A little over a year ago, I made a short reel discussing ventilating a patient in a severe metabolic acidosis. In the video, I mentioned that minimal to no PEEP might be appropriate. Not because the patient doesn't need PEEP, but because the shortened cycle time at higher respiratory rates can unintentionally generate dynamic PEEP on its own. But what if it was intentional? What if the presence of a wider pressure when the ventilator switches into exhalation would cause the air to exit more rapidly? In this podcast, I discuss my thought process with respiratory therapist extraordinare, Melody Bishop @melodybishop_rt. She helps surface some definitions and distinctions to terms like air-trapping, Set PEEP, & auto-PEEP while we discuss ventilation strategies in metabolic acidosis. Episode 240How Women Question Men?Sensers! It's never good enough! I'm not good enough! A few things the ego will sometimes convince us when engaging with other humans on this planet. However, I have a question from family back home in Virginia on the topic of why are men never satisfied? Broad but a loaded question that I can make an attempt to answer to give another perspective of understanding. “I can't live without you!” “I'm lost without you by my side.” “You are without a doubt the best thing that's ever happened to me.” This all sounds magical but fantasy has a tendency to create illusions in the minds of those who love to hear sweet nothings even if it's not how the person truly feels. Some of us are just in love with love itself. Why would someone say I'm the best for them then leave to be with someone else? Peep my stance on this topic.“Why yall men don't know how to communicate? If I had a jersey on I bet you know what to say to me .” I believe communication is very essential in maintaining a healthy relationship regardless if it's intimate, platonic, work etc. I don't necessarily believe that men struggle to communicate, I believe they struggle to communicate…with you. Now, that's not to take personal but to take notes as to why that is. Some of us are willing to talk regardless of the audience, while others must feel safe enough to do so. I don't believe its 100% proof, but what I mean by that is the only you really can control is the love you give others. It's a blessing if we're able to receive love from those we value in our lives, but how would you ever know they truly do? Truth be told, why does it matter to some of us? I believe life teaches us that unconditional love is the key regardless, but tell that to our ego (the conditioner).Have you ever thought to yourself if I just love them harder maybe they'll start to appreciate me? I believe we all believe that myth at some point in time, but how does one know when to get up from the table when respect is no longer served? I'm a firm believer in practicing unconditional love but certainly not unconditional tolerance. So when do you stop caring?
