Episode 166: Naturopathic Medicine InsightsFuture Dr. Luong talked about what she learned about naturopathic doctors (NDs). She discussed the principles of naturopathic medicine and mentioned some differences in regulations across states in the US. Dr. Arreaza shared his opinion about the pros and cons of naturopathic medicine. Written by Teresa Luong, MSIV, American University of the Caribbean. Comments and editing by Hector Arreaza, MD.You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.Arreaza: This may be a controversial topic. The term “natural” in medicine triggers strong reactions among allopathic doctors like me. Today we have a medical student who took up the challenge to talk about Naturopathic Doctors. Teresa:I am providing information based on research and living in Washington state, which is considered one of the birthplaces of modern naturopathic medicine, boasts the highest population of naturopathic doctors and a wide range of practice specialties. It's important to note that my responses are not personal opinions but rather informed insights. There are about 8,500 licensed naturopathic doctors in North America. Naturopathic Doctors may not use the title of physicians in California. What is a Naturopathic Doctor? While it's true that in California naturopathic doctors are not legally permitted to use the term ”physician” to describe themselves, some still choose to refer to themselves as “naturopathic physicians” colloquially or in their practice branding. So, confusion can arise because naturopathic doctors, while legally not recognized as physicians in California, can function as primary care providers, this overlap in roles can lead to the informal use of terms like ”naturopathic physician.“ However, it's important to recognize that legally, they are not recognized as physicians in this state. Naturopathic doctors (NDs) are healthcare professionals who embrace a holistic approach to healing, focusing on natural and non-invasive therapies to support the body's inherent ability to heal itself. Naturopathic medicine has its roots in traditional healing practices from around the world, blending ancient wisdom with modern scientific knowledge. The philosophy of naturopathy emphasizes the importance of treating the whole person—mind, body, and spirit—rather than just addressing isolated symptoms or diseases. What is an allopathic approach vs a holistic approach?Allopathic medicine: Allopathic medicine focuses on diagnosing and treating specific symptoms or diseases using pharmaceutical drugs, surgery, and other conventional interventions. Treatment is often targeted at managing symptoms or eradicating pathogens.Holistic medicine: Holistic medicine takes a broader approach, considering the whole person and aiming to address the root causes of illnesses. Treatment may involve a combination of conventional therapies and alternative modalities such as nutrition, herbal medicine, acupuncture, and lifestyle modifications. The focus is on promoting overall health and well-being rather than just treating isolated symptoms.Core Principles Naturopathic Practitioners: The six core principles of naturopathic medicine serve as guiding tenets for both diagnosis and treatment. These principles include: -first, do no harm. -the healing power of nature -identify and treat the root cause -treat the whole person, -the physician as a teacher; -and prevention as the best cure. Treatment modalities: Naturopathic doctors employ a wide range of therapeutic modalities to address the unique needs of each individual, such as:-Clinical nutrition, which focuses on using “food as medicine” to promote healing and prevent disease. -Herbal medicine utilizes the medicinal properties of plants to support various bodily systems and restore balance. -Acupuncture, everybody is familiar with acupuncture. -Other modalities may include hydrotherapy, homeopathy, physical medicine (such as massage and manipulation), and lifestyle counseling. The role of allopathic medications (NSAIDs, antibiotics) and surgical procedures.Naturopathic doctors typically prioritize natural therapies and lifestyle intervention, but may also integrate conventional medicine when necessary. Their views on traditional medications, such as NSAIDs, antibiotics, and surgical procedures vary depending on the individual practitioner and their approach to healthcare. Some may recommend them when appropriate, while others may prefer to explore alternative options first. Ultimately, their goal is often to promote holistic health and well-being. Education and Training: Pre-Medical Requirements: Aspiring naturopathic doctors typically complete undergraduate coursework in pre-medical sciences, including Biology, Chemistry, Physics, and Psychology.Naturopathic Medical Program:Naturopathic doctors must graduate from a four-year accredited naturopathic medical school. The forst two years are basic sciences courses and the last 2 years are clinical courses. After completing their naturopathic medical education, graduates must pass the Naturopathic Physicians Licensing Examinations (NPLEX) to become licensed practitioners. MDs need a residency, and NDs can start working immediately after school in some states, or they can do some additional training.Scope of Practice: The scope of practice for naturopathic doctors varies depending on the regulations in each jurisdiction. In general, NDs are trained to:- Prescribe natural therapies such as nutritional supplements, herbal remedies, and homeopathic medicines.- Prescribe prescription medications, including some, but not limited to, controlled substances.- Provide dietary counseling and lifestyle interventions tailored to individual health needs.- Provide Primary Care, such as, performing physical exams, administering vaccinations, ordering laboratory tests,- Offer acupuncture.- They can use detoxification protocols and intravenous (IV) nutrient therapy into treatment plans.- Utilize mind-body techniques like meditation, mindfulness, and relaxation exercisesIn January 2022, the recognition of naturopathic doctors (NDs) and the extent of their scope of practice vary significantly from state to state within the United States. Here's an overview of states where NDs are recognized and their scope of practice:Licensed States: In these states, naturopathic doctors are licensed to practice independently and have a broad scope of practice, including diagnosis, treatment, and prescription of medications and natural remedies within their scope. Some states may have specific restrictions or additional requirements.-California: NDs are licensed and regulated by the California Naturopathic Medicine Committee. They have a comprehensive scope of practice, including the prescription of certain medications.