Welcome to the Sustainable Clinical Medicine Podcast, where we delve into strategies for creating a more balanced and fulfilling career in the medical field. In this episode, your host, Dr. Sarah Smith, is joined by Dr. Michael Hersch, a gastroenterologist from the northwest suburbs of Chicago, who shares his compelling journey towards achieving a sustainable work life. Dr. Hersch opens up about his experiences with career burnout and the profound impact it had on his professional and personal life. Through coaching, he discovered the importance of asking critical questions about what he truly wanted from his work and life. He candidly discusses the challenge of embracing vulnerability as a strength, particularly for male physicians, and how incremental changes like a 30-minute lunch break can lead to substantial improvements in mood and efficiency. Join us as Dr. Hersch shares how redefining personal boundaries and managing time intentionally have helped him create a more rewarding clinical practice without sacrificing his well-being. Here are 3 key takeaways from this episode: Intentional Time Management: Implementing small adjustments like designating Monday mornings for administrative tasks and introducing a 30-minute lunch break led to enhanced focus and preparation for the day. Such changes can profoundly improve mood and productivity without compromising efficiency. Vulnerability as Strength: Embracing vulnerability is vital, especially for male physicians facing societal expectations. Discovering this aspect can facilitate personal growth and open opportunities to seek help, improving overall work-life balance. Navigating Skepticism: Encountering and embracing skepticism can uncover areas that require attention and transformation. My journey through skepticism led me into entrepreneurship, enabling me to assist fellow physicians in reshaping their career experiences. Dr. Michael Hersh Bio: Dr. Michael Hersh is a Gastroenterologist, husband, father, podcaster, and physician coach at Better Physician Life Coaching. He coaches physicians on creating work-life balance, goal setting, stress management, reducing overwhelm, and feeling more present at home and less annoyed at work. His goal is to help physicians stop feeling trapped in medicine so that they can finally enjoy the life they worked so hard to build. He also co-hosts a podcast called Doctors Living Deliberately, highlighting physicians changing medicine by learning to live life with intention and purpose. -------------- Would you like to view a transcript of this episode? Send us a Text Message. What happens when two accomplished physicians decide to pivot from traditional medical practice to coaching? Our guests, Dr. Michael Hersch and Dr. Arpita Gupta DePalma, reveal the transformative power of coaching and how it has rekindled their passion for helping others. Dr. DePalma shares her poignant tale of choosing her family over a demanding medical career, eventually experiencing burnout in administrative roles. The onset of the COVID-19 pandemic became a catalyst for her to discover coaching's true potential. Dr. Hersch, a seasoned gastroenterologist, gives us a glimpse into his own journey towards reintegrating humanity and compassion into his professional life.Stigma surrounding mental health and coaching can be particularly challenging within certain cultural contexts.  Over time, however, they found that coaching addressed long-standing issues like bottled-up anger and strained family ties, leading to newfound happiness and improved relationships. This section underscores the harmful belief that individuals should tackle life's challenges alone and how coaching helps dismantle these limiting paradigms for personal growth and fulfillment.Focusing on the unique challenges faced by physicians, we delve into the societal and professional expectations that discourage vulnerability, especially among male doctors. The discussion broadens to compare coaching's acceptance in other fields, asserting that while not everyone may need a coach, everyone deserves one. Personal anecdotes illustrate the journey toward recognizing self-worth beyond professional achievements. We also touch upon the importance of community, therapy, and life coaching in fostering personal and professional development. — “Life is everywhere, and we are connected to it. In the web of life, everything has a purpose and is part of the ecology of the planet. Our dogs help me see all this. It starts with their curiosity, their need to inspect every little thing on a walk and then do it again the next day. Every being has a purpose. Follow a dog or a child, explore a little, ask one question, which leads to another, and then a whole new world appears before you. Children and dogs. On a walk, the most important thing to do is be curious. We walk for all sorts of reasons. We decompress. We count steps. We listen to music. We make lists. If we are not alone, we talk. Yet if we follow our dog's nose, if we spend a bit of time wondering about where that plant, that flower, that mysterious jackrabbit — their head and ears peeking out from a bush — came from, and we do it repeatedly, the biosphere emerges before us as astonishing and interconnected. We walk in a sacred space. Life connected; life dependent. Nothing stands, walks, runs, creeps, swims, slithers, sends out