Also check out the video version on my WeitzChiro YouTube page.]  Podcast Highlights 1:48  Dried urine testing or DUTCH testing uses a dried sample for convenience, which is a big advantage over the other leading type of urine hormone testing, which is the 24 hour urine test that requires carrying around a jug of urine that you keep in the refrigerator all day.  This is very inconvenient and you lose the the changes in those hormones levels, esp. cortisol, that vary throughout the day using a 24 hour sample.  The dried urine test you collect samples throughout the day so you can look at the cortisol pattern, which correlates with what is seen on saliva in studies that have been published.  And the urine testing of hormones allows you to see the metabolites of estrogens and androgens that you can't see on blood testing.  And there is a DUTCH complete test that includes markers in urine for melatonin and B12 and B6, etc. that allows us to see a fuller picture of the hormone story.  For example, this complete testing is helpful if you have a patient who has fatigue you can find out if it is related to testosterone or cortisol or B12. 4:57  On the other hand, dried urine testing does not allow you to measure SHBG, which can be measured through blood, such as when a woman is on birth control and this drives up her SHBG, which drives down her free hormones. Urine testing is also not a good way to measure thyroid hormones. Also, if your kidneys are not functioning well, then you are not excreting urine as well as you should and this can impact urine testing. Also, saliva has some advantages for the cortisol awakening response (CAR), so DUTCH plus includes saliva for the first two samples of cortisol, since for the CAR you need an instantaneous sample when you first wake up within the first five minutes.  DUTCH uses a cotton swab that soaks up saliva instead of having to spit into a tube like other labs use and fill it with 3 ml of saliva after waking up without rinsing your mouth and without drinking any water and for most people this takes 15-20 min, which negates the value of the CAR test.                                Mark Newman, MS is a recognized expert and international speaker in the field of hormone testing. October is Breast Cancer Awareness month, so of course, we need to look at one of the best ways to assess one’s risk: DUTCH Testing! I’m joined by DUTCH Testing Medical Director, Dr. Carrie Jones, to talk about the DUTCH Test and how to naturally treat abnormal findings on the test.   Three things you can do now to reduce your risk of breast cancer: Reduce your use of plastic. If you have to, opt for BPA-free plastic. Eat the skin of your vegetables. Take a supplement like DIM.   About Dr. Carrie Jones Carrie Jones is a naturopath and an internationally recognized speaker, consultant, and educator on the topic of women's health and hormones. She is currently the Medical Director for Precision Analytical Inc.   Looking Into DUTCH Testing As the director of DUTCH Testing, one of the tests I use to detect poor estrogen metabolism in my patients, Carrie is the perfect person to have on and discuss it. She lets us know exactly what DUTCH testing is, how it’s done, and how it’s designed to detect poor estrogen metabolism. High estrogen levels can be one of the increased risk factors for breast cancer. The risk is even higher if you’ve gone through menopause and still have elevated levels of the hormone. Carrie takes us through some of the ways you can improve estrogen metabolism in your body. You’ve heard me talk about this supplement before, but I am a huge advocate for using DIM to contribute to positive breast health. There are so many benefits to taking a supplement full of cruciferous vegetables. However, Carrie explains why DIM is only useful if you have a phase one problem. In fact, she states that taking DIM with stage two or three problems could actually be detrimental to your health.   Treating Phase Two and Three So how do you treat poor estrogen metabolism with phases two and three? Carrie walks us through the natural treatments, including foods and supplementation, to safely navigate this time. Something Carrie encourages is leaving the skin on your vegetables, especially carrots. Carrots are high in beta carotene, which is beneficial for us, all the time, but especially when we’re battling something like poor estrogen metabolism. Finally, Carrie talks us through what chemicals we need to remove from our homes, diets, and beauty routines. Chemicals can contribute to increased cancer risk, so removing them from your environment early is nothing but beneficial. What’s your favorite way to eat carrots? The two/sixteen ratio, like you were saying, years ago people were really into it, has been largely debunked because now we know about four. And sixteen is estrogenic, meaning that little molecule sixteen can actually bind to estrogen receptors and, it’s not as strong as true estrogen and that’s why it’s helpful for bones. Menopausal women