Contributor: Aaron Lessen MD Educational Pearls: The case: A gentleman came in from a nursing home with symptoms concerning for sepsis. He was hypotensive, hypoxic, febrile, and mentally altered. His past medical history included previous strokes which had left him with deficits for which he required a feeding tube. Initial workup included some point of care labs which revealed a sodium of 165 mEq/L (normal range 135-145) Hypernatremia What causes it? Dehydration, from insufficient fluid intake. This might happen in individuals who cannot drink water independently, such as infants, elderly, or disabled people, as was the case for this patient. Other causes of dehydration/hypernatremia include excessive sweating; diabetes insipidus; diuretic use; kidney dysfunction; and severe burns which can lead to fluid loss through the damaged skin. How do you correct it? Need to correct slowly, not more than 10 to 12 meq/L in 24 hours Can do normal saline (0.9%) or half saline (0.45%) and D5, at 150-200 mL per hour. Check the sodium frequently (every 2-3 hours) Will likely need ICU-level monitoring What happens if you correct it too quickly? Abstract: Objective: Macrophages play a central role in the onset and progression of vascular disease in pulmonary hypertension (PH) and cell-based immunotherapies aimed at treating vascular remodeling are lacking. This work evaluates the effect of pulmonary administration of macrophages modified to have an anti-inflammatory/pro-resolving phenotype in attenuating early pulmonary inflammation and progression of experimentally induced PH. Approach and Results: Mouse bone marrow derived macrophages (BMDMs) were polarized in vitro to a regulatory (M2reg) phenotype. M2reg profile and anti-inflammatory capacity were assessed in vitro upon lipopolysaccharide (LPS)/interferon-{gamma} (IFN{gamma}) restimulation, before their administration to 8- to 12- week-old mice. M2reg protective effect was tested at early (2 to 4 days) and late (4 weeks) time points during hypoxia (8.5% O2) exposure. Levels of inflammatory markers were quantified in alveolar macrophages and whole lung, while PH development was ascertained by right ventricular systolic pressure (RSVP) and right ventricular hypertrophy (RVH) measurements. Bronchoalveolar lavage (BAL) from M2reg-transplanted hypoxic mice was collected, and its inflammatory potential tested on naive BMDMs. M2reg macrophages demonstrated a stable anti-inflammatory phenotype upon a subsequent pro-inflammatory stimulus by maintaining the expression of specific anti-inflammatory markers (Tgf{beta}, Il10 and Cd206) and downregulating the induction of proinflammatory cytokines and surface molecules (Cd86, Il6 and Tnf). A single dose of M2regs attenuated the hypoxic monocytic recruitment and perivascular inflammation. Early hypoxic lung and alveolar macrophage inflammation leading to PH development was significantly reduced and, importantly, M2regs attenuated RVH, RVSP and vascular remodeling at 4 weeks post treatment. Conclusion: Adoptive transfer of M2regs halts the recruitment of monocytes and modifies the hypoxic lung microenvironment, potentially changing the immunoreactivity of recruited macrophages and restoring normal immune functionality of the lung. These findings provide new mechanistic insights on the diverse role of macrophage phenotype on lung vascular homeostasis that can be explored as novel therapeutic targets. Abstract: Impairments in social behavior are observed in a range of neuropsychiatric disorders and several lines of evidence have demonstrated that dysfunction of the prefrontal cortex (PFC) plays a central role in social deficits. We have previously shown that loss of neuropsychiatric risk gene vCacna1cv that codes for the Cav1.2 isoform of L-type calcium channels (LTCCs) in the PFC result in impaired sociability as tested using the three-chamber social approach test. In this study we aimed to further characterize the nature of the social deficit associated with a reduction in PFC Cav1.2 channels (Cav1.2PFCKO mice) by testing male mice in a range of social and non-social tests while examining PFC neural activity using in vivo GCaMP6s fiber photometry. We found that during the first investigation of the social and non-social stimulus in the three-chamber test, both Cav1.2PFCKO male mice and Cav1.2PFCGFPGFP controls spent significantly more time with the social stimulus compared to a non-social object. In contrast, during repeat investigations while Cav1.2PFCWT mice continued to spend more time with the social stimulus, Cav1.2PFCKO mice spent equal amount of time with both social and non-social stimuli. Neural activity recordings paralleled social behavior with increase in PFC population activity in Cav1.2PFCWT mice during first and repeat investigations, which was predictive of social preference behavior. In Cav1.2PFCKO mice, there was an increase in PFC activity during first social investigation but not during repeat investigations. These behavioral and neural differences were not observed during a reciprocal social interaction test nor during a forced alternation novelty test. To