The world can be a tough place. We know that our ADHD doesn't make that any easier. But our health and well-being depend heavily on the worldview we cultivate in spite of our challenges. This week, we're talking about optimism, how we find it, and how we make it a practice in the face of struggle. We'll talk about the things that get in the way of our efforts to feel optimistic like emotional dysregulation, rejection sensitivity, an impulsivity. The thing is, in the face of all those challenges, we're already wired with our other tools to regain control. We're generally OK with therapy and strengths-based approaches to wellbeing, for example. So let's explore mental models that impact us. These include Carol Dweck's Growth Mindset, Cognitive-Behavioral models, and Martin Seligman's Explanatory Style theory. That's the one that has impacted Pete the most this month. Let's continue our conversation about treatment today with a discussion on Cognitive Behavior Therapy (CBT) for anxiety and issues with mood. CBT has shown to be an effective treatment for individuals with anxiety and depression, and research shows substantial support for CBT as an effective first line treatment for children. More research shows that up to 60% of children and teens treated with CBT experience significant decreases in symptoms that are lasting for 4 years or more. But what is Cognitive Behavior Therapy? Today's guest, Dr. Shadab Jannati, has seen firsthand the impact CBT has on children and teens with anxiety and depression. In this episode, she explains what CBT is, what a session might look like, and what this form of psychotherapy does to help children, teens, and their families. Show Notes: [3:05] - CBT is a form of psychotherapy established in the 1960s that has evolved over the years. [3:57] - This form of therapy is goal-oriented and focused on the here and now. [5:27] - Children and teens without a formal diagnosis still benefit from CBT. [6:44] - Dr. Jannati gives examples of scenarios where children and adolescents experience anxiety. [8:18] - CBT also gives kids and teens the ability to listen to body cues and their feelings. [9:49] - When these feelings come up, kids and teens can then recognize that they are anxious and change their thinking. [11:24] - Anxiety can show up in many ways, but common problems are negative thinking and “mind-reading,” which are irrational. [12:59] - The first step is being aware of the feelings and negative thinking. The next step is to find the right coping strategies. [14:15] - Like waves, feelings come and go. [15:42] - For younger children, play-based strategies are also used in CBT. [17:13] - CBT is appropriate for most ages; Dr. Jannati works with children ages 4 to 16. [20:04] - There is a sense of relief and hopefulness with CBT. Anxious feelings are validated, and children and teens can learn what to do. [22:05] - It is ideal when parents can be involved in the sessions as well. [23:53] - 80% of kids with problematic anxiety don't receive treatment. [24:43] - CBT doesn't last for years. The number of sessions varies per person but 12-24 sessions is average at about 6 months to a year. In this episode, I speak with Susan about how she came to develop Cognitive Behavioral Play Therapy (CBPT). She explained that she was originally trained in psychodynamic play therapy and found it helpful. Talking with and reflecting on a child's experience was important, but she wanted to find ways to help children gain more adaptive skills to deal with their emotions and difficulties. At the time, it was thought that you could not use CBT with young children, so she used CBT techniques and ideas and incorporated them into play. Finding ways to bring CBT into play involved modeling with puppets, dolls, toys, books and other child-oriented materials. We discussed numerous case studies using CBPT with young children, as well as the research on Cognitive Behavioral Play Interventions (CBPI), currently being used with non-clinical populations. Susan shared case examples of using puppets to model various interventions, such as Systematic Desensitization and Cognitive Change strategies, and using workbook activities, like drawing the Worry Monster/Worry Bully to help anxious and fearful children. We discuss using toys, puppets, books, movies, and art with children. She also talked about her work with parents and assessing whether the presenting problems are better treated by working with just the parents or the child and parents together in different combinations. Susan M. Knell, Ph.D. is a psychologist who received her Ph.D. in Clinical Psychology from Case Western Reserve University and did her internship and NIMH Postdoctoral Fellowship at The Neuropsychiatric Institute (NPI), UCLA, specializing in clinical child psychology and developmental disabilities. She is currently Adjunct Assistant Professor in Psychology at Case Western Reserve University, maintains a private practice, supervises graduate students in training, and is the author of the book, “Cognitive-Behavioral Play Therapy” (Jason Aronson, 1993). Susan was the first to study and write about the application of cognitive-behavioral therapy with young children. In addition to her book, she has published many chapters in edited books on play therapy, with recent chapters on creative applications of CBPT and treating young children with anxiety and phobias. She lectures throughout the country and internationally on Cognitive-Behavioral Play Therapy with preschool and early school-age children. Most recently, Susan has been working with Maria Angela Geraci, Ph.D., Meena Dasari, Ph.D. and colleagues, as part of the Cognitive Behavioral Play Therapy Institute, in Rome, Italy. That turned out to be a place for getting my feet wet in abnormal psychology because there was a whole bunch of girls there that had a multitude of problems, from kleptomania to step-dad taking girl out for the weekend and doing things to her – that particular girl tried to commit suicide and I was involved with the clean-up of that. the Math teacher tried to rape me and that's how I got back into regular high school, but in the meantime I'd had a year and a half of quite an education and I think that started me off to being observant of human behaviour and being fascinated with it. A lot of people have been traumatised in one way or another, so I do EMDR, which is eye movement desensitisation and reprocessing. That helps the brain to go back – much like cleaning out a filing cabinet – to a critical point which is often stuck, not only in a person's mind, but in their body and their cells. When we go back and re-look at that point I do a series of eye movements in front of their eyes which allows the brain to go back into all sorts of memories right around that and other places, which unsticks that point. It goes from a dramatic, colourful picture of intensity to more like a black and white picture that's still there but doesn't have that intensity anymore. I try to teach people practical things too. Firstly, I ask people to bring all of themselves. People want to compartmentalise – part of them is bad and part of them is good. Some people just need some help in their daily life, people have got very disorganised during Covid, they've got messy or sloppy or they watch too much TV. I see a lot of younger people that are very non-motivated. Their parents bring them in and ask me to “fix my teenage kid”. Well, it's usually the parents' problem, not the kid. However, what we're having trouble with is getting people to be motivated, to move forward in their life, find something they really want to do. I think screen addiction is a really big problem in that area, it tends to stop people getting out into the world. I try to help people find something they really want to do, because, once you've found that thing, it's not a job, it's a pleasure.
