Julie's debut novel, Chapel Bay Secrets (released January 2026), is set in a small town where the secrets don't belong to just one person—they ripple through an entire community.Julie shares how the seed of this novel was planted years ago, when she visited the site of the Tule Lake Internment Camp in Northern California. She describes walking through cold, dirty barracks and feeling shocked that Japanese American incarceration during World War II wasn't something she'd been taught growing up. That moment stayed with her—“like an arrow through my heart,” as she puts it—and eventually became part of the emotional backbone of her story. As Julie researched further, she also learned more about intergenerational trauma, and how the impact of incarceration can be carried through families for decades.From there, Julie built a “layer cake” of characters and issues—centered on her protagonist Brenda Cato, a Japanese American lesbian librarian whose mother was incarcerated. Brenda is grappling with grief on the anniversary of her mother's death, and she's also searching for a father she never knew—because her mother died without telling her who he was. The world of prehospital medicine is constantly evolving, driven by new research, technological advancements, and a shared commitment to improving patient care and provider well-being. As EMS professionals, staying informed about these developments goes beyond a professional obligation; it is an opportunity to improve our practice, champion our profession, and ultimately make a greater impact on saving lives. In this article, we will explore some of the latest research findings that are reshaping our field, from workplace culture to cutting-edge technology. The Culture of Care: Supporting EMS Providers Our work is demanding, both physically and emotionally, and the culture within our agencies plays a critical role in our well-being. A recent systematic review in the International Journal of Environmental Research and Public Health revealed that many EMS providers avoid using organizational mental health services due to stigma and a perception that these programs lack genuine care. The study emphasizes the need for person-centered support and a cultural shift that normalizes seeking help as a sign of strength (Johnston et al., 2025). This cultural component also impacts retention. Another study in the same journal found that agencies with collaborative, team-oriented "clan" cultures had significantly lower turnover rates compared to those with rigid or chaotic structures. For leaders in EMS, fostering a supportive environment is not just about morale. It is a strategic imperative for retaining skilled clinicians (Kamholz et al., 2025). Professional Recognition: Breaking Barriers Across the globe, paramedics are striving for recognition as integrated healthcare professionals. A qualitative study in BMC Health Services Research identified common barriers, including outdated legislation, inconsistent regulation, and insufficient funding. While the pandemic temporarily highlighted our capabilities, the momentum has waned. The study calls for targeted policy reforms and investments in education and leadership to solidify our role in the broader healthcare system (Feerick et al., 2025). Physical Demands and Injury Prevention The physical toll of our work is undeniable. A scoping review in Applied Ergonomics confirmed that musculoskeletal injuries, particularly to the back, are rampant in EMS. Tasks like handling stretchers and patient extractions are among the most strenuous. The review also highlighted fitness disparities, with male paramedics generally showing more strength but less flexibility than their female counterparts. These findings underscore the need for targeted injury prevention programs and realistic physical standards to keep us safe throughout our careers (Marsh et al., 2025). Advancements in Cardiac Arrest Care When it comes to cardiac arrest, every second counts. A study in Resuscitation reinforced the value of bystander CPR, showing that dispatcher-assisted CPR significantly improves outcomes for untrained bystanders. For those with prior CPR training, acting independently yielded even better results. This highlights the importance of public CPR education alongside dispatcher support (Tagami et al., 2025). On the scene, our interventions matter immensely. Research in The Journal of Emergency Medicine found that for traumatic cardiac arrest patients, aggressive interventions like prehospital thoracostomy can be lifesaving (McWilliam et al., 2025). Meanwhile, a study in Critical Care Medicine revealed that extracorporeal CPR (ECPR) significantly improves outcomes for patients with refractory ventricular fibrillation, emphasizing the need for early transport to specialized centers. The Role of Technology in EMS Technology is poised to revolutionize EMS, from dispatch to diagnosis. A study in The American Journal of Emergency Medicine demonstrated that large language models (LLMs) like ChatGPT could prioritize ambulance requests with remarkable accuracy, aligning with expert paramedic decisions over 76 percent of the time. This proof of concept suggests that AI could one day enhance resource allocation in dispatch centers (Shekhar et al., 2025). On the diagnostic front, machine learning is opening new possibilities. For example, a study in Bioengineering showed that analyzing photoplethysmography waveforms could estimate blood loss in trauma patients, offering a non-invasive way to guide resuscitation (Gonzalez et al., 2025). Similarly, research in Medical Engineering & Physics explored using multidimensional data to differentiate ischemic from hemorrhagic strokes in the field, potentially enabling more targeted prehospital care (Alshehri et al., 2025). Addressing Disparities in Care Equity in EMS is a cornerstone of our profession, yet recent studies highlight troubling disparities. Research in JAMA Network Open found that ambulance offload times were significantly longer in communities with higher proportions of Black residents (Zhou et al., 2025). Another study in JAMA Surgery revealed that Black and Asian trauma patients were less likely to receive helicopter transport compared to