Advocacy isn't yelling, protesting, or going viral — and for people with bipolar disorder, doing it the wrong way can actually make things worse. In this episode, we break down what real, effective advocacy actually looks like, starting where the stakes are highest: your everyday life and expanding to social and political activism — covering everything in between. From setting boundaries with family members who won't stop asking about your mental health to advocating calmly (and safely) with doctors, insurance companies, and healthcare systems to presenting in front of politicians, this episode tackles the uncomfortable truth: how you advocate matters just as much as what you're advocating for. Listener takeaways practical ways to advocate with doctors and insurance companies without risking care the difference between being firm and being perceived as aggressive why “quiet” advocacy and simply showing up still move the needle how failed advocacy efforts still lead to real, long-term change Gabe Howard, who lives with bipolar disorder, shares hard-earned lessons from decades of lived experience, including how passion can be misread as aggression, why “made-for-TV” advocacy fails in real life, and how preparation beats confrontation every time. Dr. Nicole Washington brings the clinician's perspective, explaining how advocacy can unintentionally become antagonistic — and how to communicate your needs without risking your care or personal relationships. If you've ever wanted to stand up for yourself — or for the bipolar community — but didn't know how to do it without backlash, this episode gives you a roadmap. “If you want to be an advocate, you need to develop a thick skin. Because if every time somebody criticizes a point of view, a part of your life, you lose control? Unfortunately, you're not just somebody who got angry during a debate. No, you're a mentally ill person who doesn't know how to behave in public. . .” ~Gabe Howard, Host Our host, Gabe Howard, is an award-winning podcast host, author, and sought-after suicide prevention and mental health speaker, but he wouldn't be any of those things today if he hadn't been committed to a psychiatric hospital in 2003.Gabe also hosts Healthline's Inside Mental Health podcast has appeared in numerous publications, including Bipolar magazine, WebMD, Newsweek, and the Stanford Online Medical Journal. For many people, religion is a source of comfort — but what happens when it becomes a source of fear, shame, or lifelong anxiety? In this episode, author Cassandra Brandt shares her deeply personal journey through religious indoctrination, purity culture, and the hidden mental health consequences that followed her into adulthood.Cassandra unpacks the emotional toll of being raised in an evangelical Christian environment: fearing the end times as a child, internalizing guilt as a spiritual obligation, and believing mental illness was a sign of demonic influence. She explains how these messages shaped her anxiety, self-worth, relationships, and even her sense of identity.Listener takeaways The mental health impact of purity culture on girls and women why religious trauma is often dismissed — and why that's dangerous how religious indoctrination can create lifelong anxiety, guilt, and shame Through honesty and courage, Cassandra highlights the often ignored reality of religious trauma — what it looks like, why it's misunderstood, and why so many people struggle to talk about it without being dismissed as “anti-religion.” She also shares how therapy, education, and new philosophies helped her rebuild her life, develop healthier relationships, and raise her daughter without fear-based teachings.Whether you've experienced religious trauma or want to understand someone who has, this candid conversation sheds light on an uncomfortable but essential mental health topic.“Millions of people find peace in religion. I'm not trying to throw the baby out with the bathwater. But fundamentalist evangelical theology can truly and genuinely hurt people emotionally and intellectually when it's done in this fire and brimstone sort of way, especially toward children. You can tell a child a thousand sweet stories about Jesus, but the one that's going to stick out in their head is the one that you tell them about the hell that they're going to if they don't believe. Some children won't experience sleepless nights and terror about eternal damnation. Some adults won't experience trauma later. But others will.” ~Cassandra Brandt, religious trauma survivor Our guest, Cassandra Brandt is an author, advocate, and aspiring Stoic based in rural Arizona. A steelworker prior to a spinal cord injury in 2015, Cassandra now writes full time, blogging for disability publications and writing for literary magazines. Her multiple books are available on Amazon. Cassandra speaks against ableism and for equity, and believes in the rights of animals and our obligation to the Earth. She writes about deconstruction from religion, utilizing philosophy and what it means to be human. Our host, Gabe