Kids get cuts and scrapes all the time. Have you ever wondered, should I go to the Emergency Room (ER) for this? Does it need stitches? In this episode of Real Talk With Kid Docs we are joined by Dr. Stephanie Spanos, Pediatric Emergency Medicine physician and Associate Professor of Pediatrics at the University of Utah. We focus our discussion on wounds and lacerations (the fancy word for cuts). Wounds are all unique and whether or not intervention is required depends on a number of variables including location, size, depth, and mechanism of injury.   As with any injury, if you are worried, this is always a reason to seek medical attention. If you have any thought that it might need stitches (the wound is wide open, the edges do not come together well and/or the edges cannot stay together), the sooner you get to medical attention, they better the outcome. Washing it out and addressing it as soon as you can helps prevent infection and increase the likelihood of a good cosmetic outcome.  Home care includes cleaning the wound with lots of water and applying pressure while in transit. Be aware that some areas of the body like the mouth and scalp bleed a LOT. Avoid putting anything else in the wound including soap, hydrogen peroxide, rubbing alcohol, etc. If there is a foreign body stuck on the wound, leave it in place. It is holding pressure on the wound. We hope you enjoy Dr. Spanos as much as we did and that you take away lots of practical tips.

I welcome my new friend Lina Abujamra who invites us on a compelling journey through the ups and downs of faith, the role of suffering, and the unfathomable depths of God's grace. One of my favorite quotes from her book exemplifies this. She says, “mercy is never deserved, but always desired, and the key to unlocking it is simply to receive it.” Amen! She also adds, “God loves us not because we believe it, but despite the fact that we don't.” Amen again!! As I reflect on our conversation, here's the main takeaway: Lina reminded me of the unfathomable-but-so-true depth of grace amidst the chaos of life. Her Christ-centered transformation, her candidness in sharing her struggles, and the indescribable nature of God's grace left me with a very real hope in the grace I have received in Christ alone. May we embrace grace in all its messy, beautiful, and transformative glory, knowing that God's love extends far beyond our moments of weakness and failure. Allow me to tell you a little more. And remember - Lina goes a hundred miles an hour, and our conversation reflects every bit of her 100% passion for Christ! So, as soon as we get started, she dives headfirst into the turbulent waters of suffering, highlighting her experience of God's grace amid hardship. From Lina's childhood in war-torn Lebanon to her struggles with church hurt and not one, but two broken engagements, she reminds us that grace isn't just a feel-good notion; it's a lifeline we cling to in the stormiest of seas. Through Lina's story, we see the transformative power of grace as God teaches her to turn her pain and disappointment into a purposeful journey of healing and growth. A couple weeks ago, Sam Elias texted me and said, "Hey. This ER doctor here in Louisville just sent his first 14c... He's SUPER intelligent and really researched about training, nutrition, and personal optimization. I think y'all should have him on the podcast. He has a lot to offer." Turns out this was his second--not first--14c, which I found out in my interview with him, but everything else Sam said about Thomas was spot on, and I was psyched to reach out to him for an interview. Thomas is a 36-year-old emergency medicine physician and father of 3 children from Louisville, KY who's been climbing for around 20 years. After talking to him for a while, I realized he is SUPER scientific about everything he does in climbing. This is no surprise because he's quite an overachiever in his academic/professional life as well.  He's published a bunch of academic papers, he was chief resident at the University of Louisville Department of Emergency Medicine a while back, and WHILE he was doing that, he started a medical device company, Inscope Medical, and was VP of Innovations. He also completed an IronMan while he was an intern resident. So he's an ambitious person, to say the least, and that means he has less time than some of us for climbing and training.  Only getting outside climbing around 10-15 days per year, and focusing all of that time on very hard projects, he has learned that his training and all of his days outside have to be hyperfocused and specific. He takes us through his training program, including how he trains aerobic capacity, power, power endurance, and strength. He also talks about his research (he's also a researcher!) on the use of a continuous glucose monitor (CGM) to figure out what food fuels him best, and at what times of day. He's currently doing some experimentation with Sam Elias with a CGM, and Thomas and I talked about having a part 2 to this discussion to go more in-depth about that and how he uses other monitors like the Whoop in his every day life and training. More to come on that, though. This interview and our talk afterward actually inspired me to get a Whoop myself (no affiliation, but check it out if you're interested in biodata). Oh, and we also talked about how he rehabbed not one but two pulley ruptures and came back stronger afterward. I loved this talk - it was super inspirational on all levels - and I hope we hear a LOT more from Thomas in the coming years. He will be an asset to the climbing community to take us to the next level in training scientifically, like so many other mainstream sports have already done.

Physicians train for at least 15,000 hours before we are permitted to practice independently, and one of the reasons is that it takes a long time and a lot of patient volume to be exposed to the many different presentations of disease processes to learn how to recognize true emergencies that need immediate interventions. Today's guest brings us a perfect example of the importance of this training. Karen Wilson Saintsing DVM is a veterinarian who nearly lost her arm after a nurse practitioner in an urgent care failed to recognize the onset of compartment syndrome. Fortunately, an astute emergency physician immediately recognized the condition and transferred her to a surgical center for treatment.  To help us understand compartment syndrome, we are also joined by trauma surgeon Stephanie Markle DO MPH.

Thomas Fisher is an emergency room doc in Chicago. His book, The Emergency, is an up-close chronicle of the COVID pandemic's first year in his South Side ER. It also zooms out to tell the story of his journey as a doctor: How his upbringing on the South Side fueled his desire to become a doctor. And how the realities and inequities of American health care limited his ability to help. He details how the failures of the American health care system — and the racial inequities it perpetuates — leave health care workers with a profound sense of moral injury. “Over time, when you have this conflict between what you can do and what you're supposed to do—what you wish you could do, what you're trained to do—that creates a moral conundrum….It also leads a lot of people to leave the profession ” For a time, Fisher himself stepped away from practicing medicine.
