What do 19th-century vampires, corneal scarring, and rampant chlamydia have in common? Possibly more than you'd think. I Nosferatu and spiraled (in a good way) into diagnosing one of film's creepiest figures using real ophthalmology. Along the way, I breaks down the leading causes of corneal blindness, unpack the wild world of corneal transplants, and imagine what kind of eye care Nosferartu might've needed in his castle. Takeaways: He May Be Immortal, But His Corneas Are Not: Nosferatu's cloudy eyes raise some very real medical questions—and Dr. Flanary can't let it go. Not All Corneal Scarring Comes from Contact Lenses: What causes corneal blindness around the world might surprise you… and gross you out a little. There's a Toothpick-Sized Transplant That Can Change Lives: Modern corneal transplant technology has gotten weirdly precise—and impressively effective. PBM Reform Might Finally Be Moving… a Little: A glimpse of progress in the middleman mess, with state-level action heating up. If You Watch Horror with a Doctor… Be Prepared for Diagnoses: Don't expect a relaxing movie night. Expect theories, pathologies, and a bit of eye-rolling. In this episode, I dive into a mix of strange eye conditions, pop culture moments, and one very public showdown between a doctor and a group of anti-vaxxers on Jubilee's Surrounded. But beyond the headlines, there's a deeper conversation here—about what it costs to stay online, stay visible, and speak up. You'll laugh, you'll cringe, and you might even check your eyelids before bed. Takeaways: A Surprisingly Common Condition You've Probably Never Heard Of: It flips, it flops, and it might be the reason you wake up with irritated eyes. A Debate That Left More Than a Mark: What happens when a doctor faces off with misinformation... for hours? The emotional fallout might surprise you. Engaging Online: Heroic or Hopeless? Dr. Flanary asks the uncomfortable question: Is fighting misinformation on social media actually helping—or just draining the people trying to help? Sci-Fi Fails a Real-World Eye Test: Those high-tech retinal scanners in movies? There's one big flaw they forgot to mention—and it could keep you out of your own evil lair. In this episode I share one of the more difficult days in the OR—when a routine cataract surgery didn't go according to plan. I walk through the moment everything shifted, how I kept my cool, and what came after—both technically and emotionally. If you've ever wondered what it feels like to be a surgeon in that moment, this episode is a rare, honest window into it. Takeaways: Sometimes, Even “Routine” Surgeries Aren't: Dr. Flanary opens up about a recent case that didn't go as expected—and why even years into practice, there are still moments that test every skill you've got. Staying Calm When Things Go Sideways: When a complication unfolds in real time, there's no time to panic. Dr. Flanary shares how he handled a challenging situation, and what surgeons learn to do when instincts and training have to carry the moment. Confidence Doesn't Mean Perfection: Through a powerful metaphor, Dr. Flanary reflects on how surgical confidence evolves—not as certainty that nothing will go wrong, but as the ability to keep moving when it does. Conversations After Complications Matter: How do you talk to a patient—and their family—when the procedure didn't go exactly as planned? Dr. Flanary talks about how he approaches these moments with honesty and trust. The Policies Behind the Pressure: In a classic healthcare rant, Dr. Flanary traces how systems like Medicare Advantage add stress behind the scenes—and why real reform, especially around PBMs, can't come soon enough. — We have an active survey going. In this episode I go on a private equity rant, breaking down how corporate takeovers are ruining healthcare—just like they've wrecked other industries, including Southwest Airlines. I explain how profit-driven ownership models are prioritizing revenue over patient care, forcing doctors into impossible situations and leading to worse outcomes for everyone. Takeaways: Private Equity is a Disaster for Healthcare: Dr. Flanary explains how private equity firms take over medical practices, cut costs, reduce patient care quality, and flip them for profit, leaving physicians and patients suffering. Southwest Airlines' Private Equity Problem is a Warning: The downfall of Southwest's customer-friendly policies is just another example of how corporate ownership ruins everything it touches—including medicine. Orthopedic Surgeons Really Love Pickleball: While speaking at an ortho conference, Dr. Flanary witnessed a full pickleball tournament happening inside the exhibit hall—which, honestly, makes a lot of sense for ortho docs. How ERs Handle