Podcast Show Notes: Scoliosis Treatment with Dr. Tony Nalda Episode Title: Understanding Thoracic Lordosis – What Happens When the Mid-Back Bends the Wrong Way? Episode Summary: Most people know the spine has curves—but not all spinal curves are good. In this episode of Scoliosis Treatment with Dr. Tony Nalda, we explore thoracic lordosis, an abnormal forward-bending of the mid-back. Dr. Nalda explains how this condition differs from normal spinal alignment, why it often goes hand-in-hand with scoliosis, and what you can do to restore proper spinal shape before symptoms progress. What You'll Learn in This Episode: ✅ What Is Thoracic Lordosis? Normally, the thoracic (mid-back) spine should curve backward—a shape called kyphosis Thoracic lordosis occurs when the curve bends forward instead, disrupting spinal mechanics ✅ Why It Matters Thoracic lordosis weakens the spine's ability to handle stress It can contribute to: Progressive scoliosis Neck curvature issues (cervical kyphosis) Lumbar spine instability (spondylolisthesis) Chronic back pain, poor posture, and mobility limitations ✅ What Causes It? Postural habits Congenital defects Post-surgical changes (especially scoliosis surgery with rods) Neuromuscular conditions ✅ Connection to Scoliosis A flat or reversed mid-back curve is often seen in scoliosis cases Patients with flatter thoracic spines are more likely to progress rapidly during growth The rotational nature of scoliosis can flatten or invert normal curves, worsening spinal alignment ✅ How to Fix It You can't "watch and wait" on structural conditions Treatment must target the cause and curve direction At the Scoliosis Reduction Center, Dr. Nalda and his team use: Customized in-office care Targeted therapies to restore curve balance Home rehab programs for long-term stability Key Takeaway: Thoracic lordosis isn't “just posture.” It's a structural issue that can cascade into scoliosis, nerve issues, and even spinal instability. But with proper diagnosis and proactive care, the spine can be reshaped and protected from further damage. Resources & Links:

Acne scars aren't just a skin issue—they impact confidence, self-esteem, and how we show up in the world. In this episode, Dr. Mina is joined by internationally recognized dermatologist Dr. Mehrnoosh Heidarian to unpack the full scope of acne scarring: from its emotional toll to the latest, evidence-based treatment options. They break down the different types of scars (boxcar, ice pick, rolling, keloids), why early intervention matters, and how personalized treatment plans like subcision, laser-assisted techniques, and microneedling can make all the difference. You'll also hear how social media is shaping skincare myths, the truth about collagen-boosting treatments, and what it really takes to heal acne scars from the inside out. Key Takeaways - Acne scarring has a significant emotional impact on patients. - 95% of acne patients may develop scars at some point. - Different types of acne scars require tailored treatment approaches. - Subcision is a minimally invasive procedure for treating scars. - Laser-assisted techniques can enhance treatment outcomes. - Emotional well-being is a crucial aspect of acne scar treatment. - Customized treatment plans are essential for effective results. - Combining various treatment modalities can yield better outcomes. - Understanding the patient's skin type is vital for treatment success. - The goal of treatment is to make scars as invisible as possible. Expect significant improvement, not a 100% cure for acne scars. - Ice pick scars require specific treatment techniques like cross technique. - Precision is crucial when treating darker skin types. - Microneedling is effective, but radiofrequency microneedling is preferred. - Hypertrophic and keloid scars need careful management and follow-up. - Post-treatment care is essential for optimal healing. - Trust the process and follow your dermatologist's advice. - Acne scar treatment is a journey that takes time. - Social media can spread misinformation about skincare. - Professional guidance is necessary to set realistic expectations. Dr. Mehrnoosh Heidarian, MD Board-Certified Dermatologist | Acne Scar Specialist Dr. Mehrnoosh Heidarian is a board-certified dermatologist with over 20 years of experience in cosmetic dermatology, specializing in acne scar treatment. Customized skincare is essential for effective acne treatment. Stress management is vital for overall health and skin clarity. Watch on YouTube https://youtu.be/Hv7zOyux71E Episode