The Product Experience
What I learned from unbuilding products and systems in the Public Sector - Ayushi Roy (Product Leader)

In the private sector, product teams pick their customers, generate demand, and ship into something close to a green field. In the public sector, none of that holds. Ayushi Roy — Chief Program Officer at New America's New Practice Lab and a lecturer at the Harvard Kennedy School — joins Randy to unpack what changes when your user base is already sitting in front of you, your scrutiny is congressional, and the right answer is sometimes to delete ten systems rather than build an eleventh.Drawing on her work on IRS Direct File, the Children's Health Insurance Program (CHIP), the Illinois childcare voucher system, and a text-based 911 alternative that rolled out to 800,000 students across 13 universities, Ayushi makes the case for a distinct public-sector product playbook: thin-slicing for safe failure, designing for the lowest digital denominator, separating design problems from engineering problems, and treating unbuilding as a first-class option.Chapter markers01:48 — From aid monitoring in Jordan to digital delivery03:37 — Why she built a text-based alternative to 91106:33 — From a rollout to 800,000 students to Oakland City Hall08:58 — What the New Practice Lab does, and what a CPO does inside a think tank11:06 — Why private-sector product playbooks don't transliterate14:03 — No marketing, no early adopters: latent demand and the curb cut effect14:40 — Oakland's eviction tool, MacBooks, and the lowest digital denominator17:30 — Thin-slicing IRS Direct File without losing Congress22:36 — Building executive sponsorship that allows safe failure23:41 — Product vs service: the rest of the job that isn't writing code26:09 — Illinois childcare vouchers: when modernising the form makes things worse29:22 — Design problems, engineering problems, and the laptop-hinge analogy33:18 — Can AI prototyping close the policy–implementation gap?35:40 — The FAFSA simplification crisis and the case for bilingual builders37:31 — Unbuilding: how a request for a 15th CHIP system became one to remove ten41:18 — What keeps her goingOur HostsLily Smith enjoys working as a consultant product manager with early-stage and growing startups and as a mentor to other product managers. John Schoen, Senior Clinical Manager of Evidence-Based Medicine and Drug Information in Vizient's Center for Pharmacy Practice Excellence and Vizient's vaccine subject matter expert, joins host Stacy Lauderdale to discuss key updates to the CDC's childhood Immunization schedule and what they mean for practice.   Guest speaker:     John Schoen, PharmD, BCPS   Senior Clinical Manager of Evidence-Based Medicine and Drug Information    Vizient Center for Pharmacy Practice Excellence     Host:   Stacy Lauderdale, PharmD, BCPS   Associate Vice President  Vizient Center for Pharmacy Practice Excellence   Verified Rx Host    00:00 — Introduction Announcer welcomes listeners to Verified Rx, produced by the Vizient Center for Pharmacy Practice Excellence. 00:14 — Episode Overview Host Stacy Lauderdale introduces the topic: updates to the CDC's U.S. Childhood Immunization Schedule, revised January 20, 2026. Goal of the episode: explain what changed, what didn't, and what it means in practice for providers, pharmacists, and families. Guest: John Schoen, Senior Clinical Manager of Evidence-Based Medicine and Drug Information at Vizient and vaccine subject matter expert. 01:16 — What Changed in the CDC Immunization Schedule CDC reorganized the schedule into three recommendation categories. Vaccines were reclassified, not removed. Number of diseases covered under “routine” recommendations decreased from 17 to 11 due to recategorization. 01:50 — Stated Rationale Behind the Changes Rationale provided in executive summary of scientific assessment. The supporting scientific assessment is available online and referenced for transparency (link in resources below). 03:19 — Were Any Vaccines Removed? No vaccines were removed from the CDC schedule. Some vaccines were shifted into different recommendation categories. 