In this episode of the NCS Podcast Masterclass series, Stephan Mayer, MD, FCCM, FNCS, and Jon Rosenberg, MD, welcome renowned neurointensivist Chiara Robba, MD, Ph.D., to discuss the concept of brain protective ventilation. Drawing from her experience across both pulmonary and neurocritical care, Dr. Robba explores the physiological nuance of ventilating patients with brain injury, including how to balance positive end-expiratory pressure (PEEP), tidal volume and CO₂ levels to protect both lung and brain. The trio also dives into findings from the VENTIBRAIN study and practical strategies for tailoring care using multimodal monitoring. The views expressed on the NCS Podcast are solely those of the hosts and guests and do not necessarily reflect the opinions or official positions of the Neurocritical Care Society.

Electroencephalography (EEG) is an important and relatively inexpensive tool that allows intensivists to monitor cerebral activity of critically ill patients in real time. Seizure detection in patients with and without acute brain injury is the primary reason to obtain an EEG in the Intensive Care Unit (ICU); and in response to the increased demand for EEG, advances in quantitative EEG (qEEG) created an approach to review large amounts of data instantly. Today, rapid response EEG is now available to reduce the time to detect electrographic seizures in limited-resource settings. In this week's MasterClass, our hosts Dr Jon Rosenberg and Dr Stephan Mayer are joined by Dr Lawerence Hirsch, Co-Director of the Critical Care EEG Monitoring Program at Yale School of Medicine to discuss the evolution, value, and impact of brain monitoring with EEG in the critically ill.

The field of neurocritical care and traumatic brain injury management is rapidly evolving. We are shifting from older (but not entirely outdated) monitoring techniques to novel diagnostic and therapeutic advances in critical care medicine. Managing intracranial pressure crises involves the careful tiptoeing of a line between maintaining brain circulation and yet reducing intracranial fluid volume. And many of our interventions rely on one simple mathematical construct: CPP = MAP - ICP [Cerebral perfusion pressure = Mean arterial pressure – Intracranial pressure] The importance of this formula cannot be understated. So we are spending 35 minutes today reviewing it, and discussing pearls and pitfalls of ICP monitoring and management. But believe me, it’s worth it. Produced by James E. Siegler and Jon Rosenberg. In this special episode we talk with Jon Rosenberg, managing partner and co-founder of Levrose Commercial Real Estate, about guidance and advice for buyers, sellers, tenants, and landlords dealing with real estate issues they are facing during this COVID pandemic.    For example, a lot of sellers are somewhat hesitant right now to market their properties. Jon's recommendation is for them to continue to market their properties unless they think there's going to be a significant change in their rent roll, in which case wait until some of this washes out and then put it back on market (most likely at a lower price). Another thing to consider is that there is potentially a lot more buyers now that are able to look that weren't able to look previously because of time constraints.    Jon also recommends having an open line of communication with your broker. The one thing Jon has seen at his firm and at firms through TCN is that these firms have become outstanding advisors, and they'll give the honest truth. The bottom line is that every situation is different, and firms like Jon's can help provide advice and guidance depending on your individual circumstances.

How does the relationship with your readers change when you make your webcomic an explicitly part-time job? Over the last decade there has been an implicit contract made between a webcomics author and their intended audience. "I update regularly, and in exchange you support me through encouragement, word-of-mouth advertising, and purchasing merchandise." Because this contract is implicit, however, the commitment between author and audience is a bit fuzzy. If the cartoonist feels that the project has run its course or that it's not reaching the intended goals, they can retire the project without much fuss. After all, everyone was getting it for free. As more cartoonists use Patreon or Tapastic's Support Program to subsidize their webcomic, the contract becomes a lot more explicit. Introducing support tiers and rewards creates an even more clear commitment to an audience. You're saying "This webcomic is now my part-time job, and (maybe) I hope to make it a full-time job." But a traditional part-time job, even one you take on just to make some extra income, usually promises a well-defined amount of payment. With services like Patreon and Tapastic you're making the commitment to show up for the job, but there is no promise of any specific amount of pay. So to whom are you committing? Are you committed to reaching a certain monetary goal first and foremost? Or are you committing to servicing an audience with the hope of reaching a goal? And if you fail to reach that goal, how do you navigate the friction of quitting when you've already made an explicit promise to that audience? Join us for a Lean Into Art Cast with guest +Brandon Dayton. Brandon has been navigating these concerns with his new Green Monk webcomic, and together we'll explore how things change when you make your webcomics intentions a lot more clear to the public. Rob has the week off, but he'll be back next episode! Note about this episode - normally we have a video to share with the audio version of the podcast - I'm sorry we don't have that this time.
