The Warriors have committed to staying in the NRL amid the coronavirus pandemic.Soon after the Australian and New Zealand governments had announced they would close their borders last night, George said the Warriors had met to confirm their desire to remain after spending this week in camp in Kingscliff in northern New South Wales ahead of their second-round game against the Canberra Raiders in Robina tomorrow night.They'll now follow their 'home' game at Cbus Super Stadium with a trip to Sydney to face the Manly-Warringah Sea Eagles at Lottoland on Friday, March 27.George said it was a testing time but the group had decided to stay in the competition."The entire squad is locked in and ready to go," said George. "They understand the issues that different individuals got on the table. But they're sticking together ... [The NRL] have called on us and we've stepped up to the task.""If we were told to leave your family and live in another country for an indefinite period, not many would ... we're very proud of the players."The deciding factor was that it's our time to be leaders in the community and rugby league ... we're gonna stand tall and be leaders and take it on."These guys aren't falling away from the challenge ahead." Players had to borrow gear and adapt."Our players have been asked to relocate to another country — unplanned —and continue to play in the competition, away from their families. If that is the case, then we need to make sure they have everything that their normal lives provide them, as much as is possible."That includes time with wives, partners and children, and George indicated that "a number" of families had indicated they would like to make the trip."But who pays, where do they go, where do they stay ... that all needs to be sorted out, in terms of logistics," said George.Wives, partners and children would be required to be in isolation for 14 days on arrival in Australia, and wouldn't be able to stay near the team's Kingscliff base in northern NSW for that reason. After that fortnight, they could relocate to the same area.Meanwhile, the Warriors say they are close to locking down some loan players, who could be added as early as next week.There are 23 players in Australia at coach Stephen Kearney's disposal (four are on development contracts) with another four sent over on Wednesday, although they won't be available until early April (due to isolation requirements)"There is certainly a chance it [loan players] could happen next week ... it's not unrealistic," Warriors recruitment boss Peter O'Sullivan told the Herald. "We need to re-calibrate after injuries and other things we can't foresee [this weekend]. We have some balls being juggled in the air and soon we will be clearer about what we need in the next few weeks and beyond."O'Sullivan has spoken to most clubs — "apart from the ones who are in dire straits with injuries themselves" — and believes Wednesday's cancellation of the second tier state Cup competitions could also change the dynamic."If they can't play reserve grade, the best way to get a run could be giving them a couple of weeks with us," said O'Sullivan. "If players are coming back from injury, they might need a gallop."Round two of the NRL was planned to go ahead this weekend.

Smertevidenskabs Mikkel Vind interviewer i episode 7 af Smerteuniverset Emma Line, som har levet med kroniske bensmerter i 3 år efter et fald på en trappe. Efter faldet tager Emma Line til lægen som sender hende videre til fysioterapeut som konstaterer at der ikke er noget alvorlig galt på trods af det får hun behandling 3 gange om ugen i 10 uger uden effekt. Emma Line som er uddannet feltbiolog og elsker at leve et aktivt liv, men har nu svært ved at gå ned til bussen og muskelmassen forsvinder på benet. Hun bliver opfordret til at søge ny behandler som kombinerer refleksbehandling (RAH) og styrketræning hvilket hjælper hende til at starte med, men over 1 år begynder behandlingseffekten at udeblive i takt med at smerterne bliver mere diffuse og flytter sig. Efter endt behandling klarer hun sig på egen hånd i et års tid men fordi smerterne vedbliver, skaber det bekymringer for at hun stadig har en skade. Smerterne har også spredt sig til halvdelen af kroppen og hun er overbevist om, at hun bruger sin krop forkert. Hun opsøger behandling hos Mikkel Vind med henblik på at få styr på hendes skade og forkerte bevægelser men finder ud af, at vejen til smertefrihed var meget anderledes end hun forventede. Ved første session hos Mikkel Vind som tog 1.5 time lærer Emma Line om de mange forskellige faktorer der kan bidrage til smerte og bliver udfordret på om