, we discuss how treating OCD can be challenging for therapists and caregivers because you have to be willing to invite someone to suffer in your presence. Dr. Brady discusses with us the ways he creates safe partnerships with patients to help them build courage and embrace change. And lastly, Dr. Brady gives us an overview of the best treatments currently available for OCD and what research we should all be aware of coming up in the field.

You don’t have to finish a whole sleeve of Oreo’s after a dramatic breakup in order to be labeled an emotional eater. Emotional eating is actually the act of eating without true hunger. When we eat because we are happy, sad, bored, visually stimulated or even eating out of the routine of a typical meal hour… we are engaging in emotional eating. Everyone does it so we can all benefit from deeper insight into the emotional eating world. Today Dr. Laurie Little, Clinical Psychologist, shares tips on how to be aware of emotional eating as well ways to combat it once you recognize it.Dr. Little and I worked together for more than 12 years and she is the subject of the chapter in my book about Emotional Eating.Emotional eating is deciding to eat something even though we aren’t physically hungry.Decision making engages our mind in one of three different ways:-Rational mind: We analyze a situation based purely on logic with no focus on what we feel. Think of Dr. Spock on Star Trek-Emotional mind: Polar opposite of rational mind. Logic and reason go out the window. Decisions are made purely on our emotions often when we are overwhelmed, upset or nervous.-Wise Mind: Using logic and reason but being mindful and paying attention to how we feel.When making the decision to eat something even though you aren’t truly hungry, you may not be aware of your wise mind, but you can learn to use it. Start with small decisions. For example, when deciding where to eat out. Pause and reflect about what restaurant you should go to. Your rational mind might think about the cost or distance for a particular restaurant. Your emotional mind thinks about which place has the best desserts. Reduce impulses by pausing and reflecting on what your inner wisdom tells you. Women are more socialized to please others, so they need to think if it is what they want or if they are doing it for their spouse or kids. At the end of the day you are your own best friend, so be mindful of who you are and what you want. Trust that you know what is right for you. Realize that there are no mistakes. Every decision is an opportunity to learn and grow. Decisions with negative results help you to grow stronger and learn how to do better next time.If we aren’t truly physically hungry, why do we succumb to emotional eating?An external trigger is something outside of you; such as seeing food, smelling it or hearing about it. Distraction is the best strategy to combat that. Redirect your attention onto something other than food.An internal trigger is about your feelings; mad, happy lonely, bored, etc. Distraction won’t make the feelings go away. Instead attend to the feeling. Pause, slow down, analyze how you are feeling. Give yourself permission to feel it. You don’t have to fix it right away. Talk to someone, journal or write a letter you don’t intend to send.Reducing emotional eating can benefit anyone who wants to be healthy, whether losing weight is your goal or not.
