In this episode, Dr. Sergion Zanotti discusses TEE in cardiac arrest and shock. Critical care clinicians commonly utilize transthoracic echocardiography in the ICU as part of their point-of-care-ultrasonography (POCUS) toolkit. However, there is a growing push to train intensivists in using transesophageal echocardiography (TEE) for cardiac arrest and peri-arrest situations in the ICU. Our guest is Dr. Sara Nikravan, a cardiothoracic anesthesia critical care physician with training in advanced perioperative echocardiography. Dr. Nikravan is an Associate Professor of Cardiothoracic Anesthesiology and Critical Care Medicine at the University of Washington Medical School and practices at the UW Medical Center. She is recognized as an expert and master educator in Critical Care, Perioperative echocardiography, and Point of Care Ultrasound. Cash-strapped GPs are being forced to fund an important medical tool out of pocket. The Point of Care Ultrasound (Pocus), a handheld device widely used by general practitioners overseas, is not publicly funded by Te Whatu Ora. Pinnacle Health Clinical Director Jo Scott-Jones says the tool allows doctors to get a physical view into a patient's body in order to detect conditions. "With a stethoscope, you can hear the heart murmur. With the Pocus device, you can see what's causing the heart murmur and you can determine whether it's something significant or not- you can see a lot of detail." In this conversation, we will examine the utility and functions of ultrasound within air ambulance services. We look at how the aggregation of data and familiarity with ultrasound is narrowing down and assisting time to intervention and time to definitive care. We look at the scans of preference and some of the data around pneumothoraces, tamponade and positive findings under ultrasound in the critical care patient. Importantly we will get Dan's perspective on where ultrasound is affecting patient care and where he sees the future of ultrasound going within pre-hospital care.    Dan Nevin is a PHEM consultant with London's Air Ambulance and an Anaesthetist with Barts and the Royal London NHS Trust. Dan has years of experience working in pre-hospital care in both South Africa, where he originally trained, and in the UK. His special interests include trauma, critical care anaesthesia and PHEM. Dan is also the ultrasound lead for LAA and has been leading the use of POC ultrasound within the service. In the conversation we exam: Why ultrasound has become a useful tool in trauma. Data collected on scans and relationship with intervention (surgical or otherwise). About Our Guest Dr. James Wilcox is originally from Indiana. He earned his MD from Indiana University in 2014, and completed his Family Medicine Residency at IU Health Ball Memorial Hospital in 2017. He then completed his Sports Medicine Fellowship at Western Michigan University in 2018. He first served in the rural community of Austin, Indiana, practicing the breadth of family medicine: inpatient, outpatient primary care, emergency medicine, sports medicine, and substance use treatment. He recently relocated to Indianapolis in 2020, and he is now working at Eskenazi Health in outpatient primary care. In 2021, he accepted a grant-funded position at Indiana University School of Medicine teaching Point of Care Ultrasound (POCUS) and coordinating POCUS integration into the 4-year curriculum as the HRSA PRIME POCUS Thread Director, and Indiana University School of Medicine Assistant POCUS Director. "92% of healthcare workers experienced workplace violence in April 2022" (Security Magazine)  In this episode, Amie & Sara discuss the current rise of violence towards healthcare workers. In an article posted by the Amercican Medical Association, it was noted that "...violence against healthcare employees has markedly increased since the onset of the pandemic. The American Hospital Association (AHA) reports that healthcare workers suffer more workplace injuries because of violence than any other profession".  It is not just verbal but physical violence as well. Our guest this week, Dr. Resa Lewiss, discusses the pervasiveness of violence she has seen and experinced in her own profession.  Dr. Resa E. Lewiss MD is a Professor of Emergency Medicine and Radiology at Thomas Jefferson University Hospital in Philadelphia PA.  She directs the Point-of-Care Ultrasound Section. She publishes and speaks on equity, health design, point-of-ultrasound, medical education, global health, and navigating the workplace, including leadership and mentorship. Resa has led point-of-care ultrasound education courses regionally, nationally, and internationally.  After graduating Brown University with a degree in Sociology and Racial Ethnic Studies, Resa attended the University of Pennsylvania School of Medicine. She spent one year at the NIH in Bethesda, MD, as a Howard Hughes Research Scholar. Resa is a graduate and former chief resident of the Harvard Affiliated Emergency Medicine Residency program in Boston, MA. She graduated in the first emergency ultrasound fellowship class at Mount Sinai St. Luke's Roosevelt Hospital Center in New York, NY.  Resa was the Fellowship Director and Chief of the Emergency Ultrasound Section there from 2003-2014.  From 2014 – 2017, she led the Point of Care Ultrasound program at the University of Colorado. Resa publishes, speaks, and is committed to creating a safe, equitable, and dignified work environments in healthcare. She is the creator and host of the Visible Voices podcast coving topics of Healthcare, Equity, and Current Trends. The utilization of point-of-care ultrasound and other non-invasive cardiac output monitoring technologies varies because of knowledge, resource availability and cultural practices. In this  Clinical Challenge in Surgery episode from the Surgical Critical Care team at Behind the Knife, we provide a brief history of the use of cardiac-output monitoring in the ICU, introduce a few clinical scenarios in the context of point of care ultra-sound and other less-invasive cardiac-output monitoring technologies. Learning Objectives:  In this episode, we review the historical uses of central venous pressure monitoring, pulmonary-artery catheters and the more frequently utilized point-of-care-ultrasound (or POCUS) in managing complex ICU patients. In this episode, I sat down with a technology leader Sandeep Akkaraju and physician leader Joseph Minardi to discuss point of care diagnostic imaging. Prior to founding Exo, Sandeep was Founder and President of Jyve Inc. (acquired by a major semiconductor corporation at an exit valuation of more than $60M). Previously, he was CEO of IntelliSense, a leading provider of micro-mechanical integrated circuits (MEMS) and nanotechnology-based software and solutions. Under his guidance, IntelliSense expanded its physical presence into China and India and its sales presence into 30+ countries. Sandeep helped the company rapidly grow from a startup to its eventual acquisition at a valuation of $750 million. He helped develop and run the first national courses in Critical Care Ultrasonography in the U.S., and served as a Director of these courses with the American College of Chest Physicians for several years.