Howard, is an award-winning writer and speaker who lives with bipolar disorder. This episode of West of Knowhere digs into the week's biggest headlines: the U.S. operation in Venezuela and questions about fentanyl and oil, state crackdowns on SNAP purchases and the food-stamp debate, and the rise of AI-generated content that makes the internet untrustworthy. Along the way the hosts riff on pop culture, controversial viral moments, a parenting 'Am I the Asshole' dilemma about choosing a daughter's game over a funeral, and thank listeners and supporters. [It’s winter break, and everything has come to a halt here, including podcasting. We’ll be back soon, promise!] Reddit's “Am I the Asshole?” makes us question everything Reddit's “AITA?” brings out the best and worst in people—and this week, we're analyzing some choice cases through the lens of med school. M1s Srishti Mathur, Sahana Sarin, Maria Schapfel, and Mahaasrei Ghosh debate whether people in these scenarios are truly in the wrong or just victims of someone having a very bad day. We break down the pressure on pre-meds and med students to do research, the value of publications, and the “gunner” mentality. Is bench research a necessary evil, or are pre-meds wasting their time? When is it appropriate to insist on being called ‘Doctor?' Is it okay to go nuclear to take someone down a notch when they need it? Is a degree in design and marketing as important as an MD? It's a chaotic mix of ethics, egos, and existential crises—so strap in. We Want to Hear From You: YOUR VOICE MATTERS! We welcome your feedback, listener questions, and shower thoughts. Being told to “calm down” has never calmed anyone down — especially if you are experiencing symptoms of bipolar disorder. In this surprisingly funny episode, Gabe Howard (who lives with bipolar disorder) and Dr. Nicole Washington break down why the world's most common advice backfires… and what truly helps instead. Whether you're spiraling at 2 a.m., overwhelmed in your car, or suddenly flooded with anxiety for no clear reason, the ability to de-escalate yourself is a core skill for managing bipolar disorder. But knowing how to calm down — without shame, judgment, or dismissive clichés — isn't something most of us were ever taught. Listener takeaways how to build your own personalized calm-down toolkit how naming emotions instantly reduces their intensity how to challenge spiraling thoughts before they take over the difference between managing emotions versus invalidating them So, take a listen as our hosts share practical, stigma-free tools you can start using immediately, from deceptively simple breathing exercises to naming emotions, reframing intrusive thoughts, and building a personalized “calm-down buffet” of strategies that actually work for you. “And here's another pro tip that shouldn't be as earth-shattering as it is, but: admit it. Just admit that you need to calm down. Admit that you're anxious, agitated, overwhelmed, angry, elevated, whatever. Just admit it. Don't judge it. Practice some radical honesty with yourself and admit that you need to take a beat and that you need a moment. Don't try to deny it. And also don't assume that it's because you live with bipolar disorder. You're a human with real emotions, but you still need to manage this.” ~Gabe Howard, Host Our host, Gabe Howard, is an award-winning podcast host, author, and sought-after suicide prevention and mental health speaker, but he wouldn't be any of those things today if he hadn't been committed to a psychiatric hospital in 2003.Gabe also hosts Healthline's Inside Mental Health podcast has appeared in numerous publications, including Bipolar magazine, WebMD, Newsweek, and the Stanford Online Medical Journal. Hey friend — welcome to our latest People's Court episode where we dive into an Am I the Asshole post about a 19-year-old bridesmaid who finally got her ears pierced and wore one-inch silver hoops to her sister's wedding, only to be called "trashy" and accused of disrespecting the bride. Spoiler: we're not surprised by how wild this got. We walk through why that reaction felt wildly over the top — the bridesmaid is an adult making a small, tasteful choice, and calling your sibling trashy at your wedding? Cold. We compare it to the exact opposite energy of chill brides who gift matching earrings and set clear, reasonable boundaries, and we laugh (a little cruelly) about how dramatic family weddings can be. Have your own juicy wedding drama? The uncle who threw up, the brother-in-law who got handsy, or the cousin who caused a scene — we want to hear it. Send us your stories at truecrimesagainstwine@gmail.com or find us on TikTok and Instagram — we'll read the best ones on the show and might even send you some swag. Pour a drink, settle in, and let's judge some people together — because honestly, it's way more fun from the guest table.

