Get your FREE Sample of VMG (US listeners Only) https://theessentialoilscoop.com/vmgsamples Scoop of the Day Bath Salt Recipe Ingredients: 1 cup Epsom salt (soothes muscles and detoxifies) ½ cup sea salt or Himalayan pink salt (adds minerals) ½ cup baking soda (softens skin and neutralizes acids) 10–15 drops of your favorite essential oils Instructions: In a bowl, mix the Epsom salt, sea salt, and baking soda. Add essential oils and stir well to distribute evenly. If using dried herbs, mix them in. Store in an airtight glass jar. Use about ½ to 1 cup per bath. DIY Essential Oil Bath Bombs Ingredients: 1 cup baking soda (softens skin, fizzes) ½ cup citric acid (creates the fizz) ½ cup Epsom salt (relaxes muscles) ½ cup cornstarch (helps harden the bath bomb) 2½ tbsp coconut oil (or another carrier oil, adds moisture) ¾ tbsp water 10–15 drops essential oils (choose based on your needs) Instructions: In a large bowl, whisk together baking soda, citric acid, Epsom salt, and cornstarch. In a separate small bowl, mix melted coconut oil, water, essential oils, and food coloring. Slowly pour the wet ingredients into the dry mixture, whisking continuously to prevent fizzing. The mixture should feel like damp sand and hold its shape when squeezed. Pack the mixture firmly into bath bomb molds, pressing both halves together tightly. Let the bath bombs dry in the mold for at least 24 hours before removing. Store in an airtight container and use within a few months. DIY Nourishing Bath Oil Ingredients: ½ cup carrier oil (We use Fractionated Coconut Oil) 10–20 drops essential oils Instructions: In a small glass bottle, pour your carrier oil(s). Add essential oils and shake gently to mix. If using, add vitamin E oil for extra skin benefits. Store in a dark glass bottle to preserve the potency of the essential oils. Contributor: Meghan Hurley, MD Educational Pearls: Gastroenteritis clinical diagnoses: Diarrhea with or without vomiting and fever Vomiting in the absence of diarrhea has a large list of differential diagnoses, so the combination of diarrhea and vomiting in a patient is helpful to indicate the gastroenteritis diagnosis Symptom timeline is usually 1-3 days, but can last up to 14 days – diarrhea persists the longest Treatment for mild to moderate dehydration: oral or IV rehydration Begin orally to avoid unnecessary IV in a pediatric patient Administer ODT Ondansetron (Zofran) to prevent vomiting Meta-analysis showed that 2-8 mg orally, based on body weight, decreased vomiting quickly Wait 15-20 minutes for the medication to take effect Use streamlined method for oral rehydration: Fluids such as over-the-counter Pedialyte, Infalyte, Rehydrate, Resol, and Naturalyte may be used If patient weighs less than 10kg: administer 5mL of fluid per minute for 20 minutes If patient weighs 10kg or more: administer 10mL of fluid for 20 minutes If the patient can keep the fluid down, double the fluid volume and repeat If the patient once again keeps the fluid down, double the fluid volume and repeat If successful with each attempt, the patient may be discharged home Can prescribe ODT Zofran for 1-2 days at home If the patient vomits more than once during this oral rehydration process, intravenous rehydration must be initiated References Churgay CA, Aftab Z. EMS A to Z: Post-Resuscitation Care Show Notes: From your hosts, Dr. Josh Gaither, Dr. Amber Rice, and Dr. Rachel Munn What is the best next step to take once identifying a cardiac arrest patient has ROSC? Hemodynamic support: Patients with ROSC are typically in “shock” with unstable vital signs, hypotension, hypoxia, etc. Obtaining vitals and identifying how to support the patient's hemodynamics is key. IV fluids Dopamine: Dosing range is 5 – 20mcg/kg/min --> There are a number of ways taught to dose dopamine (the clock method, etc.). As an example, if you choose to start your dose at 10mcg/kg/min, in a 80kg person that's 800mcg/min. If your concentration is 1600mcg/mL (standard), then that's 0.5mL/min or 30 drops / min if you're using 60cc tubing. Dopamine is incredibly titratable. Keeping 30 drops a minute (1 drop every 2 seconds) in the back of your mind is a great starting point for *most* patients, and you can increase / decrease based on BP from there. The key is starting dopamine early! Most of these patients will still have a little epi circulating and can crash precipitously when that wears off if another vasopressor hasn't been started. What are other considerations for evaluation / treatment? ECG: A post-ROSC ECG can help identify a STEMI, which may change your transport destination. Airway: A functional iGel / SGA can stay in place. Support respirations with BVM in sync with patient's breathing to the best of your ability. ETCO2: Monitoring ETCO2 can clue you in to an airway displacement, or hemodynamic compromise indicating impending re-arrest. CAUSES: Of course we're thinking about causes during the arrest, but don't forget to consider things like hypothermia, renal failure / hyperkalemia, etc. That treatment could be initiated in the prehospital setting. Interview with Chris Reed, CEO of Neometals (ASX:NMT), and Accompanied by Darren Townsend, Chief Development Officer, and David Robinson, General Manager.Neometals' management team's track record of success extends to the Mt. Marion Lithium Project, where they timed their exit to a tee, selling their 13.8% stake for AU$103.8m. We've interviewed them several times. Each time, they've described a project that fits neatly into a green, profitable narrative. It's a cookie-cutter approach of finding value that's gone under the radar, acquiring it for a steal, and developing it efficiently. We're amazed the market hasn't clocked the potential of Neometals yet, with their market cap valued at less than the cash they currently hold, irrespective of their