The federal Office of Management and Budget (OMB) has proposed rule changes that would bring a major shift in how scientific grants are awarded by U.S. government agencies. Under the current process, researchers submit grant proposals that are then vetted and scored by a committee of experts in that scientific field, with top-scoring proposals recommended for funding. If its proposed changes are enacted, the OMB would insert a political review into the process, allowing administration officials to determine whether grant proposals are aligned with administration priorities, regardless of their scientific merit. Those proposed rules are now in a public comment period. Holden Thorp, editor in chief of the Science family of journals, joins Ira to explain why he called the change “another red alert for American science” in a recent editorial. It's no secret that the Trump administration wants to have more control of who federal agencies hire and fire. A new proposed rule from the Office of Personnel Management would provide it a new tool to do so, allowing it to examine the suitability and fitness of an employee any time, not just during the hiring process, as it is currently used. As you can imagine, there are some against the idea. One of them is Joe Spielberger, policy counsel for the Project on Government Oversight. CMS has proposed significant DME regulation changes in the 2026 Home Health Payment Update Rule that focus on combating fraud, improving supplier compliance, and enhancing efficiency through increased oversight and expanded requirements. The proposed changes would dramatically impact DME operations through annual accreditation surveys, expanded competitive bidding, and enhanced data reporting requirements.• Annual accreditation surveys instead of the current three-year cycle would increase costs and operational burden• Expanded competitive bidding to include continuous glucose monitors, insulin pumps, and certain medical supplies• Prior authorization exemption process for providers with 90% or higher claim approval rates• Enhanced oversight requirements and increased data submission for both providers and accrediting organizations• Significant financial impact through decreased reimbursement rates paired with increased costs• Comments are due by August 29, 11:59 PMWe strongly encourage DME providers to review the proposed rule and submit comments with data, beneficiary stories, and constructive suggestions. There are serious ramifications for the continued payment reductions in the Medicare home health regulations, Steven Landers, MD, MPH, CEO of the National Alliance for Care at Home, said. Beyond the lack of access to home health services, the whole payment structure has been decimated, he said. This year, the home health payment program will total just over $15 billion, down from $17.8 billion in 2019. The cuts are making it harder for agencies to have the purchasing power to staff agencies. Another negative development for home care is the passage of the “Big Beautiful” bill, which will place pressure on states to reduce home- and community-based services. The Medicaid work requirements in the law may hurt personal care workers who use Medicaid as their health insurance, he noted. While home care is not the most powerful lobbying contingent in Washington, “sometimes right beats might,” Landers said. Landers' past positions, which included his role as director of home care at the Cleveland Clinic, helped him see the value of home care and how policy making in Washington affected the care regular people receive. As part of his tenure, he would like to see home care providers be able to deploy new care models such as the Medicare Care Choices Model. More than 130 people are dead after a terrorist attack Friday night at a concert in Moscow. An offshoot of the Islamic State known as ISIS-K claimed responsibility, and U.S. officials said there's evidence to support that claim. Four suspects from Tajikistan were arrested. But Russian President Vladimir Putin instead pushed the idea that Ukraine was involved in the attack, despite the fact that there's no evidence to support it.The Texas Medical Board on Friday released its proposed definition for what would constitute an “emergency medical exception” to the state's strict anti-abortion law. The board left the rule purposefully vague, however. Molly Duane, a senior staff attorney for the Center for Reproductive Rights, explains how the proposed definition could make things even worse for Texas patients and doctors.And in headlines: Monday is the deadline for former President Donald Trump to cough up the $454 million fine he owes in his New York civil fraud case, the Princess of Wales said she's undergoing chemotherapy to treat an undisclosed form of cancer, and indicted former Rep.
