I'm on call July 4th for the first time in what feels like forever, and I have fully lost perspective on what call should feel like. 2018 me, fresh out of an Iowa trauma center, would be horrified at how nervous private practice me has gotten about it. I'll report back after the holiday. Today's main event is World Cup eye injuries. Every professional sport needs an ophthalmologist on the sideline, soccer included, and I walk through three I'd be ready for. First, a 50 MPH ball off Erling Haaland the Viking's foot straight to the face, hyphema, open globe risk at the equator and limbus, retinal dialysis in young patients. Second, a player smashing their face on a goalpost, orbital floor fractures, the workup for extra-ocular muscle entrapment, and a full breakdown of eyelid laceration repair: above brow, below brow, eyelid margin, and canalicular. Third, the soccer flop, where grass exposure can deliver a corneal abrasion or an allergic reaction, and the drops I reach for. I've got more on deck if the tournament keeps giving me material. Takeaways: A high-velocity soccer ball to the face can cause hyphema (sheared vessels in the anterior chamber), an open globe at one of the eye's two weak points (the equator behind the extra-ocular muscles and the limbus at the edge of the cornea), or a retinal dialysis, especially in younger patients Hyphema management is driven by eye pressure: normal pressure means strict couch-potato rest until the blood clears, because a second bleed is always worse than the first; very high pressure may require surgical evacuation A face-into-goalpost impact most commonly causes an orbital floor fracture; suspected extraocular muscle entrapment is a PANTS patient, the ophthalmologist comes in, the orbit gets scanned, and forced duction testing rules entrapment in or out Eyelid laceration repair depends on location: above the brow is straightforward, below the brow uses skin-only sutures because there's an eyeball underneath, eyelid margin lacerations require specialized technique, and canalicular lacerations need temporary tube placement Even a soccer flop can cause real eye injury, grass exposure can lead to corneal abrasion or an allergic reaction in the fornix, treated first-line with antihistamine drops like Patanol, Pataday, or Zaditor, with short-course topical steroids for severe cases To Get Tickets to Wife & Death: You can visit Glaucomflecken.com/live  We want to hear YOUR stories (and medical puns)! Eyelids flutter, and we sense—more than we see—how differently light looks than when we fell exhausted into bed. Awareness jabs at everything—the too-hard pillow; the blanket thin against the chill; the shoulder sore from hours of unmoving. Awake—too soon; too late; too urgently. The undone stuff of yesterday grabs our first thoughts. Oh no! Not that! How much? How soon? And in those fitful moments, the impulse to be grateful for our lives so easily departs—chased out by hot adrenaline. Should we—could we—offer thanks for grumpy children shepherded to school; for spouses facing drama at the office; for traffic ribbons of red taillights? Yet waking up is still a grace, and drawing breath is still a gift. Everything we count as sameness and routine is proof that life still offers possibilities; that things don't stay just as they were; that hope—and hopeful people—still endure. Grace saves more than souls and minds—the planned, deliberate parts of us. Grace floods our zone with oxygen; with joys too small to write them down; with love as wordless as an infant's fingers curled about our own. And gratitude—perhaps a prayer we've memorized; an easy sigh of heavenward contentment—gratitude equips us for the journey of these hours, this life, and on to life eternal. “I have come that they may have life, and that they may have it more abundantly” (John 10:10), Jesus says to all who put their mornings in His care. A sedated teenager with a swollen face, red eyes, and a whole lot of chemosis needs help, and I show up with… my ophthalmologist luggage. What I found is something every ICU clinician has seen but doesn't always recognize: exposure keratopathy, the completely preventable eye emergency that happens when the lids won't close, the tears disappear, and the cornea gets sacrificed to hospital airflow. Takeaways: How facial edema and chemosis in the ICU create the perfect setup for exposure keratopathy and why it often goes unnoticed. Why artificial tears twice a day are nowhere near enough for a sedated patient who can't blink. The simple bedside tools ophthalmologists bring to evaluate non-responsive patients (including the portable slit lamp). Why corneal abrasions in hospitalized patients can become dangerous fast and how to stop them before they start. The easiest ICU intervention ever: ointment, moisture chambers, and remembering that eyelids exist for a reason. He's also got a lot of experience as a lead blocker for a running QB, but that's probably not what he came here for. WRs are Donaven McCulley and then a big drop to Just-Guys who probably were better than they looked last year. Think Semaj Morgan should be the #2 since he gives you something—F.Moore, Bell, O'Leary just guys, hearing things about the freshmen doesn't give Brian happy feels. Tight end is the strength of the offense. The spectrum goes fullback: Max Bredeson, h-back: Jalen Hoffman, inline: Marlin Klein, flex: Hogan Hansen. Bredeson kickouts force defenders to overcommit, create bounce opportunities. Klein is an underrated blocker (watch the Reliaquest Bowl). Hansen was open last year, Davis Warren was allergic to throwing at him. Hoffman might be Underwood's secret binkie, can run WR routes and block like an OL. 3. Offensive Line Starts at 1:18:36 PLEASE do not activate our rule that if two guys are battling for a job and it's won by a third guy it's a very bad sign. Concerning that OL was an issue, and player evaluation was an issue, when the HC was the OL coach. It's not a hot take that Andrew Babalola is going to start at Ohio State, Brian. If Evan Link gets fixed and Gio El-Hadi truly feels more comfortable at LG that'd be great; the latter is more likely than the former, but they don't have better options so Link to start. Crippen is who he is; if his issues were mental then could see Juan Castillo having an effect, but Crippen is mentally there and just gets physically dominated. RG and RT are two really good stories for the future, as Efobi and Sprague (especially Sprague) are on track for excellent careers. Depth at G. 4. Hot Takes, Special Teams and Chip Starts at 1:46:36 Takes hotter than Sabrina Carpenter. Thank you to the Michigan fan who left Dominic Zvada out of the Groza semifinalists so he would come back to be the best kicker in Michigan history again. Punter…wasn't great last year but it's not like Hudson Hollenbeck was better in the one game we saw him, and attempts to get more guys in the portal is a little concerning. Returns should be Semaj, I'Marion Stewart, and hearing things about the UMass transfer Anthony Simpson. Chip's offense is about building his passing game onto your running game. They'd like to be an outside zone one way/rollout the other way team but they probably can't run outside zone. Need to hit the downfield shots to keep safeties off their screen game; last year they were set up at 7 yards. Hey everyone...in honor of our upcoming 100th episode (!!!), I'll be re-releasing a few of my favorite past conversations on Tuesdays.This one was a lifelong dream for me: getting to talk to DEVO's Gerald V. Casale about their body of work--a band whose musical and visual identity showed teenage me that it was o.k. to be different, weird, and skeptical. Jerry does not suffer fools gladly, so I was quite nervous going into the interview, but once he realized I wasn't going ask about the flower pot hats, we both relaxed and took a serious deep dive into how he and DEVO altered the world forever, as well as how lonely that could be for them.
