Is there any evidence that TEAM can help patients with Borderline Personality Disorder (BPD)? 3. How do you get patients with BDP to stop jumping from problem to problem? 4. How do you get them to stop endless venting during therapy sessions? Greg asks: 5. What comes first, thoughts or feelings? 6. Can't a genuinely negative or tragic event directly cause negative feelings, without having to have negative thoughts?   Dear Dr. Burns: 1. I would like to use your BMS but I mostly work with patients in Mexico. Has there been any standardization of your tests in any Spanish speaking country? David and Rhonda address this.  You can email Victoria Chicural, who is one of the TEAM-CBT leaders in Mexico (along with Silvina Carla Bucci), at victoriachl@yahoo.com and ask her about access to TEAM-CBT forms that have been translated into Spanish. 2. I am wondering if TEAM has proven to be effective in the treatment of BPD (Borderline Personality Disorder). I use it a lot, but I have found quite a few challenging elements. David describes his published work, indicating an excellent response to TEAM-CBT in patients with BPD. 3. People suffering from BPD usually have trouble prioritizing tasks and activities. The same happens when it comes to setting objectives. Because of their emotion dysregulation, they usually decide to work on one objective, and later on, they sometimes say: "Well, this objective is not THAT important anymore. Let's do another." For them, doing the specificity part can be really challenging because their perspective changes very quickly and they usually go back to the former objective when they're being challenged by a similar situation!!! How do you get them to prioritize objectives and not to switch from one to another so quickly? Or, do you think I could be making a mistake when setting objectives? David describes the strategies he has developed for coping with this type of clinical problem, including the development of his Concept of Self-Help Memo that he required every new patient to fill out prior to their first therapy session. 4. BPD usually come up with a lot of material to the session. They may be facing complex PTSD but also dysfunctionality at work, at school, etc. They want to say everything in a single session even if we have agreed to follow one single objective. Many sessions turn into endless talking without getting anywhere - some of them argue they need to vent out what they feel - but as time goes by, they complain that therapy is not working! How do you deal with a patient who is overwhelmed with numerous factors in a session where you have a previously set objective? David describes the strategies he has developed for coping with this type of clinical problem, Carlos S Bouchanm, Clinical Psychologist David's Response Hi Carlos, I think these would make for excellent Ask David podcast questions. If so, can we use your name and read your questions? I reported on the effectiveness of the forerunner of TEAM in the treatment of BPD is the Journal of Clinical and Consulting Psychology in the 1990s. TEAM was specifically developed for this population, since 28% of my patients in Philadelphia had BPD. In the live podcast, I will address the excellent questions you asked about treating individuals with BPD. Thanks! David From: Greg Hi David, Thanks for everything you do and for the great podcast! I have another couple questions possibly for the “Ask David” segment of the podcast. 5. Can you say some more about automatic thoughts? CBT is based on the idea that we're thinking things that produce feelings, but with an automatic thought it just kind of pops up and is there. It's not like actively, intentionally thinking it. Other schools of thought (for example Somatic Experiencing) posit that feelings from the nervous system occur first and that the thoughts are actually the product of that, which seems to run counter to the CBT view. This has been a little challenging and confusing. David and Rhonda discuss this, including new research on the causal links between emotions and thoughts. 6. How do you apply TEAM CBT to worries about real and true things, like a real diagnosis or a tragic event? It would seem that it's not just one's thoughts about it, but an actual threat or upsetting event causing feelings because that is simply how one would feel about. Maybe the thinking is accurate? This, too, has been particularly challenging and confusing, so I'd love to hear more on this. David and Rhonda discuss how thoughts trigger all of your feelings, even after a genuinely tragic event. This week we speak to our wonderful guest Kabir about their experience with BPD (Borderline Personality Disorder) and how BPD affects relationships. We also discuss being queer, how this relates to having BPD and what their experience was like growing up.We begin by hearing how BPD affects Kabir's daily life, and what BPD feels like, before getting into how Kabir's relationships have been formulated and impacted by having BPD and being queer. We hear a little bit about what dating with BPD has looked like for Kabir, and they tell us that casual dating with BPD can be difficult, and how embracing their queerness has been instrumental in learning to know and love themself for who they truly are.Growing up with any type of mental illness or personality disorder can be difficult, and it can be made more difficult when you're very visibly queer, especially as a brown person in a predominantly white, straight, and rich circle of West London. Kabir tells us about how their embodiment of queerness growing up was somewhat protective, and how this impacted their relationships with those around them. "Yeah, I'm definitely learning as this pandemic it goes through its phases and me getting back into the work field and dealing with customer service, I'm noticing a lot of negative interaction with customers and things like that. And it really affects my mood and I'm realizing how one little thing can set my whole day off for me. And I was diagnosed with BPD almost a year ago now, and it hasn't even been a year yet since I was given that diagnosis. So it's been something that I've been working on and yeah, I'm noticing a lot in the customer service industry." We asked Dr. Kobernick to come back (her first episode is here: https://hopetorecharge.com/episode/dialectical-behavior-therapy-dr-chaya-lieba-kobernick/) because I have been getting tons of questions about Radical Acceptance, which is a building block of Dialectical Behavior Therapy (DBT). I've also noticed that our most popular episodes have been those about BPD (Borderline Personality Disorder) and DBT so, I knew we needed an expert to come in and talk about it some more.Radical acceptance is the most difficult skill to master in DBT. Normally, the best way to go about it is to start with small, immediate problems like burnt food or missing the bus. Once you are able to accept inconveniences like those, you can start working on accepting the more difficult parts of life like trauma or illness.