Healthcare is often a tangled web of bureaucracy and inefficiencies. In this week's episode, both of our storytellers share their experiences navigating its many flaws. Part 1: Zoe Wisnoski's takes matters into her own hands when her son has months of ongoing fevers. Part 2: During the pandemic, epidemiologist Bryon Backenson becomes disheartened when the public stops cooperating with public health authorities. Zoe Wisnoski is a seeker of stories, adventure, travel, and moments that stick with you. She stumbled into the world of storytelling through a training put on by Story Collider. Her passion for activism buoyed by a penchant for oversharing has finally found a home. Formerly a feminist policy analyst with a Masters in Public Policy from the Humphrey School of Public Affairs, now a full time - still feminist - mother, Zoe spends her time attempting to create joy amidst utter chaos. When her son was diagnosed with the super rare, to date minimally understood, genetic disease Tatton Brown Rahman Syndrome (TBRS), Zoe reoriented her educational and professional background to meeting his needs and volunteering with the TBRS Community, the nonprofit aimed at supporting families and advancing research for TBRS. In 2024 she joined the board of directors and continues to search for answers. Bryon Backenson is an epidemiologist. He is currently the director of the Bureau of Communicable Disease Control at the New York State Department of Health. He and his team investigate, respond to, and research infectious disease outbreaks. He is also a professor in the University at Albany College of Integrated Health Sciences, where he teaches in the Department of Epidemiology and Biostatistics. When not thinking about disease, he spends his time hiking, fishing, and reading. While he talks about epidemiology and infectious diseases all the time in classes, meetings, and webinars, this is the first time he's tried to tell his own story in this kind of format. Kaylee Stewart of the Albany College of Pharmacy and Health Sciences has done a study of environmental stressors near local elementary schools in Albany such as proximity to fighting, loitering, homeless shelters, and liquor stores. While such factors are often more prevalent in low-income and communities of color, it is especially high near Giffen Elementary School in the South End of Albany. This includes its nearness to the Capital City Rescue Mission, which is seeking to expand. Such stressors can negatively impact upon a child's learning capacity. Ms. Stewart talks to Mark Dunlea of Hudson Mohawk Magazine.

A new, real-world evidence study of patients with Gram-negative infections showed that early intervention with Fetroja® (cefiderocol) significantly reduced in-hospital all-cause mortality. This real-world evidence, drawn from a retrospective analysis of 275 seriously ill patients, emphasizes the critical importance of timely treatment.    The retrospective multicenter observational study showed earlier treatment resulted in a reduction in in-hospital all-cause mortality (IHACM) in patients with Gram-negative infections who previously received other antibiotics, including those with difficult-to-treat pathogens that were resistant to other antibiotics.   Here is a link to the press release with a summary of the results presented by Shionogi at ID Week in October.   We interview Thomas Lodise, PharmD, PhD, Associate Professor, Albany College of Pharmacy and Health Sciences, to delve deeper into these results. Dr. Lodise discusses the implications of this study for hospital pharmacists as it sheds light on the potential clinical utility of Fetroja in the treatment of difficult-to-treat adult patients with culture-confirmed Gram-negative infections. A new review paper was published in Oncotarget's Volume 14 on March 31, 2023, entitled, “Crosstalk between triple negative breast cancer and microenvironment.” Although many advances have been made in the treatment of breast cancer, for triple negative breast cancer (TNBC) these therapies have not significantly increased overall survival. Tumor microenvironment (TME) plays an essential role to develop and control TNBC progression. Many preclinical and clinical studies are ongoing to treat patients with TNBC disease, but effective therapies are currently not available. In their new review, researchers Karly Smrekar, Artem Belyakov and Kideok Jin from Albany College of Pharmacy and Health Science discuss recent progress in our understanding of TNBC, advancements in defining mechanisms of TNBC therapies and potential therapeutic strategies to overcome TNBC. “Technological advancements such as genomics and epigenomics have provided us with vast insight about the complexity of breast cancer. However, one thing has remained the same, the need for the evaluation of three markers. These three markers; the expression of estrogen, progesterone, and HER2, are all molecular targets for treatment regimens, and are relied on by clinicians [1]. Chemotherapy is the staple treatment for TNBC patients. However, they lack the expression of three key therapeutic markers. The lack of therapeutic markers leads to poorer outcomes in TNBC.” DOI: https://doi.org/10.18632/oncotarget.28397 Correspondence to: Kideok Jin - kideok.jin@acphs.edu Sign up for free Altmetric alerts about this article - https://oncotarget.altmetric.com/details/email_updates?id=10.18632%2Foncotarget.28397 Subscribe for free publication alerts from Oncotarget - https://www.oncotarget.com/subscribe/ Keywords - triple negative breast cancer, tumor microenvironment, current therapy About Oncotarget Oncotarget is a primarily oncology-focused, peer-reviewed, open access journal. Dr. Nardine Nakhla's career has had one common denominator since her graduation in 2007 from Albany College of Pharmacy and Health Sciences — striving to help as many people as possible. That desire is seen in her work with students, as a faculty member at the University of Waterloo's School of Pharmacy. Dr. Nakhla's patient-centered approach also comes through in her research, which has focused on issues like smoking cessation and high-risk, non-prescription drug interactions in older people. Her professional writing also reflects her desire to help patients. Dr. Nakhla has authored four chapters for the Canadian Minor Ailments reference, one chapter for the American Handbook of Nonprescription Drugs, and one chapter for the Encyclopedia of Pharmacy Practice and Clinical Pharmacy.     