Eye Emergencies: Not every ER has an on-call ophthalmologist, but level 1 and level 2 trauma centers are required to have one available 24/7—so if you have a serious eye emergency, that's where you should go. The Internet Really Wants to Talk About Spleens: After joking about replacing the spleen with a second liver, Dr. Flanary responds to fan concerns that spleens can “explode”, clearing up how spleen ruptures actually happen. — We have an active survey going. In this episode I unpack the wildest eye surgery you've never heard of—a procedure that uses a patient's tooth to hold an artificial cornea in place. This rare surgery restores vision in people with severe corneal damage. I also react to Goldie Hawn mentioning her cataracts at the Oscars, discussing why cataracts are a normal part of aging and why most people should have them removed by their 70s. Takeaways: Tooth-Based Eye Surgery Sounds Crazy, But It Works: The osteoodontokeratoprosthesis (OOKP) procedure involves implanting a patient's tooth into their cheek, then using it as a corneal support. It's used in extreme cases of corneal blindness when traditional transplants won't work. Goldie Hawn's Cataract Revelation at the Oscars: A throwaway comment at the Oscars sent cataract searches skyrocketing, highlighting how common cataracts are after age 60—and why most people should consider surgery by their 70s. Should Cataract Surgery Be Delayed? Dr. Flanary breaks down why waiting too long can make cataract surgery riskier, especially when glare, reading issues, or night driving problems start affecting daily life. Australia Wants to Learn from America's Healthcare Failures: Ahead of his keynote speech in Sydney, Dr. Flanary shares how Australia is facing growing privatization threats—and why they don't want to end up with a U.S.-style system. Healthcare Comedy as a Teaching Tool: Dr. Flanary floats the idea of a stand-up show explaining U.S. healthcare, mixing satire and real policy breakdowns to educate audiences worldwide on how we got here. — To Get Tickets to Wife & Death: You can visit Glaucomflecken.com/live We want to hear YOUR stories (and medical puns)! In this episode I share a wild VA investigation story involving misplaced suspicion, misinterpreted medical images, and seven months of wasted time. I also dive into measles-related eye complications, explaining how this highly contagious virus can lead to conjunctivitis, corneal scarring, and even blindness. Takeaways: The VA's Seven-Month Investigation Fail: Dr. Flanary tells the almost unbelievable story of how VA police spent months investigating "pornographic images"—only to realize they were larynx photos from ENT exams. Measles Can Cause Blindness: He explains how measles-related conjunctivitis and keratitis can permanently scar the cornea, making it one of the leading causes of childhood blindness in low-income countries. Spleens, Extra Organs, and Evolution: Responding to fan comments, Dr. Flanary revisits the spleen vs. second liver debate and expands on which organs would be most useful if humans could regenerate them. Do We Really Need Multiple Skins? In a bizarre but hilarious discussion, Dr. Flanary considers the logistical nightmare of humans molting like reptiles, questioning what we'd even do with billions of discarded skins. Get Vaccinated Against Measles: With rising measles cases in the U.S., Dr. Flanary strongly emphasizes the importance of the MMR vaccine to protect against severe illness, eye damage, and preventable deaths. — To Get Tickets to Wife & Death: You can visit Glaucomflecken.com/live We want to hear YOUR stories (and medical puns)! In this episode I answer frequently asked eye health questions, tackling topics like why your eyes hurt after removing contacts, whether it's possible to reverse pupil dilation with Rizumvi, and how UV light therapy for seasonal depression affects eye safety. Takeaways: Why Contact Lenses Should Never Hurt: If your eyes hurt after removing contacts, it's a sign of severe dryness, poor oxygen flow, or corneal irritation. Overwearing contacts can worsen long-term eye health, so taking breaks is essential. Reversing Eye Dilation with Rizumvi: A listener introduced Dr. Flanary to Rizumvi, an FDA-approved medication for reversing dilation. He explains why it's expensive, slow to act, and not widely used, meaning most people still have to wait it out. UV Light Therapy and Eye Safety: Seasonal affective disorder (SAD) lamps can help mood regulation, but they must have UV filters to prevent potential eye damage. Without proper filtering, they can increase the risk of photokeratitis (sunburn of the eye). Cataract Surgery and Timing: Not everyone needs cataract surgery at 60—it depends on how much the cataracts