shoots or roots independently. Every living creature has a past, a history that takes it back to those single-cell ancestors four billion years ago. It is the great Oneness.” Valeria interviews Hersch Wilson — He is the author of  “Dog Lessons: Learning the Important Stuff from Our Best Friends, Play To Win: Choosing Growth Over Fear in Work and Life, Firefighter Zen: A Field Guide to Thriving in Tough Times and Test of Faith - A Novel of Faith and Murder in the Southwest.” Hersch Wilson has worked (as in jobs he was actually paid for) as a corn pollinator, a ferris wheel operator, a short-order cook, a ballet dancer, an outdoor educator, a soccer coach, a leadership consultant, a pilot, and a writer. The ferris wheel operating was the strangest job. Lots of physics involved. Along the way, he was also a volunteer firefighter-EMT for thirty-three years which culminated in the award-winning book, “Firefighter Zen, A Field Guide for Thriving in Tough Times.“ Most importantly he is the partner of Laurie Wilson, father of two daughters and grandfather of two. For over sixty years he's also been a dog guardian. He has co-written three national business bestsellers with his father, Larry Wilson, including the award-winning, “Play to Win: Choosing Growth Over Fear in Work and in Life”. Hersch attended Colorado College and quit his junior year to follow his passion, dance. He performed in Canada, Switzerland, and the United States. After that career ended, he graduated with a BA in English from the University of Minnesota. Hersch and Laurie live in Santa Fe, New Mexico with their daughter Sully and two rescue dogs, Toby, a Great Pyrenees, and Maisie, a terrier-chihuahua mix. His latest book is “Dog Lessons, Learning the Important Stuff from Our Best Friends.” You can follow on Medium.com (@HerschWilson), on X (Twitter) (@Bravingfires), on Instagram (@herschwilson)and on his website, HerschWilson.com. In Dark Star: A New History of the Space Shuttle (MIT Press, 2023), Dr. Matthew Hersch challenges the existing narrative of the most significant human space program of the last 50 years, NASA's space shuttle. He begins with the origins of the space shuttle: a century-long effort to develop a low-cost, reusable, rocket-powered airplane to militarize and commercialize space travel, which Hersch explains was built the wrong way, at the wrong time, and for all the wrong reasons. Describing the unique circumstances that led to the space shuttle's creation by President Richard Nixon's administration in 1972 and its subsequent flights from 1981 through 2011, Dr. Hersch illustrates how the space shuttle was doomed from the start. While most historians have accepted the view that the space shuttle's fatal accidents—including the 1986 Challenger explosion—resulted from deficiencies in NASA's management culture that lulled engineers into a false confidence in the craft, Dark Star reveals the widespread understanding that the shuttle was predestined for failure as a technology demonstrator. The vehicle was intended only to give the United States the appearance of a viable human spaceflight program until funds became available to eliminate its obvious flaws. Hersch's work seeks to answer the perilous questions of technological choice that confront every generation, and it is a critical read for anyone interested in how we can create a better world through the things we build.  This interview was conducted by Dr. Miranda Melcher whose forthcoming book focuses on post-conflict military integration, understanding treaty negotiation and implementation in civil war contexts, with qualitative analysis of the Angolan and Mozambican civil wars. What to Watch For When You Take On Capital As a Restaurant Startup

In today's podcast, we interview Robert Hersch of Mastodon Ventures and discuss the market for raising capital and what you should be doing to prepare. We break down a variety of ideas. What are some areas you are watching for capital deployment in 2024 within the restaurant sectors? How the market will respond to consumer spending pressure? Will brands grow in 2024?Is franchising a viable option for early-stage brand growth? How do you go about selecting founders and understanding the business mission? What's most important to invest in brands or assist brands to recover? What are the best capital structures for startup brands? My 8-Step Plan for Raising Capital As a startup, this can be a daunting task, but it's also an essential part of growing your business. Here are some of the best steps for taking on capital and what to watch out for:1. Know your funding options. There are a variety of ways to raise capital for your startup, including:Bootstrapping involves using your money and/or reinvesting profits to fund your business.Friends and family: This is used for a pre-proof of concept and can be a good way to get early-stage funding from people who know and trust you.Angel investors: These are high-net-worth individuals who invest in early-stage startups and are more likely to invest in you vs the startup so make sure you have all your accolades and history out there and easy to find.Venture capitalists are professional investors investing in startups with high growth potential, looking at full return on investment. This is where your P&L performance comes into play; make sure you have a top-notch accounting firm