Derek is the general manager of the gym behind our facility called Choice Fitness. If you are local then you may be familiar with other gyms that is has been in the past. Previous to Choice Fitness, it was known as Boston Sports Club (BSC), Latitudes, and even before that it started as a Gold's Gym. We have known Derek over the last 2 years since Choice Fitness has moved in and we are very grateful for all of the changes that they have made. The way that BSC left it was unacceptable and still leaves a sour taste in people's mouths. Choice Fitness has not only created a better community environment over there, but they have also been making a lot of changes to best benefit their members. Dr. Chad has been a member of all of the gyms it has been and we will say that this gym is probably in the best condition that it has been in years. Not to mention that they have adapted to the times of today and offer strength and conditioning types of training as well as boxing classes. J Darrin Gross I'd like to ask you, Maxwell, Wu, what is the BIGGEST RISK? Maxwell Wu I think the BIGGEST RISK, you know, right now in this market It is price fluctuation, right? investors do not like uncertainty. It's almost like a natural thing that the universe wants some order there. And we don't have a threshold to go off of a baseline they go off of it's very hard to see what the future looks like. What do all those words mean? Right? You know, again, you mentioned insurance. And I think a lot of investors, and I'm sure a lot of your listeners have noticed insurance costs skyrocket 3050 100%, of what it was previously. And that's for a multitude of reasons that, you know, we can drone on about, and maybe that's something you're able to add in here. But besides in middle class materials, labor, unexpected, you know, natural events, natural disasters happening. These are all things where, you know, folks didn't think there'd be such a destructive hailstorm in Texas, right. You know, these are things that truly anomalies, black swan events. Again, market does not like uncertainty. But how do you buttress against that against these headwinds? It's you got to Proform it appropriately and do that scenario analysis. And whether we do that for line item expenses, or really on a more macro basis, what happens if interest rates go up another 50 bibs? 100, VIBs 150 bibs, what to do to the property, right? So, sensitizing is a is a great way that we see the kind of hedge against risk or rather go in eyes wide open and say, if this does happen, what's the result? What can we do, right? To your point, what is preventable weather things that you can transfer risk on. So that's, that's how we view risk. And that's how we assess it. And that's how we had it. But in terms of, again, answering your question, what do we think the biggest risk is, is, you know, the highest levels of uncertainty, right. But if you want to be more specific with regard to real estate insurance is is is certainly out there. And making sure that you have the right folks that has seen have seen, you know, where policies were previously in the high inflationary times. And in times of stress, or for limited credit supply, look, or liquidity in the market, right? All these things inform premiums and cost of really insuring these properties, which any lender you go to any good lender you go to is going to require that you have a very strong insurance policy on the property.

This podcast episode is a special one and followed our most recent breathwork and cold exposure workshop that we had at our facility ProForm Physical Therapy. In this episode Dr. Nick and Karyssa conduct an exclusive interview with Danny Massa and Frannie Marin from Survivor season 44. Danny and Frannie ran this workshop for us and we not only had an amazing time, but we had over 30 people in attendance. Breathwork and cold exposure is all the rage right now, but until you actually have someone coaching you through it then it can be hard for some to understand and feel the benefits of the breathwork and how to appropriately apply it in a physically and mentally challenging environment. Danny and Frannie make an unbelievable team compliment each other well. Danny brings the energy and has a great way of explaining how to execute the techniques of proper breathwork and its application to real life scenarios. Frannie has an amazing background in research and is able to break down the reasoning as to how this breathwork is working on a physiological level. In addition to diving into breathwork and cold exposure we learn a little more about Danny and Frannie on a personal level as well as some of their experiences on Survivor. We also talk about the the vision that both Danny and Frannie have in store following