The DSM is widely used by psychiatrists, psychologists, and other mental health professionals to not only diagnose individuals with mental health disorders, but also to submit codes for insurance reimbursement for those receiving treatment. As important as it is, there are some major flaws with its structure. Dr. Abramowitz takes a deep dive into some of the major flaws of this classification system, including the lack of an underlying theory and issues with reliability and validity. Multiple examples of the current issues with the DSM are applied to clinical situations, and Dr. Abramowitz begins to discuss solutions to these problems. Specifically, they take an in-depth look at anxiety and obsessive compulsive and related disorders. Dr. Abramowitz discusses the main differences between OCD and other disorders, such as hair-pulling (trichotillomania), and how focusing on the mechanisms of each disorder is the most important part so someone can receive effective treatment. He also shares information about an alternative model to the DSM focused on biological processes; however, this model is not without its own issues. Despite its many problems, Dr. Duncan and Dr. Abramowitz concede the DSM is the best tool clinicians and other mental health professionals have, but it should continue to be criticized so it can be improved for years to come.     Dr. Scott Waltman on common pitfalls learning CBT and Socratic questioning

    On this episode of “Sanity” Dr. Jason Duncan and Dr. Scott Waltman discuss some of the pitfalls mental health professionals make when implementing cognitive-behavioral therapy (CBT). The most common pitfalls identified in research completed by Dr. Waltman included socratic questioning and guided discovery. Dr. Waltman and Dr. Duncan discuss socratic questioning, its origin with the Greek philosopher Socrates, and how it can be an effective tool in therapy. They provide numerous benefits to this intervention, including helping clients “become their own therapists.” Dr. Waltman discloses the framework for honing the skill of socratic questioning from his new book, Socratic Questioning for Therapists and Counselors: Learn How to Think and Intervene Like a Cognitive Behavior Therapist. Dr. Waltman also identifies a number of other pitfalls therapists make when implementing CBT, such as setting an agenda, pacing the session, receiving feedback from clients, reviewing and developing individualized homework, trouble selecting when to implement interventions and deciding where to intervene. He describes his own therapeutic techniques and how to combat some of these pitfalls. Dr. Duncan and Dr. Waltman both emphasize the importance and difficulty of learning these important skills. They described numerous resources for therapists interested in learning more about CBT, including websites, research studies, books, trainings, and certificates. Dr. Waltman is a clinical psychologist, international trainer, and practice-based researcher. His interests include evidence-based psychotherapy practice, training, and implementation in systems that provide care to underserved populations.     On this episode of Sanity, Dr. Jason Duncan and Dr. Katherine Dondanville cover post-traumatic stress disorder (PTSD) and its various treatments. Dr. Dondanville is board-certified in cognitive and behavioral psychology, and she specializes in a specific treatment for PTSD called cognitive processing therapy (CPT). CPT is a time-limited (around 12 sessions) approach which provides clients with background information on cognitive theories, explores negative beliefs, called “stuck-points” they developed related to their trauma, and then teaches them skills to challenge these beliefs. Dr. Dondanville explains CPT also addresses five themes often affected by traumatic events: safety, power/control, esteem, intimacy, and trust. Other treatment options for PTSD, such as prolonged exposure (PE) and eye movement desensitization and reprocessing (EMDR) treatment are also covered. Dr. Dondanville provides numerous examples of how the components of CPT are delivered in therapy. She also shares a multitude of resources accessible to clinicians or people who are struggling with PTSD themselves. Traditionally, exposure therapy was believed to work due to habituation, or the diminishing of a response to the feared or anxiety-provoking stimulus. Dr. Loerinc Guinyard describes how recent research suggests exposure therapy works because an individual is learning the previously feared stimuli no longer produces an anxiety response, known as the inhibitory learning model. Another aspect of this model includes an individual learning even if they are in an anxiety-provoking situation, they can tolerate and cope with those emotions. Positive Affect Treatment, a new therapy developed in part by Dr. Loerinc Guinyard, is also covered. This therapy is a transdiagnostic approach, meaning it can be useful for a number of different disorders, and its focus, not surprisingly, is on increasing positive emotional experiences, thoughts, and enhancing compassion.     On this episode of Sanity, Dr. Jason Duncan and Dr. Elizabeth Turk-Karan discuss how exposure therapy works, and how it is used to treat a number of different anxiety-related disorders. Dr. Turk-Karan shares how we all need some levels of anxiety to function properly, but sometimes, our anxiety can be triggered inappropriately. When this happens, exposure can be used to reduce the anxiety we feel. Dr. Turk-Karan explains how exposures are set up collaboratively with patients, and they include a hierarchy, from least to most anxiety-provoking. Patients are asked to “climb the ladder” and repeat these exposures for homework. Exposure works best when it induces sufficient anxiety, the patient's anxiety levels drop, and when they are done repeatedly over time, not just in the office. As Dr. Turk-Karan explains, while this therapy sounds tough, trained therapists are there to support their patients every step of the way, and they would never ask a patient to do something they would not do themselves.
