Don't forget about the blunt trauma that may be associated with an assault! 2.  Don't miss injuries- always start with the ABCs and do a thorough head to toe exam  3. For stab wounds to the torso- hemodynamic instability, evisceration, peritonitis, impalement, or gross blood should go to the OR. 4. The three general clinical pathways for patients without a clear indication for the OR, include serial abdominal exams, local wound exploration, or diagnostic imaging. 5. Serial abdominal exams require frequent monitoring ideally by the same team member every time to detect changes early. 6. Local wound exploration requires adequate lighting and retraction to visualize the anterior rectus fascia. A negative LWE rules out an intra-abdominal injury, but a positive LWE does not necessarily rule it in. 7. Left thoracoabdominal stab wounds require evaluation of the diaphragm to rule out a traumatic diaphragm injury. 8. If there are no clear indications for the OR, diaphragm evaluation should be performed via laparoscopy after a period of 8 - 12 hours from injury. 9. A negative pericardial ultrasound does not rule out a cardiac injury in patients with a left-sided hemothorax. 10. Patients with flank and back stab wounds should be evaluated with CT scan to rule-out retroperitoneal injuries References 1. Join us for Part 2 of the powerful discussion we started at the end of last season (Season 4, Episode 6), with our 2 experts on caring for people experiencing houselessness, James O'Connell, MD, MACP (Boston Healthcare for the Homeless) and Rachel Solotaroff, MD, FACP (Central City Concern- Portland, Oregon). From the HIV epidemic to the rise of multi-drug resistant TB and the opioid crisis, those caring for people experiencing houselessness and housing insecurity saw the rise of each of these epidemics in their patient populations long before the rest of us.  The devastating impact of the COVID-19 pandemic was particularly harsh for those in shelters or unhoused. Some say that caring for those experiencing houselessness reveals the fractures in our healthcare system earlier and more clearly than healthcare in other settings.  In our discussion, Drs. Whereas other animals rely primarily on hearing, smell, or touch to understand the world around them, human beings rely in large part on vision. A large part of our cerebral cortex is devoted to seeing, and we have substantial visual skills. Seeing begins when light falls on the eyes, initiating the process of transduction. Once this visual information reaches the visual cortex, it is processed by a variety of neurons that detect colours, shapes, and motion, and that create meaningful perceptions out of the incoming stimuli. The air around us is filled with a sea of electromagnetic energy: pulses of energy waves that can carry information from place to place. Electromagnetic waves vary in their wavelength— the distance between one wave peak and the next wave peak — with the shortest gamma waves being only a fraction of a millimeter in length and the longest radio waves being hundreds of kilometers long. Humans are blind to almost all of this energy — our eyes detect only the range from about 400 to 700 billionths of a meter, the part of the electromagnetic spectrum known as the visible spectrum. When people think of emotions they usually think of the obvious ones, such as happiness, fear, anger, and sadness. This module looks at the knowledge emotions, a family of emotional states that foster learning, exploring, and reflecting. Surprise, interest, confusion, and awe come from events that are unexpected, complicated, and mentally challenging, and they motivate learning in its broadest sense, be it learning over the course of seconds (finding the source of a loud crash, as in surprise) or over a lifetime (engaging with hobbies, pastimes, and intellectual pursuits, as in interest). The module reviews research on each emotion, with an emphasis on causes, consequences, and individual differences. As a group, the knowledge emotions motivate people to engage with new and puzzling things rather than avoid them. Over time, engaging with new things, ideas, and people broadens someone's experiences and cultivates expertise. The knowledge emotions thus don't gear up the body like fear, anger, and happiness do, but they do gear up the mind—a critical task for humans, who must learn essentially everything that they know. PODCAST SUMMARY Fatigue is one of the most disabling symptoms of multiple sclerosis and occurs in approximately 80% of people with this condition. In an article that was published in the Journal of Neurological Sciences in 2021, it was found that fatigue was the number one factor influencing self-reported ability to work. In this 2021 study, fatigue was significantly associated with an increased likelihood of missing work, low work productivity, and decreasing ability to work. When fatigue was combined with depression, there was a higher likelihood of people reducing their work hours or not working at all. And when fatigue and anxiety were present, fatigue was associated with