their season 44 journey. It is not only very inspiring, but it is going to help so many people in the process. Hayden graduated from the University of Massachusetts, Amherst with his undergraduate Bachelor of Science degree in 2020 and then went to continue his education at the Massachusetts College of Pharmacy and Health Science to graduate with his Doctorate of Physical Therapy in 2023. Hayden was a member of the Track and Field team as a sprinter, where he was a captain in his junior and senior year. Alongside this experience, he also coached small, and large group training sessions at Mike Boyle Strength and Conditioning gym where he earned his Certified Functional Strength Coach Certification (CFSC) and had the opportunity to work with clients of all different capabilities. We not only have a discussion with Hayden as a new member of the team, but also as a PT colleague. The topic of today is a good way to end 2022 and start 2023. Its the topic of health shaming with an emphasis on respecting your own health as we dive into 2023. We also dive into some tips to help you navigate the upcoming holiday. In 2022, we saw so many people in our community make huge strides on their health journey, and we certainly want to keep this rolling… However, in 2022, we also witnessed a culture that criticizes and shames you for giving unhealthy people a hard time, yet it “is totally fine” to make fun of people for “being healthy” by doing the boring small behaviors with pristine consistency. There is a double standard in our culture, especially in the US. We are told to be okay with your body, no matter what! But this discounts the reality that one's current weight/body fat content could very well indicate diminished poor metabolic health!! This is NOT OK!!! It is one thing to be mentally stable with where you're at and have plans in place to make strides toward a healthier you. We cannot continue on this train of “be okay in your own skin” if that means you're continuing to live an unhealthy life! Ultimately, we want people to respect their health in 2023! We are NOT saying...go body shame people for being overweight! We want people to optimize their health. In this Episode Chad and Nick get a little reckless In The RACK!!"If You Want It To Go Away...Don't Move" This is what a patient was told by her medical doctor. Both Dr. Chad and Dr. Nick dive back into the use of language as well as how this statement could not be farther from the truth! Dr. Chad talks about a patient that he had recently that was treated with the classic diagnosis of spinal stenosis. Not only was this patient not having relief with her treatments, but she was not being treated as an individual. Dr. Nick talks about a situation in which he had a phone conversation with someone that was very much misdiagnosed by his medical provider. This was a situation in which Dr. Nick saved this person from going through an unnecessary surgery. This happens all too often, especially when a patient has surgery prior to exploring conventional treatment like physical therapy. Having a patient stop a particular movement may not be bad advice as long as there is a plan to re-introduce that movement when appropriate. To tell someone that they should never do a particular movement ever again is not only bad advice, its reckless!! We feel that strong feet do improve quality of life - it makes sense, as foot issues related to weakness could causing mobility issues and limit physical activity, etc. We feel that this could be just as important if not more important as a predictor of your longevity...Just a theory lol!From an evolutionary perspective, we've been lifting things, climbing and hanging from trees forever. But in our modern day society we are becoming more "lazy". For example we now have the option to have our groceries being delivered to our home. This is an even more important reason to work on that grip strength. Now go out there and hold something. Matt also discusses his involvement with the company which started while still at school and then developed further as his progress in elite-level sport drove his research into healthy diet options to provide the essential fuel his body required.Our discussion includes how Proform Foods is managing to avoid some of the major criticisms of plant-based meats while still keeping their products enjoyable to eat and maintaining their associated health benefits. This naturally includes a discussion of the company's R&D investments, the patents they hold and the custom machinery they've developed for use in their new manufacturing facility.Having been a contract manufacturer of plant-base protein products for other brands, Proform Foods have now launched their own consumer range of products under the brand MEET, which