Mental health is one of the greatest health risks encountered by children and adolescents in today's world.  Stigma can be one of the biggest barriers to children being evaluated and treated for mental health problems, particularly in communities that are marginalized.  In the final episode of the miniseries on Health Equity in Children, we bring mental health into your everyday conversations to diminish stigma and give mental health the time it deserves to help your patients grow up happy and healthy. We're back for Season 2!!! Hannah and Easton take a trip back to 1882 when a group of women from the town of Westfield, IN took their frustration with the opening of a local saloon into their own hands. Abstract: SCN2A encodes Nav1.2, an excitatory neuron voltage-gated sodium channel and major monogenic cause of neurodevelopmental disorders, including developmental and epileptic encephalopathies (DEE) and autism. Clinical presentation and pharmocosensitivity vary with nature of SCN2A variant dysfunction with gain-of-function (GoF) cases presenting with pre- or peri-natal seizures and loss-of-function (LoF) patients typically having infantile spasms after 6 months of age. Here, we established and assessed patient induced pluripotent stem cell (iPSC) - derived neuronal models for two recurrent SCN2A DEE variants with GoF R1882Q and LoF R853Q associated with early- and late-onset DEE, respectively. Patient-derived iPSC lines were differentiated using a Neurogenin-2 overexpression yielding populations of cortical-like glutamatergic neurons. Electrophysiological and transcriptomic profiles were assessed after 2-4 weeks in culture. Increased neuronal activity at both cellular and network level was observed for R1882Q iPSC-derived neurons at three weeks of differentiation. In contrast, R853Q neurons showed only subtle changes in excitability after four weeks in vitro. In alignment with the reported efficacy in some GoF SCN2A patients, phenytoin (sodium channel blocker) reduced excitability of neurons to the control levels in R1882Q neuronal cultures. Transcriptomic alterations in neurons were detected for each variant and convergent pathways pointed at the shared mechanisms underlying SCN2A DEE. Copy rights belong to original authors. In this episode I had the honour of chatting with Simon Heighway. An incredibly gifted individual, Simon is the author of the book Time after Time, a geniusly simplistic piece of writing detailing just some of the many spiritual moments of awakening Simon has experienced throughout his life. Pinball: After a brief mention of Stern Pinball's Iron Maiden LE gameplay (provided by Deadflip), we move into a couple legal/business updates. The first is Zidware and pinball designer John Popadiuk's legal defeat in a civil suit regarding his failure to produce pinball machines. The second is the collapse of Heighway Pinball, its ongoing liquidation, and the rumors/drama surrounding the failure of yet another pinball company. The pinball segment concludes with the results of the Slam the Top 101-200 and the setup of Round 4 of The Worst of the Fall. Video Games: It's BattleTech day. Tony walks through his hands-on experience with the latest game in the franchise. There is also a bit of discussion regarding Mad Max and Prey.