03:40 — Category 1: Routine Childhood Vaccinations Vaccines still routinely recommended for all children include: MMR (measles, mumps, rubella) Polio Tdap / DTaP Hib (Haemophilus influenzae type B) Pneumococcal HPV Varicella (chickenpox) 04:27 — Category 2: Vaccines for High-Risk Populations Vaccines recommended for children who meet specific high-risk criteria: RSV monoclonal antibodies (mAb) Hepatitis A Hepatitis B Quadrivalent meningococcal Meningococcal group B Dengue 05:19 — What Changed vs. Stayed the Same Hepatitis A, Hepatitis B, and quadrivalent meningococcal moved from routine to high-risk RSV mAb recommendations are effectively unchanged, as high-risk infants are defined as those born to mothers who did not receive the maternal RSV vaccine. Dengue remains risk-based. Meningococcal group B remains a mix of risk-based and shared clinical decision-making. 06:31 — Category 3: Shared Clinical Decision Making (SCDM) Defined by ACIP as an individualized decision made jointly by provider and parent/guardian. Allows vaccination when risk-based criteria are not met but benefit is still considered. 06:52 — Vaccines Under SCDM Vaccines now include: Influenza COVID-19 Rotavirus Hepatitis A Hepatitis B Quadrivalent meningococcal Meningococcal group B 08:05 — What's New in SCDM Influenza and rotavirus moved from routine to SCDM. Hepatitis A, hepatitis B, and quadrivalent meningococcal also shifted. COVID-19 moved to SCDM in September 2025 for individuals ≥6 months. 08:28 — Insurance Coverage Implications No expected changes in coverage. Vaccines recommended by CDC as of December 31, 2025 remain: Covered without cost-sharing under Affordable Care Act (ACA) plans. Covered by Medicaid, Children's Health Insurance Program (CHIP), and Vaccines for Children (VFC) program. 09:14 — Impact on Pharmacy Vaccine Access Pharmacists are considered healthcare providers under CDC SCDM definitions. Authority to administer vaccines primarily determined by state laws. Standing orders, protocols, and collaborative practice agreements may need to be updated, as applicable, to address language related to ‘routine' immunizations for children. 10:27 — Recommendations for Providers & Organizations For providers: Become familiar with schedule changes. Be prepared for patient and parent education. Recognize differences between CDC and other professional guidelines. For organizations: Review EHR documentation and order sets. Consult local state regulations to ensure compliance with vaccine administration practices. Review standing orders/protocols and collaborative practice agreements to determine if revisions are needed. Monitor vaccine utilization and adjust inventory accordingly. 11:24 — Resources & Closing Additional CDC and Vizient resources will be linked in the show notes. Announcer closes with subscription and feedback reminder. Proposed Cuts to Medicaid Could Mean Proposed Cuts to Sexual and Reproductive Health

Medicaid, the United States' largest public health insurance program, currently insures over 72 million people with low incomes. Medicaid covers a host of health needs, including those related to sexual and reproductive health (SRH). Madeline Morcelle, Senior Attorney at the National Health Law Program (NHeLP) and co-chair of the National Coalition for Gender Justice in Health Policy, sits down to talk with us about how Medicaid is indispensable for SRH coverage and how recent proposed cuts would impact those who are enrolled.Medicaid plays an essential role in securing SRH for those with low incomes and other underserved communities, including Black, indigenous, and other people of color, people with disabilities, women and LGBTQI+ people, young people, and people with limited English proficiency. States are required to cover family planning services and supplies, pregnancy-related care, and abortion within the Hyde Amendment's exceptions. The House recently passed a budget resolution that requires at least a minimum of $880 billion in budgetary cuts from the Energy and Commerce Committee—who's vast majority of financial jurisdiction is centered in Medicare, Medicaid, and the Children's Health Insurance Program (CHIP). Assuredly, proposed cuts would cause states to limit eligibility for Medicaid insurance and impact million's access to SRH care. Find Intersectionality Matters where ever you get your podcasts: https://pod.link/1441348908Support the showFollow Us on Social: Twitter: @rePROsFightBack Instagram: @reprosfbFacebook: rePROs Fight Back Bluesky: @reprosfightback.bsky.social Email us: jennie@reprosfightback.comRate and Review on Apple PodcastThanks for listening & keep fighting back!