affect vision. Dr. Flanary explains how eye doctors determine when surgery is actually necessary. Watching Live Eye Surgery: A listener shared their experience of watching a live cataract surgery through a viewing window in a clinic. Dr. Flanary discusses why some clinics allow this and why most patients prefer NOT to see their own surgery up close. — To Get Tickets to Wife & Death: You can visit Glaucomflecken.com/live We want to hear YOUR stories (and medical puns)! Kristin shares her journey of supporting her husband through two testicular cancer diagnoses and a cardiac arrest in 2020. She emphasizes the need for healthcare providers to better recognize and support co-survivors. Kristin talks about the emotional impact of these experiences, including the trauma of performing CPR on her husband. She urges medical professionals to improve communication with families and involve them more in the care process. She also advocates for changes like providing social workers, chaplains, or support groups to help families during and after crises. Kristin also emphasized the importance of addressing the mental health needs of co-survivors, as many struggle with recovery on their own. She encouraged healthcare providers to show empathy and build stronger connections with both patients and their families. BIO After her husband suffered a sudden cardiac arrest, Kristin Flanary, also known as Lady Glockenflecken, became a prominent advocate for trauma, resilience, and co-survivorship. Drawing from her personal experiences, Kristin has focused on raising awareness about the challenges faced by caregivers and co-survivors during medical crises. With a master's degree in experimental psychology, she leverages her expertise to advocate for greater recognition and support for caregivers and families impacted by their loved ones' health challenges. Her efforts emphasize addressing the often-overlooked emotional and mental health needs of these individuals while promoting resources like HeartSite and CASA (Cardiac Arrest Survivor Alliance) to provide practical guidance and community support. In this episode I give listeners a behind-the-scenes look at my call week as a private practice ophthalmologist. With a mix of routine calls and complex cases, I go into unexpected consults for Kaiser-Fleischer rings related to Wilson's disease, diagnosing giant cell arteritis, and managing diplopia with no clear cause. I also share my diagnostic process for distinguishing between common post-surgery floaters and more serious retinal issues. For anyone curious about the daily challenges of eye care on call, this episode offers both humor and insight into the life of an ophthalmologist. Takeaways: Kaiser-Fleischer Rings and Wilson's Disease: Dr. Flanary shares a rare consult for a Kaiser-Fleischer ring to help diagnose possible Wilson's disease, highlighting the role of ophthalmologists in identifying systemic conditions through eye exams. Floaters After Cataract Surgery: He discusses common post-surgery floaters and the importance of differentiating them from signs of retinal detachment, which may require urgent attention. Giant Cell Arteritis and Vision Loss: A past patient with biopsy-negative giant cell arteritis raises questions on how best to assess for potential vision-threatening conditions and the typical diagnostic approach for such cases. Diagnostic Process for Diplopia: Dr. Flanary walks through his approach to a challenging case of double vision (diplopia), outlining potential causes from cranial nerve palsies to myasthenia gravis and thyroid eye disease. The Life of On-Call Ophthalmology: Balancing sleep disruptions, complex patient calls, and intricate diagnoses, Dr. Flanary shares the ups and downs of ophthalmology on call, giving a humorous and realistic view of the role. — To Get Tickets to Wife & Death: You can visit Glaucomflecken.com/live We want to hear YOUR stories (and medical puns)! Today, we are revisiting one of our favorite episodes from last year - The Forgotten Patient. Dr. Zanotti focuses on the loved ones and families of critically ill patients and explores the topic through the lens of a cardiac arrest co-survivor. His guest is Kristin Flanary, a cognitive neuroscientist and social psychologist. Kristin is a co-survivor of cancer(twice) and a co-survivor of sudden cardiac arrest. She is also known for her social media alter ego – Lady Glaucomflecken. In 2022 Kristin co-founded Glaucomflecken LLC, where she works to advocate for caregivers and co-survivors of medical trauma and critical illness, community CPR and AED use, and sudden cardiac arrest survivorship. She has written about her experience as a co-survivor in medical journals and has received an EMS Cardiac Arrest Save Challenge