Transcript Cheryl McColgan (00:01)Hello everyone, welcome to the Heal Nourish Grow podcast. Today I am joined by Meg Gage. I hope I got that right, it's spelled a little differently. And she has a really interesting health history. She's a holistic nutritionist. She specializes actually in acne, I guess, prevention and treatment. And as you can see by her skin, I mean, it looks amazing here if you're watching on video. So if you can't see it, maybe pop over to YouTube. Meg (00:08)You did. Cheryl McColgan (00:30)But it wasn't always this way Meg, right? So I'd love it if you could just start by sharing a little bit of your history, your health journey, and then we'll eventually get into how you came to figure out like how to get rid of acne. So welcome. Meg (00:45)Thank you. Yeah, it's quite the journey. feel anyone who becomes a healer always has quite their own story as to how they got there. But yeah, ever since I was five, I just dealt with multiple traumas and that kind of stress of living in prolonged kind of fight flight breathe since I was little showed up very physically for me in a lot of symptoms, one of which was long standing acne. since I was 12 and others were chronic pain, chronic fatigue, a lot of misdiagnosed disease of like fibromyalgia and chronic fatigue syndrome. All the while at that point when I was 12 being put on hormonal birth control without any kind of questions asked, that's just what we did back in those days. So, you without having many cycles of my own, I was just put on these synthetic hormones as well. And kind of throughout dealing with major health issues over the years and a couple of car accidents and whiplash and PTSD and concussions, my acne was always such a consistent thing in the background. And when I came off hormonal birth control without any prep work or understanding that after a decade, this might have crazy impacts on my hormones when they try to kind of rebalance themselves, my acne exploded into very cystic hormonal me all over my face, all over my back, all over my bum, very painful. And it really just started my quest in kind of the natural healing world. And for the next five years, I tried everything from, you know, naturopaths to nutritionists to Chinese medicine practitioners to herbalists to acupuncture, all in a quest kind of to heal my skin and nothing worked. And it was a very frustrating journey. I was spending upwards of $30,000 on this. It felt like a full-time job, distress when you're kind of putting 100 % of your effort into healing something for it not to work. And then just seeing so many practitioners that were telling you so many, so many varying things. And I just… got to a point where I was like, need to do this myself.

I need to tell you about Brenda. When she first came to me, she was done. Exhausted, burned out, frustrated. She felt like she was living in a body she didn't recognize anymore — and no matter how “healthy” she tried to be, nothing was working. ✔️ Her hair was thinning. ✔️ Her weight was climbing. ✔️ She couldn't sleep. ✔️ Her energy was nonexistent. ✔️ Her cycles were all over the place. And she kept thinking, “Maybe it's just stress… maybe it'll pass.” But it didn't. What Brenda didn't realize was that years of unaddressed hormone shifts — from birth control to motherhood to relentless career demands to a devastating divorce — had sent her system into survival mode. Her cortisol was sky-high, her estrogen detox pathways were struggling, and her body was holding on to everything because it didn't feel safe. Here's what changed everything for her: We tested first. Not at the end of the program. Not as an afterthought. Right up front. We used a DUTCH hormone test to see exactly what was happening inside her body — what her cortisol patterns looked like, how she was metabolizing estrogen, and whether her body was trying to keep up or completely tapped out. Then we built a customized plan specifically for her body. No cookie-cutter protocols, no generic advice. ✅ Nutrition tailored to her cycle. ✅ Targeted, effective supplements. ✅ Stress management strategies for real life. ✅ And ongoing testing to track real, measurable progress. And today? Brenda is thriving. ✔️ Her energy is back. ✔️ Her cycles are regular. ✔️ She's sleeping through the night. ✔️ The stubborn weight has started to release. ✔️ And, most importantly, she feels like herself again. I share Brenda's story because I know so many women are walking around right now in burnout mode, thinking this is just how it is. It doesn't have to be.

