The Government Accountability Office (GAO) is recommending expansion of the Medicaid Recovery Audit Contractors (RACs) program, to include auditing of payments made to managed care organizations (MCOs), according to a GAO report released in June.Launched in 2011 as a consequence of the Patient Protection and Affordable Care Act (PPACA) by the Centers for Medicare & Medicaid Services (CMS), the Medicaid RACs were purported to be aggressive in their recouping of alleged Medicare Fee-for-Service (FFS) overpayments. They could now be poised to expand into the rapidly growing MCO plans.  During the next live edition of Monitor Mondays, Dennis R. RACS's Trauma Committee has been pivotal in helping prevent acute trauma in the community. That includes encouraging surgeons to pursue a trauma specialty. Instrumental to its success is Dr Li Hsee, who stepped down as Chair of the Aotearoa New Zealand Trauma Committee in late 2022, after eight years in the role. Among other leadership roles in trauma both at home and internationally, Dr Hsee's responsible for the development and implementation of the first Acute Surgical Unit in Aotearoa New Zealand. He's also the Clinical Director in General Surgery at Auckland City Hospital. WorldPride is the biggest event on the world's LGBTQIA+ calendar. This year, it's being hosted by Sydney – the first time WorldPride is held in the Southern hemisphere – coinciding with the iconic Sydney Gay & Lesbian Mardi Gras. This year, RACS is participating in WorldPride alongside several other medical colleges. The involvement has come about through the advocacy of a new LGBTQIA+ community, Pride in Medicine. Two key figures of the group are Dr Matt Marino, a surgical Fellow who is President of Pride in Medicine and Dr Sarah Rennie, Aotearoa New Zealand Surgical Advisor and a driver of this initiative for RACS. Both participated in the Mardi Gras parade. The College's participation demonstrates its advocacy on behalf of the health and wellbeing of LGBTIQIA+ communities. It is also part of the College's strategic focus of building a culture of respect and embracing diversity as well as serving all communities equitably to build sustainable surgical services.            In S 3 E 11 I am delighted to welcome Associate Professor Rhea Liang to the podcast.  Rhea  is a general and breast surgeon, surgical educator, and diversity advocate. She is Surgical Discipline Lead and Clinical Sub-Dean at Bond University, and  notably she is the immediate past Chair of the RACS Operate With Respect education committee.      A third of medical trainees report that they have experienced or witnessed workplace bullying or harassment, including racism in 2021 according to the AHPRA annual Medical Training Survey.  After a sentinel event in 2015 RACS  launched Building Respect.  In this conversation we delve into the nuts and bolts of what it actually takes in terms of planning,  investment, resources, expertise and supports to role out a strategy and system designed to change culture.        Rhea gives wonderful insights from her international learning and experience across disciplines and industries designing and implementing change in complex  systems.  The conversation zooms in and out from big picture systems thinking to on the ground clinician experience, from the professional to the personal and from the logistics of process to the truly practical. Rhea explains the mechanics of having peer " cup of coffee" conversations to address disrespectful behaviours, the evidence-base behind this work and some real world examples. A new foundational course in surgical robotics

Technology in surgery continues to evolve in leaps and bounds, particularly the advances of robotics. A little under 20 years ago, Professor Tony Costello performed the first radical prostatectomy using robotics in Australia. Since then, he has performed 2,500 robotic procedures and has trained dozens of international robotic surgeons at International Medical Robotics Academy (IMRA), an institution he founded and of which he is currently Executive Director and CEO. RACS has partnered with IMRA to deliver ‘The Foundations of Robotic Surgery' online introductory course, which provides foundational theory and principles required to safely perform robotic surgical procedures. This course is the first of its kind to have surgeons instructing surgeons on how to use robots in surgery. As a new Fellow, Dr Zainab Naseem was hoping to achieve a more comfortable work-life balance when she came across an advertisement for the New Fellow Rural Placement – or NFRP – on the College's website. The NFRP is an initiative delivered by RACS through the Specialist Training Program, funded by the Australian Department of Health. Dr Naseem was at Sydney's Royal North Shore Hospital at the time when she applied for the rural placement at Griffith Base Hospital, in the Riverina region of central New South Wales. Dr. Sally