-Washington: NDs are licensed and regulated by the Washington State Department of Health. They have a limited formulary for prescription medications.Unlicensed States (for example, Georgia and Virginia): In states where naturopathic medicine is not regulated, NDs have to work as healthcare consultants or working alongside other licensed healthcare providers, like MDs. The scope of practice in these states may be more limited, and NDs may not have the authority to diagnose, treat, or prescribe medications independently.Pros:Naturopathic doctors utilize natural and holistic therapies, which may have fewer side effects compared to conventional medications. These therapies can complement conventional treatments, especially if they are collaborating with an MD.If Naturopathic doctors work collaboratively, it can offer complementary care options to patients, which can provide patients with treatment modalities and perspectives beyond conventional medicine. This can be particularly beneficial for patients who are a bit apprehensive to traditional medicine for whatever reason.Naturopathic medicine emphasizes preventive care and strategies to promote health and prevent disease. So they try to catch underlying imbalances and risk factors early, and see patients multiple times a week, which can help patients reduce their risk of developing chronic illnesses and improve their overall quality of life. By seeing a patient this frequently, it promotes stronger doctor-patient relationships and improves patient satisfaction.Cons:Lack of Regulation and Standardization: Naturopathic medicine is not regulated to the same extent as conventional medicine, and there may be variability in the education, training, and qualifications of naturopathic doctors. This lack of regulation and standardization can raise concerns about the quality and safety of naturopathic care.Limited Scientific Evidence: Some naturopathic treatments lack rigorous scientific evidence supporting their efficacy and safety. Without robust clinical research, it can be challenging to assess the effectiveness of certain naturopathic interventions and differentiate between evidence-based practices and unproven therapies.Potential for Harmful Practices: In some cases, naturopathic doctors may recommend treatments or interventions that have the potential to be harmful, particularly if they lack scientific support or are based on unfounded beliefs. Patients may be at risk of receiving ineffective or unsafe treatments without proper oversight and regulation.Delayed Access to Conventional Care: So if a patient relies solely on naturopathic care, it may delay access to conventional medical treatments that are necessary for managing serious or life-threatening conditionsCost and Insurance Coverage: Naturopathic services may not be covered by health insurance plans, or coverage may be limited compared to conventional medical services. This lack of insurance coverage can make naturopathic care inaccessible to some patients, particularly those with limited financial resources. This information is provided to increase your knowledge on this topic, it is not intended to convince you to go against or support naturopathic medicine. Our goal is to inform you and you can draw your own conclusions about it. It is important to educate ourselves on this topic because naturopathic doctors are rising in California and we may encounter them in our future practices. ________________________________Conclusion: Now we conclude episode number 166, “Naturopathic Medicine Insights.” Future Dr. Luong explained that naturopathic doctors receive training in naturopathic medical schools and receive a degree before they sit for a board exam. Many NDs may be working in outpatient primary care or even in hospital care. Dr. Arreaza shared his opinion about the pros and cons of having naturopathic medicine available for patients. This week we thank Hector Arreaza and Teresa Luong. In today's episode I get to introduce you to my friend, colleague, and mentor Dr. Tori Hudson. Dr. Tori Hudson has been in practice as a Naturopathic Doctor for almost 40 years in Oregon. She is a nationally recognized naturopathic physician, speaker, educator, researcher, clinician and the first woman in the United States to become a full professor of naturopathic medicine. She is also the author of the Women's Encyclopedia of Natural Medicine: Alternative Therapies and Integrative Medicine for Total Health and Wellness, and has served as a Medical Director, Associate Academic Dean, and Academic Dean at the National University of Natural Medicine (NUNM). Most recently, she has released her new book: The Menopause Companion. In addition to founding and running her own clinic, A Woman's Time, Dr. Hudson is the founder and co-director of the Naturopathic Education and Research Consortium (NERC), a non-profit organization for accredited naturopathic residencies. She currently serves as the program director for the Institute of Women's Health and Integrative Medicine. Dr. Hudson also co-founded Vitanica, a supplement company offering formulations based on her decades of clinical experience. Today we talk about women's health later in life and how we can help women in new ways using a naturopathic approach and natural medicine. How Has Women's Healthcare Evolved Over the Last 20-25 Years? There's been a considerable shift in menopause management. Specifically, the rise and fall, and rise again, of menopausal hormone therapy. Natural medicine started to gain more recognition in the late 80s'. Dr. Michael Murray was very influential in bringing to light the botanical medicine research out of Europe and other parts of the world. Dr. Tori attributes to him the initiation of an awareness of the world of botanical medicine research and the influence it has had on naturopathic practice and the development of integrated physicians. The shift began in using supplements to both reduce the risk of disease and treat disease. Women's health in particular is seeing an immense expansion of options, which can be fantastic, but also more confusing to navigate. People are now able to research different approaches online and they're able to access different practitioners. So, women are more likely to say “I want something different” or “I don't just want a cookie cutter suggestion”. Today, patients are more informed and have more opportunities to be informed. They come more often with their own thoughts, ideas, and opinions. Most the time that can lead to a collaborative process of determining their treatment plan. Women today have a lot more tools to help themselves. They have new ways to get what they need from their healthcare providers, and they have a lot more ability to question if they really need a treatment or procedure. Dr. Tori loves when a patient comes in and asks for a second opinion. Recently, she had a patient asking her if she really needed a particular surgery for her bladder and she was able to reassure her that surgery was the necessary next step. Naturopathic doctors are trained to recognize the value of conventional medicine and how to incorporate that into a natural approach and a naturopathic medicine context. It's important to recognize that every paradigm has its strengths, and every paradigm has its weaknesses. That's what is unique about the modern naturopathic physician - we can really help patients navigate the best opportunities for their healthcare. Naturopathic Medicine as an Individualized Approach for Menopause Perimenopause starts at least a decade before the period ends and then comes post menopause. So, for women living till their mid-80's it's really most of their lives. It's a lot of years of our lives to be navigating hormone shifts, and potential symptoms and health issues, and women really do need help navigating that. Especially because there's still not enough research, and the information that is available is not always consistent and can be confusing. Dr. Tori reminds us that there's rarely a one size fits all approach, which is the beauty of naturopathic medicine. We try to individualize the approach for each person. If there are 10 different women who have hot flashes, there's going to be some recommendations that are common for them all, but there's also the nuance of suggestions that match for the rest of their health. So, hot flashes occur in a context of: does the patient have arthritis, what's their risk for diabetes, do they have high blood pressure, what's their risk for Alzheimer's? There is the quality-of-life issues which can be fairly simple to resolve, and then there's the disease prevention issues and the disease treatment issues which need to be