Around 80% of adults experience low back pain, and most are not candidates for surgery. Prentiss Lawson, Jr., M.D., and Christopher Paul, M.D., both anesthesiologists who specialize in pain medicine, discuss a promising new durable therapy for low back pain called restorative neurostimulation. Unlike existing low back pain management options, neurostimulation addresses a root cause, a weak or inactive multifidus muscle. The doctors explain the minimally invasive procedure, which involves sending electric pulses to the multifidus nerves. Learn how this treatment uniquely restores patents' spinal stability and thereby reduces chronic pain.

Even in a non-military population, chronic back pain is overwhelmingly the number one cause of disability and quality of life reduction worldwide. In this episode, Dr. Gabriel Pagani, a Mayo Clinic-trained, double-board-certified interventional and surgical pain management neurologist, discusses the prevalence and significance of chronic low back pain within the military. Gabriel discusses the link between military service and chronic back pain, attributing it to multifidus muscle dysfunction and its impact on spine stability. He underscores the challenges faced by active-duty personnel and veterans, emphasizing the role of ReActiv8, a minimally invasive procedure addressing multifidus dysfunction, in improving both physical and mental well-being. Despite its proven benefits, the slow adoption of ReActiv8 in the military community prompts Dr. Pagani to call on healthcare providers to explore this innovative treatment. Tune in and learn about the revolutionary ReActiv8 technology, a potential game-changer in restoring spinal function and improving the quality of life for those suffering from chronic low back pain! In this Physiotutors podcast episode, Canadian researcher Yannick Tousignant-Laflamme introduces the Pain and Disability Drivers Management Model, a groundbreaking approach to understanding and managing chronic musculoskeletal pain, particularly low back pain. This model identifies five key domains across three dimensions, going beyond biomechanics to reveal the root causes of pain and disability. It underscores the significance of interdisciplinary collaboration and effective communication among healthcare professionals. By integrating classification systems and prognostic tools, this model optimizes its effectiveness. Classification systems pinpoint patient subgroups, while prognostic tools screen for psychological factors affecting pain outcomes. Clinicians are encouraged to utilize these tools for screening, delving deeper for medium or high-risk patients. The incorporation of questionnaires and patient-reported outcome measures informs evidence-based decision-making. The ultimate aim? Personalized, evidence-based care for musculoskeletal conditions, addressing both physical and psychosocial factors. Curious to learn more about this innovative approach? Tune in to the full episode for a comprehensive exploration. Guest Canadian researcher Yannick Tousignant-Laflamme is a professor at the School of Rehabilitation at the University of Sherbrooke. How similar are the neuromuscular responses to pain with chronic low back pain and GI pain? Join Susan and Erica in a great discussion of the role of the diaphragm and how this changes with pain and inhibition/over-recruitment in the system. In chronic low back pain and abdominal bloating/distention the diaphragm becomes a postural control muscle which greatly limits the respiratory ability along with an ineffectual model for spine stiffness and visceral organ pusher. Psychologist and senior researcher in the field of low back pain research, Professor James McAuley discusses the historical development of psychological models in pain treatment. In this episode, he highlights that psychology, while essential in pain management, may not be highly successful as a standalone treatment for low back pain. Professor McAuley and his team have been working on new treatment approaches for chronic low back pain, including cognitive functional therapy and pain reprocessing therapy. Their research focuses on improving self-efficacy in patients and utilising techniques like tactile acuity training to enhance sensory perception in the context of pain management. Kent et al. Pilates exercise seems to help people with chronic low back pain...but how? What are the causal mechanisms underpinning the apparent effectiveness of Pilates? Does it activate the multifidus and transeversus abdominis muscles? Does it reduce fear and improve confidence? Both or neither? We're continuing the conversation this week with Greg Potter, PhD., but with a plot twist. We've had Greg on the podcast many times discussing all facets of sleep and circadian biology, but today the topic is low back pain. Greg has had personal experience with this problem since the age of 12 and has devoted extensive time and effort to researching and trying various treatment methods to find relief for himself and his coaching clients. He's quick to admit he's not an expert in low back pain, but it's clear he's well-versed on the topic. Greg thoroughly examines low back pain in this podcast, including causes, treatments, and scientifically-validated approaches for relieving chronic discomfort. He discusses time-tested treatments and cutting-edge options, including therapies, exercises, and supplements. He also recalls the book that started his healing journey and the best ways to give your back a break during daily movement and activities.