Podcast Show Notes: Scoliosis Treatment with Dr. Tony Nalda Episode Title: Avoiding Surgery – Bianca's Scoliosis Success Story Episode Summary: When Bianca was diagnosed with scoliosis at a young age, she was initially told that surgery was her only option. With a 65-degree spinal curve, multiple doctors recommended spinal fusion. However, through intensive scoliosis care and dedicated home exercises, Bianca was able to reduce her curve to 45 degrees, avoiding invasive surgery altogether. In this inspiring episode of Scoliosis Treatment with Dr. Tony Nalda, Bianca shares her personal journey—how scoliosis affected her daily life, the difficult decision to reject surgery, and how conservative scoliosis treatment helped her take control of her health. Dr. Nalda also explains why scoliosis is not just an injury, but a developmental condition that requires specialized treatment beyond traditional chiropractic care. Key Topics Covered: ✅ Early Scoliosis Detection – How school screenings helped Bianca catch her scoliosis early. ✅ The Traditional Approach to Severe Scoliosis (Surgery Recommendation): Orthopedic surgeons recommend surgery for curves over 45 degrees, especially in growing children. Spinal fusion can lead to stiffness and chronic pain, affecting quality of life. ✅ Why Conservative Scoliosis Treatment Works: Intensive therapy can halt progression and reduce the curve naturally. Customized exercises, chiropractic care, and rehabilitation helped Bianca improve her spine alignment. ✅ Avoiding Surgery with Hard Work & Commitment: Bianca's curve reduced from 65 to 45 degrees through consistent treatment. Daily 30-minute home exercises helped maintain and further improve her progress. ✅ The Role of Lifestyle & Posture in Scoliosis Progression: Playing the banjo put asymmetrical weight on her spine, potentially influencing her curve. Understanding postural habits can help prevent worsening over time. ✅ Bianca's Passion for Music & Overcoming Challenges: Bianca and her sister have turned their struggles into music, releasing an album titled "Growing Wings". Their song "Limitless" is about pushing past obstacles and achieving goals—a message that resonates with anyone facing challenges like scoliosis. Key Takeaways: ➡️ Scoliosis is NOT just an injury—it's a progressive, developmental condition that requires specialized treatment. ➡️ Surgery is not the only option, even for severe cases—dedicated scoliosis care can significantly reduce curves. ➡️ Early intervention leads to better results, but even larger curves can be improved with the right approach. ➡️ Hard work now means better long-term health—Bianca chose a proactive approach over a quick fix and is healthier because of it. The amazing people we talked to in the Ivoclar (https://www.ivoclar.com/en_us) Ballroom during the 40th anniversary of LMT Lab Day Chicago just keep coming and this week we have a stellar line up. First up is Eric Kukucka. Eric came on the podcast years ago to talk about his multiple denturist practices in Canada. But he has since sold them and has joined the Aspen Dental (https://www.aspendental.com/) group as their Vice President of Clinical Removable Prosthetics & Design Technologies. Eric talks about going from making dentures for hundreds of patients to working with a team to produce quality dentures for millions. Then we chat with Chelsea Homire, who has years of dental assistant experience, but when given the oppritunity to learn the in-office lab work, she took off. With a passion for printing, Chelsea is now teaching other offices (and even a few labs) how they can optimize their digital workflow. We wrap up the episode with Brittany Mitchell. You might know her becuase she designed (and made the denture) on our "Just Say No to Brushing" shirt, but she's also a talented removable technician. Brittany talks about starting in at the front desk, drawing a picture that got notices and placed in the lab, falling in love with it, and making digital dentures for a 12 doctor dental group. Don't miss this incredible webinar with Tanya Little, a dentist from beautiful Vancouver, Canada. We would like to invite you to a special webinar that she's hosting with Ivoclar (https://www.ivoclar.com/en_us) and LMT (https://lmtmag.com/) on Wednesday, May 7th, titled "Reflections from a Denturist: 20 Years of Crafting Dentures!". In this program, she will review the incredible history of removables with Ivoclar and focus on the new and exciting evolution based print resin and of course, the PM7. Her goal is to provide you with several key takeaways that will allow you to work smarter and not harder. You can join on May 7th by registering at https://lmtmag.com/webinars/reflections-from-a-denturist-20-years-of-crafting-dentures Are you a dental lab in need of more talent to improve your bottom line and keep production on schedule? Burnout is affecting everyone in farm credit, from individual contributors and managers to senior leaders. The recent FCCS Employee Engagement survey revealed that while everyone is feeling burnout, the why behind burnout varies based on each individual association This episode of the Forward Thinking Podcast features FCCS VP of Marketing and Communications Stephanie Barton and FCCS Organizational Development Consultant Angie Coleman unpack the data from this survey, define what burnout looks like across different roles, and explore what organizations can do to combat burnout, improve employee wellbeing, and drive engagement.    