addressed. A naturopathic doctor should help you look at the big picture and then help gather the information to help you make the best decisions possible about what to take and how to support your health. Navigating The Ladder of Intervention in Women's Health Dr. Tori's new book outlines what changes might be happening in your body, how to ask the right questions of your health care provider, what might be problematic in the world of tests and treatments, and more. She compares it to a map, as opposed to a strict sequence. There are many options for how to get on from point A (where you are starting) to point B (feeling better). This is important because we are all coming to the table with a different set of cards, and we may all have similar pathways to navigate, but at the end of the day, each of our approaches needs to be individualized. It is tempting to talk to other women and hear a particular intervention that worked well for them, but your menopausal journey will probably look very different from your friends' or even your family members' journey – and what may have worked well for them, may not have the same impact for you. It's great to share stories and share experiences with other women, but no one else's experience will be your exact prescription. That's where the ladder of intervention becomes helpful. On this ladder you can consider where you are starting and what makes sense in your case. Consider what symptoms you are experiencing, what your family history predisposes you to, your own medical history, and what you've already tried to help you see where you should begin. The beauty of this process is to determine the most minimally invasive intervention we can introduce that will lead to the intended improvements. It's not just a hierarchy for all circumstances, as in we do not always start on the first rung of the ladder. Each person or situation will have their own hierarchy. Maybe you've already been implementing diet and exercise but still experiencing night sweats, or hot flashes, or have osteoporosis. In these cases, we move up the ladder of intervention to see what steps are needed to produce the necessary changes. Multitasking Herbs for Menopause Since our body is an ecosystem, symptoms do not usually occur in the singular. Taken together, the totality of symptoms can provide a more complete picture of what herbs might be most helpful. Dr. Tori refers to these combinations as twofers, three-fers, and four-fers. For instance, if a woman is experiencing hot flashes we will also ask, is she struggling with her mood, does she have achy joints as well, or is she experiencing memory issues. Then we look for herbs or formulas that will cover the most issues for a specific patient. Black Cohosh, for example, has by far more research than any other single herb, but that doesn't mean it's going to work for everyone. Maybe curcumin will be the remedy for someone who has hot flashes as well as joint pain. Better Understanding of Hormone Replacement Therapy Is hormone replacement therapy right for you? Women are often hesitant about hormone replacement for one reason or another, as it can feel drastic. There's newer research that's helping us, even as providers, become more comfortable with hormone replacement therapy. If you have been following a treatment plan, and you've tried herbs and formulas that can help with perimenopausal symptoms, but you're still experiencing night sweats or vaginal dryness and your symptoms are not improving, then we can start talking about some form of bioidentical hormone replacement therapy. It's important to understand the benefits and the risks of menopausal hormone therapy. Age will be one of the most important determining factors. How old is the patient, when did she become menopausal, does she have other coexisting health issues or what other coexisting health problems does she have, and what are her risks for what Dr. Tori calls the big three: Osteoporosis, Heart Disease, and Alzheimer's Disease. Additionally, there are three buckets of information that are important to evaluate: what are your quality-of-life issues, what diseases are you at increased risk for, and what conditions or diseases do you already have. All of that informs the decision making on the benefits and the risks of menopausal hormone therapy. If you're within the first ten years of your last menstrual period, less than 60 years old, you haven't had breast cancer, a stroke, or clot, or DBT, then the risks of hormone replacement therapy are negligible. There are a few other risks to consider, like fatty liver, but those are the fundamental considerations. From there we can determine whether menopausal hormone therapy is safe for that person. The risk of breast cancer from going on menopausal hormone therapy is very low if you're in the group just described. In fact, it is much more likely to get breast cancer if you drink alcohol every day, smoke, you don't exercise and you're overweight. There are very few women for whom systemic menopausal hormone therapy is actually contraindicated. There are cautions on delivery methods, for instance if you have a gallstone, we shouldn't give oral hormones, and if you have fatty liver or elevated triglycerides, we shouldn't give oral, but we can give transdermal hormones instead. It's important to work with a practitioner who understand all the options, delivery methods, and ways to customize your dose based on your health. What About Local Estrogen for Vaginal Health? Vaginal estrogen can be a miracle worker for some women. Vaginal estrogen for the purpose of local vulvovaginal problems doesn't increase the risk of anything if it's done properly. Typically, it is prescribed for local dryness, itching, irritation, painful sexual activity, and/or leaky bladder. In these cases, it's often a waste of time to attempt vitamin E, moisturizers, or lubricants. You can skip up the ladder of intervention to vaginal estrogen since it is still very minimally invasive. There are, of course, still instances where caution should be exercised, for instance if breast cancer is or was present. In that case, a non-hormonal option would usually be the beginning recommendation. But even gynecologist societies and menopause societies say you can use tiny doses of vaginal estrogen safely in a breast cancer patient for vaginal symptoms in many cases. A low dose of vaginal estrogen has also been found to be helpful for women who tested positive for HPV and have abnormal cells on their pap smear. With vaginal estrogen, you're improving the vaginal biome and that means you can prevent bacterial vaginosis or vaginal infections. We're finding that the vaginal biome and the species of lactobacilli living in the vagina help fend off HPV as well. Vaginal estrogen gets you a healthier ecological environment and balance of microorganisms. A healthy vaginal biome relies on healthy estrogen levels in the cells. We might first notice vaginal dryness most acutely during sex, but vaginal dryness can be an indicator of other health issues and affect more than just your sex life. Local estrogen can be a preventive medicine that can help with vaginal dryness and a range of other health issues as well. The dryness and discomfort put you at an increased risk for vaginitis, urinary tract infections, possibly HPV, bladder leakage, and overactive bladder. You can prevent those things from happening most of the time by using a small dose of vaginal estrogen twice per week. So, it's not just for sexual comfort, it's related to your overall bladder and vaginal health as well. What Are Some Good Supplements for Menopause and Women's Health? As we mentioned before, Dr. Hudson co-founded Vitanica, a supplement company