Independent Black Woman I believe this is a phrase, perhaps not coined by us, but one repeated by us as a salve to ease the distress of being unloved, unprotected and unprovided for. Let's define our term: Independent -not needing another, not reliant on another How is this a healthy means to live?  More to the point, is it even possible to exist without relying on others?  I should say it is not. We take WIC (Women Infants and Children) a federal program giving us food for our babies. We use our state's Children's Health Insurance Program (CHIP) to obtain healthcare.  We use food banks and take advantage of school nutrition programs. We take federal student loans to get educated. We take bank loans for cars and homes. We take subsidized housing to offset rental costs. Where all the independence that we are so proud of? If we are honest, we will admit that we use the term independent pretty much in relation to black men and and relationships.  Because it is too painful to say, “I am lonely” or “I want companionship” or “I want love”  we boast of our independence because we can pay our own bills. While that may be true, our education cannot protect us, our corner office won't love us and our boss babe hustle will not nurture our children.   I think this is part of the conditioning we have had to manage our disappoints and hurts as it relates to not having our fathers and the fathers of our children present.  I also believe we have to break the shackles of this conditioning that puts us against ourselves and against our men. We and our Black men are in fact two halves of a whole. If you enjoy Nurah Speaks, please share the podcast with friends, family, and colleagues. Every time you share, you help the show's growth. The health and safety of children is a top priority for families. Enrolling in health insurance makes going to the doctor much more accessible. But it can be difficult to find an insurance plan that meets your little ones' needs and fits your family budget. We visit with Mark Schafer, the Program Support Administrator at the Arizona Health Care Cost Containment System, to discuss the low-cost health program called the Children's Health Insurance Program (CHIP). Learn the ins and outs of coverage options and how your family can enroll.Host:Jessica Stewart-Gonzalez is the Chief of the Office of Children's Health at the Arizona Department of Health Services. Healthcare insurance in the U.S. has a long history. However, the earliest forms of healthcare insurance in the U.S. were largely private plans that covered hospital and medical care for individuals or employers.  Today, most Americans obtain health insurance through their employers or government plans such as Medicare and Medicaid. In today's episode, I will start by talking about the private healthcare system. Private care in the U.S. allows individuals to purchase health insurance coverage from private companies instead of relying on public insurance sources like Medicaid or Medicare.  Most private health insurance plans are offered through employers and are known as employer-sponsored health insurance.   Private health insurance usually covers a wide range of healthcare services, such as doctor visits, prescriptions, mental health care, hospitalization, and emergency care. Depending on the plan, private health insurance might include coverage for vision and dental care, alternative medicine, and other services. These plans can vary significantly in cost and coverage, from basic plans that cover essential services like doctor visits and prescription drugs to more comprehensive plans that can cover costly treatments like surgery.    Other options In addition to health insurance, there are several other options for accessing healthcare. You can access healthcare through community health centers or free or low-cost clinics if  uninsured.    Health Marketplace For instance, the Health Marketplace is a resource where people can shop for and purchase health insurance coverage. It provides access to various health insurance plans, including private and Medicaid and the Children's Health Insurance Program (CHIP). It also provides information about subsidies and tax credits that can help reduce the cost of premiums.   Get the transcript on my website:SpeakEnglishPodcast.com/podcast

We are super excited today to be joined by the chief strategy officer of CMS Innovation Center for Medicare & Medicaid Services, Purva Rawal. Purva will be sharing a bit about CMS, the agency's priority to drive innovation that tackles the health systems challenges, and Purva's role at CMS. About CMS In order to find ways to raise healthcare quality while lowering costs for the Medicare, Medicaid, and Children's Health Insurance Program (CHIP) programmes, Congress established the CMS Innovation Center in 2010. In response to this challenge, the CMS Innovation Center has sped up the transition from a healthcare system that pays for volume to one that pays for value through its models, projects, and Congressionally mandated demonstrations. The Centers for Medicare & Medicaid Services (CMS) Innovation Center, also known as “CMMI,” develops and tests new healthcare payment and service delivery models to:1. Improve patient care.2. Lower costs.3.
