In this episode, we welcome Dr. Robert Dunne, who is the EMS Medical Director for the Detroit Fire Department, as well as the Detroit East Medical Control Authority.  Dr. Dunne is here to describe why fire departments and EMS agencies should be prepared to treat patients with smoke inhalation injuries.  Specifically, Dr. Dunne is here to put a spotlight on the toxic twins, cyanide (CN) and carbon monoxide (CO), which are two major contributors to the injury and death of smoke inhalation patients.  During this episode, Dr. Dunne will describe why fire departments and EMS agencies should be prepared to treat patients with smoke inhalation.  In addition, doc will describe the impact of cyanide (CN) and carbon monoxide (CO) on the human body.  Then, he will list and describe the assessment and treatment priorities for smoke inhalation injuries.  Finally, Dr. Dunne will describe the benefits of hydroxocobalamin for patients with signs and symptoms of cyanide toxicity.  Support the show

In this episode, Dr. Mark Piehl and Dr. Peter Antevy discuss why EMS must adopt a more thoughtful approach to fluid resuscitation for critically ill or injured patients.  Specifically, patients in shock. It turns out that EMS needs to take a more of a proactive approach to recognizing the need for and administering fluids for certain patients. Dr. Piehl, and Dr. Antevy are here to put a spotlight on the value of fluid resuscitation in the pre-hospital setting, walk us through the data, and discuss the how EMS can provide the right amount of fluid in the right amount of time to make a big impact for our patients.  Dr. Peter Antevy is a Pediatric Emergency Medicine Physician, as well as an EMS Medical Director for several fire-based EMS agencies in South Florida.  Dr. Mark Piehl is a Pediatric Intensivist at WakeMed Health & Hospitals.  He is also the Assistant Medical Director for WakeMed Mobile Critical Care, and an Associate Professor of Pediatrics at the University of North Carolina School of Medicine. EMS should use lights and sirens selectively, the same as any clinical intervention for our patients. On Feb. 14, 2022, leaders of many EMS associations published a statement calling for decrease in use of red lights and sirens for EMS responses and transports.  Data from NHTSA and Dr. Doug Kupas's seminal paper on the use of red lights and sirens in EMS that fewer than 7% of patients transported to the hospital with lights and sirens received a life-saving intervention upon arrival in the ED. Rob and Hilary interview two of the leaders of research and change on this topic: Brooke Burton, Quality Improvement & Controlled Substances Manager at Unified Fire Authority in Salt Lake City, Utah and Bryan Wilson, EM/EMS Physician at St. Luke's University Health Network and EMS Medical Director for City of Bethlehem EMS.  They discuss the need to educate the public and local governments that clinical care is more important than response times. Dr. Jay Lemery, Professor of Emergency Medicine at The University of Colorado School of Medicine, joins me today to discuss the relationship between climate change and human health, the growing field of climate medicine, as well as the advocacy required to affect realistic, positive change for our planet and ourselves. Bio: Dr. Lemery is an Associate Professor of Emergency Medicine at the University of Colorado School of Medicine and is Chief of the Section of Wilderness and Environmental Medicine. He is a Past-President of the Wilderness Medical Society and has provided medical direction to National Science Foundation subcontractors operating at both poles, most recently serving as the EMS Medical Director for the United States Antarctic Program. Dr. Lemery has an academic expertise in austere and remote medical care as well as the effects of climate change on human health. He serves as a consultant for the Climate and Health Program at the Centers for Disease Control and Prevention and sits on the National Academy of Medicine's (IOM) Roundtable on Environmental Health Sciences, Research, and Medicine. He is a Fellow of the American College of Emergency Physicians was a term member of the Council on Foreign Relations. He is the co-editor of Global Climate Change and Human Heath: From Science to Practice (2015) and co-author of Enviromedics (2017). He serves as an advisor to the organization Climate for Health (ecoAmerica) and the George Mason University Center for Climate Change Communication. From 2005-2012, he was the Director of Cornell Wilderness Medicine and a member of the Global Health Steering Committee at the Weill Cornell Medical College. Dr. Lemery was an Echols Scholar at the University of Virginia and received his MD from the Geisel School of Medicine at Dartmouth. From 2003-04 he was chief resident in Emergency Medicine at NYU & Bellevue Hospitals. Dr. Peter Antevy, innovator of the Handtevy Pediatric Resuscitation System, is a nationally recognized lecturer and expert in the field of pre-hospital pediatrics. He is extremely passionate about improving both adult and pediatric pre-hospital care and evaluating and improving upon currently accepted medical practice.  Dr. Antevy received his medical degree from the University of Miami School of Medicine in 1999 and then continued on to complete his pediatric training at the Children's Hospital of Los Angeles. He then achieved a subspecialty degree in Pediatric Emergency Medicine at the Children's Hospital of Pittsburgh before returning back to Florida where he has been a Pediatric Emergency Medicine Physician at Joe DiMaggio Children's Hospital since 2005. Dr. Antevy also currently serves as the EMS Medical Director for Davie Fire-Rescue, Coral Springs Fire Department, Southwest Ranches Fire Rescue and American Ambulance, and is the Associate Medical Director for Palm Beach County Fire Rescue, Miramar Fire Rescue and Seminole Tribe Fire Rescue. Faizan H. Arshad, MD, is the EMS Medical Director for Vassar Brothers Medical Center, part of NuVance Health. Dr. Arshad completed his EMS fellowship with the Fire Department of New York City while serving as Assistant Medical Director of Manhattan. He began his career in EMS over 30 years ago, has worked in three states as a paramedic, and retains his active paramedic license today. He teaches extensively and has authored multiple articles on EMS issues in both peer-reviewed and industry journals.