offering formulations to support women's health. Some of my favorites are: Black Cohosh: The most effective formulation I've ever come across for hot flashes and other menopausal symptoms. Chaste Tree Berry: An herb that supports ovulation and helps with symptoms of premenstrual syndrome, breast pain associated with the menstrual cycle, infertility, and perimenopausal menstrual changes. Slow Flow: A formula of herbs and nutrients for heavy menstrual bleeding. CCDG Blend: Tried and true for HPV cases, CCDG contains curcumin, coriolus mushroom, DIM and green tea, all in one. Cranstat Extra: Herbal and nutrient formula for bladder infections. Candidastat: To support a healthy biome and prevent yeast overgrowth. I've been working with these formulas for 23 years (Dr. Doni speaking), and I've seen time and time again how helpful they are for women. That's why I recommend them in my practice. As always, I am here and would love to help you wherever you are in your health and menopausal journey. I'm licensed to help with hormone replacement therapy in several states. On Naturopathic medicine In this episode of Stepsero we chat about naturopathic medicine and human interconnectedness with Dr. Antonella Aguilera Ruiz, ND. Below are a few main takeaways from our discussion: Naturopathic medicine is a relatively new discipline and is sometimes perceived as an antagonist to Orthodox medicine. In the US, there are only five accredited colleges. It is Dr. Antonella's hope that Naturopathic Medicine and Orthodox Medicine will manage to coexist in the near future. Ultimately, we should we able to combine different schools of education so that patients have access to the best possible care. Human beings are complex interconnected systems, made of different delicate parts which function in relation with each other. One aspect of particular importance is social connection, which is however rarely taken into account, despite having a significant impact on our well-being. Social connection can look very different for different people (e.g. an introvert vs an extrovert), and it can be expressed in many ways, such as being part of a religious community or going for a hike with a small group of friends. Agency and Control are two crucial concepts and yet they are often misunderstood. While we all are mortal beings, we do have agency over certain aspects of our life that can create a greater sense of well-being and purpose. On this note, we should be careful with telling people that they have total control over their health. Agency can actually help us rewire and reshape our brain (neuroplasticity). Through positive neuroplasticity our brain can learn and grow. This means that, for example, with the right behavior we change the structure of our brain, adjust our fear response, and develop greater resilience. She went on to become a founding physician in the development of the SIBO Center and SIBO Symposium at National University of Natural Medicine. Once in Southern California, she spent time with leading SIBO physician, Dr. Mark Pimentel, MD, and his team at Cedars-Sinai's GI Motility Program and the Pimentel Laboratory. While in naturopathic school, Dr. Melanie discovered the cause of her longtime digestive problems was actually SIBO, or rather, an excess of methogens. She eventually treated it successfully, but it took multiple tries. Now, she shares this knowledge with her patients, for whom she develops highly individualized treatments. In this episode, Melanie primarily shares how she was able to treat SIBO without antibiotics. In my conversation with Melanie you'll hear… How she diagnosed herself About the different types of SIBO Why methane SIBO isn't actually small intestinal bacterial overgrowth How too-low or too-high stomach acid contributes to SIBO or methanogens The type of testing required to assess stomach acid levels The steps she took to treat SIBO How addressing her stomach acid levels made all the difference for her The role of diet and why long-term restrictive diets are not recommended How she prevents SIBO relapses Episode web page: www.rebuildingmyhealth.com/sibo-without-antibiotics Learn more about Melanie at https://www.sibosolution.com/ or https://www.intuitiveedgedoctor.com/. October is Breast Cancer Awareness month, so of course, we need to look at one of the best ways to assess one’s risk: DUTCH Testing! I’m joined by DUTCH Testing Medical Director, Dr. Carrie Jones, to talk about the DUTCH Test and how to naturally treat abnormal findings on the test. Three things you can do now to reduce your risk of breast cancer: Reduce your use of plastic. If you have to, opt for BPA-free plastic. Eat the skin of your vegetables. Take a supplement like DIM. About Dr. Carrie Jones Carrie Jones is a naturopath and an internationally recognized speaker, consultant, and educator on the topic of women's health and hormones. Dr. Jones graduated from the National University of Natural Medicine (NUNM) in Portland, Oregon where she also completed a 2-year residency in women's health, hormones, and endocrinology. Later, she graduated from Grand Canyon University's Master of Public Health program. Recently, Dr. Jones became board certified through the American Board of Naturopathic Endocrinology. She was adjunct faculty at NUNM for many years, teaching gynecology and advanced endocrinology/fertility. While in practice, Dr. Jones served as Medical Director for 2 large integrative clinics in Portland. She is currently the Medical Director for Precision Analytical Inc. Looking Into DUTCH Testing As the director of DUTCH Testing, one of the tests I use to detect poor estrogen metabolism in my patients, Carrie is the perfect person to have on and discuss it. She lets us know exactly what DUTCH testing is, how it’s done, and how it’s designed to detect poor estrogen metabolism. High estrogen levels can be one of the increased risk factors for breast cancer. The risk is even higher if you’ve gone through menopause and still have elevated levels of the hormone. Carrie takes us through some of the ways you can improve estrogen metabolism in your body. You’ve heard me talk about this supplement before, but I am a huge advocate for using DIM to contribute to positive breast health. There are so many benefits to taking a supplement full of cruciferous vegetables. However, Carrie explains why DIM is only useful if you have a phase one problem. In fact, she states that taking DIM with stage two or three problems could actually be detrimental to your health. Treating Phase Two and Three So how do you treat poor estrogen metabolism with phases two and three? Carrie walks us through the natural treatments, including foods and supplementation, to safely navigate this time. Something Carrie encourages is leaving the skin on your vegetables, especially carrots. Carrots are high in beta carotene, which is beneficial for us, all the time, but especially when we’re battling something like poor estrogen metabolism. Finally, Carrie talks us through what chemicals we need to remove from our homes, diets, and beauty routines. Chemicals can contribute to increased cancer risk, so removing them from your environment early is nothing but beneficial. What’s your favorite way to eat carrots? How have you already reduced your plastic use? Let me know in the comments on the episode page. Quotes “If I was to be concerned about any of the dials, it’s the four, for sure. The two/sixteen ratio, like you were saying, years ago people were really into it, has been largely debunked because now we know about four. And sixteen is estrogenic, meaning that little molecule sixteen can actually bind to estrogen receptors and, it’s not as strong as true estrogen and that’s why it’s helpful