In 2002 a group of surgeons got together and radically changed surgical training forever. They just got together again... They call themselves the Blue Ribbon Committee. When they met in 2002 they addressed critical issues such as residency structure, specialization, work-life balance, and simulation-based training. Now, with rapid advancements in medicine and education, BRC II has revisited these topics and introduced new priorities to ensure that surgical education meets the demands of the modern era. In this episode, we are joined by Dr. Steven Stain, co-chair of BRC II and a leader in surgical education. Dr. Stain shares insights into the motivations behind the second committee, its key findings, and how competency-based education, entrustable professional activities (EPAs), and other innovations are shaping the future of surgical training. James R. Korndorffer, Jr. MD MHPE FACS is Associate Professor and Vice Chair of Education in the Department of Surgery at Stanford University. Previously he served as Vice chair of Surgery at Tulane University Health Sciences Center in New Orleans. While at Tulane he also was the Surgery Residency Program Director, the assistant dean for Graduate medical education and the Medical Director of the Tulane Center for Advanced Medical Simulation and Team Training. He received his undergraduate degree in Biomedical Engineering from Tulane University, his Medical Degree from the University of South Florida College of Medicine and his Masters in Health Professions Education from the University of Illinois Chicago. His general surgery residency was completed at The Carolinas Medical Center in Charlotte, North Carolina and his Advanced Laparoscopic Fellowship was completed at Tulane University.He is actively involved in numerous national societies including the American College of Surgeons, the Society for Gastrointestinal and Endoscopic Surgeons, the Association for Surgical Education, and the Association for Program Directors in Surgery. Imagine a world where a specialist surgeon on one continent could be virtually transported into any operating room of another surgeon specialist, anywhere in the world, to collaborate with one another visually and practically. That is now a reality with the Proximie system, an augmented reality telehealth platform and innovation in education and global surgery. Dr. Nadine Hachach-Haram if the Founder of Proximie. She shares the story of her childhood and the events that made a lasting imprint on her memory growing up in Lebanon. She learned a lot about community, people coming together and sharing and supporting each other. Seeing the resulting injuries of war and the effects it had on friends who experienced injuries led her to understand the importance and role of doctors. Her family had a profound influence on her trajectory into medicine, innovation and narrowing the gap in access to care. This became of great interest to Nadine as her own career in plastic surgery progressed. We discuss the collection of knowledge in the library database of the Proximie system and the ongoing value it has in surgical education and learning. I mention to Nadine the honor I had speaking to Dr. Joe Dusseldorp and Dr. Minas Chrysopoulo about their Proximie experience during a DIEP flap procedure. It was seamless connectivity for these two surgeons and delightful to share their experience using Proximie across continents. At the heart of this it is patients for access and quality of care. Nadine has accomplished this by reimagining the future of healthcare through the Proximie system. Challenges continue to plague the residency application process, with programs receiving increased application volume that makes holistic review difficult, and medical student applicants burdened with application costs and uncertainty about what programs best align with their values. As a specialty, Ob/Gyn is facing these problems head on, and have united within their field to create a new independent residency application system, ResidencyCAS. While substantial changes to an already stressful process can be intimidating, the new platform offers many advantages that hope to improve the residency application experience for applicants, programs, and the Ob/Gyn community as a whole. We're joined by Dr. Maya Hammoud and Dr. AnnaMarie Connolly, two of the leaders in this effort, to break down the creation of ResidencyCAS, plans for its implementation starting in the 2024-2025 application cycle, and the reaction of the Ob/Gyn and medical education communities to this change. Application season is almost here! Whether you are about to apply to surgery residency or are part of a program recruiting applicants, there are several recent changes to the process that you should be aware of. Residency recruitment continues to see increased numbers of applications that create challenges for both students and programs. ERAS and other organizations are trying to address these issues with alterations