We're dropping this bonus episode because something genuinely historic happened in oncology and we couldn't wait until our regular schedule to talk about it. Dr. Mark Lewis, GI medical oncologist, pancreatic cancer survivor, and one of our absolute favorite guests. He just got back from ASCO, the Super Bowl of oncology, where a new drug called daraxonrasib received one of only about six standing ovations in the conference's history, including applause that broke out mid-sentence when researchers showed the survival data on-screen. The drug targets a mutation that drives the vast majority of pancreatic cancer and has been considered "undruggable" for decades and it works not by attacking the mutation directly, but by cutting off the downstream signals it sends, like snipping the wire instead of fighting with the switch. The results are remarkable. In patients who had already received standard chemotherapy, daraxonrasib roughly doubled survival time and delivered it in pill form rather than an IV every two weeks, a meaningful quality-of-life difference for people who are already facing the hardest year of their lives. Mark walks us through the science, the side effects (rash, because RAS proteins live in skin too), the path to FDA approval, and what this means beyond pancreatic cancer, the same KRAS mutations show up in about 30% of lung cancers and 40% of colon cancers. Takeaways: Pancreatic cancer has been devastatingly hard to treat, 85% of patients are incurable at diagnosis because it spreads silently and there's no good screening, leaving most patients with a median survival of about a year on IV chemotherapy. KRAS, the mutation driving nearly all pancreatic cancer, was long considered "undruggable", the protein was so smooth and spherical that no drug could bind to it, and researchers were actively discouraged from pursuing it as a target. Daraxonrasib works by cutting the power rather than fighting the switch, instead of binding to the KRAS protein itself, it uses molecular glue to interrupt the downstream growth signals the mutation sends, an approach that took decades to develop and wasn't taken seriously until now. The trial results roughly doubled survival and the treatment is a daily pill, not an IV, patients who had already been treated with chemotherapy gained approximately an additional year of life with improved quality of life, which represents one of the most significant advances in pancreatic cancer treatment in decades. This breakthrough has implications far beyond the pancreas, KRAS mutations drive about 30% of lung cancers and 40% of colon cancers too, and proving the target is druggable opens the door to a new generation of treatments across multiple cancer types. Professional snowboarder Kimmy Fasani was still breastfeeding her second child when she found a lump under her armpit. Just days later, she was diagnosed with an aggressive form of breast cancer and found herself preparing for chemotherapy. Having lost both her parents to cancer, it was a terrifying moment for her, but she tried not to let her family history loom over her as she began treatment. Instead, she focused on the lessons learned from years spent tackling mountain peaks—to stay flexible and adaptable when her plans were upended and to find beauty in the adventure even if she hadn't asked for it. At the time of her diagnosis, Kimmy and her husband Chris were filming a documentary following five years of their lives and careers. As she faced the challenges of chemotherapy, radiation, and a double mastectomy, she opted to keep the cameras rolling, documenting the highs and lows of her treatment with unflinching honesty. As difficult as it could be to show the world her most vulnerable moments, Kimmy came out on the other side inspired and hopeful that she could remind others like her that they aren't facing cancer by themselves. When Molly Hones was diagnosed with fibrolamellar cancer for the second time in less than two years, she made two critical decisions. First: She wouldn't focus on the unknowns of her condition and would focus on the positives instead. Second, she would give back by participating in research. And so, in June 2023, Molly traveled to the Johns Hopkins Hospital, where oncologist and Conquer Cancer grant recipient Marina Baretti, MD, was running a clinical trial for people with fibrolamellar cancer. “I like to say I'm donating my body to science while I'm still alive,” Molly says. “And how cool is it to see my legacy when I'm still around?” Which is how she found herself in Baltimore, Maryland, on the day she received an unthinkable phone call: Her husband Grant had passed away at age 45 from a brain aneurysm. Even faced with such devastating news, Molly remained committed to moving forward with her treatment, connecting with other fibrolamellar patients, and celebrating life whenever she could. Being brave and facing challenges with a smile on her face, she says, is her way of honoring the people she's lost and the people she's met through her cancer journey. As a young person starting over in a new country—one where she didn't even yet know the language—young Yelena faced no shortage of challenges. But she also found opportunity she believes might not have existed in her native country. “As an Armenian individual growing up in Azerbaijan, going by my parents' and my family's experience, I don't think I would have had an opportunity to be a physician there,” Dr. Janjigian says. “There was a clear limitation on who got to be a physician, and it's a prestigious position anywhere in the world. As a relative minority, I wouldn't have been able to do that. My parents certainly had the courage it took to leave and to come to a foreign country.” That same brand of courage led Dr. Janjigian to her current work as a gastrointestinal medical oncologist at Memorial Sloan Kettering Cancer Center. A specialist in esophageal and stomach cancer, she presented the results of her gastric cancer clinical trial during the plenary session of this year's ASCO Annual Meeting in Chicago. The presentation was the culmination of more than eight years of work involving nearly 1,000 patients and collaborators. Much of that work, she says, started when she received her first grant from Conquer Cancer, a Young Investigator Award. Receiving that funding, she