‘Dialectical' means that there are two things in opposition to each other that simultaneously exist. The major dialectic in DBT is acceptance and change. We can both fully accept the way things are right now and work toward changing ourselves and our situation for a better future, but the acceptance part is not done in order to bring about change. It is done for the parts of life that we cannot necessarily change the outcome ourselves, at least not in this very moment, and we need to accept this moment.Everyone experiences pain. It is inevitable. Suffering, however, is not. When we choose to accept our pain, we can experience freedom instead of suffering. Fighting against what's happening and focusing on how unfair and unjust it is only leads to misery. We can be sad about our current situation and grieve loss and still accept that it is reality. You'll know you have accepted a situation when you stop fighting for reality to be different or hiding from what's happening. Radical acceptance is almost never a one-time decision. After you have chosen to accept something in your reality, you will be reminded of the difficulty of it again and again, and you must choose to accept it every time. It should gradually get easier for each problem.To experience less distress, we need to realize that all emotions are welcome and needed in life. They serve different purposes. Happiness or bliss should not be one's goal. A happier life includes every emotion at its proper time. The goal of DBT is to build a life worth living, not to feel only happiness.Dr. Kobernick works with teens who have multiple problems and need the support of full DBT treatment. Teen treatment always includes family members because it is almost impossible for a kid to implement the changes needed in their life without the whole family system cooperating or at least one advocate being in the home with them. BPD is a type of ‘personality disorder’. It is an illness that makes you struggle with your emotions and this can affect your relationships with other people. Around 1 in 100 people have BPD. It seems to affect men and women equally, but women are more likely to have this diagnosis. This may be because men are less likely to ask for help. Everyone will experience BPD differently. If you have BPD, you may have problems with:feeling isolated or abandoned by others,self-harming or suicidal thoughts,coping with stress,getting on with other people,strong emotions that you find hard to cope with,misusing alcohol and prescription drugs,illegal drugs and substances,understanding other people’s points of view,staying in work,having a long-term relationship, orbeing able to maintain a home.It is called ‘borderline’ because doctors used to think it was on the border between two different disorders: neurosis and psychosis. Doctors no longer like to use these terms to describe mental illness. It is sometimes called Emotionally Unstable Personality Disorder (EUPD). Some people feel that this describes the illness better.Some people with a personality disorder think that the name is insulting or makes them feel labeled. Could you or someone you know be a victim or a perpetrator of parental alienation? In this episode I interview Jon Smith. He is a targeted parent of parental alienation. In 2015, he had his 3 children dramatically taken from his home. Ever since that day he has tried to make sense of his world being flipped upside down by his estranged ex-wife. Fighting to have equal rights to his children, the judicial system in these cases; have failed him and millions of other families over the decades. Jon now advocates for this movement and is working hard to get the laws changed. Parental alienation is a form of spousal domestic violence, whereas innocent children are used as pawns in a relationally destructive adult mind game through pathogenic parenting techniques. Parental alienation begins with a psychologically distressed parent having severe feelings of inadequacy and relational dysfunctions stemming from their own childhood trauma pertaining to a caregiver This typically results in the development of a personality disorder such as NPD (Narcissistic Personality Disorder) and/or BPD (Borderline Personality Disorder) whereas the individual develops an emotional cutoff to normal range empathy. They retaliate against their inner demons by acting out in ways which are destructive themselves as well as to the family unit, such as excessive overspending, substance abuse, adultery and/or psychotic behavior which often leads to divorce. They are then unable to process the psychological mourning surrounding divorce and project their inadequacies on to their ex, who becomes the targeted parent. The targeted parent is slandered and persecuted by their ex as a form of emotional restitution through the re-enactment of the NPD/BPD parent's childhood scenario. In this fantasy role play, the targeted parent must become the presumed villain and the children must become the presumed victim in order for the personality disordered parent to play the restitutional role of the hero that they never had. The children become trapped in this vicious cycle and are either obstructed or completely erased from having normal range relationship dynamics with their otherwise loving, targeted parent. This pathogenic style of parenting is a form of psychological child abuse and the related trauma hinders the child's emotional development. Pathogenically parented alienated children are more likely to develop their own life long attachment based disorder as this psychological sickness is passed onward to the next generation and becomes a further breeding grown for non empathetic, widespread relational destruction of family values. Targeted parents additionally suffer from PTSD type symptoms including depression and anxiety which often leads to physical health issues, substance abuse or even suicide. Parental alienation is literally a mental health care crisis estimated to affect over 22 million families in the United States alone.