forkerte bevægemønstre eksisterer. Det var en meget anderledes behandling end Emma Line havde forestillet sig, men efter behandlingen som stort set udelukkende bestod af samtale var hun smertefri på dagen og flere dage efter. I de opfølgende sessioner udfordrer Emma Line sit ben med tung styrketræning og sine overbevisninger omkring smerter som hjælper hende med at genvinde tilliden til hendes krop og komme frygten for hendes smerter til livs. Hovedpunkter for syvende episode: Kan man bevæge sig forkert? Skal genoptræning bestå af specifikke øvelser? Frygt, bekymringer om smerter kan bidrage til smerter. Manglende information omkring andre faktorer der kan bidrage til smerter end skade i behandlingssystemet. Flere behandlere flere diagnoser og kropsfejl. Jagten på diagnose og vished. Frustrationen over ikke at have en diagnose eller forklaring på sine symptomer. Kan man bilde sig selv smerter ind? Betydningen af tillid til den smertefulde kropsdel. Styrketræning som redskab til at genvinde tilliden til kroppen. Friheden ved at forstå sine smerter og ikke være bange for dem. Er skade = smerte og smerte = skade? In this episode, we’re talking about making sense of beliefs and behaviors in pain care. Our expert guest is Dr. Samantha Bunzli who is a physiotherapist and has worked in clinical practice for several years before undertaking a PhD with Professor Peter O’Sullivan at Curtin University in Australia. Her PhD research was entitled, “A Qualitative Investigation of Pain-Related Fear in People with Low Back Pain.” She is a post-doctoral researcher at the University of Melbourne working to optimize treatment outcomes for people with osteoarthritis. Dr. Bunzli’s work is part of an international effort to change the narrative in musculoskeletal pain care. She’s been awarded major grants by the National Health and Medical Research Council of Australia to improve evidence-based decision-making among orthopedic surgeons and implement non-surgical care pathways for people with end-stage osteoarthritis. In this episode, you’ll learn why the beliefs people hold about their musculoskeletal pain matter, where do unhelpful beliefs come from, and how the common-sense model could help us understand unhelpful beliefs and behaviors. Samantha describes findings from her qualitative research based on data collected through interviews from both patients as well as clinicians. She is keen to emphasize the qualitative research is more than just the collection of stories. It involves robust methods of data collection and analysis. Rather than going into detail about these methods, Samantha has provided free access to all of her research in which she explains the science behind qualitative research and describes the contribution qualitative approaches can make in the field of pain science. This includes some of her first work in the Journal Of Orthopedic and Sports Physical Therapy. Steadying Our Understanding with the Evidence Low back pain is one of the most prevalent, costly, and complex issues we face in rehab and medicine as a whole. It has been proposed that a large portion of back pain cases are due to core weakness or, as it’s commonly referred to, instability. This idea originated from the work of Paul Hodges and Peter O’Sullivan in the early 1990s. The idea caught like wild fire, infiltrating university level classes, continuing professional education courses, and subsequently shaping societal beliefs about backs and back pain causes. Fitness, rehabilitation, and medical professionals frequently blame back pain on an unstable or weak core. Consequentially, patients believe that, if they experience back pain, they must be weak and unstable in their spines. Yet, what does the current best evidence tell us about this topic? Has this theory strengthened, or has it failed to keep up with more rigorous investigation? Finally, what implications could this clinical framework have on patient outcomes and resilience surrounding the most common musculoskeletal complaint in the world? (Recorded at PAINWeek 2018)