Medication nonadherence in schizophrenia is often framed as defiance — but that narrative misses the truth and harms the very people it claims to help. Guest host Rachel Star Withers (who lives with schizophrenia) unpacks why up to 70% of people with schizophrenia have trouble taking medication as prescribed — and why the reasons are far more complex than “noncompliance.” From severe side effects and cognitive symptoms to stigma, trauma, access issues, and being dismissed by providers, this conversation exposes the real barriers standing in the way of effective treatment. Rachel shares deeply personal experiences of both intentional and unintentional nonadherence, including how side effects like extreme fatigue and tardive dyskinesia shaped her decisions — and how lack of education nearly cost her years of quality of life. The episode also explores how medication refusal can be weaponized, stripping people with schizophrenia of autonomy under the guise of “lack of insight.” Later, Rachel sits down with board certified toxicologist and psychopharmacologist Dr. Alberto Augsten, who brings clarity, hope, and science to the discussion. Together, they explore long-acting injectable antipsychotics (LAIs), new treatment advances, realistic expectations, and how education — not force — improves outcomes. If you've ever wondered why medication adherence is so difficult in schizophrenia — and what actually helps — this episode delivers answers rooted in empathy, evidence, and lived experience. Our guest, Dr. Alberto Augsten, is a board certified toxicologist and psychopharmacologist. He is a consultant and expert witness specializing in psychopharmacology and clinical toxicology. Offering professional, impactful, and strategic consulting services, Dr. Augsten caters to legal, healthcare, and clinical practice development. His expertise encompasses many areas, ensuring clients receive top-notch guidance and insights. Dr. Augsten specializes in providing tailored consulting services to assist in the development and optimization of clinical practice standards. His expertise covers many areas crucial for clinic development, allowing for standard of care reviews concerning those matters. Dr. Augsten's influence extends beyond his clinical practice. He offers consultation and education services to pharmaceutical companies, contributing to the advancement of pharmaceutical knowledge and practices. Through his contributions, he is helping to improve patient outcomes and ensure the highest quality of care. Our guest host, Rachel Star Withers, creates videos documenting her schizophrenia, ways to manage, and let others like her know they're not alone and can still live an amazing life. She has written “Lil Broken Star: Understanding Schizophrenia for Kids” and a tool for schizophrenics, “To See in the Dark: Hallucination and Delusion Journal.” Learn more at RachelStarLive.com. Traveling is supposed to be fun — but when you live with bipolar disorder, even a short trip can disrupt sleep, routines, and stability. In this episode, Gabe Howard (who lives with bipolar) and Dr. Nicole Washington get honest about why vacations can feel overwhelming, how to plan without fear, and the exact safeguards that make travel not just possible, but enjoyable. From jet lag to family expectations, from “what if I'm not stable in 6 months?” to navigating early recovery setbacks, this conversation gives you practical, nonjudgmental guidance you can use immediately. Whether you're planning your first overnight away or a bucket list vacation, you'll walk away with tools to travel confidently, all without sacrificing stability. Listener takeaways how travel disrupts routines — and which disruptions matter most tips for setting realistic expectations and reducing travel anxiety the difference between “I'm too sick to go” and “I'm just nervous” how to create a personalized travel routine that supports stability If you've ever wondered, Can I actually travel with bipolar disorder? the answer is yes. And this episode shows you how. "I have so many stories of me losing my mind out in public and needing to be calmed down and, you know, given water. I have so many stories of getting ready to go to the show, the concert, the play, the, the event, the whatever, and then refusing to leave and ruining it for everybody else. So I don't want anybody to think that this just came naturally. . ." ~Gabe Howard, Host Our host, Gabe Howard, is an award-winning podcast host, author, and sought-after suicide prevention and mental health speaker, but he wouldn't be any of those things today if he hadn't been committed to a psychiatric hospital in 2003.Gabe also hosts Healthline's Inside Mental Health podcast has appeared in numerous publications, including Bipolar magazine, WebMD, Newsweek, and the Stanford Online Medical Journal. Hallucinations are the most recognized—and most misunderstood—symptom of schizophrenia. Movies depict them as dramatic, terrifying commands or cinematic visions, but the lived reality is far more complex. In this episode we unravel what hallucinations actually are, why they happen, and how people learn to live with them. This episode is a special feature from our sister show Inside Schizophrenia. Hosted by Rachel Star