5.10 Mood Disorders (Depression and Bipolar) Psychiatry Review for the USMLE Step 1 exam Mood disorders are persistent disruptions in emotion, categorized into bipolar disorders and depressive disorders. Bipolar disorders are characterized by manic or hypomanic episodes, while depressive disorders feature periods of depression. Major Depressive Disorder (MDD) is characterized by feelings of sadness, guilt, worthlessness, and anhedonia lasting for at least two weeks. SIG E CAPS is an acronym used to remember the symptoms of depression: Sleep changes, Interest loss, Guilt, Energy loss, Concentration difficulties, Appetite changes, Psychomotor retardation, and Suicidal ideation. In MDD, at least 5 of the SIG E CAPS symptoms must persist for more than 2 weeks. Persistent Depressive Disorder (dysthymia) is a chronic, low-grade form of MDD that lasts for at least two years. Mania is a symptom common to all bipolar disorders, characterized by elevated or irritable mood and increased activity or energy. Mania lasting for at least 1 week is called a manic episode, while hypomania is a less severe form lasting longer than 4 days. The symptoms of mania can be remembered using the mnemonic DIG FAST: Distractibility, Irritable mood/insomnia, Grandiosity, Flight of ideas, Agitation/increased activity, Speedy thoughts/speech, and Thoughtlessness. Bipolar I involves manic episodes, and bipolar II involves hypomanic episodes and major depressive episodes. Bipolar I requires at least one manic episode, while bipolar II requires depressive episodes. Mood stabilizers such as lithium, valproate, carbamazepine, and lamotrigine are used to treat bipolar disorders. Litium is the best (notable exeptions however). Antidepressants are contraindicated for bipolar patients due to the risk of flipping into mania. The risk of suicide is high in bipolar patients, with 25-50% attempting suicide and 10-15% dying by suicide. Cyclothymia is a less common form of bipolar disorder characterized by cycling between hypomania and mild depression over many years.

The term cyclothymia was coined in the late 19th century by German psychiatrist Karl Ludwig Kahlbaum. It describes a clinical situation which has many similarities with bipolar disorder, although its symptoms are less severe. We talk about something being a disorder when it has an impact on everyday life, and that's the case with cyclothymia. It usually comes on during adolescence or early adulthood. Those affected are highly sensitive and can quickly change moods in an intense way. During the depressive phase, they may suddenly lose all enthusiasm for something that interested them before, like exercise or work. At other times, they may have inflated self-esteem, be more talkative, hyperactive or unpredictable. This phase is known as hypomania and usually lasts four or more days. Dr. B here, with our series on Demystifying the DSM. Now I'm going to sum up the other bipolar type of diagnosis. I don't personally study the DSM, and so it's kind of funny that I'm doing this Demystifying the DSM series. I personally study Dr. Stahl and his symptoms and receptors, and any professionals out there, that's who I recommend that you study from. These videos are more for our clients to understand these diagnosis, and when I'm talking to my clients, I'm very specific about “don't own this”, right? This is just a cluster of symptoms, we can work with this, The Mentally STRONG Method and the cognitive behavioral approach to get your thinking and your choices in line with what you want, and if there is a neurobiology symptoms and receptor, we can treat that with medication. So, I'm going to quickly go through some of these other bipolar types. Cyclothymia is the next one here in the DSM, and it's basically a milder bipolar II. Right, so you don't really meet criteria for hypomania. You don't really meet criteria for major depressive disorder, but you still have these pretty significant fluctuations in mood and I would challenge that maybe that we can do that with therapy. Or is there a borderline personality or some other personality disorder contributing to that fluctuation. So that's one of the differential diagnosis with Cyclothymia. The other thing that we often see is substance use. And substance use can often look like bipolar, but it can also mask true bipolar. So really, when there's substance abuse going on, the number one goal is sobriety, and there's several mental health clinics that won't treat someone's mental illness until they have become sober. I actually will treat the mental illness while I'm trying to encourage sobriety, because I think it would be very difficult if I'm severely depressed to stop using my substance if I can't get treatment for my depression, right? So, there are two schools of thought on that, but substance abuse can always, always, always complicate the diagnosis and often can look like bipolar. Also, you can have a fluctuating mood disorder secondary to a medical condition. The first thing I tell people, especially if they're coming in at 50 years or older with their first significant mood disorder symptoms, I'm really looking hard at, Is there a medical reason for this? Because most mental health conditions start in childhood, teens, 20s, rarely, early 30s. After that, if there's no mental health history and then someone has significant problems, let's look closely at medication that they've been prescribed. Steroids are a big culprit. I've had several people who have post steroid mood disorder that looks like bipolar and sometimes we do have to treat that for a period of time and sometimes it can take six months to a year post steroid use to feel stabilized from that. So, anytime someone is presenting with some other medical conditions, look at the medications that they're on, as well as any kind of neurological or inflammatory conditions can actually present as mood problems. Then we have the bucket of unspecified bipolar, I use this diagnosis frequently because I don't know the client well enough to say for sure If it's bipolar I or bipolar II. We haven't been able to really pinpoint it yet, and that's there in the DSM and then it goes through trying to show you all these little boxes of additional modifiers on that bipolar: bipolar with psychotic features. Bipolar with melancholic features. Bipolar with catatonic features. We delve into Matt's parenting journey, and how this was something he wasn't sure he wanted for a long time. What is cyclothymia? Thanks for asking!The term cyclothymia was coined in the late 19th century by German psychiatrist Karl Ludwig Kahlbaum. It describes a clinical situation which has many similarities with bipolar disorder, although its symptoms are less severe. It affects 6% of people but to get diagnosed takes 10-12 years on average and seeing several different doctors.We talk about