Coin, the Citizen CPR Foundation's '40 Under 40' award, and the American Heart Association's Resuscitation Champion award. First: an update on our recent two-parter with the writer John Green, about the global, decades-long fight to make an important tuberculosis drug more widely available. Just two days after we posted part 2, the activists waging that battle scored a major victory. John Green was kvelling on YouTube, of course. We'll get you up to speed. And for the meat of this episode, we've got a guest a lot of you have been asking for: Physician/comedian Will Flanary, AKA Dr. Glaucomflecken. His punchy videos satirizing the absurdities and cruel complexities of the American health care system have been a fan favorite for years among An Arm and a Leg listeners (and us too). We're sharing a delightful and moving conversation with Dr. G and his wife, educator Kristin Flanary (AKA @LadyGlaucomflecken online), from our pals at The Nocturnists, a podcast about the experiences of health care workers. As the Glaucomfleckens tell Nocturnists host Dr. Emily Silverman, the inspiration behind Flanary's most biting videos. came from the couple's experience dealing with health insurance after he suffered a near-fatal heart attack.Check out the Nocturnists here or wherever you get your podcasts, and Dr. Glaucomflecken's videos on TikTok, Instagram, and YouTube. Dr. Will Flanary is an ophthalmologist and stand-up comic who moonlights as the hilarious “Dr. Glaucomflecken” on social media. The Glaucomflecken videos have earned him an enormous worldwide following, as he has gently parodied physicians of many specialties (including pathology). He and his wife Kristin (aka "Lady Glaucomflecken") join me to share their inspirational story, with NO blood bank learning whatsoever! Even so, you've gotta listen!
Emily speaks with ophthalmologist-comedian Will Flanary and educator Kristin Flanary (aka Dr. Glaucomflecken and Lady Glaucomflecken) about their medical comedy platform, and how Will's personal experience with cardiac arrest impacted the way they view their work as healthcare satirists and content creators.

In this episode of BackTable, Dr. Bagrodia interviews Dr. William Flanary, a physician-comedian popularly known as Dr. Glaucomflecken, about lessons he has learned as a two-time testicular cancer survivor and the importance of humor in medicine. --- CHECK OUT OUR SPONSOR RADPAD® Radiation Protection https://www.radpad.com/ --- EARN CME Reflect on how this Podcast applies to your day-to-day and earn free AMA PRA Category 1 CMEs: https://earnc.me/VJvXZx --- SHOW NOTES First, Dr. Glaucomflecken shares about his first diagnosis of testicular cancer. During his third year of medical school he felt a lump in his testicle, which led to a quick workup, diagnosis, and a full orchiectomy. The diagnosis was emotionally difficult, as he was in his mid-twenties and healthy. He returned to comedy, a skill he had developed in high school and college, to cope with his diagnosis. This time, however, he started to practice medical-based comedy with his new experiences as a medical student. He recounts other discussions he had about his cancer, such as fertility, the possibility of chemotherapy, and active surveillance. Four years after his first orchiectomy, he received his second diagnosis of testicular cancer during his last year of residency. He recounts feeling distraught and overwhelmed, as questions about fertility, hormone replacement, medical expenses, and postponing residency became more serious. He decided to have a full orchiectomy and testosterone replacement therapy, which solved his issues with fatigue and irritability. Additionally, his wife got him involved in testicular cancer support groups and foundations, including one called First Descents, an organization that encourages young adults with cancer to explore the outdoors. He notes that young patients are often overlooked in cancer support groups and encourages cancer patients to find their support networks outside of friends and family as well. Then, Dr. Flanary discusses his experience with suffering from cardiac arrest in 2020, which led to his wife doing ten minutes of chest compressions to keep him alive. He reflects on this event and concludes that it taught him how to be a better physician to his patients by making sure he involves patients' families and encouraging him to address medical insurance issues directly. Finally, Dr. Flanary discusses how he uses humor to advocate and educate patients on social media. He notes that comedy can stimulate conversation and debate and encourages physicians to have social media presence. --- RESOURCES Knock Knock Hi Podcast https://podcasts.apple.com/us/podcast/knock-knock-hi-with-the-glaucomfleckens/id1659572053 First Descents https://firstdescents.org/

