As Scripture reminds us, pestilences are part of a broken world (Luke 21:11) - COVID-19 was not the first and will not be the last.Dr. Annelies Wilder-Smith has devoted her career to emerging infectious diseases, particularly those impacting low- and middle-income countries. Her path was shaped during two decades in Asia, where she was at the forefront of the SARS outbreak, and investigated the H5N1, dengue and Zika virus outbreaks. During the COVID-19 pandemic, she served as an external advisor to the World Health Organization, where she developed COVID-19 vaccine policies for the global use.Annelies is Honorary Professor of Emerging Infectious Diseases at the London School of Hygiene and Tropical Medicine, Past President of the International Society of Travel Medicine, and Editor-in-Chief of the Journal of Travel Medicine. Her academic career resulted in 380 publications and many research grants. She leads the Lancet Commission on Dengue and was Principal Investigator of EU-funded research consortia.Her awards include the Myron Levine Vaccinology Prize and the CDC Honor Group Award. Annelies is also the author of Travel Medicine: Tales Behind the Science and Grasping Heaven, a biography of Dr. Tami Fisk. Dr. Billick is the lead author of Five things to know about dengue, and Dr. Vaughan is the lead author of Five things to know about Oropouche virus, both published in CMAJ.Dr. Maxime Billick describes the dramatic surge in dengue cases in 2024 and outlines the major drivers behind the virus's global spread. She explains the urban-dwelling mosquito vectors that make dengue difficult to contain, discusses the virus's four serotypes, and emphasizes the risk of antibody-dependent enhancement with reinfection. She reviews key clinical warning signs, diagnostic tests, and why identifying dengue—despite limited treatment options—still matters for patient counselling and care.Dr. Stephen Vaughan introduces the less common but emerging Oropouche virus. He explains its current geographic range, including recent Canadian travel-related cases, and the role of biting midges in its transmission. Mosquito season has begun in earnest, and until November, there is an increased risk for mosquito-borne illnesses, including dengue fever. CDC has urged U.S. doctors to watch for dengue, particularly in people who've traveled to countries where the virus is endemic. But several states, including Texas, have reported locally acquired cases of dengue over the last fifteen years.

Dengue fever is in the news right now after the death of a young Samoan boy at Starship Hospital last week. As many New Zealanders travel to the Pacific Islands, the recent outbreak means it's something to consider. The other interesting fact about dengue fever is as well as being relevant to the Pacific, it's becoming more common around the world due to climate change – increasing temperature and humidity driving mosquito population and leading to more people being infected. What is it? A mosquito-spread virus. You get it when a mosquito bites you that is infected with dengue fever. Unlike malaria, it's carried by the small mosquitoes that come out during the day. Not the evening/early morning mosquitoes that typically carry malaria. It's becoming more common around the world with climate change – with hotter more humid temperatures. Endemic in Pacific, which is presently going through an outbreak. Can actually now be found in places like far North Queensland. It could it reach New Zealand one day – possibly with climate change. What happens if you are bitten by a mosquito carrying dengue fever? Symptoms occur 4-10 days after the mosquito bite. Typical symptoms: High fever, severe headache, pain behind the eyes, muscle and joint aches (has been referred to as ‘break bone fever'), nausea, vomiting, and skin rash. Often people recover after 1-2 weeks. Severe form is called haemorrhagic fever with bleeding, cardiovascular shock, and sometimes death. Typically occurs when you get an infection a second time – not the first. What can we do treat it? Important: There is no vaccine to prevent the disease, once you have it there is no treatment. Just treat the symptoms: Paracetamol, not ibuprofen or aspirin as this can increase bleeding risk. Plenty of fluids and rest. A majority will get better, however, if symptoms are severe, you need to see a doctor and may need the hospital. How do we prevent it? Be aware that Pacific destinations Fiji, Tonga and Samoa have had recent outbreaks. Outbreaks are often every 2-5 years with year-round risk. Resorts often put in place protocols to reduce/eliminate mosquitoes. If mosquitoes are around, protect against getting bitten with insect repellents, long sleeve shorts/pants, and mosquito nets. These include climate change, urbanization, international travel, and other factors.