for bones. Menopausal women who have a higher sixteen maybe have better bone quality. Ultimately, what the big thing is, is it’s because it’s estrogenic that we have to look at the woman’s whole estrogen picture. Then when it comes to cancer, it’s that four hydroxy pathway that, if it gets out of hand, really causes DNA damage and we don’t want that.” [16:38] “DIM is helpful for that phase one, only. DIM is only helpful if you have a phase one problem. If you have a phase two or three problem, DIM will actually make you worse. You want to be very careful and you want to test to know.” [20:22] “You can, ladies, have estrogen issues because you have gut issues. It’s a recycle thing. So working on your gut health, cleaning up your gut health, improving constipation, etc will really help your estrogen.” [30:25] In This Episode What DUTCH testing is [5:00] How to reduce your estrogen to have the least cancer-causing risk [14:00] How the DIM supplement can benefit your breast health [19:30] How to take DIM safely [21:45] How to treat phases two and three of breast cancer [22:25] Why you should eat the whole carrot, skin and all [32:45] What chemicals should you remove from your life [33:50] Links & Resources Check out the full show notes for this episode here Learn More About Your Longevity Blueprint: The Course Learn more about the DUTCH Test Find Dr. Carrie Jones Online Follow Dr. Carrie Jones on Instagram Find Your Longevity Blueprint Online Follow Your Longevity Blueprint on Instagram | Facebook | Twitter | YouTube | LinkedIn Get your copy of the Your Longevity Blueprint book and claim your bonuses here Find Dr. Stephanie Gray and Your Longevity Blueprint online Follow Dr. Stephanie Gray on Facebook | Instagram | Youtube | Twitter | LinkedIn Integrative Health and Hormone Clinic Podcast Production by the team at Counterweight Creative Related Episodes Episode 20: Breast Cancer Conqueror Dr. Veronique Desaulniers Episode 17: Recovery from Breast Implant Illness with Sarah Phillipe Episode 19: How to Heal from Endometriosis and Pelvic Pain with Dr. Jessica Drummond

Estrogen metabolism dysregulation may be linked to symptoms of bloating, constipation, menstrual problems, weight gain, low libido, and also conditions like pms, acne, endometriosis, polycystic ovarian syndrome, and breast cancer. The link could be explained by the Estrobolome. Which was first mentioned in 2011 by Plottel and Blaser in 2011. as “the aggregate of enteric bacterial genes whose products are capable of metabolizing estrogen." Simply put some bacteria in the gut may be causing increased estrogens to recirculate into our blood instead of being eliminated. When this happens, it can be not so good for us. In this episode, I welcome Dr. Carrie Jones an international expert in Women's Health and Hormone health to help us unpack this topic. In this episode we cover: What is the Estrobolome. Symptoms of estrobolome dysfunction Phase 1 detoxification Phase 2 detoxification Phase 2.5 detoxification Phase 3 detoxification How is bile involved with estrogen detoxification? What lab tests would identify detoxification issues Beta-glucuronidase Other hormones affected by gut health How to correct imbalances. Welcome to the Keto Kamp Podcast Episode 78 with Dr. Carrie Jones Today, I am lucky to have here with me, Dr. Carrie Jones, a functional medicine women's health and hormone doctor. Dr. Carrie Jones believes in the importance of both men and women understanding their hormones in a simplified manner so they can feel more empowered to take control of their health, especially when they are told everything is "normal." Still, they don't feel "normal." Dr. Carrie is a Naturopathic Physician with a Master's in Public Health, having over 12 years in the field of functional and integrative medicine. As Adjunct Faculty for the National University of Natural Medicine (NUNM), she has taught courses in both Gynecology and Advanced Endocrinology. Dr. Carrie has been the Medical Director for two large integrative clinics in Portland, Oregon. Dr. Carrie is currently the Medical Director for Precision Analytical, Inc, creators of the most cutting edge hormone test on the market, the DUTCH Test. As a result, Dr. Carrie consults with the most fantastic health care practitioners all over the world on the hormone status of their patients, and lecture/teach/frequently write on the same topic. Understanding adrenal, thyroid, and sex hormones are complicated. Dr. Carrie's job is not to judge. Her job is to educate and empower. In this episode, Dr. Carrie Jones speaks about common women's health issues. Lack of energy is a common complaint which could be due to a variety of issues. It is essential to get morning sunlight to combat the lack of energy. If you do not have access to sunlight, Dr. Carrie says to get a full-spectrum light. Then, Carrie explains the importance of healthy fats on our hormones. If women are not eating healthy fats, they stop ovulating, their skin breaks out, and their hair is in poor health. Later, Dr. Carrie advises on how to prevent constipation and heavy periods. [00:45] About Dr. Carrie Jones Carrie always knew she wanted to get into the healthcare industry. In Portland, Dr. Carrie found naturopathic medicine. She learned all the naturopathic principles, diet, lifestyle, sleep, and risk-factor mitigation. Carrie uses a lot of herbal medication, essential oils, and acupuncture. She does not put herself in any specific box. There are a time and a place for surgery and antibiotics. [05:25] The Most Common Problem With Women’s Health There are metabolic and hormonal problems. Most women’s issues will fall into one of these two categories. A majority of women complain about their energy. Dr. Carrie says there is a lot of reasons for low energy. It can be cortisol, B12, thyroid, poor sleep, etc. Dr. Carrie does a series of tests to determine what could be causing low energy. [07:50] The Importance of Morning Sunlight Morning sunlight is light and day difference. Open your curtains. The sunshine in your eyes will wake up your brain. You have twenty to thirty minutes when you switch from unconscious to alertness. If you have overcast, it’s still okay. If it’s dark when you wake up, you will need a full-spectrum light. It will give you the sensation of being outside. Turn it on for ten minutes to a few hours. You want it within eye view – don’t just set it on the counter and walk away. When you are getting ready, set it on the bathroom counter. [15:15] Teas for Adrenal Health and Morning Energy Licorice blends and other supportive herbs. Detox teas are typically a joke. However, many detox teas have nutritive and supportive supplements. Every morning, Dr. Carries drinks green tea. Jasmine green tea is her favorite. Midmorning, Dr. Carrie drinks macha tea. [17:00] The Role Healthy Fats Play With Our Hormones Cholesterol is the backbone of all our healthy hormones. If you don’t eat fat, a lot of things can happen to your body: You won’t ovulate, so you can’t get pregnant. Your skin will break out. Your hair will be too greasy or too dry. Fat will make hormones and increase cellular health. Athletes will often not have their periods. However, it is completely destroying their hormones. Which will also make it tough on the brain, skin, and hormones. [21:35] Dr. Carrie’s Favorite Fats and Least Favorite Fats Carrie responds well to olive-oil, avocados, and fish oil. Bad fats are things like fried foods. Eating a basket of fries will not make you feel healthy. Avoid industrial seed oils. Do the best you can and take baby steps. [25:00] About DUTCH (Dried Urine Test For Comprehensive Hormones) It’s a test in which you need to urinate on paper four or times a day. The test