Bird's eye view of molecular clouds in the Milky Way: II Cloud kinematics from sub-pc to kpc scales by Andri Spilker et al. on Wednesday 14 September The kinematics of molecular gas are crucial for setting the stage for star formation. One key question related to the kinematic properties of gas is how they depend on the spatial scale. We aim to describe the CO spectra, velocity dispersions, and especially the linewidth-size relation, of molecular gas from cloud (parsec-) scales to kiloparsec scales in a complete region within the Milky Way disk. We utilise the census of molecular clouds within 2 kpc from our earlier work, together with CO emission data for them from the literature. We study the kinematics and the Larson's relations for the sample of individual clouds. We also mimic a face-on view of the Milky Way and analyse the kinematics of the clouds within apertures of 0.25-2 kpc in size. In this way, we describe the scale-dependency of the CO gas kinematics and Larson's relations. We describe the spectra of CO gas at cloud scales and in apertures between 0.25-2 kpc in our survey area. The spectra within the apertures are relatively symmetric but show non-Gaussian high-velocity wings. At cloud-scales, our sample shows a linewidth-size relation sigma_v=1.5*R^{0.3pm0.1} with a large scatter. The mass-size relation in the sample of clouds is M_{CO}= 794*R^{1.5pm0.5}. The relations are also present for the apertures at kpc-scales. A suggestive dependency on galactic environment is seen, with apertures closer to the Galactic centre and the Sagittarius spiral arm having slightly higher velocity dispersions. We explore the possible effect of a diffuse component in the survey area, and find that such a component would widen the CO spectra and could flatten the linewidth-size relation. The course explores how misinformation is created, why it persists, and explores what we can do about it.   And as an added bonus, Ricardo and Mary are joined by Regina Revazova, a journalist who grew up in Russia with direct experience of how important mediums of communication for honest and critical journalism can be to a free society. In this episode, host Angie Gust talks about the use of essential fatty acids in combatting chronic inflammation which can lead to illnesses such as heart disease, diabetes, cancer and bowel diseases to name a few. Recent research has found that essential fatty acids are converted by the body to a novel genus of lipid mediators called specialized proresolving mediators. Specialized proresolving mediators represent a portion of the omega-3 fatty acid spectrum that has a big effect on reducing inflammation during the ending phase of the immune response. Importantly, research has shown that taking aspirin helps omega-3 fatty acids combat inflammatory diseases.  Turning to the environment, in November last year the U.S. Department of Energy launched a climate program called the Better Climate Challenge. The Challenge commits to reducing greenhouse gas emissions by at least 50% within 10 years. More than 90 organizations across the U.S. have joined the challenge thus far. Medicaid Paid Benefits to the Dead: Incompetence or Fraud? INDIANA The state of Indiana paid some $1.1 million in Medicaid-related payments in 2016 and 2017 to managed-care organizations (MCO) on behalf of beneficiaries who were dead, according to federal auditors reported by the United States Department of Justice. The audit, carried out by the Department of Health and Human Services Office of Inspector General (OIG) and released on February 13, 2020 revealed that in a random sample of 100 so-called capitation disbursements made to MCOs, the state of Indiana “made 95 unallowable payments.” The Office of the Attorney General found: Indiana made capitation payments on behalf of deceased beneficiaries. We confirmed that 70of the 71beneficiaries associated with the 100 capitation payments in our stratified random sample were deceased. Of the 100 capitation payments, Indiana made 95 unallowable payments totalling $79,403 ($58,773 Federal share). On the basis of our sample results, we estimated that Indiana made payments totalling at least $1.1 million ($862,097 Federal share) to MCOs on behalf of deceased beneficiaries during our audit period. Indiana did not always fully process Medicaid beneficiaries' death information in the MMIS. Although the State agency's eligibility systems interfaced with Federal and State data exchanges that identify dates of death, the State agency did not enter the dates of death in the MMIS for 48 of our sampled beneficiaries. Additionally, the State agency did not recover the capitation payments for 22 sampled beneficiaries that did have a date of death in the MMIS. The organizations that received the unlawful payments are part of the Medicaid Managed Care health care delivery system. Medicaid agencies and managed care organizations (MCOs) that accept a set per member per month (capitation) payment for these