Send us a textIn this episode, we speak with Claudia Battison, a Psychiatric Rehabilitation Technician (PRT) in the mental health system in Trieste Italy. I am joined in this interview by Dr. Joy Agner, Assistant Professor at the USC Chan School of Occupational Therapy and Occupational Science at USC. Heart Forward has become particularly interested in the potential role that occupational therapists -- if empowered to practice their profession to its full potential -- could play in mental health support settings in the U.S. Unfortunately, the way that OT services are primarily funded (through short-term, medically oriented reimbursement systems) constrain their ability to come alongside people in their recovery journey over the long term. This topic was already approached in a Season Four podcast with Dr. Deborah Pitts from USC's Chan School. In this conversation, we learn about the ways in which the PRT engages with the system users. Three stark differences emerge: Time. There are no deadlines. They are afforded the time necessary to get to know the user and tease out the life plans/goals (also referred to as a personal rehabilitation project) that are meaningful to the user.Friendship. The relationship is described more like a friendship than what might be more typical in an American context. This equates with the ethos of coming alongside people in horizontal relationships that eschew the power dynamics associated with “professional” more verticalized relationships.Team. The PRT is part of a broader team – an équipe of other “operators” (their word for staff) – in the Community Mental Health Center. The other team members can help to weigh in on how to support the system user; the PRT is not left to his or her own devices.And, or course, all of this is grounded in the belief that a mental health system must support a person in all three pillars of one's life: casa, lavoro e socializazzione, or housing, work/purpose and community. The PRT must pay attention to each of these pillars to provide support for recovery. As we have researched this further, it appears that this role if fairly unique to Italy and was created to augment the psycho-social support that is an underpinning to the Italian model. Vanessa M. Blackstone and Olivia S. Sinaiko, LPC, authors of The Pain Reprocessing Therapy Workbook, join us to talk about how we can use the brain's neuroplasticity to break the cycle of chronic pain. Vanessa, citizen of the Eastern Band of Cherokee Indians Nation, is executive director of the Pain Psychology Center. She earned her MS in social work from the University of Southern California. Following her own personal recovery from chronic pain, Vanessa began her career as a therapist in 2018. She was one of the first clinical consultants at the Pain Reprocessing Therapy Center, and has helped train hundreds of practitioners in pain reprocessing therapy (PRT). In addition to chronic pain treatment, she specializes in sex therapy, substance use and recovery, mindfulness-based relapse prevention, and works on film sets as an onset wellness professional. Outside of her professional roles, Vanessa is a former foster youth who advocates for current and former foster youth by sharing her personal experiences in public speaking events. She lives in Los Angeles, CA. Olivia leads the behavioral health pain program at the tribal health organization, Southeast Alaska Regional Health Consortium (SEARHC). She has been a featured presenter in the American Society of Addiction Medicine's (ASAM) training modules on nonpharmacological approaches to pain treatment and interdisciplinary care in the treatment of chronic pain, and has presented to the Pain Psychology Center on the relationship between attachment trauma and chronic pain. She earned her BA from Stanford University and her MA in counseling psychology from The Wright Institute, as well as a JD from Yale Law School. She lives in Juneau, AK. As interest rates rise, so has interest in pension risktransfer (PRT)—and litigation. Nevin & Fred take a look at what's underlying (and undermining) that focus, aswell as a new suit alleging aggressive participant marketing.Pension risk transfer is an aptly named process undertakenby an organization that wants to transfer its pension obligations to another entity, typically an insurance company. The process itself is a fiduciary decision requiring carefulconsideration of the entity to which those obligations are transferred—and therein lies the basis of a recent spate of litigation regarding those choices (and during a period of time in which PRT volumes have been setting records). For those not familiar with the underpinnings of the pensionrisk transfer (PRT—because we need another acronym), IB 95-1, issued by the Department of Labor in 1995 (in the wake of the Executive Life collapse), outlines the fiduciary standards to be used in selecting an annuity provider for a pension risk transfer. That includes considerations of the provider's investment portfolio, size relative to the annuity contract, level of capital and surplus, liability exposure andavailability of state government guaranty associations.In 2024, and in accordance with the provisions of the SECURE2.0 Act of 2022, the Labor Department basically concluded that while it was open to, and still considering, potential updates to Interpretive Bulletin 95-1, it felt that that document “continues to identify broad factors that are relevant to a fiduciary's prudent and loyal evaluation of an annuity provider's claims-paying ability and creditworthiness.” Additionally, EBSA found it “desirable for guidance in this area to remain principles based.” In this episode, Nevin (Adams) and Fred (Reish) talk about the trends and issues here. In this episode of the Postural Restoration podcast I am joined by Yoshi Mitsuyama, who currently resides in Nashville, TN. Yoshi started his athletic training career in Rolla, Missouri after graduating from Northern Colorado. While working as a collegiate athletic trainer in Rolla he had the opportunity to intern with Sporting KC and step into the world of professional soccer. It was here that Yoshi was introduced to PRI concepts from Kentaro Ishii who was working there at the time. Yoshi discusses seeing strange activities being performed early on in his internship and became interested in the reasoning behind the techniques he saw being performed. After being introduced to the science of PRI Yoshi signed up for all three primary courses at the same time, completing them in 3 months over the summer. Yoshi discusses his early years of implementing PRI and having a new tool to provide individualized programming. After several years implementing PRI within collegiate and professional settings in MO, Yoshi earned his PRT in 2018. Shortly after, he accepted an opportunity to move back to Colorado and accept a position with the Colorado Rapids. Building on his experience in MO, he was now able to provide one on one attention to professional soccer athletes while working alongside performance teams and other professionals who were apart of these athletes lives. Yoshi discusses the challenges of implementing a new approach within this team setting, and making sure that others involved were complimenting the PRI programming that these athletes were utilizing. One of the biggest aspects of bridging the gap between the performance side and "rehab" team, along with the athletes individual involvement, was determining what factors outside of their sport were affecting their performance. While performing at a high level is done day in and day out, the activities that are attempted outside of sport such as driving, sitting, walking etc. often cause tension, or autonomic tone, until addressed.Yoshi has also spent time clinically working alongside PRC Lisa Kelly, and through their partnership of "Tri-Planar Movement Therapies" he was able to see and implement PRI within a more clinical setting along with a totally new client population. This time together allowed Yoshi to gain appreciation for ongoing vestibular, dysautonomic issues that not only were these clinical patients experiencing but also the athletes alike. We discuss what some of the symptoms are that these populations are dealing with whether from concussions, orthostatic issues, past traumas or the umbrella term of dysautonomia. Since his time in CO, Yoshi has since moved to Nashville initially working with Nashville, FC and now working as a private consultant within his community. This has presented him with yet another population of people seeking out PRI treatment and he has began working alongside Erika Young (voice coach) to help singers gain control of their voice via PRI concepts of position, respiration and retraining their systems to allow proper pressure regulation for the production of sound. Throughout these diverse populations Yoshi has been a part of, We discuss how all of these cases have similar needs when it comes to ground sense, breathing mechanics and sense of ones self, whether kicking a soccer ball, singing on stage, or just trying to get out of bed. Yoshi will be presenting a live case study alongside Torin Berge as part of the 2025 PRI Interdisciplinary Studies Summit in April.