White patients. These findings are a call to action for all of us to examine our systems and biases to ensure equitable care for every patient (Mpody et al., 2025). Looking Ahead The research discussed here represents just a fraction of the advancements shaping EMS today. From improving workplace culture and injury prevention to leveraging AI and addressing systemic inequities, these findings have real-world implications for our protocols, training, and advocacy efforts. As EMS professionals, we have a responsibility to stay informed and apply these insights to our practice. Why do I feel exhausted even though I'm successful? You've built something impressive. You've proven yourself over and over. But you're exhausted, your body is breaking down, and no matter how much you achieve, it never feels like enough. She was over-giving in every direction, having emotional reactions that would take her out for days, and her body was physically breaking down from carrying the weight of it all. Alex had gone from being hyper-independent and emotionally unavailable to swinging completely to the other extreme: over-functioning, people-pleasing, and seeking partnerships to fill the gaps she believed existed in herself. She was operating from scarcity, not abundance. She was trying to earn love by being helpful and valuable enough. And she was attracting emotionally unavailable people so she could over-compensate by being the caretaker. The breakthrough came when Alex realized she was having outsized emotional reactions because she had given every ounce of energy to everyone else. There was nothing left for her. She had to face the uncomfortable truth: her patterns of over-functioning weren't making her a better leader or partner. They were destroying her health, her relationships, and her ability to feel the success she'd built. What we talk about in this episode: How over-functioning creates under-functioning in others. When you're constantly doing everything, carrying all the weight, and being the strong one, you're actually teaching the people around you to do less. You create the exact dynamic you resent. What it's like to attract emotionally unavailable people when you're trying to earn love. Alex shares how her pattern of seeking partners from scarcity (looking for people to fill her perceived gaps) versus partnering from abundance completely shifted once she did the deep work on her worthiness. The moment you wish someone would save you while hating yourself for even thinking it. This is the rock bottom moment for high-functioning, capable people. When you're so exhausted that you just want someone to rescue you, and you despise yourself for having that thought because you're supposed to be strong. Why your "buttons" getting pushed reveals your unhealed wounds. Alex explains how the people closest to us push our buttons not because they're trying to hurt us, but because they're the only ones allowed close enough to reach those wounds. Her business partner was pushing the exact buttons related to her "not enough" story from childhood. How to set boundaries without over-explaining yourself. Learning to say no as a complete sentence. Learning to set a boundary and hold it without justifying, defending, or convincing. This is the work of self-respect. What self-trust actually means and how to rebuild it. Self-trust isn't built through grand gestures. It's built by keeping the small promises you make to yourself. Every time you break a promise to yourself, you're teaching yourself you don't matter. The shift from "not enough" to "I am so in love with myself." Alex shares what it's like on the other side of the deep emotional work: the peace, the clarity, the ability to move through the world without constantly proving yourself or seeking external validation. How success and freedom get redefined once you stop abandoning yourself. For Alex, freedom used to mean location independence. Now it means the freedom to prioritize her health, be where she wants when she wants, work with people she chooses, and have the bandwidth to do work that matters without worrying about compensation. The spiral metaphor: why it looks like you're going in circles but you're actually going up. Alex has a spiral tattooed on her wrist because from one angle, personal growth looks like you're just repeating the same patterns. But shift your perspective and you see you're actually ascending, going around and up with each iteration. ABOUT TODD SNIDERCelebrated singer-songwriter Todd Snider has continued the troubadour legacy of mentors like John Prine, Jerry Jeff Walker, and Ramblin' Jack Elliott, while putting his uniquely clever, wry, sly, and often irreverent spin on folk, rock, country, and Americana. Launching his career on Jimmy Buffett's Margaritaville Records, Snider has spent more than two decades touring relentlessly, both on his own and with legendary artists such as Emmylou Harris. Along the way he's made a splash with fan favorite songs such as “Talking Seattle Grunge Rock Blues,” “Alright Guy,” “Can't Complain,” “Beer Run,” “Statistician's Blues,” and “Play a Train Song.” He has released well over a dozen albums, including The Devil You Know and Agnostic Hymns and Stoner Fables, both of which were named to Rolling Stone's list of the Top 50 Albums of the Year. He also formed the group Hardworking Americans, and published a memoir called I Never Met a Story I Didn't Like: Mostly True Tall Tales. His songs have been covered by Garth Brooks, Gary Allan, Mark Chesnutt, Tom Jones, Jerry Jeff Walker, Billy Joe Shaver, Elizabeth Cook, Warren Haynes, Loretta Lynn, and Elvis Costello. When we first spoke to him he'd recently released his experimental funk-influenced album called First Agnostic Church of Hope and Wonder. Early .577 Snider cartridges were made from a composite design using paper and brass foil with a stamped metallic base and primer, much like the first generation of Martini-Henry cartridges. Later cartridges (after the design had been proved with the Martini-Henry cartridges) were made from drawn brass, much like modern small arms ammunition. The .577 Snider cartridge was eventually replaced in service by the .577/450 Martini–Henry cartridge in the 1870s.