project portfolio.We've previously questioned Neometals' marketing strategy; however, today we're questioning its latest in a long line of green projects. As of last week, Neometals has entered an agreement with unlisted (but public) Scandinavian mineral development company, Critical Metals, to consider developing a recycling facility to recover and process high- grade vanadium products from vanadium-bearing steel by-product in Scandinavia. The green component has already been ticked off! The by-product is slag, not tailings, though we'll get into that in a moment.So, what exactly does the agreement entail? Reed states that what Neometals believes it has acquired is a "call option to acquire what is the world's largest stockpile of high-grade vanadium slags." The 27-month consideration period will allow Neometals to conduct its extensive dude diligence on the potential vanadium plant, which will result in a feasibility study to be completed by 31st December 2022. At this point, a final investment decision will be made. If Neometals agrees to go ahead, the company will enter into a 50/50 arrangement, with the certainty of a supply agreement from SSAP, the largest fuel maker in Scandinavia. Total CAPEX prior to FID will be A$5ML: a figure Neometals can easily afford, and one that fits into the project generator narrative the company has been pushing.Slag vs tailings? Tailings is the waste rock at a mine after the raw ore has been processed to remove the metal, whereas slag is a glass-like by-product left over after a desired metal has been smelted from its raw ore. The process of dealing with the slag is not only immensely economic but is also a remediation process, as the glass itself will change colour and no longer be a sea-adjacent eyesore.What is the upside in terms of revenue? The steel slag with vanadium content has a standard benchmark vanadium grade of 3.9% V2O5, 2-3X higher than any producer can make in a concentrate before a similar level of processing. There is no mining cost, no crushing plant, no grinding plant, no magnetic gravity separation plant, and no kilns for the traditional salt-roast-leach process. 50% of the CAPEX had been wiped out straight away. Moreover, while vanadium is a volatile commodity that hasn't had its best few years, the macro story, including fundamental demand for stainless steel, and the potential growth of VRFBs, still places vanadium as a promising commodity.Once the process is completed, what happens to the inner pile of largely calcium/iron-bearing residues? Reed states Neometals will take ownership and either safely dispose of it, or reinsert it back into the front end of the steelmaking process. The green thematic is consistent throughout Neometals' operations.Reed actually sees COVID-19 as the element that forced Neometals and Critical Metals to get the deal over the line when it did but also says it will have little impact on operations, especially considering mining is considered an essential service, and the population density of Australia is low. Narcan and Synthetic Opioids: vive la résistance? Probably not. Read this absolutely brilliant piece from The Tox & The Hound here. (They did all the hard work and we stole their sources.) Opioid "resistance" to naloxone is most likely not a thing, per se. The reported effect from synthetic and novel opioids are unlikely to be due to the agent's binding affinity for receptors within the brain, but rather from an ability to rapidly permeate the blood brain barrier much faster than "traditional" opioids such as heroin. Even in cases where a synthetic opioid agent was identified, the vast majority of cases did not need more than 4mg of naloxone to achieve reversal. Synthetic opioids don't bind any more "tightly" to receptors than naloxone. Synthetic opioids will usually cross the blood brain barrier faster than traditionally encountered agents. Most available evidence shows that synthetic opioid toxicity does not require significantly more naloxone to achieve clinically significant effect. Ergo, the traditional serial naloxone dosing algorithm does not need much modification. 0.04mg -> 0.4mg -> 2mg -> 4mg -> 8mg -> 10mg Not all that "overdoses" is an opioid. Consider all other causes of altered mental status or coma. Namely: hypothermia, hypoxia, and hypercarbia. Acidosis may potentiate the effect of opioids, highlighting the demand for timely and effective ventilation. Polypharmacy or adulteration is increasingly common. Consider intoxication by additional agents. Anchoring bias is a dangerous phenomenon: don't get burned! The Nose Knows. Or does it? Intranasal (IN) naloxone is popular among many EMS agencies as well as law enforcement, fire departments, and bystanders. IN naloxone has been shown to be effective in several randomized controlled trials for successful reversal of opioid intoxication. However... There are important pitfalls to be cognizant of when choosing this option for delivering naloxone. Intranasal naloxone has poor bioavailability when compared to IV or IM dosing, so higher doses may be required to achieve clinical effect. This is further potentiated by the maximum volume able to be absorbed by the nasal mucosa (around 0.5mL). Patients administered intranasal naloxone may have a variable or delayed response in achieving reversal. Protect Ya Neck Standard isolation precautions are adequate protection against the overwhelming majority of overdose scenes. In the rare instance where respiratory or splash exposure is a concern, a properly fitted N95 mask and goggles will be sufficient. To date, there has yet to be a laboratory confirmed case where a first responder or emergency healthcare provider has suffered a clinically significant opioid intoxication (bradypnea, hypoxia ) as the result of an occupational exposure to fentanyl or its analogues.