Langley is a plastic and reconstructive surgeon, who is based in Christchurch, Aotearoa New Zealand. She has worked in both public and private surgery in Christchurch and the West Coast of the South Island for more than 30 years. Her work has covered the whole spectrum of plastic surgery including craniofacial, cleft lip and palate, head and neck, paediatric, reconstructive including microsurgery, hand surgery, as well as skin cancer and breast surgery. She has has experienced being part of plastic surgery visits to a number of developing countries including Samoa and Tonga with Interplast Australia and New Zealand, Vietnam with the New Zealand Vietnam Heath Trust, Bangladesh, India, Indonesia, and Mali with Interplast, rebranded as ReSurge. Her last trip was to Mali in 2012. She has been an EMSB instructor in Papua New Guinea. Dr. Langley has been involved with surgical education and training throughout her career. She was an examiner in plastic and reconstructive surgery for nine years and for two years she served as the New Zealand deputy Chair of the Court of Examiners, the entity which conducts the Fellowship exam of the Royal Australasian College of Surgeons (RACS). She is a faculty member for the Surgeons as Leaders course. Dr. Langley is a former president of the New Zealand Association of Plastic Surgeons. In 2014 she was elected member of the RACS Council where she chaired the Professional Standards and Development Board from 2019 till 2021. Since May 2021 Dr Langley is the RACS President. Sally is married to Don Miskell. Sally's son Sam is married with one son and Don's three children are married all with children. That makes nine grandchildren. Sally has an ageing mother who she attends to. Since 2010, the College has awarded the John Corboy Medal in recognition of exceptional service of surgical Trainees. The medal is named after Dr John Corboy, former Chair of RACS Trainee Association, who gave generously to his peers and who offered energetic service for surgery despite facing personal adversity. On the recommendation of the Trainee's Association, the medal is awarded annually to a Trainee who demonstrates the characteristics for which Dr Corboy was admired. The award is made to a candidate for one or more of the following qualities: outstanding leadership, selfless service, tenacity, and service to Trainees of the College. RACS is considering a proposal to change the name of the College following a request from our Fellowship. RACS Council appointed a working party to progress the matter and it was agreed that an electronic vote of members on the proposed name change will be conducted later in 2022, allowing ample time for member and stakeholder education and engagement. The working group has agreed to go with the name, Royal Australian and Aotearoa New Zealand College of Surgeons (RAANZCS). In this episode, otolaryngologist Dr Rachelle Love discusses the details. Dr. Sarah Rennie has a portfolio career as a general surgeon and surgical endoscopist, locuming around Aotearoa and is one half of the Aotearoa New Zealand Surgical advisors for RACS along with Professor Spencer Beasley. She was the first SET trainee to be a grandma, and was a registrar for 18 years, in and out of training in New Zealand and the UK. Episode 6.14 Avalanche Control 101: techniques and technology used in the North American avalanche industry Have you ever wondered what exactly are RACS? What is a Wyssen tower, or a Gazex exploder? What on earth is a Daisy Bell or an O'Bellx? In this episode guest host Dom Baker sits down with a panel of Canadian avalanche professionals to talk through many of the common methods of avalanche control. Joining Dom are Mark Vesely, Robb Andersen and Val Visotzky, who bring with them a wealth of experience and have used all of the different techniques and technology discussed in this episode. The aim of the episode is to provide some background and context for many of the awesome avalanche control stories you may have heard on the podcast over the past 6 seasons. Mark Vesely is an Avalanche Technician with over twenty years of experience in operations, fieldwork, and planning. He has worked in a wide range of sectors including lift access ski areas, industrial work sites, industry training, and guiding. His work experiences have included a variety of contexts in western Canada, the United States, and Chile. The core of Mark's career has been centered around managing the avalanche safety program at Fernie Alpine Resort from 2005 to 2018. He is a professional member of the Canadian Avalanche Association and a ski guide with Association of Canadian Mountain Guide membership. Controlling avalanches with a wide variety of startegies and tactics has been a big part of Marks career. Robb Andersen is the senior Manager of the BC Ministry of Transportation Avalanche and Weather Program since 2016. Prior to that he ran the Kootenay Pass avalanche program for 