Episode Insights Include: The FCCS Employee Engagement survey  The survey highlights trends by region and size.  The survey will be conducted annually.  2024 survey findings included a national and global decrease in employee engagement and an increase in management satisfaction.  Burnout is continuing to increase, despite distance from the pandemic.    Burnout at every employment level Middle management is most negatively affected by burnout.  Senior leaders feel burnout the least, possibly because of their level of control, but also because they understand the ‘why' behind change.  Communications breakdown is a major player behind burnout.  Leaders rarely overcommunicate with their teams.  There is no clear correlation between burnout and company size, region, weather, or challenges.   The manifestation of burnout Middle managers are carrying the heaviest emotional load as they support both upper management and individual employees.  Burnout needs to be addressed from an emotional labor standpoint as well as a task-oriented perspective.  Leaders can ask themselves what specifically they are worrying about with their teams. If everything is a crisis, then employees can't get excited about anything.    A customized approach to burnout The why behind burnout shows that a generalized approach to combatting burnout will not be effective.  Customized training, targeted strategies and one-on-one meetings are essential to productive change.  Managers can note specific employee concerns and address then at each one-on-one meeting.    Successful burnout combat strategies Communicate survey results to the entire employee population at the same time in the same way.  Set clear bullet points for leaders to address in one-on-ones and team meetings to enhance purpose behind communication.  Consider if you are taking things off employees plates as often as you are putting things on.  Leaders can use the findings from the surveys to connect with managers and employees about specific questions in one-on-one meetings.    This podcast is powered by FCCS.   Resources Connect with Angie Coleman — Angie Coleman   Get in touch info@fccsconsulting.com   “If you [as a leader] feel like you're tired of communicating, you're probably just scratching the surface on what your employees need in terms of their level of communication.” — Angie Coleman   “While everyone is feeling burnout, the why is completely up to the individual association and what's happening in that space.” — Angie Coleman   “It's helpful not to just approach burnout from a task-oriented perspective, but also recognizing how we can be supportive of that from an emotional labor standpoint.” — Angie Coleman   “If everything is a crisis, then really nothing is.” — Angie Coleman “It is critical for leaders to ask the right questions so they can meet employees where they are.” — Angie Coleman

Episode 186: Exercise PrescriptionsDr. Sandhu and future Dr. Daoud explain the way to prescribe exercise, what are the general guidelines for exercise and how to overcome barriers to exercise. Dr. Arreaza emphasized the importance to screen our patients before exercise and using the term “physical activity” to improve receptivity by patients.  Written by Wessam Daoud, MSIV, Ross University School of Medicine. Edits and comments by Ranbir Sandhu, MD, and Hector Arreaza, MD.  You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.Arreaza: I'm Dr. Arreaza, and today, we will talk about a topic that is both simple and powerful: exercise. Previous episodes: 158, 100 (sexercise), 95, We all know exercise is good for us, but how do we prescribe it like we do medications? How can we tailor exercise recommendations to our patients' needs and lifestyles? To help us unpack this, I'm joined today by Dr. Ranbir Sandhu, and Medical Student, Wessam Daoud, who has a passion for preventive medicine. Welcome to the show!Ranbir: Thanks, Dr. Arreaza! We're excited to be here and to discuss something so fundamental to health.Segment 1: Understanding Exercise PrescriptionArreaza: Let's start with the basics. In medicine, we prescribe medications with precise instructions—dosage, frequency, duration. But how do we apply this concept to exercise?Ranbir: Great question! Before we prescribe exercise, we have to make sure that it is not contraindicated. We can use a system to stratify our patients based on risk factors, such as older age, smoking, baseline level of activity, etc. For