Today, we're going to talk about virtual reality and how it could be used to treat depression. We're talking with psychiatrist Kim Bullock, the founding director of Stanford's Neurobehavioral Clinic and Virtual Reality & Immersive Technologies (VRIT) program. Dr. Bullock — a physician certified in Neuropsychiatry, Psychiatry, and Lifestyle Medicine — calls herself a "radical behaviorist." Like other practitioners of cognitive behavioral therapy (CBT), she sees the troublesome thoughts and emotional states of many psychiatric disorders as just another form of behavior, which can be reshaped through self awareness and practice — much like you might work at avoiding junk food or not biting your nails.Of course, one of the biggest challenges is the practice part. It's no easy task for patients to practice experiencing the world in a more positive, healthy way. Diagnosing Post-Acute Infection Syndromes: Special Series from The Cohen Center for Recovery from Complex Chronic Illnesses at Mount Sinai“Tick-borne illnesses are incredibly stealthy and complicated and if I wasn't living and breathing it every day and seeing the intensity of these symptoms in patients, I would never believe it,” says Shannon Delaney, a psychiatrist in private practice in New York City. The complexity of such illnesses and the symptoms they cause is a key reason it can often take years to reach a diagnosis, which is the focus of this episode of Raise the Line, part of a special series on post-acute infection syndromes that Osmosis from Elsevier has created in partnership with the Cohen Center for Recovery from Complex Chronic Illnesses (CoRE) at the Icahn School of Medicine at Mount Sinai Hospital. In addition to Dr. Delaney, host Dr. Raven Baxter of CoRE welcomes Dr. Amy Kontorovich, an associate professor in Cardiology and Genomic Medicine at the Icahn School and Dr. David Putrino, director of Rehabilitation Innovation at Mount Sinai, to explore the limitations of current diagnostic tools and protocols and what changes are needed to improve patient care. Key themes include developing better tests for pathogens and educating providers to listen more carefully to patients. “In a typical medical encounter, the biggest dropped ball is completely disregarding an acute infection in the medical history,” stresses Putrino. All agree that providers need to be more comfortable with uncertainty and resist the urge to develop treatment plans that don't address root causes. “If more doctors could just say, ‘I don't know' I think it would do a great service to patients," adds Kontorovich. It is our great pleasure and deep honor to host Dr. Marsel Mesulam who is a giant in the field of Neurology and one of founders of OHBM. Dr. Mesulam is Chief of Behavioral Neurology and the Ruth Dunbar Davee Professor of Neuroscience at Northwestern University Feinberg School of Medicine, and Professor of Behavioral Neurology at the Weinberg College of Arts and Sciences.Dr. Mesulam received his MD from Harvard Medical School in 1972, and in 1976 completed residencies at Boston City Hospital and Beth Israel Deaconess Medical Center in Boston. After a 1 year postdoc at Harvard University he began his tenure in Chicago at Northwestern. Dr. Mesulam's work has been both prodigious and impactful over the years, as his almost 1000 papers have been cited over 140 thousand times. He has written the seminal book, Principles of Behavioral and Cognitive Neurology, and has produced many landmark papers - a few of which we'll discuss in the podcast. One paper that we consider a masterpiece was published in Brain in 1998 and titled From Sensation to Cognition. This can be considered as a required reading for everyone in the field of brain mapping as it lays out so concisely and eloquently, a breathtaking perspective of the structure and functional organization of the human brain. Dr. Mesulam's research is extremely broad and diverse, having impacted such areas as neural networks and functional imaging, Dementia, Alzheimer's Disease, Primary Progressive Aphasia (PPA), Cholinergic Pathways, Acetylcholinesterase Studies, Cognitive Psychology, Neurology, and Neuropsychiatry. He also developed, early in his career, a neuronal marker, Tetramethyl benzidine, that profoundly impacted research in this area. In this inspiring conversation, Peter and Marsel discuss his early career and what was important for his success, delve into research culture and the value of opportunistic research, and the value of having the freedom and resources to try many things and rapidly change directions that follow interesting leads. They also discuss some of the exciting early days of Neuroimaging and OHBM. We know that exercise does have a positive impact on the brain. We know that exercise can optimize brain function and prevent cognitive decline in adults. But what can it do for children and adolescents that learn and think differently? Today's guest is Dr. John Ratey, an internationally recognized expert in neuropsychiatry. Dr. Ratey became interested in the science of exercise and the brain early on in his career and has seen first-hand the benefits not only to one's body, but also behavior, executive functioning skills, and even attention. In today's episode, Dr. Ratey digs in and explains how exercise and being physically active can change more than your physical health.   Show Notes: [2:38] - Dr. Ratey became interested in the impact of exercise on the brain as an athlete in medical school and seeing studies comparing exercise and antidepressants. Episode 7: Bleeding-Edge Mental Health Treatment: Ketamine, Psilocybin, MDMA, TMS, VNS