is handy because it will alter depending on your circadian rhythm. It will test your hormones and where your hormones are going. Our hormones have to go somewhere. We can be proactive when we learn our results from this test. [31:40] Preventing Constipation Hydration is huge. Most people are underhydrated. Diet is also essential. We lack in fiber. Eat fiber and vegetables to move your stool. Everything can be done through diet and lifestyle changes. Get good sleep. When we are not sleeping well, we do not wake up repaired, and it slows our systems down. [36:10] Eating the Keto Diet Without Restricting Hormones Hormones will tank out if women strictly eat keto. Women might not be doing the diet correctly. However, doing 100% keto will affect some women. Seven days before the cycle, progesterone should be the highest. People will need flexibility with the diet whose hormones are out of whack. [41:50] Preventing Heavy Periods Heavy periods can happen because of low progesterone, iron deficiency, or low thyroid. A lot of women are not in tune with their cycle. Some women don’t know if they are releasing eggs. When we have enough progesterone, our bleeding is easy and straightforward. Some women have too much estrogen. This will result in heavy periods and lots of clots. Find out if you ovulate. Track temperatures and look at symptoms. If you want to ovulate, you need B vitamins. [47:00] Post-Menopausal Women Once the ovaries are done, estrogen and progesterone get low. Be diligent about your stress response, self-care, and sleep cycle. Women will need to be more diligent post-menopause. If you have always had the same kind of diet, start changing up your diet. AND MUCH MORE! Episode 47 is one of our favorite episodes yet. We are chatting with Tyler Jean, the guy behind instagram's @functional.foods and the creator of eatfunctionalfoods.org Tyler is an aspiring Naturopathic Doctor, who attends the National University of Natural Medicine (NUNM) in Portland, Oregon. He believes that we, as a society, are starting to move toward a more preventative and natural approach to medicine. With the rise in chronic disease and the increased patient frustration from minimal treatment options, now, more than ever, people are starting to get second opinions from alternative medicine practitioners like Naturopaths. Tyler says that while it can be easy to get caught up in the latest fads and dietary trends, when it comes to food choices, his golden principle is this: Eat nutrient-dense, unprocessed foods, that celebrate plant diversity, emphasize quality, and encourages a balanced, adequate, and varied diet. By making this simple shift, you can make a profound impact on your health and vitality. In this episode Tyler shares the one thing all diets should have in common. He also shares some of his 10 food guidelines he lives by as well as how he overcame anxiety & panic attacks [by adhering to an elimination diet for 30 days and removing gluten] and why we should all be getting back to the basics aka: cooking more from scratch. For professional Inquiries contact - Jessica Dogert, Registered Dietitian Nutritionist: jessdogert@gmail.com; or Marisa Moon, Certified Primal Health Coach: marisa@marisamoon.comDisclaimer: Consult with your doctor or functional medicine practitioner before trying any of the remedies or protocols mentioned in this episode. Your body is pretty smart… and has evolved over millennia to survive harsh conditions through feast, famine, and disease. We were built to procreate. Despite the fact that we all want to look good naked, we can't outsmart the bodies inherent drive to simply survive. So much so, that when our brain perceives stress, the last thing it wants to do is burn body fat. Rather, it will redirect all necessary resources to ensure survival… Be it lack of sleep, over exercise, under eating, finances, poor relationships, and any other actual, perceived, or anticipated stressors, all have the ability to significantly impact how our body functions. On today's show, I speak with Dr. Carrie Jones about how stressors impact our natural hormonal rhythms and why our present day lifestyles can set us up for hormonal sabotage, cause us to store body fat, decrease energy, and a plethora of other symptoms. "Everyone vilifies cortisol, but actually cortisol helps your blood sugar, is anti-inflammatory to a point and it actually helps to trigger your own T cells that have been tagged as autoimmune to be killed. So if your cortisol rhythm is dysfunctional you can actually increase your risk for autoimmunity or you can increase the autoimmune symptoms you already have." - Dr. Carrie Jones Dr. Carrie Jones is an internationally recognized speaker, consultant, and educator on the topic of women's health and hormones. Additionally we talk about our personal experience with implementing a Fasting Mimicking Diet as we both coincidentally are on day 5 (the last day, thank goodness!!) at the time of this recording. There's a plethora of research based benefits from fasting, including: Increase insulin sensitivity Decreased insulin resistance Decreased blood glucose Increased visceral fat utilization (in longer term fasting) Increased ketone production decreased autoimmunity Decreased inflammatory cytokines Decreased blood lipids (more triglycerides mobilized for fuel) Increased memory and learning Increased Brain Derived Neurotrophic Factor (BDNF): basically better brain function through brain hippocampal mitochondria production Despite all of these incredible benefits, In my opinion one of the biggest benefits of fasting is simply getting a better understanding of your body, how you feel, your hunger levels, how to manage hunger and why it's just not necessary to eat as much and as frequently as we Americans tend to. In this episode of The Dr. Hedberg Show I interview Dr. Carrie Jones of Precision Analytical on hormone balancing and testing. We cover a lot of detail about reproductive and adrenal hormones including testosterone, estrogen, progesterone, cortisol, DHEA and more. Dr. Jones explains the causes of elevated and decreased hormone levels and some strategies on balancing these issues. We also discuss the best way to test hormones including the DUTCH test which is my favorite hormone test offered by Precision Analytical. Dr. Carrie Jones, ND, MPH is an internationally recognized speaker, consultant, and educator on the topic of women’s health and hormones. She graduated from the National University of Natural Medicine (NUNM), School of Naturopathic Medicine in Portland, Oregon where she also completed her 2-year residency in women’s health, hormones and endocrinology. Later she graduated from Grand Canyon University’s Master of Public Health program with a goal of doing more international education. She was adjunct faculty for many years teaching gynecology and advanced endocrinology/fertility and has been the Medical Director for 2 large integrative clinics in Portland. She is the Medical Director for Precision Analytical, Inc, creators of the DUTCH hormone test. Dr. Hedberg: Well, welcome everyone to the Dr. Hedberg Show. This is Dr. Hedberg, and I'm excited today to have Dr. Carrie Jones, on the show. I've been listening to her and reading her material for some time now. And I've also heard her speak in person. So Dr. Jones, is a naturopath and she's definitely an expert on hormones, we'll be talking about all the different hormones today. So Dr. Jones welcome to the show. Dr. Jones: Thank you Dr. Hedberg, I appreciate you having me on, hormones is my favorite subject. Dr. Hedberg: Great. So, why don't you just fill everyone in on what you're working on these days and your area of expertise. Dr. Jones: Yeah, so like you said