In this Bell Work Talks Dr. Goolsby discusses the processed used to develop consensus recommendations for healthcare in mass shootings. This project was based in part on the fact that in 2021 702 people died in mass shooting incidents in the US. To define best healthcare response to these incidents, the Uniformed Services University's National Center for Disaster Medicine and Public Health hosted a consensus conference of emergency medical services (EMS) clinicians, emergency medicine physicians (EM), and surgeons who provided medical response to six of the nation's largest recent mass shootings. The eight recommendations addressed readiness training, public education, triage, communication, patient tracking, medical records, family reunification, and mental health services for responders. Craig Goolsby, MD, MEd, MHCDS, FACEP is Chair of the Department of Emergency Medicine at Harbor-UCLA Medical Center and a Professor of Clinical Emergency Medicine at the David Geffen School of Medicine at UCLA. He is a member of the International Liaison Committee on Resuscitation's First Aid Task Force, the American Red Cross Scientific Advisory Council, and is a leader in medical education and military-to-civilian knowledge transfer, particularly for the Stop the Bleed Campaign. His multi-million-dollar research portfolio has focused on bleeding control, tourniquet usage, first aid and public response, mass casualty incidents, and developing novel educational programs and tools. Dr. Goolsby is a distinguished graduate of the United States Air Force Academy (BS), an AOA graduate of the Tulane University School of Medicine (MD), earned his Master of Education in the Health Professions degree from Johns Hopkins University, and completed a Master of Healthcare Delivery Science degree at Dartmouth College's Tuck School of Business. He has received many awards celebrating his work. Dr. Goolsby served in the Uniformed Services University's Department of Military and Emergency Medicine for a decade prior to his current position. He also previously served as an active-duty military officer, including two tours as the flight commander and medical director of the Air Force's emergency department in Balad, Iraq. Dr. Goolsby is the author or co-author of more than 70 peer-reviewed articles, chapters, editorials, and other scholarly publications. He is a board-certified, practicing emergency physician. Professor Mara Kyrgiou is a Professor at Imperial College London and a Consultant Surgeon in Gynaecology and Gynaecological Oncology at the West London Gynaecological Cancer Centre, Imperial Healthcare NHS Trust. She is the Head of Section of Gynaecological Oncology at Imperial College London. Her main clinical interest is minimal access and complex gynaecological cancer surgery. Highlights: - Local treatment techniques for cervical pre-invasive and early invasive disease were previously thought to be interchangeable with regards to treatment failure rates and associated with minor and infrequent complications. - The findings of a network metaanalysis (Athanasiou Lancet Oncology 2022) suggest that more aggressive treatments (i.e., CKC, laser conisation) are associated with lower recurrence rates but higher risk for subsequent preterm birth. - The risk of reproductive morbidity is directly associated with the length of the cone. - This paper by ESGO/EFC/IFCPC describes a consensus terminology of the cone dimensions to make studies addressing effectiveness and safety of SIL/CIN treatment comparable and facilitate their use to tailor surveillance and antenatal management. - The 2022 terminology should replace all previous terminologies, which will facilitate communication between clinicians and foster tailored treatment guidelines that balance obstetrical harm against therapeutic effectiveness. - Universal agreed reporting will further facilitate communications among clinicians and pathologists, promote audit of practice and future research, and improve the quality of future meta-analyses.

Dr. Kara Radzak interviews Dr. Brianna Mills, Ron Courson, Dr. James Ellis, and Dr. Stanley Herring about their recent papers titled "Consensus Recommendations on the Prehospital Care of the Injured Athlete With a Suspected Catastrophic Cervical Spine Injury" and "Best Practices and Current Care Concepts in Prehospital Care of the Spine-Injured Athlete in American Tackle Football."   Dr. Brianna Mills is a Research Scientist at the Harborview Injury Prevention and Research Center at the University of Washington. Dr. Mills was responsible for leading the the consensus building process and served as the first authors of the consensus paper discussed in this video.   Ron Courson has served as Director of Sports Medicine with the University of Georgia Athletic Association since 1995. He previously served as Director of Rehabilitation at the University of Alabama from 1991-1995 and Director of Rehabilitation with Samford University from 1989-1991. Courson received his undergraduate degree in education/physical education from Samford University, performed two years of graduate work at the University of Tennessee-Chattanooga, and graduated with honors from the Medical College of Georgia with a degree in physical therapy. Courson is a national registered advanced emergency medical technician as well as a certified strength and conditioning specialist by the National Strength and Conditioning Association.   Stanley A. Herring, MD, is senior medical advisor and co-founder of The Sports Institute at UW Medicine, medical director of Sports, Spine and Orthopedic Health for UW Medicine, and co-medical director of the Sports Concussion Program, a partnership between UW Medicine and Seattle Children's.   Dr. James Ellis specializes in emergency medicine in Greenville, SC and has over 35 years of experience in the field of medicine. He graduated from Louisiana State University School Of Medicine In New Orleans with his medical degree in 1985. He is affiliated with numerous hospitals in South Carolina and more, including Ghs Greenville Memorial Hospital.

Diet and physical activity before and during pregnancy affect short- andlong-term health of mother and child. The energy needs at the end ofpregnancy increase only by about 10% compared to nonpregnant women. Anexcessive energy intake is undesirable since maternal overweight andexcessive weight gain can increase the risks for a high birth weight andlater child overweight and diabetes. Maternal weight at the beginning ofpregnancy is especially important for pregnancy outcome and childhealth. Women should strive to achieve normal weight already beforepregnancy. Regular physical activity can contribute to a healthy weightand to the health of pregnant women. The need for certain nutrientsincreases more than energy requirements. Before and during pregnancy,foods with a high content of essential nutrients should bepreferentially selected. Supplements should include folic acid andiodine, iron (in case of suboptimal iron stores), the omega-3 fatty aciddocosahexaenoic acid (in case of infrequent consumption of ocean fish)and vitamin D (in case of decreased sun exposure and decreasedendogenous vitamin D synthesis). Pregnant women should not smoke and notstay in rooms where others smoke or have smoked before (passivesmoking). Alcohol consumption should be avoided, since alcohol can harmunborn children.

This paper reviews current knowledge on the role of the long-chain polyunsaturated fatty acids (LC-PUFA), docosahexaenoic acid (DHA, C22:6n-3) and arachidonic acid (AA, 20:4n-6), in maternal and term infant nutrition as well as infant development. Consensus recommendations and practice guidelines for health-care providers supported by the World Association of Perinatal Medicine, the Early Nutrition Academy, and the Child Health Foundation are provided. The fetus and neonate should receive LC-PUFA in amounts sufficient to support optimal visual and cognitive development. Moreover, the consumption of oils rich in n-3 LC-PUFA during pregnancy reduces the risk for early premature birth. Pregnant and lactating women should aim to achieve an average daily intake of at least 200 mg DHA. For healthy term infants, we recommend and fully endorse breastfeeding, which supplies preformed LC-PUFA, as the preferred method of feeding. When breastfeeding is not possible, we recommend use of an infant formula providing DHA at levels between 0.2 and 0.5 weight percent of total fat, and with the minimum amount of AA equivalent to the contents of DHA. Dietary LC-PUFA supply should continue after the first six months of life, but currently there is not sufficient information for quantitative recommendations.