and attorney to prep for these engagements.Crowdfunding: This involves raising money from many people, typically through online platforms. While somewhat new, this model is gaining more popularity, especially if you have a brand and a following - you may be able to tap into your most loyal customers, fans, and advocates.Grants: These are non-repayable funds typically awarded by the government or non-profit organizations. This will need a professional grant writer and an angle that provides significant benefit to society along with a vision and business plan that is easy to understand.2. Determine how much funding you need. This will depend on your stage of development, your business model, and your burn rate (the rate at which you are spending money). This is also an area that can become very sticky with valuations of your business or idea, which you may think is more than the investor - get a third party to help you evaluate.3. Write a business plan. This is a document that outlines your business strategy, including your target market, your competitive landscape, and your financial projections. You matter more at this stage, and the narrative is the killer app, so know what your mission is and understand the total addressable market along with the competition and a good SWAT plan is always welcome.4. Identify potential investors. Research investors with a track record of investing in startups in your industry. This is critical and is kind of like choosing a family member - choose very wisely this can come back on you very easily.5. Network and make connections. Attend industry events, join online forums, and connect with other entrepreneurs. But best of all referrals and high-value connections are the ones that will make a difference in getting your idea out there to those who matter most.6. Practice your pitch. Your pitch is a short presentation that you will give to potential investors to explain your business. Focus on the mission and revenue opportunity - don't get too tactical, and try to keep it simple.7. Be prepared to negotiate. Investors will want to make sure that their investment is protected, so be ready to negotiate terms such as equity and board representation. Structure your deal or term sheet with a full understanding of what you are giving up as a founder - don't write yourself out of the deal just to get it going, unless of course, you are looking for an exit.8. Be patient. Raising capital can take time, so be patient and persistent. Here are some things to watch out for when taking on capital:Dilution: This is giving up ownership of your company in exchange for funding. Make sure you understand the dilution terms of any investment before you agree to it.Warrants: These are financial instruments that give investors the right to buy shares of your company at a specific price in the future. Be aware of the terms of any warrants before you accept them.Liquidation preferences: These terms dictate how investors will be paid if your company is sold or goes bankrupt. Make sure you understand the liquidation preferences of any investment before you agree to it.Raising capital for your startup can be a complex process, but it is also essential to growing your business. Make sure to get good legal and financial representation to start the process right, you will thank me later if you do this alone!

Have you played any of these modern classics at family game night? You have Brian Hersch to thank. Now a General Partner at Hersch and Company, with over $850 million in sales under his belt, Brian didn't set out to design games. His first career was in real estate development, where he honed a keen sense for business. Then came Trivial Pursuit. When the trivia board game first burst onto the scene in the 1980s, Brian was a natural, so much so that his friends urged him to use his creative energies to invent a game of his own. Bringing his business experience to bear on the idea, he set himself to doing some market research, and found that the game's key feature—its social interactivity—was its most undervalued and under-marketed aspect."I'd played games my whole life," says Brian. "It was second nature. Some people watch television and say, 'I want to go into television.' Nobody who plays games says they want to go into games, at least not from our generation."Brian (a fan of the show who's currently listening to Rabbi Yoshi's interview with Betsy Borns) now runs Hersch and Company with his brother, and is famous for being a creator and proponent of social interactive games.Hersch has said that the reason he went into adult social games was because they served as a lubrication for rusty social skills. Children are less inhibited, and are more ready, willing, and able to have fun at any moment."They don't carry the burdens of life, the responsibilities, the weight of memories both good and bad," Brian says. "Can we facilitate a good time amongst a group of people who may or may not know each other? Can we find the common spark that they share so that they discover that there's something they can enjoy with the same feeling and laughter and pleasure? That's the biggest part of it. My games have always been group games."His titles have now sold over 45 million copies through strategic relationships with companies such as Milton Bradley, Parker Brothers, Mattel, and Western Publishing.