something being a disorder when it has an impact on everyday life, and that’s the case with cyclothymia. It usually comes on during adolescence or early adulthood. Those affected are highly sensitive and can quickly change moods in an intense way.During the depressive phase, they may suddenly lose all enthusiasm for something that interested them before, like exercise or work. At other times, they may have inflated self-esteem, be more talkative, hyperactive or unpredictable. This phase is known as hypomania and usually lasts four or more days.So how is cyclothymia different from bipolar disorder? Why is it so difficult to diagnose then? Is cyclothymia a curable disorder? Lately, I've been learning about some of the challenges in my life related to ADHD. Cyclothymia has been a beast that I've been taming and feeling confident with. My mood disorder is an every 3-month thing but now that I'm learning about ADHD, I'm realizing how much this affects my day-to-day life, my commitments, relationships, and work. I'm having to revisit quite a few moments in my past and learn which scenarios in which I'm "allowed" or capable of speaking up for myself as reasons for my challenges, not excuses for my behavior. I personally feel that as long as we're doing the best we can then our reasons remain valid. I'm a little low energy this week, so I appreciate the patience of getting through this episode but it's a new important process that I'm very much willing and happy to share with you

I've just come out of one of the most difficult mental health spikes I've experienced in recent memory. The depression side of things wasn't so bad, as I've learned to manage my mental energy with gratitude practices, mediation, and overall giving myself fewer expectations during those difficult times. Mania, however, is a completely different story. Hypomania, which comes along with my Cyclothymia, is a state of being in which I crave a "hit" of some sort. I need a high to keep me going and usually that comes in the form of overspending, over-drinking and an accelerated, and sometimes frightening, sex-drive. The mania sides of things hasn't been easy to manage. So much so that I spent most of my mental health spike in bed trying to stave off these horrible feelings by watching Dragon Ball Super on my phone. But the truth is, in order for me to work through it I've had to spend more time focusing on my emotional difficulties in order to get through mania.  In either case, I need to continue to look at my mental health as an opportunity to grow. What is my mental health telling me that I need to work on? Am I putting too much value on negative emotions? Am I putting too much value on positive emotions? Am I not giving myself enough of an opportunity to breath and remove expectations of myself? Or am I working on my mental and physical health enough in my every day life? While paleo can be a very helpful diet for people who are suffering, it's not a panacea. The Paleo View (TPV), Episode 199, Diana Rodgers and Embracing Modern Medicine Intro (0:00) News and Views (1:18) We are joined by Diana Rodgers, newly minted Registered Dietician and blogger at Sustainable Dish Diana and Emily Deans just posted an amazing article called Unicorns and Rainbows: The Myth of the Paleo Panacea Paleo will not necessarily solve all of your medical issues There is no shame in seeking medical attention! The goal of paleo is not to make anyone feel "less than" Diana was inspired by a patient who had Cyclothymia and was worried about going on an antidepressant If you had a heart condition would you stick it out and hope paleo would make it go away? No way! Modern medicine, with all its criticism from the Real Food Movement, has some great stuff going on! Remember: What's your goal? Because none of us are making it out of here alive. The search for the optimal human diet is not instead of modern medicine, it's in addition to it. Find a balance and take advantage of medicine when it is appropriate. The good news: there is no chronic condition that a healthy diet, proper sleep and sufficient activity will not improve. You have not failed if you have to take medication! No moral judgment for medical intervention Even if you choose to go off medication, do it with doctor supervision to monitor for the potential bad effects of going off medicine Read Matt's blog post about his psychiatric conditions here and here. Remember: It's super hard to stay off medication for him! It's amazing how we now have tons of medical professionals in paleo now! Diana is now a Registered Dietician after a long process Controlled nutrition and IV nutrition save lives! How do you sit in a classroom and listen to things you disagree with? It was really hard for Diana to "waste her time" learning for her RD. By the third week, Diana couldn't hold her tongue all the time. Luckily, she could text Mat Lalonde (Harvard lecturer, scientist, Kraken) (she trade organ meat from her farm for unlimited texting!) Everything in moderation drives us crazy! It's not great advice that candy and junk food can be a part of a healthy diet because most people can't effectively moderate. Diana is a big believer in a period of cleansing like Whole30. The saturated fat is fine new ideas hasn't made it into her RD classes. Studying for test is hard! She had to figure out their "right" answer because anything she believed would probably be wrong! They wouldn't accept many of her undergrad classes and she had to start over at Biology 101! Even though she was a Nutritional Therapy Practitioner in practice, none of her experience would count towards her RD. She had a hard time getting an internship because she couldn't move due to family and farm. So she made sure to get all A's so she could get an internship in Boston, but was still rejected a lot (maybe due to her paleo presence on the internet She got into an ISPP (Individualized Supervised Practice Pathways) instead. Allowed her to pay for supervision and allowed her to set her own flexible internship. She paid 6500 dollars! She worked in a community capacity, in a food service capacity and in a medical capacity rotation. Part of her rotation was with Dr. Alessio Fasano, a noted scientist and doctor that studies Celiac disease! Sarah is amazed by that! It was really depressing to talk with dying patients without any opportunity for real food resources. Diana has a couple of offices she works out of, and is taking patients! She's glad she has a Nutritional Therapy and real food background before she started getting her RD. Diana's post is "How I Became a Real Food Nutritionist/RD" Over half the states say that only an RD can give nutrition advice, so you have to be careful when writing about nutrition on the internet. And now she can give advice as an RD, and she can accept insurance! She also works with patients online!
