In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Professor Alejandra Martinez to discuss the Chemotherapy Response Score (CRS) in ovarian cancer. Prof. Martinez has been the Deputy Head of the Surgical Oncology Department at Toulouse University Cancer Center (Institut Universitaire du Cancer de Toulouse) since 2016. She has served as the Director of the ESGO fellowship program of the IUCTO since 2017 and as a professor in oncology since 2021. Highlights: - CRS is an easy and reproductible biomarker that can help to stratify a patient's treatment. - CRS 1-2 is associated with higher tumor load, more extensive surgical procedures, residual tumor after cytoreduction, and early relapse. - Pathology response measured by the CRS is associated with disease-free and overall survival in patients with stage IIIC/IV treated with neoadjuvant chemotherapy irrespective of the number of cycles.

In this episode, Natalia Garzon sits down with Alejandra Martinez and Jorge Sosa, who wrote and composed "Buford Highway Dreams" a 10-min opera inspired by the life of Guillermo Cesario, who lives and works on Buford Highway. We interviewed him on this podcast in 2021. This project was done in collaboration with We Love Buford Highway as part of the 96 Hour Opera Project, organized by the Atlanta Opera in June of 2022. This is the first opera ever created about Buford Highway immigrants. The two artists talk about the operatic art form, why it is important, how they created "Buford Highway Dreams," and how the themes of the piece complexify the migrant narrative in the area. The first part of this conversation gives a brief overview of the work, the art form, and some of the elements the creators found memorable and meaningful. They also open up conversations about the "American Dream," a concept that deserves more reflection and complexity in our day and age. Dr. Martinez-Gomez is an author of the Lead Article for IJGC’s January 2021 issue, “Urinary diversion after pelvic exenteration for gynecologic malignancies” (https://ijgc.bmj.com/content/early/2020/11/22/ijgc-2020-002015). Dr. Martinez-Gomez is a gynecologist oncologist at Institut Universitaire du Cancer de Toulouse, France, from 2016, where he performed the ESGO fellowship program under the supervision of Drs. Gwénaël Ferron and Alejandra Martinez. His areas of interest are immunology of cervical and ovarian neoplasms, advanced ovarian cancer, reconstructive surgery after pelvic exenteration, and surgical teaching. Highlights: • Pelvic exenteration is a demanding surgery requiring mastery of organ removal combined with functional reconstruction • Pelvic exenteration should be performed in high-volume referral institutions by well-trained multidisciplinary teams • Incontinent diversion represents a good option for unfit patients or those refusing the constraints entailed by continent diversions (self-catheterization, postoperative learning, engagement) • Postoperative complications of both types of reconstruction are comparable, but the quality of life issues would seem to support continent diversions in young and fit patients • Continent and non-contingent techniques should be offered to patients, with a final decision based on their preferences

At a young age Alejandra became a teen mom however, no matter what others thought of her situation, she didn't allow it to hold her back. She defeated all odds and went on to attain her bachelor's degree in Business Administration concentrated in Human Resources from Texas A & M University – San Antonio. She started her professional career in the corporate world as a Corporate Trainer in 2014 and after 5 years she knew there was more to life than just the 8-5 job. Alejandra and her husband decided to pursue real estate investing as another source of income and to eventually reach financial freedom. She then went on to attain her Realtor License to help with the investments but soon after realized she loved educating and helping others, so now she's helping buyers, sellers, and investors. Alejandra has now acquired 9 doors in just under 2 years since making the decision to invest in real estate. Alejandra and her husband have recently made their business official, GNA Properties! She certainly makes juggling the corporate world, realtor, property manager, real estate investor, wife and being a mother sound easy, however she has stated it's not easy but her WHY is strong for her family.

Alejandra talks about the incredibly challenging conditions faced by people who are migrating and seeking asylum and the inhumane U.S. policies such as an illegal waitlist, the highly controversial family separation, and the MPP-Migrant Protection Protocol law, also known as the “Remain in Mexico” policy, as well as how U.S. asylum procedures are constantly changing and unpredictable. She explains that she and her team provide a safe, supportive space in Tijuana, where they acknowledge the trauma and long journey many asylum seekers and migrants have experienced. Alejandra discusses how the Border Rights Project provides legal orientation and a “Know Your Rights” session for asylum seekers, as well as connects them to additional services such as shelters and medical care. She tells how Al Otro Lado recently reunited 29 families who had been separated, and she shares how she got into this work. It's effectively creepy and gives the Viewer access into how this Raul (I will explain in a moment) sees the world. This camera lens is a sort of protective shield from, well, Realism. We rarely see him without it and when he speaks, it's almost childlike "Little bird, little bird.. where are you?" He also carries a small dog under one arm. The thing that truly ASTONISHED me as I was watching was that I thought to myself "This guy is an oddball, creepy, yes, but harmless.. And he has a dog!" Raul lives in an empty studio apartment with bare-white walls, completely devoid of any furniture. His window is blocked out with newspaper and tape. He runs in circles around the room every now and then pausing at the covered window. Hesitant, he lifts a taped corner and there SHE is.. Alejandra Martinez, twenty-nine, the object of Raul's obsession, hanging her lacy delicates on the clothesline just outside his window. Alex and Alejandra Martinez live in the flat below. Raul has constant audio surveillance on the couple, portrayed by Isabel Orizaga and Sergio Valdez, recording and listening in on their every conversation. He learns of their marital problems, how they met at the Circus, every intimate detail of their lives. The audio surveillance is more than just a part of the plot, Ruiz uses it to facilitate a unique piece of sound design. Raul often fast-forwards through anything that doesn't feed his fantastical fetish. Mundane conversation is sped-through resulting in high-pitched gibberish, while other words or phrases, particularly those spoken by Alejandra captivate his attention and he will play them over and over again as if consuming her words and making them a part of his story. It's an effective tool of showing how the editing of emotional matter in the lives of others becomes immersed within him and when his darker, predatory self takes over, he is able to hear only what he wishes. (Well- demonstrated when the audio is out of sync or altered as the characters speak.) Then we meet another Raul – this Man struts up the street with the confidence of a gigolo. He wears shades, carries a gun and runs around the rooftop of his building firing empty chambers. This is a different man, perhaps not yet dangerous but getting impatient and ready for action. The Music changes when Raul shifts from his reluctant voyeuristic self to the brazen, greedy, predator who is preparing to make his move. The film is designed to provoke interpretive discussion. Every shot, every element of set design, every choice of music or editing sequence is to reflect Raul's inner turmoil and eventual consumption by his dark desires and violent machinations. Something that I found most unique and remarkable is a technique of freezing frame so that we are watching a series of still shots while the dialogue occurring in real time continues without pause. It's an effective technique to further show the disassociation of Raul from making any genuine connections to the world/people/friends around him. Overall, this is an impressive film with many layers and a compendium of stylistic, directorial maneuvers. Ruiz is not afraid to go off the border and explore different genres of film. "The Neighbor" is a provocative thriller with an avant-garde twist that also explores some serious sociological and cultural issues with identity and learned behavior according to what society deems politically correct. Why is it that we are so concerned with being "polite" that we would go against our human instinct so as not to be perceived as rude when we innately feel a threat is near? With the characterization of the Martinez's, Ruiz makes some disturbing ruminations on why we so often ignore our primal senses when something sinister is close at hand. It might be just next door