hormones is it. So I went to school and did my residency in all things women's health, hormones and gynecology. And expanded a little bit into men's health and hormones, just by default as women would bring the men in in their life, and they would say, "He has a hormone problem too." So I always joke and my ongoing joke is that if something's wrong with your child, don't ask me. And if you hurt yourself, like you hurt your knee, don't ask me that either. But if you're a hormonal mess I can help you with that, that's what I'm good at. Dr. Hedberg: Right. So you work for Precision Analytical which is the lab that I use for hormone testing and they do the Dutch test. So why don't we jump in and excuse me. Why don't we jump into the first hormone which is, I always like to start with start with progesterone. So why don't we start with cycling females. So when you talk a little bit about what may be some of the reasons why we would either see high or low progesterone, in a cycling female? Dr. Jones: Yeah, and actually will start with a level because I think that's well, a lot more common. So there are two reasons that a cycling female will be low. One is she does not ovulate. So when a woman releases an egg she has two sets of cells around her follicles, and they convert into a third set of cells called the lutein cells. And that's what makes progesterone. So if she doesn't release the eggs, then she's not gonna get that little conversion and she's not going to make progesterone. Now the other reason the second big reason is she may ovulate. She might release an egg, she feels it, she notices it, her mucus changes what have you. She does the you know, ovulation predictor kit from the grocery stores, it's positive. And still her progesterone might be pretty low, pretty weak. And then the reason for that is that those cells that are supposed to make progesterone, they themselves are weak. And so we have to do something to pump up the cells.
Dr. Carrie Jones is a naturopathic doctor and a medical director for Precision Analytical, which is the company that handles all the dutch hormone testing that is run for women in the hormone project and through my practice. Listen to this episode to unwrap knowledge about the work of hormones in your body, stress connection, estrogen and androgen dominance and the impact on your thyroid. Take your time to absorb and digest this information, and if a lot of what is said today resonates with you, you should take a look at the Hormone Project starting in January. Why are hormones so critical to our overall health? Hormones are made in glands, they basically give the message to other glands in your body to do or not to do something, so when there is a overproduction or, on the contrary, not enough hormones, the symptoms start to show. It’s all about balance because hormones are literally the chemical messengers that tell your body what to do and help you feel better. There needs to be an appropriate amount of messages and they need to be received by the right gland! “My test results are fine… but I don't feel well.” Usually lab test cover the basic aspects of your health, they go over functions and signals just to make sure you are not going to die. Short doctor’s consultations don't help either! But you can ask for more, go deeper than a simple blood test! Ask for other lab tests and sometimes you even have to consider changing doctors. How DUTCH Panel testing helps to describe what is going on in our bodies? It is an easy to collect urine test where you get all the traditional hormone results, but also all the pathways that these hormones are travelling (which blood, saliva or stool lab tests don’t show). The Estrogen Path Your body makes estrogen and when this production is over, the estrogen needs to be broken down and excreted. In the first stage of this transit, estrogen can be directed through one of three differents paths called: 2, 4 and 16. Number 2 is the healthy path, 4 can potentially increase your risk for cancer and 16 is called prolific. In the second stage your body has to neutralize the estrogen, this phase is called the detox stage. These two phases need to be addressed differently. Estrogen Dominance As a woman, your estrogen rises right before ovulation, and then it goes down for later rising back up again before the following ovulation. The second time it goes up, your progesterone needs to be high and healthy. But if you have low progesterone production, your estrogen will be unattended and run wild, which translates into heavy periods, tender breasts, mood changes...and it is called estrogen dominance (since progesterone is not enough). Sleep, Sleep, Sleep During REM sleep you dream and during deep sleep your body heals! Cortisol goes down, you lose weight, men produce more testosterone. Just sleeping makes you healthier! Androgen Dominance You can either make an excess of testosterone that stimulates androgen receptors, causing symptoms such as greasy skin, cystic acne, chin hair...or you can be making normal amounts of testosterone but going in an androgenic pathway, creating imbalance. To restore your hormonal balance, start by slowing down, insulin has to be kept low, lift weight, check your diet, have the right supplements, include more Zinc in your diet. Check your Thyroid Are you converting T4 to T3 well? Your TSH might look ok if your pituitary is fine, but you can have a thyroid problem, running a TSH, T3 and T4. T3 can be deactivated to reverse T3 (RT3). You have to investigate the whole pathway, dig deeper, ask the right questions! I encourage you to invest your time and make the right choices to heal your hormones and restore their balance, hoping that this information has contributed to get you closer to your health goals. Ever been told, after getting a hormone test back - whether it was testosterone, estrogen, thyroid hormones, cortisol or anything else, that everything's 'normal' and 'you're fine?' - yet you feel totally NOT FINE? In this week's show I chat to the wonderful Dr Carrie Jones who makes the trickiest of hormone challenges (male or female) feel like a fun puzzle to get to the bottom of - and I'm serious. Imagine turning the talk of hormone woes on its head and making it instead an interesting fun thing to figure out - it was this attitude of Carrie's from following her instagram feed and the generous sharing she does there, that said: I have to have her on the show and shake up the hormone conversation, that's always such a Debbie downer! Carrie is a Naturopathic Physician with a Master's in Public Health having over 12 years in the field of functional and integrative medicine. As Adjunct Faculty for the National University of Natural Medicine (NUNM), she has taught courses in both Gynaecology and Advanced Endocrinology. Today we’re excited to welcome soon-to-be Holistic Nutritionist and Instagram celeb, Jasmine Galimov to the show! Jasmine’s Instagram, “@Berryfitt” is extremely popular and showcases her beautiful food creations which are not only vibrant, but nutrient packed. Jasmine describes herself as a “huge foodie” and that her passion for food is what made her Instagram so successful. She demonstrates how eating healthy can be easy and when it looks pretty, people are going to want to eat it! Jasmine’s Tips on Perfectly Roasted Veggies: Preheat oven to 375 degrees Lay down a piece of parchment paper for easy clean up Put veggies on baking sheet and drizzle them with avocado oil (with high heats you want to stay clear of olive oil so you can maintain the nutrients and structure of the oil) Sprinkle veggies with Himalayan salt and roast for 10-15 minutes Flip and bake for another 10 minutes and ENJOY! Jasmine says, “My eating philosophy