Withers (who lives with schizophrenia), with Gabe Howard as co-host. (Don't worry, new Inside Mental Health episodes return in 2026.) In this episode, Rachel shares her own experiences, from everyday “simple” hallucinations like sounds or shifting faces, to more intense, emotion-laden complex hallucinations. She challenges the assumption that hallucinations are always violent or dangerous—and breaks down the critical differences between hallucinations and sensory disturbances. Expert guest Dr. Paul Fitzgerald joins the conversation to explain how the brain creates these perceptual misfires, why hallucinations in schizophrenia differ from those caused by grief, sleep deprivation, or drugs, and what current research reveals about how universal these experiences are across different cultures and countries. Listener Takeaways The difference between simple vs. complex hallucinations Why hallucinations in schizophrenia feel different from drug- or grief-based ones Why reducing—not eliminating—hallucinations is often the realistic recovery goal How CBT and coping strategies help reduce fear and regain control Whether you live with schizophrenia, love someone who does, or are simply curious about how the brain works, this episode offers clarity, compassion, and surprising insights you won't forget. Guest, Professor Paul Fitzgerald, completed his medical degree at Monash University and subsequently a Master of Psychological Medicine whilst completing psychiatric training. He then undertook a Clinical and Research Fellowship at the University of Toronto and The Clarke Institute of Psychiatry, Toronto, Ontario, Canada. On returning to Melbourne, he worked as a psychiatrist and completed a PhD in transcranial magnetic stimulation in schizophrenia. Since completing this PhD, he has developed a substantial research program including a team of over 25 psychiatrists, registrars, postdoctoral researchers, research assistants, research nurses, and students. Professor Fitzgerald runs a research program across both MAPrc and Epworth Clinic using brain stimulation and neuroimaging techniques including transcranial magnetic stimulation, functional and structural MRI, EEG, and near infrared spectroscopy. The primary focus of this program is on the development of new brain stimulation-based treatments for psychiatric disorders. Guest host, Rachel Star Withers, creates videos documenting her schizophrenia, ways to manage, and let others like her know they're not alone and can still live an amazing life. She has written “Lil Broken Star: Understanding Schizophrenia for Kids” and a tool for schizophrenics, “To See in the Dark: Hallucination and Delusion Journal.” Learn more at RachelStarLive.com. Why does happiness always feel one step away? In this episode, we welcome back positive psychology expert Jodi Wellman to explore why so many of us keep “moving the goalposts” on our own success, and learn why accomplishments that once thrilled us eventually feel ordinary. Jodi breaks down why we convince ourselves that one more thing — a new job, a relationship, a move, a big purchase — will finally deliver lasting happiness. Using Gabe's own podcasting journey as a case study, the conversation exposes a mental trap familiar to anyone who's ever felt like their wins “don't count” for long. But this episode isn't just about naming the problem — it's about fixing it. Jodi offers practical, research-backed strategies to help you savor your accomplishments, find a healthier balance between ambition and contentment, and stop letting your inner critic erase your progress. Listener takeaways why humans believe they're always “one thing away” from feeling fulfilled what the hedonic treadmill is and how it sabotages our happiness how to balance ambition with genuine joy so you can grow without feeling miserable If you've ever wondered, “Why don't I feel successful?” this is the episode that can finally help you understand. “We like to dupe ourselves. It's just one of the features of our psychology. It all comes down to this idea called the hedonic treadmill. We think, and we go for it. We'll buy the pill, we'll make the move, we'll take the job, we'll buy the car, we'll do a thing. And usually it's external stuff, but sometimes it's internal, like, I will become the person that I've been meant to be and that will make me happy. But then, because we adapt so well, this is this hedonic adaptation, hedonic treadmill, we adapt so reliably, damn it. And we end up, over time, some of us shorter than others. Naturally, meh. Okay, well, how come it's not doing it for me anymore?” ~Jodi Wellman, MAPP Our guest, Jodi Wellman, MAPP is a speaker, author, and facilitator on living lives worth living. She founded Four Thousand Mondays to help people make the most of the time they are lucky to be above ground. With 25 years of corporate leadership experience (most recently as Senior Vice President of Operations at a leading health and lifestyle organization), Jodi has led private CEO advisory boards and coaches teams to work well and live even better.