holds a 4.5 Star Health Star Rating. (He ran a 2:16:40 and finished in 19th place, and went on to make the 1992 Olympic team and become a world championship medalist.) She also shares what it was like being home-schooled for most of her life, and opens up about how running went from something she did because her parents did it to something she really discovered for herself. Neely made her marathon debut at the 2016 Boston Marathon, where she ran a 2:35:00 and finished ninth (and was the first female American finisher), and followed it up with a 2:34:55 finish at the 2016 New York City Marathon, where she finished eighth. Cathepsine sind lysosomale Cysteinproteasen, die neben der allgemeinen Proteindegradation in Lysosomen auch spezifische Funktionen ausüben, die eine limitierte Proteolyse erfordern. Zudem werden Cathepsine sezerniert, weshalb man sie auch im Extrazellulärraum findet, wo sie ebenfalls an verschiedenen biologischen Vorgängen, wie etwa der Zellmigration/Invasion, teilnehmen. Über Cathepsin X, einen relativ neu entdeckten Vertreter dieser Proteinklasse, war zu Beginn der Promotionsarbeit noch wenig bekannt. Die Struktur und das Aktivitätsprofil konnten zwar bereits gelöst werden, über mögliche (patho-)physiologische Funktionen gab es jedoch noch keine Erkenntnisse. Das Hauptziel meiner Untersuchungen war daher, mittels geeigneter Methoden nähere Aufschlüsse über die Rolle von Cathepsin X oder seiner Proform innerhalb und außerhalb der Zelle zu erlangen. Dies sollte vorwiegend durch die Analyse der Expression und Sekretion dieser Protease, sowie durch das Auffinden von Interaktionspartnern erfolgen. Wie sich in Vorversuchen zeigte, wird Cathepsin X in humanen Leukozyten unterschiedlich stark exprimiert. Da eine hohe Expression insbesondere in Monozyten vorlag, wurde für weitere Analysen das Zellmodell THP-1 eingesetzt, das auch für die Differenzierung zu Makrophagen-ähnlichen Zellen durch Stimulation mit Phorbol-12-Myristat-13-Acetat (PMA) oder all-trans Retinsäure (ATRA) verwendet werden kann. Interessanterweise zeigten diese Agenzien unterschiedliche Auswirkungen auf die Expression und Sekretion von Cathepsin X. So wurde mit PMA eine starke intra- und extrazelluläre Erhöhung der Protease verzeichnet, während mit ATRA das Gegenteil der Fall war. Da eine differenzielle Expression von Cathepsin X in Leukozyten auf eine mögliche Funktion in der Entzündungsantwort hindeutet, schien eine Untersuchung der Wirkung von proinflammatorischen Zytokinen und extrazellulären Matrix (EZM)-Proteinen sinnvoll, weil diese Faktoren ebenfalls die Sekretion von Proteasen beeinflussen können. Die untersuchten Zytokine hatten allerdings keinen Effekt auf die Sekretion von Cathepsin X aus THP-1-Zellen, wohingegen mit dem EZM-Protein Vitronektin eine Verdopplung der Cathepsin-X-Konzentration im Medium beobachtet wurde. In diesem Kontext konnte nachgewiesen werden, dass Vitronektin durch die Interaktion mit dem Zelloberflächenrezeptor Integrin avb3 den Sekretionsapparat der Zelle beeinflusst, wobei offensichtlich das Sequenzmotiv Arginin-Glyzin-Aspartat (RGD), welches in Vitronektin enthalten ist, für diesen Vorgang entscheidend ist. Neben Cathepsin X wurde auch für die Cathepsine B und L eine erhöhte Freisetzung nach Inkubation mit Vitronektin gemessen, was zeigt, dass dieser durch das EZM-Protein ausgelöste Mechanismus nicht auf Cathepsin X beschränkt ist. Umgekehrt ließ sich in einem weiteren Zellmodell (HUVEC) durch den Einsatz von „small-interfering RNA“ (siRNA) die Expression von Cathepsin X erniedrigen, was zu einer verminderten Migration der HUVEC in einem Invasionsversuch führte. Dies deutet auf eine Funktion von Cathepsin X in der Zellmotilität hin. Weil Cathepsin X, ähnlich wie Vitronektin, ein exponiertes RGD-Motiv in seiner Proregion aufweist, sollte nun eine mögliche Interaktion mit Integrinen untersucht werden. Tatsächlich ließ sich eine RGD-abhängige Interaktion von Procathepsin X mit dem Integrin avb3 zeigen. Somit werden in dieser Arbeit zwei wesentliche neue Aspekte in der Regulation der Sekretion und seiner Beteiligung an Migrationsvorgängen gezeigt, wobei die Interaktion von Procathepsin X mit dem Integrin avb3 eine besondere Rolle zu spielen scheint. Ob diese beiden Vorgänge miteinander gekoppelt sind, konnte mit den bisherigen Ergebnissen noch nicht bewiesen werden. Insgesamt deuten die Ergebnisse jedoch darauf hin, dass extrazelluläres (Pro)Cathepsin X neben seiner Rolle als Protease auch nicht-proteolytische Funktionen, beispielsweise als Ligand bestimmter Zelloberflächenstrukturen ausüben kann. Dieser Aspekt könnte im Hinblick auf eine therapeutische Inhibition von Angiogenese und Metastasierung von Tumorzellen durch Antikörper gegen Cathepsin X und/oder Integrine von großem Nutzen sein.