to the application process. Today, we'll discuss how the ERAS supplemental application, as well as new concepts like a single interview release period and decoupled rank lists, can help. We're joined by Dr. Jennifer LaFemina, one of the leaders in these efforts within the general surgery recruitment process. Charles J. Yeo is a hepatopancreaticobiliary surgeon here at Jefferson. He was born in East Orange, New Jersey, and attended Spring Valley Senior High School in Spring Valley, New York. He received his undergraduate degree from Princeton University in 1975, summa cum laude with an A.B. in Biochemistry. Dr. Yeo graduated in 1979 from the Johns Hopkins University School of Medicine, being awarded the Upjohn Achievement Award and he was elected to Alpha Omega Alpha and Phi Beta Kappa. While completing his internship and residency in General Surgery at the Johns Hopkins Hospital, he pursued a one year Research Fellowship at the S.U.N.Y. Downstate Medical Center in Brooklyn, N.Y. Dr. Yeo joined the faculty of the Johns Hopkins University as an Instructor and Assistant Chief of Service in the Department of Surgery in 1985, and rose to the rank of Professor of Surgery in 1996. In 1997, he became a Professor in the Department of Oncology. Dr. Yeo directed the Pancreatic Cancer Interdisciplinary Working Group at Johns Hopkins, and served as the Surgical Clerkship Coordinator and Surgical Curriculum Consultant. In 2002, Dr. Yeo was named to an endowed chair at Johns Hopkins, becoming the inaugural John L. Cameron M.D. Professor for Alimentary Tract Diseases. On October 1, 2005 Dr. Yeo was named the 8th Samuel D. Gross Professor and he assumed the chairmanship of the Department of Surgery at Jefferson (now Sidney Kimmel) Medical College of Thomas Jefferson University in Philadelphia, Pennsylvania. He currently serves on the Board of Trustees of the Thomas Jefferson University Hospitals, and as the Senior Vice President and Chair of Enterprise Surgery for Jefferson Health (an 18 hospital system). Dr. Yeo has authored over 500 peer reviewed scientific papers, 105 book chapters, and over 15 books or monographs. Dr. Yeo is also known as the “Whipple King” having completed over 1700 Whipple procedures and treated 2100 patients with pancreatic and related cancers. We are joined by Dr. Teodor Grantcharov, one of its creators. The OR Black Box is a system that collects, stores, and analyzes a large amount of data from the operating room beyond just surgical video, such as video and audio of the operating room and patient physiology data. Using the system for feedback through self-directed review, coaching, and integrated AI analysis has changed the way we can learn and teach in surgery, and may have implications for the future of evaluation and credentialing.  Learning Objectives Listeners will describe the value that accessible data review and analysis adds to surgical education. The latest podcast episode features Dr. Jonathan Dort, the Director of Surgical Education for the Inova Health System and Program Director of the Surgical Residency for the Department of Surgery at the Inova Fairfax Medical Campus. “I never understood how surgeons had egos, because it is the most continuously humbling business I can imagine being in…” Dr. Dort left private practice to join an academic program. As program director, he has seen residency education change significantly over the years with increasing regulations and requirements. Residents are changing, too, and learning in different ways. Gone are the days of “go read a textbook.” Residents are learning from online sources, SCORE, podcast for example. He had great advice for those interested in applying for residency: - There is no magic formula for getting into residency - Do the activities you are passionate about - Be yourself on the application. He is looking for the real you. You will be appreciated by the program you are the best fit for, and that is a path that is sustainable -When the blood is welling in the OR, the name on your certificate is not going to bail you out Tips for succeeding as a resident: - “Your attitude more than your aptitude will determine your altitude” - Wake up and decide what kind of day you are going to have - Reminding yourself, “This is hard, I will learn from my mistakes, I am going to make mistakes, I am going to use the support from the people around me, and I am going to support the people around me, and those are the people succeed. It is a team approach” -Burnout is not caused by hard work, it is caused by futility If you are interested in becoming a program director: - You have to love to teach and mentor - The relationships you form with the mentorship last a lifetime - You have to see three steps ahead- considering new technology (like robotics), conscious of the needed rotations - There is gratification from taking a resident from “unconsciously incompetent to consciously incompetent, to consciously competent, then unconsciously competent.”