example, a patient who had a heart attack within the last 6 weeks should not exercise yet, a person with heart failure exacerbation, asthma exacerbation, uncontrolled heart arrhythmia, etc. Wes: Exercise prescription follows a structured approach, similar to medications. We use the FITTE mnemonics to guide recommendations: Frequency – How often?Intensity – How hard should the patient work?Time – How long should each session last?Type – What kind of exercise is best?Enjoyment – Does the patient enjoy this activity?By adjusting these components, we can tailor exercise to each patient's needs, whether it's improving cardiovascular health, managing chronic disease, or building strength.Segment 2: How Much Exercise Do Adults Need?Arreaza: Now, when we talk about exercise, there's a lot of conflicting advice out there. What do the official guidelines say about how much adults should exercise?Ranbir: The American College of Sports Medicine (ACSM) and CDC provide clear guidelines:Aerobic Exercise: At least 150 to 300 minutes of moderate-intensity exercise per week, OR 75 to 150 minutes of vigorous-intensity exercise (or a mix of both).Muscle Strengthening: At least two days per week of resistance training targeting major muscle groups.Balance & Flexibility: Particularly important for older adults to reduce fall risk. These guidelines are adaptable, meaning patients can break them into shorter sessions throughout the week.Arreaza: For weight regain, you may need to exercise a little bit more, about 300 minutes/week, and >2 days of resistance activity.Segment 3: Choosing the Right Type of ExerciseArreaza: With so many options—cardio, strength training, yoga—how do you guide patients in choosing the right type of exercise for them?Wes: It depends on the patient's goals, health conditions, and personal preferences. Here's how we might break it down:For cardiovascular health: Activities like brisk walking, jogging, cycling, or swimming. For strength and bone health: Resistance exercises, bodyweight exercises, or weightlifting.Ranbir: For flexibility and balance: Yoga, Pilates, or tai chi, especially for older adults. For chronic disease management: Customized plans—e.g., low-impact options for arthritis or supervised exercise for heart disease. The key is finding something they enjoy, because sustainability is the most important factor.Arreaza: We can use our physical therapy friends to design an appropriate plan for our patients.Segment 4: Overcoming Common Barriers to ExerciseArreaza: I hear this all the time in the clinic—patients want to exercise but struggle to stay consistent. What are the biggest barriers, and how do we help patients overcome those barriers?Wes: Absolutely. Some common barriers include:Lack of time: Patients think they need hours at the gym, but even short bouts of 10 minutes throughout the day add up.Low motivation: Encouraging goal setting and accountability, such as a workout buddy or an activity tracker, helps.Arreaza: Instagram post from Ranbir: Go to the gym even if you don't want to go. Wes: Pain or chronic illness: We can adapt exercises—low-impact options like swimming or chair exercises work well.No access to a gym: Many exercises require no equipment—walking, stair climbing, bodyweight exercises.Ranbir: As physicians, we need to normalize movement as part of daily life rather than an all-or-nothing approach.Segment 5: The Role of Healthcare Providers in Exercise CounselingArreaza: We often focus on medications and procedures, but exercise is one of the best treatments we have. What role should physicians play in promoting physical activity?Ranbir: Our role is critical! Exercise is preventive medicine and can reduce the risk of heart disease, diabetes, obesity, and even depression. As physicians, we can:-Ask about exercise levels at routine visits.-Provide specific, personalized exercise prescriptions rather than just saying 'you should exercise more.'-Address patient concerns by modifying recommendations to their abilities.-Follow up and reinforce progress like we would with any other treatment.-Even a brief conversation about physical activity can significantly impact patient motivation and adherence.Arreaza: Exercise vs physical activity. Ask your patients as a routine. Closing Thoughts & Call to ActionArreaza: Ranbir andWessam, this has been a fantastic discussion. Any final thoughts for our listeners?Ranbir: My biggest takeaway is that any movement is better than none. Exercise doesn't have to be perfect—it just has to be consistent. Start small, find an activity you enjoy, and build from there!Arreaza: Any take-home message, Wes?Wes: Same for me, find an activity you enjoy, start where you are, and keep moving!Arreaza: Great advice! If you found this episode helpful, share it with your colleagues and patients. Ranbir: Until next time—stay active and stay healthy!Even without trying, every night you go to bed a little wiser. Thanks for listening to Rio Bravo qWeek Podcast.
