Send us a textProf. Dr. Niels Riedemann, MD, Ph.D. is Chief Executive Officer and Founder of InflaRx ( https://www.inflarx.de/Home/About-Inflarx/Team~Niels-C.-Riedemann~.html ), a biopharmaceutical company focused on applying its proprietary anti-C5a and C5aR inhibitors to the treatment of life-threatening or debilitating inflammatory diseases with high unmet medical need. Prof. Dr. Riedemann has over 15 years of experience in the biotech industry and drug development, as well as over 20 years of experience in complement immunology research. He founded InflaRx in 2007 and has served as Chief Executive Officer since inception of the company. He has been instrumental in and led numerous private and public financing rounds of the company and has been the responsible lead for its Nasdaq IPO in 2017. He is named inventor on several internationally granted core patents of InflaRx. As physician, Prof. Dr. Riedemann was appointed Vice Director (“Leitender Oberarzt”) of Intensive Care Medicine, and led a 50-bed University ICU unit for over 6 years at Friedrich Schiller University, Jena, Germany until 2015. Before that, he received his board certification as General Surgeon upon completion of his surgical fellowship at MHH (Hannover Medical School, Germany) in 2007 where he also received his habilitation (equivalent to Ph.D.) and where he still holds an Adjunct Professorship (APL Professor). He spent three years as postdoctoral research fellow at the University of Michigan, USA until 2003. 2.14 Acute Respiratory Distress Syndrome (ARDS) Pulmonary system review for the USMLE Step 1 Exam Acute respiratory distress syndrome (ARDS) is an acute, diffuse, inflammatory form of lung injury that is life-threatening for patients. ARDS is a response from the lungs to severe injury, most commonly caused by sepsis, but can also be caused by pneumonia, trauma, drowning, toxin inhalation, and pancreatitis. The lung injury from sepsis causes systemic inflammation, damaging lung tissue and causing diffuse alveolar damage, reducing surfactant production and causing alveolar edema. Patients with ARDS will present with progressive cough, progressive dyspnea, and hypoxia, and usually deteriorate quickly, 12-24 hours after symptom onset. Diagnosis of ARDS is based on imaging findings of bilateral lung infiltrates seen as white-out on chest X-ray and ground glass opacities on CT, as well as a decreased Pao2/Fio2 ratio to less than 300. Patients with ARDS are placed on mechanical ventilation with low tidal volume and positive end expiratory pressure (PEEP) to keep the alveoli open at the end of the respiratory cycle. Treatment of ARDS involves addressing the underlying cause of the disease, such as sepsis or pneumonia.

Although not yet FDA approved, clinical trials of clazakizumab for treatment of COVID-19 are under way worldwide. Samantha Gambles Farr, MSN, NP-C, CCRN, RNFA, is joined by Bonnie E. Lonze, MD, PhD, to discuss the article, A Randomized Double-Blinded Placebo Controlled Trial of Clazakizumab for the Treatment of COVID-19 Pneumonia With Hyperinflammation, published in the September 2022 issue of Critical Care Medicine. Dr. Lonze is a faculty transplant surgeon at NYU Langone in New York, New York, USA, focusing primarily on kidney transplants. As vice chair for research of the NYU Langone Transplant Institute, she leads the research group on clinical trials and regulatory oversight.

What happens when your patient has been given a cric or intubated but continues to decline… SpO2 continues to slowly drop despite taking control of the airway. You have placed your patient on a ventilator and slowly adjusted the PEEP up to 20cmH20… which quickly leads to hypotension. Do you go lower? Higher? Change volume or rate? You are out of bottled O2 and your oxygen concentrator does not seem to have much effect. The SpO2 continues dropping. Telemedicine is not available. You try positioning the patient by sitting them up. You try a couple other recruitment maneuvers you heard about. Nothing is working. What would Doug do? Prone the patient??? Your patient may be suffering from ARDS, Acute Respiratory Distress Syndrome caused by a number of etiologies such as pneumonia or other lung injury. Carefully turning your patinet on their stomach may improve oxygenation by recruiting alveoli formerly compressed and “drowned” as demonstrated in the picture below. Positioning your patient on their stomach in the prone position must be practiced with anyone who will be helping you. Put someone else in a similar position and have the team with which you plan to help move the real patient do a couple rehearsals. You don't want to flip them over only to lose your IVs, IOs and yank the airway out. Check out this Brazilian article which includes a proning checklist and some informative pictures and tips. You also don't get an automatic win by flipping them on their belly and calling it a day. You will have to be even more vigilant about any potential complications with a dedicated airway person as is is a little harder to recognize a patient in distress if you are not used to it. You will also have to do more nursing care on the delicate skin of the face and other surfaces not normally on the down side: Shoulders, hips, knees tops of the feet. Put yourself in this position for a few minutes on a litter and you can quickly tell where the major pressure points will be. All of these complications increased along with the benefits of the study. While no prolonged field care patient should be on a bare litter, there is even more reason to move them to a mattress or other more comfortable padded surface. The role of #oxygen at times of #pandemic have a vital position, it's a major source of an element which is needed for #treating patients. Coronavirus second wave has impacted lives as many people succumbed to death or in dire need of oxygen as this new variant virus crippled the human body leading to #deaths in many cases. The ongoing surge in new #cases every day shows a direct impact on covid-19 management and this therapy where shortness of breath progresses to a more acute condition in patients who are affected by #virus. This can progress to a more #severe and systemic disease characterized by Acute Respiratory Distress Syndrome. Low levels of oxygen triggered by Covid-19 are #inflammatory markers. Severe cases are defined in three categories: severe #pneumonia, #acuterespiratory distress syndrome, and #sepsis. The clinical management protocol recommends oxygen therapy at 5 liters/min.