Photo / Getty"Thousands of kids must be waking up in New Zealand today with the No 7 jersey, wondering what the heck is going on. Interview recorded as part of the low back pain course on Physiopedia Plus. Peter O’Sullivan is from physiotherapist originally from New Zealand. He completed later his post graduate manipulative training in Australia. He is a musculoskeletal professor at University of Curtin in Perth, Australia. He is well known internationally as a clinician, researcher and professor of musculoskeletal pain disorders. During the most recent Physiopedia interview, Peter discusses the multidimensional components of pain. He discusses pain beliefs and behaviors and how it affects clinical practice and research. His research focuses on the cognitive functional approach in the lumbar spine largely due to the number of cases that present clinically. Peter discusses how movement related to pain is largely behavioral. Over protecting painful structures because of fear often times causes compensatory issues. Physiotherapists should consider address retraining movement into a means of moving without compensatory pain patterns. Ultimately retraining the concept that movement is safe is key. There are three components of pain that should be addressed: Cognitive behavioral beliefs-pain beliefs Functional behavioral aspect-movement patterns Lifestyle adaptations due to pain and avoidance of activities Peter discusses how picking up a shoe demonstrates motor control patterns that may be pain related. He recommends using a questioning for helping patients to problem solve a more effective pattern of movement without driving compensatory pain such as tensing up when doing particular movements. Peter additionally discusses the difficulty of imaging with MRI. People who pursue imaging typically will have some pathoanatomical finding. This can further complicate pain processing and behaviors within movement patterns. People tend to grasp onto imaging findings and act in a protective manner towards what they perceive to be a significant finding on imaging despite our beliefs that it may at times be incidental. Peter discusses how when patients discuss the lack of benefits with therapy and how it is more helpful in the learning process. In these cases, stress may negatively impact pain and cause flares. Educating patients to focus on extra care towards their body in more stressful times is important. Focusing on good sleep patterns, eating a health diet, avoiding alcohol, maintaining an exercise program, utilizing relaxation techniques are strategies helpful in decreasing sensitivity within the body. It is important for us as physiotherapists to educate our patients on these principles.

He is recognized internationally as a leading clinician, researcher and educator in the management of complex musculoskeletal pain disorders.   In this episode, we discuss: -Why you should validate your patient’s pain experience, understand their beliefs and fears, and disconfirm them through behavioral learning -The link between a practitioner’s language and self-efficacy -The informal and non-threatening art of Peter’s initial examination -Maintaining professional boundaries with chronic pain patients and avoiding burn out -And so much more!   One of the strongest influences to better treatment outcomes for chronic pain patients is trust in the therapeutic alliance. “You’ve got to build a strong therapeutic relationship,” Peter suggests if you want to see patient’s engage in their program and take more control over their pain.   Treating chronic pain patients can be challenging. With the right evaluation framework and understanding of neuroscience, Peter believes you can make instant impact for the patient. Peter stresses, “The nervous system is so damn plastic. If you can get to the heart of what someone is thinking and feeling. Validate it and take them on a journey—it can break that schema up.”   Peter is critical of therapeutic techniques in physical therapy when in fact a majority of patients would benefit from relaxation strategies and progressive loading. That’s a ‘gold’ podcast (>5000 listens) and Peter O’Sullivan now extends those ideas into the realm of sports physiotherapy. “We can massively create health problems” warns Prof O’Sullivan speaking about the way clinicians may explain pathology to patients. Over-diagnosis, over-treatment and industry influence. Surely not in sports medicine? Ray Moynihan is an internationally renowned contributor in the battle to limit any inappropriate influence of pharmaceutical, diagnostic, and technology companies on good clinical practice. He shares his concerns about the role of drug/implant companies but also argues that clinicians may well be contributing to suboptimal care – almost subconsciously. This is relevant for conditions such as femoroacetabular impingement (FAI). The issue of ‘preventive’ surgery to prevent future arthritis in currently pain-free hips is a massive issue in sports medicine of course. How do we know which patients need this treatment? Enjoy this podcast from a voice outside BJSM’s traditional network of physiotherapists, physicians, massage therapists, clinicians working with active people.