Synopsis On today's date in 2002, Mariss Jansons led the Pittsburg Symphony in the premiere performance of the Second Symphony written by a then 32-year-old American composer named Michael Hersch. Hardly a child prodigy, Michael Hersch was introduced to classical music at age 18 by his brother Jamie, who showed him a videotape of Georg Solti conducting Beethoven's Fifth. That experience shook him. "It scrambled everything.” Hersch recalled. “That's when I knew that I was to be a composer... My whole life started over at that moment." Hersch certainly made up for lost time, exhibiting an uncanny ability to master both the piano and the intricacies of contemporary compositional techniques in less than a decade. His first success as a composer came when his Elegy for Strings won a major prize and was conducted by Marin Alsop at Lincoln Center in New York in 1997. Since then, his works have been commissioned and performed by many other leading orchestras and performers. Hersch's Symphony No. 2 has no stated program, but it was composed shortly after the events of September 11, 2001, and knowing that, it's hard to disassociate the score's violent opening and subsequent elegiac mood from that tragic moment in American history. Some of the greatest minds in physics have grappled with how superconductivity works to drive electrical resistance to 0. The 1972 Nobel prize in Physics was awarded to John Bardeen, Leon Neil Cooper, and John Robert Schrieffer "for their BCS theory of superconductivity. Now the race is on to get the highest temperature superconductor possible; another Nobel Prize was awarded just for getting the temperature up to 35K or -396 Fahrenheit! Seymour Hersh, a journalist with a checkered record of sensational, but not always accurate, reporting claims that on September 26, 2022, a covert action ordered by President Joe Biden blew up three of four Russian undersea pipelines known as Nordstream 1 and 2. Based upon what appears to be a single source, Hersch describes how months earlier, Navy divers executed a complex and highly classified operation to clandestinely put in place remotely controlled explosives.  While this account may or may not prove to be true, we do know that forty years before, another president ordered the CIA to take steps that spectacularly sabotaged a Siberian gas pipeline – causing the most massive non-nuclear explosion ever and catalyzing Ronald Reagan's take-down of the Soviet Union and its evil empire. Joe Biden is no Ronald Reagan. But did he reprise one of Reagan's greatest achievements?  This is Frank Gaffney.

EPISODE 47 A-Block (1:46) SPECIAL COMMENT: Trump's appeal of the "Special Master Compromise" is just a stalling tactic so he can time his 2024 announcement AFTER the Mid-terms but BEFORE he's indicted (2:16) He knows announcing before 11/8/22 will undercut many election deniers he needs in place in '24. He may even believe the DOJ won't indict him if he's an announced candidate. (4:00) Whatever the specifics, the appeal to Clarence Thomas at the Supreme Court is nonsense: it might as well be 35 blank pages (6:11) The NYT notes he didn't even appeal the ruling that lets DOJ continue using the classified stolen documents in its investigation of him. (7:27) Trump could also be deflecting legal attention from his next real problem: The trial of The Oath Keepers (11:00) Prosecutors played a tape of a November 2020 meeting to plan a violent insurrection and if they can tie it to Trump's inner circle, he's meat. B-Block (14:45) EVERY DOG HAS ITS DAY: Ranger in Greenville, Tennessee. (15:48) POSTSCRIPTS TO THE NEWS: The abortion story won't hurt Herschel Walker. His son's videos will. And what happened to the lawsuit, Hersch? (20:08) IN SPORTS: Our long national nightmare is over: Aaron Judge is the new home run champ. And if you don't think so, ask yourself: which is the all-time Strikeout Record: Nolan Ryan's 383 in 1973 or Matt Kilroy's 513 in 1886? (24:19) THE WORST PERSONS IN THE WORLD: Mehmet Oz, torturer of puppies, competes with Roger Waters and David Zaslav of CNN for the honors. C-Block (30:50) THINGS I PROMISED NOT TO TELL: It's the 15th Anniversary of the day MSNBC management told me they'd never let me use Rachel Maddow as my guest host, nor give her her own show, because no woman could get ratings in prime time, and we didn't need a second liberal show, and no LGBTQ woman host would ever attract male viewers! See omnystudio.com/listener for privacy information.