who have a higher sixteen maybe have better bone quality. Ultimately, what the big thing is, is it’s because it’s estrogenic that we have to look at the woman’s whole estrogen picture. Then when it comes to cancer, it’s that four hydroxy pathway that, if it gets out of hand, really causes DNA damage and we don’t want that.” [16:38] “DIM is helpful for that phase one, only. DIM is only helpful if you have a phase one problem. If you have a phase two or three problem, DIM will actually make you worse. You want to be very careful and you want to test to know.” [20:22] “You can, ladies, have estrogen issues because you have gut issues. It’s a recycle thing. Dr. Carrie Jones is a naturopathic doctor and a medical director for Precision Analytical, which is the company that handles all the dutch hormone testing that is run for women in the hormone project and through my practice.   Listen to this episode to unwrap knowledge about the work of hormones in your body, stress connection, estrogen and androgen dominance and the impact on your thyroid. Take your time to absorb and digest this information, and if a lot of what is said today resonates with you, you should take a look at the Hormone Project starting in January.     Why are hormones so critical to our overall health? Hormones are made in glands, they basically give the message to other glands in your body to do or not to do something, so when there is a overproduction or, on the contrary, not enough hormones, the symptoms start to show. It’s all about balance because hormones are literally the chemical messengers that tell your body what to do and help you feel better. There needs to be an appropriate amount of messages and they need to be received by the right gland!   “My test results are fine… but I don't feel well.” Usually lab test cover the basic aspects of your health, they go over functions and signals just to make sure you are not going to die. Short doctor’s consultations don't help either! But you can ask for more, go deeper than a simple blood test! Ask for other lab tests and sometimes you even have to consider changing doctors.    How DUTCH Panel testing helps to describe what is going on in our bodies? It is an easy to collect urine test where you get all the traditional hormone results, but also all the pathways that these hormones are travelling (which blood, saliva or stool lab tests don’t show).    The Estrogen Path Your body makes estrogen and when this production is over, the estrogen needs to be broken down and excreted. In the first stage of this transit, estrogen can be directed through one of three differents paths called:  2, 4 and 16. Number 2 is the healthy path, 4 can potentially increase your risk for cancer and 16 is called prolific. In the second stage your body has to neutralize the estrogen, this phase is called the detox stage. These two phases need to be addressed differently.   Estrogen Dominance  As a woman, your estrogen rises right before ovulation, and then it goes down for later rising back up again before the following ovulation. The second time it goes up, your progesterone needs to be high and healthy. But if you have low progesterone production, your estrogen will be unattended and run wild, which translates into heavy periods, tender breasts, mood changes...and it is called estrogen dominance (since progesterone is not enough).    Sleep, Sleep, Sleep During REM sleep you dream and during deep sleep your body heals! Cortisol goes down, you lose weight, men produce more testosterone. Just sleeping makes you healthier! Androgen Dominance You can either make an excess of testosterone that stimulates androgen receptors, causing symptoms such as greasy skin, cystic acne, chin hair...or you can be making normal amounts of testosterone but going in an androgenic pathway, creating imbalance. To restore your hormonal balance, start by slowing down, insulin has to be kept low, lift weight, check your diet, have the right supplements, include more Zinc in your diet. Check your Thyroid Are you converting T4 to T3 well? Your TSH might look ok if your pituitary is fine, but you can have a thyroid problem, running a TSH, T3 and T4. T3 can be deactivated to reverse T3 (RT3). You have to investigate the whole pathway, dig deeper, ask the right questions!   I encourage you to invest your time and make the right choices to heal your hormones and restore their balance, hoping that this information has contributed to get you closer to your health goals. Take the necessary steps to balance your hormones and make sure they’re going down the good pathway. Dr. Carrie Jones said: “Your genes, they dictate a lot, but there’s a lot you can do to change it. And just because you have the mutation, it doesn’t mean everything is 100%... Just because you have the mutation, it’s entirely possible other things have made up for it.” “Hormones are a luxury but stress is not. The body is designed to deal with fight or flight, and whether or not you ovulate has no bearing on you living.