evaluate a potential deficit in reward-related processes, we tested mice in a three-chamber test wherein the social stimulus was replaced by food. Behavioral testing revealed that both Cav1.2PFCWT and Cav1.2PFCKO mice showed a preference for food over object with significantly greater preference during repeat investigation. Interestingly, there was no increase in PFC activity when Cav1.2PFCWT or Cav1.2PFCKO first investigated the food however activity significantly increased in Cav1.2PFCWT mice during repeat investigations of the food. This was not observed in Cav1.2PFCKO mice. In summary, a reduction in Cav1.2 channels in the PFC suppresses the development of a sustained social preference in mice that is associated with lack of PFC neuronal population activity that may be related to deficits in social reward. This week we discuss placental inflammation and heart development with postdoc Eleanor Jayne Ward and PI Suchita Nadkarni (@SciSuchita) from Queen Mary, University of London. In this preprint, the authors show that during embryonic heart development, neutrophils (innate immune cells) can sometimes cause placental inflammation. This inflammation leads to a loss of barrier function. In turn, this allows maternal monocytes (innate immune cells that develop into macrophages) to migrate into the embryonic heart. These monocytes alter the tissue resident embryonic macrophages and damage cardiac tissue. This cardiac damage continues postnatally. We discuss this and how limiting placental inflammation was enough to promote normal cardiac development. We also talk about the precarity of postdocs and why making research/postdoc a recognised profession would be an important step forward. Modern end-user computing platforms such as smartphones (e.g., Android and iOS)and smart home systems (e.g., SmartThings and NEST) provide programmable interfaces for third-party integration, enabling expressive and popular functionality that is often manifested in applications, or apps. Thus, for the last decade, designing security systems to analyze apps for vulnerabilities or unwanted behavior has been a major focus within the security community. This approach has continued well into the smart home, with researchers developing systems inspired by lessons from Android security to inspect IoT apps developed for popular platforms such as SmartThings. However, emerging characteristics of smart home ecosystems indicate that IoTapps may not represent automation in real homes, and may even be unavailable in the near future. That is, while API misuse by third-party developers is an important problem, the approach of analyzing/instrumenting IoT apps may not offer an effective or sustainable solution.In this talk, I will describe the challenges for research in the backdrop of the unsuitability of IoTapps for practical security analysis, and motivate three alternate research directions. First, I will describe the need to develop an alternative artifact for security analysis that is representative of automation usage in the wild. To this end, I will introduce Helion, a system that uses statistical language modeling to generate natural home automation scenarios, i.e., realistic event sequences that are closely aligned with the real home automation usage in end-user homes,which can be used for security or safety analysis. Second, I will illustrate the need to improve the security of mobile companion apps, which often form the weakest link in smart home deployments, and the important position of security analysis/compliance tools in ensuring the development of secure companion apps. To this end, I will present the mSE framework, which automatically and rigorously evaluates static program analysis-based security systems using mutation testing. Our work on mSE (and its successor, MASC) culminated in the discovery of critical security flaws in popular tools such as FlowDroid, CryptoGuard, Argus, and Coverity that affect the reliability and soundness of their analysis. Finally, I will conclude the talk by describing our current efforts to build system-level defenses into IoT platforms that are agnostic to IoTapps, i.e., independent of their visibility or mutability, thereby potentially providing a lasting solution to API misuse by third-party developers. About the speaker: Adwait Nadkarni is an Assistant Professor in the Department of Computer Science, and director of the Secure Platforms Lab (SPL) at William & Mary. Prof. Nadkarni's primary research domain is security and privacy, with a focus on emerging platforms, and the areas of operating systems and software security. Prior to joining William & Mary, Prof. Nadkarni earned his Bachelor of Engineering (BE) in Computer Engineering from the University of Mumbai in July 2011, followed by his Ph.D. and M.S. in Computer Science from the Computer Science Department at the North Carolina State University in May 2017 and December 2012respectively, both with Dr. William Enck. At NC State, Prof. Nadkarni was a founding member of the Wolfpack Security and Privacy Research (WSPR) Lab, and served as its Lead Graduate Student until May 2017.