New developments in the treatment of mental health challenges have been painfully slow to arrive. Talk therapy, lithium, prozac, electroshock therapy have all developed in the past 150 years. There have been a number of new medications developed but each has its own profile of positive and negative benefits. And few are highly effective (i.e., help more than 50% of the population they are intended to serve). In the past 10 years, however, there have been some noticeable improvements from some unsuspecting sources. These include psilocybin, ketamine, vagal nerve stimulation, Transcranial magnetic stimulation (TMS), Electroconvulsive therapy (ECT) and MDMA. Tune in to this fascinating conversation with Dr. Rebecca Allen, who heads up the Seattle Neuropsychiatric Treatment Center which is comprised of 5 treatment centers throughout Washington. Recent research out of Johns Hopkins, published in the prestigious journal Nature, found that critical windows of learning were re-opened in mice for 2,3 and 4 weeks depending on which psychedelic drug they were administered (and no, I have no idea how you discover the proper dosage of psilocybin for a mouse!). The mice were given either ketamine, MDMA, psilocybin, LSD or ibogaine. Ketamine kept the social reward learning period active for 48 hours, psilocybin and MDMA opened the critical learning window for two weeks, LSD for 3 weeks and ibogaine opened it for four weeks. This duration aligns loosely with the self-reported after effects of each psychedelic drug in humans. This post-treatment period could offer a valuable window of opportunity for maintaining the learning state. It appears that these drugs impact the RNA and thus, the genetic level, turning on or off genes associated with social learning. So these drugs seem to be impacting at a spiritual level, an interpersonal level, a cognitive level, a neurological level and at a genetic level. It is research such as this (as well as research in larger mammals, i.e., humans) that has heralded a new age in the treatment now called the Psychedelic Renaissance. Topics Covered In This Episode:Transcranial magnetic stimulation (TMS). This week's podcast features Rebecca Allen, MD. Rebecca is a neuropsychiatrist who completed her residency and fellowship at Harvard in 2017. Her specialty is interventional psychiatry, including treatments such as Transcranial Magnetic Stimulation, Electroconvulsive Therapy, ketamine, and Vagus Nerve Stimulation. Her work has included research on quality of life in multiple sclerosis, sleep and memory, and authoring a book chapter on sleep for a Harvard textbook.   Our conversation covered a lot of ground (as usual) because Rebecca is currently participating in research on Vagus Nerve Stimulation, comparing techniques in brain stimulation, electro compulsive therapy for bipolar disorders, schizophrenia, and the other psychotic disorders. It's quite a fascinating field and she is an expert with a lot of experience and education in this area. She is on the forefront and cutting edge of the current treatments and technology available which allows her to provide the best treatments available for her patients.  She can make suggestions but as with all fields of medicine it is ultimately up to the patient to make their own decisions. Her advice is to not just take what she says, go talk to other people, do some of your own research before making a decision about treatment. Very good advice in all things medical! Dr. Allen graduated with a B.A. in Psychology with honors from Stanford University. She attended Oregon Health and Science University for medical school where she also earned a Masters of Public Health in biostatistics and epidemiology. She completed psychiatry residency at the Harvard Longwood Psychiatry Residency Training Program, a joint program between Brigham and Women's Hospital, Beth Israel Deaconess Medical Center, and Massachusetts Mental Health Center. She served as Chief Resident of the psychiatry consult service at Beth Israel Deaconess Medical Center. She then went on to complete a 2-year fellowship in Neuropsychiatry at Brigham and Women's Hospital. She has participated in research on sleep and memory consolidation, quality of life in multiple sclerosis, efficacy of different TMS protocols, EEG biomarkers of TMS response, the RECOVER VNS trial, and other projects. She has authored several papers.Her practice is Seattle Neuropsychiatric Treatment Center, the oldest and largest provider of TMS, ECT, VNS, and esketamine in the Pacific Northwest. She is one of three Partners and serves as the Director of Neuropsychiatry and Research. She is the Site Principal Investigator for multiple clinical trials.She does multiple educational talks every year on neuromodulation, including nationally (Clinical TMS Society) and locally (UW medical events, Washington State Psychological Association).