In this episode of Occupation Station, Dr. Nakhla talks about how her career path evolved. She entered ACPHS thinking that she would devote herself to community practice. While she and her husband do own a community pharmacy, she has achieved so much more than she initially envisioned. Hear why she calls her time at ACPHS “magical” and how the school has led to lifelong friendships and meaningful connections with professors. Sabrina spent hours reading through fellowship position/functional area descriptions, and that process opened her eyes to many new opportunities that she had never considered to be the right fit for her initially based on title alone. Conversation starts @ 7:26 This conversation begins with a discussion of the banisteriopsis vine (ayahuasca) featured in the background. We explore Dr. McKenna's first adventure down to South America to study and explore psychedelics used in the Amazonian medico-religious process. This endeavor would eventually connect him with several elder medicine men who created mixtures for Dennis to study – “We want to learn everything that you know about ayahuasca.” We discuss how the indigenous folks of the Amazon responded to the scientists who sought to investigate their sacramental plant, the rubber boom of the early 20th century, lost aspects of the Amazonian ethnomedical tradition, the hard problem of consciousness, the limitations of reductionism, the value of science, limitations of science, an overview of the journey to La Chorrera, building the temple for “spiritual” experience, or ecstatic/mystical experience, the body as apothecary, theories of consciousness, the molecule and the “trip,” scientific bias of the west, the Default Mode Network, the value of psychedelics to the scientific community, and the McKenna Academy. Bio: Dennis McKenna is an American ethnopharmacologist, research pharmacognosist, lecturer and author. He is a founding board member and the director of ethnopharmacology at the Heffter Research Institute, a non-profit organization concerned with the investigation of the potential therapeutic uses of psychedelic medicines. Dennis also serves on the Advisory Board of the American Botanical Council; as Founder and Executive Director for the Institute for Natural Products Research; as an Independent Research Consultant to the Phytomedicine and Nutraceutical Industry; was formerly on the Editorial Board of Phytomedicine, International Journal of Phytotherapy and Phytopharmacology; and is an adjunct professor in the Center for Spirituality and Healing at the University of Minnesota. In that role, he has taught graduate courses in Botanical Medicines and Ethnopharmacology and an inter-session ‘intensive' in Hawaii each January called Plants in Human Affairs. He has also taught summer field courses in Ecuador for the University of Arizona, and courses in the Amazon and Andes for Pharmacy doctoral students at the University of Kansas and the Albany College of Pharmacy and Health Sciences. Dr. McKenna received his Master's Degree in Botany from the University of Hawaii in 1979, his Ph.D. in Botanical Sciences from the University of British Columbia in 1984, and continued into post-doctoral research fellowships in the Laboratory of Clinical Pharmacology, National Institute of Mental Health (NIMH), and in the Department of Neurology, Stanford University School of Medicine. His research has included the pharmacology, botany, and chemistry of ayahuasca and oo-koo-hé. He has also conducted extensive ethnobotanical fieldwork in the Peruvian, Colombian, and Brazilian Amazon. Since 2019, he has been working with colleagues to manifest a long-term dream: the McKenna Academy of Natural Philosophy, a non-profit organization founded in the spirit of the ancient Mystery Schools and dedicated to the study of plant medicines, consciousness, intelligence in nature, preservation of indigenous knowledge and a re-visioning of humanity's relationship with Nature. During his eight-year tenure at Albany College of Pharmacy and Health Sciences, Dr. Greg Dewey accomplished many great things. In this episode of Occupation Station Dr. Dewey reflects on The Collaboratory, the Stack Family Center for Biopharmaceutical Education and Training (CBET), and the creation of two student-run pharmacies.     The Collaboratory, a “laboratory for collaboration,” offers services which are delivered to residents in Albany by ACPHS's Public Health Pharmacy Team and Trinity Alliance's Wellness Advocates Linking Communities (WALC) team. Programs at The Collaboratory address social justice and social determinants of health with the shared goal of improving population health. CBET is an evolutionary leap in health sciences education and biomanufacturing workforce training. Dr. Dewey is perhaps most proud of the creation of two student-run pharmacies both situated in what would be deemed “pharmacy deserts.” The pharmacies, which are located in Albany and Schenectady, N.Y., are prime examples of social entrepreneurship and are designed to have a lasting impact on the communities they serve, while also helping to develop skills that students will benefit from. Dr. Dewey explains that social entrepreneurship involves being self-sustaining while doing public good.      Because of his relationships with faculty, staff, and students, coupled with the development of these crucial initiatives, Dr. Dewey has found his work at ACPHS to be greatly gratifying. And because of his enormous contributions at ACPHS he will be greatly missed!