many more work-related challenges. While fatigue can develop directly from multiple sclerosis. It can also be caused by or exacerbated by many other factors, including things like vitamin deficiencies, depression, and physical exertion. Another huge contributor to MS-related fatigue is poor sleep. Although fatigue and sleep can exacerbate one another. They can also be independent of one another. Sleep disorders are extremely common in people with multiple sclerosis. They can be caused by MS symptoms like bladder urgency at night, restless legs, chronic pain, or temperature dysregulation. But other conditions like sleep apnea are also more common and contribute to daytime sleepiness. In this podcast course, you will learn from two psychologists. The first is Dr. Anna Kratz. She's a research psychologist at the University of Michigan. She discusses new ways to assess fatigue, as well as ways to treat or mitigate its effects. Dr. Abbey Hughes from the Johns Hopkins University School of Medicine discusses different types of sleep disorders seen in individuals with multiple sclerosis, as well as a current study she's conducting to assist patients to improve their sleep. She offers advice for mental health professionals on how to work with their clients on sleep-related issues. Our patient voice and advocate is Kathy Chester. Boards or panels made from magnesium oxide (MgO) are showing up on projects across the country to act as a subfloor, sheathing, wall board, or underlayment material. In the process, they are bringing enhanced fire resistance and acoustical performance to many building types, most notably multifamily projects. Today we are talking to Kate Andrews Land, General Manager, EXA COR MgO Products, and Ben Richardson, the Technology Manager for EXACOR™ MgO products at Huber Engineered Woods. By the end of this podcast you should be able to: Learning Objectives: Identify the physical characteristics of magnesium oxide (MgO) panels in terms of their makeup and basic performance attributes for use in floor and wall assemblies in multifamily projects. Jessica Combs joins Mark to in a Q&A style discussion to address recent questions on healthcare trends. Mark addresses each question on various topics including private equity interest in healthcare, the opioid crisis, neutral site payments, healthcare technology, and revenue cycle management. Questions We have heard much discussion in the media and throughout this election season on the opioid crisis, but I’m curious if this is something of a significant impact yet on physicians and medical groups or hospitals. Have you observed any specific instances where the talk in the media and Washington has resulted in major changes – positive or negative – for healthcare entities?  We have been monitoring the issue of “neutral site payments” all year, as this could have a significant impact on hospital payments for those entities that have spent a lot of time and dollars investing in outpatient services. Can you comment on what has happened with this matter and perhaps make any predictions for how the results of the midterm elections could influence the outcome on this issue? In listening to your episode on what hospital boards should be thinking about concerning healthcare technology, we have been paying a lot of attention to the frequent stories that emerge about fines and legal issues (not to mention PR concerns) that many hospitals are dealing with in protecting records, maintaining security in the information systems, and lacking appropriate documentation. However, there are so many facets of protecting data that an organization like a hospital – even a relatively small, rural community hospital – must consider that it’s challenging to think about where to start. How would you suggest a leadership team begin to confront this issue to ensure we have as much protection as possible from some of those risks? Our organization has been struggling from revenue stagnation (and even drops in some cases) that we know are not tied to negative changes in payer contracts, fee schedule changes, expense management, or even the billing and collections component of the revenue cycle. So, we think the issue could be related to coding; however, in the past, our focus on coding has mainly been to ensure our providers are not up-coding. How could coding issues play into the broader revenue cycle management procedures? As it has become commonplace to see hospitals buying physician groups, some local markets have seen significant shifts as a result of larger groups selling to hospitals. Meanwhile, new players, such as large corporate systems or groups backed by private equity investors are entering markets and competing with the hospitals for acquiring major physician groups, which can significantly shake up the dynamic between the physicians within those groups and the hospitals they interact with on a regular basis at a local level. My question is twofold: 1) Have you seen this take place; and, 2) how might this trend potentially shake up local healthcare markets more in the future?