services,” the Medicaid program site states. MCOs use capitation payments to manage health care costs, utilization, and quality. The OIG concluded that the State agency made capitation payments on behalf of deceased beneficiaries. OIG confirmed that 70 of 71 beneficiaries associated with the 100 capitation payments in our sample were deceased. Of the 100 capitation payments, the State agency made 95 unallowable payments totalling $79,403 ($58,773 Federal share). The State agency did not recover any of the 95 sampled capitation payments. On the basis of the sample results, the DOJ estimated that the State agency made payments totalling at least $1.1 million10($862,097 Federal share) to MCOs on behalf of deceased beneficiaries for service dates during the audit period. As Pharmacists, we're all familiar with Medicaid and Medicare services to an extent. We've processed a claim through them before, have told patients about Open Enrollment, and hear about it quite regularly. However, what's it like being a pharmacist for human services and health care programs. Dr. Fatima Ali is the Senior Pharmacy Director of Medicaid Pharmacy Services at Commonwealth of Kentucky Cabinet for Health and Family Services. She graduated from Philadelphia College of Pharmacy in May 2018 with her PharmD. Since then, Fatima has worked in the managed care setting. She completed a one-year residency at Horizon NJ Health in Pennington, NJ. Horizon NJ Health is the largest Medicaid MCO in New Jersey serving over 800,000 members. Thereafter, Fatima worked for RxAdvance, a small PBM based out of Massachusetts. In August 2020, Fatima joined Kentucky DMS as the Associate Pharmacy Director where she managed the pharmacy benefit for MCO and FFS members.

A month before the start of the Pandemic, Dr. Pate stepped into a new role at Lifebridge. As the Chairman of the Division of Psychiatry and Behavioral Health at LifeBridge Health, he was charged with uniting the behavioral health leadership of the 5 hospitals for psychiatry. We touched on a number of interesting topics throughout our discussion. Cost of care: Maryland has a total cost of care model that uses a hospital rate setting commission to set rates across the state on a yearly basis in order to create budgets. While it has resulted in reasonable cost containment, it excludes mental health, psychiatry, and outpatient care. Telehealth: Prior to the pandemic, Maryland Medicaid had a narrow definition of who could receive or deliver tele-health, which made it very restrictive, and inaccessible to most. That definition was relaxed based on CMS guidelines during the Pandemic, and allowed patients who couldn't travel to continue getting treatment. One of the main points Dr. Pate sought to drive home was that in psychiatry, there is no statistical difference in the effectiveness of care based on the mode of delivery. Though change came slowly, and was brought about in large part due to the Pandemic, there is now legislation in MD to make tele-health a permanent change. One of the more shocking stats that Dr. Pate shared was that although there is a 2-4x higher cost associated with a co-morbid psychological diagnosis, as a country, we're spending half of what we spent 30 years ago on behavioral health services.One of the quotes from our conversation that stuck with me touched on the disparity between the way physical and mental health are addressed in this country. "There are 2 health care systems in this country, which are separate and unequal. We have a medical system which is more open and permissive in the way its allowed to bill and admit and treat patients, and then we have a mental health system which is incredibly restrictive in terms of access to quality and to services. "When I asked who was to blame for this, though Dr. Pate believes for-profit-payors are a large part of the problem, he also holds himself and his peers responsible.  He noted that for a long time, his profession refused to meaningfully participate in a more medicalized approach to treatment, and in measurement based care. When I asked Dr. Pate how we could make things better, he had several solutions:Reintegrate mental and physical health services. We should federally legislate medical necessity criteria, which currently have nothing to do with quality, and were created by MCO's.We have to legislate the types of organizations that are able to manage the healthcare of our systems - we should not allow for-profit insurance companies to be in the Managed Care business. Tens of billions of dollars in pure profit are being drawn out of the pockets of patients who have the greatest needs, since most of the big insurers manage most of the medicare and medicaid across the country. If we can provide folks with incentives to produce high quality care, that is one of the solutions to our health care criss. The more we can globally budget and include all services, and move towards national healthcare, the more we can improve healthcare.