6 of the 12 years he spent there. Robb Grew up in Banff and moved to Nelson in the early 1990's for “just one season” – he's still there to this day! He got his start in the avalanche world at Whitewater ski resort on the ski patrol, where he worked from 1994 to 2004. Rob had a working CARDA avalanche rescue dog from 2002 to 2010 and was the President of the Canadian Avalanche Association from 2010-12 and Vice President from 2012-16. Val Visotzky was born in Montreal. He started working as a ski Patroller in 1993 at Sunshine Village, also worked at Norquay, The Remarkables in New Zealand and St.Verans in France. He started his MOTI avalanche career in Stewart in 1999, moved to the Coquihalla program in 2003, and Revelstoke in 2010. He started as the 2ic and started running the Revelstoke program in 2013. Associate Professor Rhea Liang is a general and breast surgeon on the Gold Coast. She is a surgical educationalist and Surgical Discipline Lead at Bond University and the immediate past Chair of the Operating With Respect Committee, RACS. She also researches, advocates and consults widely in diversity and equity issues. On multiple occasions (including recently!) she has been told she is unsuited for surgery for a whole range of reasons including being small, being a woman, being too feminine, being not feminine enough, having kids, being an academic, disliking early starts, getting hungry easily, being too  outspoken about bullying in surgery, and being not outspoken enough about bullying in surgery.In this episode, we discuss her journey into general/breast surgery, with a focus on her work in Operating with Respect (OWR) with RACS, key techniques for combating bullying and racist/sexist microaggressions at work, and persisting in the surgical career as wāhine. After 30 years as a plastic surgeon, Chris Edwards transitioned smoothly into a career as a sculptor. Chris is a former president of the Australasian Society of Aesthetic Plastic Surgery, who initially chose plastic surgery all those years ago because of his interest in art. He says learning and practicing art has improved his powers of observation. Chris firmly believes studying art or photography helps elevate the skills of a plastic surgeon. Chris’s artwork will appear on the virtual gallery at this year’s RACS Annual Scientific Congress in May. Also, as part of the Congress’s opening plenary, Chris will give a presentation titled ‘The art of being a creative surgeon’. Blame the RACs for the ALJ Backlog

With little in the way of reporting on the backlog of appeals at the Administrative Law Judge (ALJ) level, you could conclude that such hearings are on hiatus.But that’s not the case, according to senior healthcare consultant Frank Cohen, who will be reporting the lead story during the next live edition of Monitor Mondays. Cohen, director of business intelligence at DoctorsManagement, is expected to report that while he had testified at four ALJ hearings in all of 2019-2020, he has already testified at four so far this year, with four more between now and June – all pertaining to cases from 2014-2015. In this episode, I am speaking with Sarah Dale - the Centre Director and Principal Solicitor for RACS Australia. RACS (Refugee Advice & Casework Service) provide critical free legal advice, assistance and representation, for financially disadvantaged and vulnerable people seeking asylum in Australia, including: women who have experienced or fear domestic, family or gender-based violence members of the LGBTIQA+ community stateless people children who are seeking protection without their parents people seeking family reunion in Australia They advocate for systemic law reform and policy that treats refugees with justice, dignity and respect. In the episode, I speak with Sarah about the history of the asylum seeker policy in Australia, the critical support that RACS provides to the community's vulnerable and the impact that COVID-19 has had on Australia's refugee and asylum seeker population, both on-shore and in off-shore detention. The refugee sector at large has been under constant threat in recent times, with federal funding decimated. RACS does not receive any federal funding to deliver services and are currently the only organisation providing free legal advice dedicated to supporting people seeking asylum. It's for this reason that I wanted to invite Sarah on the show to share how we can support these people in our community and shed light on the hidden crisis facing refugees and asylum seekers in Australia. Juul Labs, the e-cigarette behemoth partially owned by Altria, has today announced a new POS age-verification system that it will require all Juul retailers to comply with by May 2021. The Retail Access Control Standards program, or RACS for short, raises the standard for age-restricted POS systems, automatically locking the POS each time a Juul product is scanned until a valid, adult ID is scanned.