We're honored in this episode to talk with Kristin Flanary, MA (aka Lady Glaucomflecken). She shares her experience when her famously hilarious husband (Will Flanary, MD aka Dr. Glaucomflecken) unexpectedly went into cardiac arrest while sleeping. Her quick action saved his life, but she talks about how that experience changed her and her subsequent journey coming back from what she calls The Quiet Place. In healthcare, so much on the patient in the gurney, but often overlooked are the family/loved ones/friends who are also affected. Some have labeled them The Forgotten Patients or Co-Survivors. In many cases, they get pushed to the side, but they've just had a significant trauma in their own right as well. Kristin also touches on some of the experiences with healthcare in the days after the event that could have been improved with minimal effort and just a little thoughtfulness. We know you'll love Kristin as much as we did. Enjoy. Want more like this? Join our Weekly Well Check by clicking here. The enthusiasm will come through your ear phones when you hear this week's guest, Allinee, “shiny” (small “s” on her website) Flanary, founder of Come Thru Market. If you check her website, you will see a true system of farm-to-market education and assistance focused on black and indigenous farmers and makers. There step-by-step tutorials to walk participants through the process of developing a product, readying it for market and getting into marketplaces. Also, shiny books private consultation sessions for those wanting to accelerate their projects. Come Thru Market is open the 1st and 3rd Monday of the month from May through October, 3-7pm. She also takes her Scrapberry farm to Portland's Montavilla Market which is mainly the last Sunday of each month. Scrapberry Farm is shiny's medicinal herb farm, which is heavy on spicy peppers. Her journey began with her own chronic diseases and disabilities. She pursued herbalism to heal herself and quickly discovered the false claims and fraud in the industry. Many products had the right idea for healing, but sourced their herbs from agribusiness entities which produced inferior herbs. To correct this, shiny started a small garden in the front yard of her Portland, Oregon home. The longer she tended the garden and consumed her own herbs, the more the neighbors in her community commented on how much better she looked and felt. They said she seemed “shinier”. At the time, she was a college professor and librarian, with absolutely no experience in farmers' markets and no real desire to be part of them. But the black and brown herb growers and farmers she knew pushed her hard to pursue a path to farming. Her first foray was a small plot in a hilly, rather cold part of the state with a growing window of about two months. It helped her see that food was much more difficult to grow than herbs and they became her focus. So now that shiny had a product, where to sell it? And that was the entrée into farmers' markets. Of course the path is never easy and she had to navigate state regulations while also having the FDA watching over her herb claims. At the same time, shiny wound up bumping into more and more black and brown farmers who needed help marketing their food and herb products. So as difficult as the regulations were, as complex as the steps to running a successful farmer's market, shiny saw the big opportunity to put together a farmer's market, complete with the “how to” educational component and offer a path for the farmers who were looking for a sales outlet. With all systems “go” in her comprehensive business, shiny juggling lots of plates but certainly has the energy to keep them in the air. The market: IG - @comethrupdx. Have you ever wondered what it's like when a doctor becomes a patient? I thought I was unlucky when I got breast cancer but then I spoke to my next guest, the American ophthalmologist Dr Will Flanary. He had testicular cancer twice - first a med student and then as a junior doctor. But that's not all. In 2020 Will had a cardiac arrest and his amazing wife did CPR for ten minutes to bring him back to life. We spoke about how he's used his comedy alter-ego Doctor Glaucomflecken to cope with everything life has thrown at him. Will is fund-raising for First Descents, a charity that provides free outdoor adventures for young adults impacted by cancer and other serious health conditions. He donates all his speaking fees to the organisation in aim to raise $50,000. You can read more about Will's story here: https://www.drgcomedy.com/bio and watch his hilarious videos here. Will's entry for the Jar of Joy was his new puppy. What are you putting in the jar this week?