is to eat what my body needs.” Jasmine got her undergraduate degree from the University of Colorado in Boulder, majoring in Communications with a Business and Philosophy minor. She realized after graduating that she still had no clue what she wanted to do. After taking a year off for some soul searching, she discovered that it had been in front of her the entire time. Jasmine explains that she’d spent her whole life consumed with searching for alternative, holistic remedies and she found her passion for nutrition and health through her own journey of learning about her body. Knowing she wanted to go down the holistic nutrition route rather than becoming a dietician, Jasmine was thrilled to find the National University of Natural Medicine (NUNM) here in Portland, Oregon and relocated immediately to attend the program. Growing up, Jasmine remembers eating healthier than all her friends because her mom felt so helpless about her young daughter’s health issues. After years of being extremely sick, Jasmine was diagnosed with celiac disease in middle school. It was at this time that Jasmine and her family shifted to a healthier way of eating and lifestyle. Jasmine says her mom has continued to “nerd out” on holistic health and since then each of her family members has done their own exploration to find what best fits their individual body. Jasmine says one of the most powerful principles she’s learned is that food is medicine. She says, “When finding what foods work for your body we hold all the power to heal ourselves in our kitchens.” Why Shouldn't We Wait Until the New Year to Get Healthy? Jasmine explains that the mentality of needing to ‘get it out of your system’ while binging on unhealthy foods before getting healthy creates a horrible habit for people. This mindset makes it more likely that when we do slip up or enjoy certain foods that it can quickly take us down a binging spiral because we tell ourselves we’ll just start on Monday, or next week, or even next year. Jasmine says, “Every day is a new day and a new opportunity to be focused on our health and to take care of yourself.” Tips to Bounce Back: Take 5-30 minutes each day away from your TV or social media to go for a walk, grocery shop or to cook healthy meals. Christa and her husband like to do a three-day fast for a full mind, body reset whenever they’ve escaped their normal routine. Jasmine tries to do what she can for herself, when she can, by being mindful, aware and present. Whatever negative statement comes into your mind, reframe it and tell yourself the opposite message. Christa says, “Even if you don’t believe it initially, like telling yourself you have more than enough time in the day to cook and that you enjoy it. Just keep repeating it to yourself because ultimately your brain is going to create a new thought pattern and that new thought pattern is going to guide you in your day-to-day to go and do those things.” Set good boundaries by recognizing how you show up for others and make it a focus to show up for yourself too. Simplify by enjoying a whole foods' diet so you don’t feel restricted or deprived. If you’re finding that you still mainly crave processed foods, chips and cupcakes when you’re trying to eat intuitively, then a program designed to help you remove these types of foods is necessary to help you get out of that addictive cycle. Christa is offering her 30 Days to Thriving program starting again in January to help you fight the hard battle against our reliance on refined carbs, sugar and alcohol. Make sure to sign up today if you’re ready to meet your healthiest YOU, while fighting against diseases and inflammation. Also, check out this past episode about redefining what it means for us to “diet”. Jasmine encourages the listeners not miss out on upcoming holiday events and family functions but to simply do what you can to take care of yourself. She personally likes to meditate for five minutes when she wakes up in the morning to set herself up for a successful day. Jasmine also pre- her meals before going to an event so she’s not as tempted by unhealthy goodies. Lastly, she stays focused on gratitude; specifically how lucky she is to be celebrating and enjoying time with the people she loves. Being present and enjoying the moment feels good and fires up the brain’s reward center which helps us make healthier, happier choices. Give us a five star review on iTunes and Stitcher and tell us your tips for staying healthy this holiday season!

Try a Free Sample of 180 Superfood Protein Blend: http://180nutrition.com.au/free-sample And this week our wonderful guest is Dr. Allison Siebecker. Now Allison has worked in the nutritional field since 1988, and is a 2005 graduate of the National University of Natural Medicine, where she got earned her doctorate in Naturopathic Medicine, and her Masters in Oriental Medicine. Boy, is Allison a wealth of information. I, again, biasedly, love this episode. This topic is actually a bit close to my heart, and after listening to what Allison had to say, today, I'll be going to get a SIBO Test. If you have no idea what SIBO is, don't worry about it. I still think it's a really relevant topic for us all, to at least be aware of, and especially if you're having symptoms of bloating, in the gut, all the time. Symptoms of IBS, and things like anxiety, as well, and depression, can even stem from SIBO, which is just fascinating. It is Small Intestinal Bacterial Overgrowth. [00:01:30] Anyway, we cover all areas today. We wanted to make this episode relevant for the listener, as well as maybe, the practitioner, that wants to learn more about SIBO, as well. I'm going to get a test and if you do fancy getting a test done, yourself, on SIBO, just drop us an email through 180, and we can help direct you in the right direction, if you choose to do so. And you probably will. [00:02:00] Anyway, the other thing I wanted to mention, as well: You might've heard me talking about giving away free samples of our 180 Superfood. Where you can just have a smoothie in the morning, get a complete natural protein, wholefood hit. Replaces poor meal choices, it's a really quick and convenient way of doing so, which we love. With the free samples, plus shipping, I'm trying something different today, so what I didn't realize, after podcasting for 4 and a half years, is that, if you're actually listening to this podcast right now, on your phone, you can actually go to where it's playing. Just below the play button, on the podcast app, there's a 'Show Notes' button. If you click that, you can, within 3 seconds, click the link that directs you to free samples, and, like I said, all you have to do is pay a small shipping and handling fee. If you want to try them, and you haven't yet, I suggest you do so. That is the easiest way to do it. Well there you go, eh. You learn something new every day. [00:02:30] Anyway, lets go over to Allison. I love this episode. Enjoy. Hi, this Guy, I'm joined with Stu, as always. Good morning, Stewart. Stu Ow' yah goin'? How are you? Guy And our lovely guest, today, is Dr. Allison Siebecker. Allison, welcome to the show. Dr. Allison Thank you. Guy Well, what a topic. This is a topic, we've not covered in depth before. I tell you what, I am pumped for this episode, today. I truly am. Dr. Allison (laughs) Guy [00:03:00] Now, we ask a question to every guest that comes on the show. That is, "If a complete stranger stopped you on the street, and asked you what you did for a living, what would you say?" Dr. Allison I'd probably say, "I'm a doctor." But you know, that's a very simple answer, but these days I do a lot more teaching. So maybe I should say I'm a teacher. Or maybe I should say, "I'm a doctor that teaches."
