Have you ever wondered what it would be like to go back to the heyday of the muscle car? To see new Charger's, Chevelle's and Mustangs right off the dealer lot rolling around as everyday cars? For many of us, being a kid in the late 1960's and early 1970's would have been a dream, but for Wade Kawasaki, the President and CEO of Legendary Companies (formerly of the Coker Group), it was a reality. That led him to SEMA (Specialty Equipment Market Association) and a chance meeting with one of their Vice Presidents and from there, let's just say his career flourished. Episode 48 - A lot of physicians I’ve talked to have had some bad experiences with private equity. Maybe they got a big payout but they lost so much autonomy and had their workload increased so much that they ended up retiring or leaving the practice because they just couldn’t stand the working conditions. But, I’ve talked with other docs that said the private equity deal they did was fantastic. They got a big payout, they’ve grown their business faster than they thought possible, and the equity company is a great partner that just wants them to be able to practice great medicine on the doctors’ terms. I'm grateful to have Mark Rieboldt of The Coker Group on the podcast today to talk about private equity deals.Not every private equity deal is bad. If you do your due diligence, a private equity deal could end up being a great opportunity for your business. I would encourage you to not reject every PE deal out of hand, but take each opportunity on a case by case basis. In summary, the new guidelines affect only new and established office/outpatient E/M codes (99201-99215); however, they will significantly alter documentation of the patient visit. These changes will remove the history and exam as key components, and base code-level selection on either Medical Decision Making (MDM) or time. First, most hospitals see the essential goal of a hospitalist program as enhancing throughput (timely discharges, reduced length-of-stay, etc.). Paying hospitalists based on their productivity can be counter to these key goals, as it incentivizes them to do more. Second, the market data for hospitalist wRVU production is imprecise, given that hospitalists vary significantly in their productivity levels as a result of staffing/census dynamics. We believe the best measure of productivity for hospitalists is the patient census, not wRVUs. Key Trends from Industry Data The majority of hospitalist groups work 12-hour shifts (day and night). Working additional hours per day equates to the need for fewer shifts per year to equal a 1.0 FTE. Episode Synopsis Mark, Max and Michael discuss transitioning from a first-to-second generation transaction and industry trends affecting hospital-physician transactions. Throughout their discussion, they touch-on the top 10 lessons they have learned from these types of alignment initiatives. Top 10 Lessons Learned from First-to-Second Generation Transactions Physician market share does not mean success, so health systems have become more selective relative to transactions. (PSAs are much more popular now than 3 years ago.) Incurring losses in physician transactions may not be offset by downstream revenue (increased site-neutral payments). Ensure organization’s productivity and revenue goals are being met. (Fee-For-Service still prevails.) Consider infrastructure required to support non-productivity incentives. (Fee-For-Value payments must exist to require such.) Gradually begin tapering off productivity goals and replace with alternative incentives. (Mirror reimbursement trends.) Tie new incentives to organizational goals, such as cost-savings, quality, etc. and prepare to respond to MACRA/MIPS requirements. Begin tying incentives to reimbursement changes, such as shared savings and/or the quality/CPIA/ACI metrics being measured for MIPS. Leverage newly prioritized metrics to reap additional reimbursement incentives (i.e., commercial or MIPS increases). Continually reassess compensation structures in light of regulatory and industry trends. Welcome to the inaugural episode of Coffee with Coker, a healthcare business podcast published by Coker Group. Tim’s father is Wade Kawasaki, a past guest on Cars Yeah and President and COO at Coker Tires and he’s also the 2017 SEMA Chair elect.
Wade Kawasaki is the President and Chief Operating Officer of the Coker Group, overseeing twelve Coker Group brands and six operating companies. The Coker Group is a family of brands that includes The Coker Tire Company and many other companies offering products in the automotive marketplace. Wade is also the Founder and CEO of Exports International, an exports management company specializing in wholesale distribution of American made automotive specialty aftermarket products. He is also the Chair-Elect for SEMA, the Specialty Equipment Market Association Board of Directors and will become their Chairman in 2017.