recalls, was a career-defining moment. It started—as these stories so often do—with fatigue. At first, Jace Yawnick simply chalked it up to a busy life and a job requiring frequent travel. But then he noticed something: There were times the fatigue kicked in when he hadn't just gotten off a plane or done anything else particularly strenuous. “Intuitively,” Jace says, “something within me just knew something wasn't right.” Then came the coughing and the back pain. It would be months—and more than one misdiagnosis—before doctors found the problem: Hodgkin lymphoma. As he processed the news of his diagnosis, Jace realized something: Facing cancer was not something he wanted to do by himself. And so, when he started to chronicle his cancer experience online, he was simply looking for catharsis and camaraderie. He got both—along with a community of 700,000-plus followers who've followed his entire journey in hopes of seeing the moment he fulfilled a cherished goal: to ring the bell and declare himself cancer-free. It took a long time after I first read David Menconi's work to finally meet him, and longer still to sit with him for an extended conversation, but fortunately, the time has finally come. And if the saying “good things come to those who wait” applies here, if I was indeed waiting years for that Tuesday afternoon interview in late September 2024, then it is doubly appropriate now that our conversation sees the light of day in this podcast nearly another half a year afterwards. Decades after first reading his work, more than a year after first meeting him, and another stretch of months after interviewing him, now is the moment for all of this to come together. It all feels a bit fortuitous, a feeling which is echoed in our conversation, especially regarding the subject of David Menconi's latest book. David Menconi was a staff writer at the Raleigh News & Observer for 28 years, beginning in 1991, when the music scene in the region was exploding nationwide. He has also written for Rolling Stone, Billboard, Spin and New York Times. His latest book is titled Oh, Didn't They Ramble: Rounder Records and the Transformation of American Roots Music. A label formed by three idealistic folkies fresh out of college, it went on to champion the music of artists ranging from Alice Gerard to Alison Krauss, from the Blake Babies to Billy Strings, and from Ted Hawkins to Tony Rice. The Appalachian Outlaw Trails is adding more than 2,000 acres of trails designed for ATVs, dirt bikes, side-by-sides, and full-size vehicles like Jeeps and trucks. This expansion aims to attract tourists and outdoor adventurers from West Virginia and the entire East Coast. Currently featuring 25 miles of trails in the Upper Kanawha Valley, AOT has ambitious plans to double this mileage. The goal is to develop one of the most extensive off-roading parks in the nation, covering over 100,000 acres in the long term. The ultimate plan is to develop a comprehensive outdoor recreation resort complete with cabins, a concert venue, mountain biking trails, hiking paths, horseback riding areas, and clay shooting venues. The Kanawha County Commission is collaborating with AOT on this multi-phase project, with the initial focus on developing trails in the Quincy area. But of all the George Washington showcases, this reporter's favorite is a national park in the small town of Berkeley Springs, West Virginia. That's where America's first president was said to bathe in an open-air brick tub that reads, not surprisingly, “George Washington's Bath Tub.” It was filled with warm natural mineral spring water emanating from the surrounding mountainous countryside. What makes traveling to Berkeley Springs most enticing is that we can join, at least figuratively, Washington in sitting in a large indoor bathtub that can easily accommodate two bathers and is filled with 104-degree mineral water. It's situated in what's called the Old Roman Bath House at Berkeley Springs State Park. The bureau and Lewis won the Excellence in Tourism Development award at the 2025 Hospitality University conference held Jan. 26-28 at the Greenbrier. The increase in tourism has enabled them to double the attendance at the Oil and Gas Museum and at Henderson Hall, while the main draw continues to be Blennerhasset Island Historical State Park. According to guitarist Alex Wade, the album is designed to be “shockingly menacing and brutal.” With guttural lows, eerie atmospheres, and crushing riffage, Hymns In Dissonance is poised to be one of the heaviest releases of 2024. In this week's episode of The Loaded Radio Podcast, we sit down with Ben Savage to break down the album's concept, the band's unique approach to songwriting, and what went into crafting this monstrous new record. The Concept Behind Hymns In Dissonance Unlike their previous album, Kin (2021), which explored deeply personal themes, Hymns In Dissonance is a concept album centered around a cult leader gathering followers to commit heinous acts in worship of their dark lord. Frontman Phil Bozeman describes the album's title as a direct contradiction: “Hymns are melodious and harmonious. Dissonance is the opposite. Dissonance represents evil. These tracks serve as hymns for the cult, each one symbolizing a deadly sin.” The record follows a 10-track journey that builds upon Whitechapel's legacy while introducing some of their lowest tunings and most intricate guitar work yet. Whitechapel's Heaviest Riffs Yet? One of the most exciting elements of Hymns In Dissonance is its brutal guitar work. In the interview, Ben Savage breaks down how Whitechapel pushed their sound further than ever before. One key highlight is the album's title track, which drops to Drop F tuning in its final breakdown—the lowest tuning Whitechapel has ever used. According to Wade, the shift in tuning helps create an “overwhelmingly crushing” finale that fans will undoubtedly feel in their bones. The album's lead single, “A Visceral Retch,” was the first taste of this new sound, immediately receiving high praise from fans for its sheer intensity. The Making of Hymns In Dissonance Unlike previous albums, the writing and recording process for Hymns In Dissonance was entirely in-house, with Zach Householder handling production duties. Having worked closely with producer Mark Lewis over the years, Householder was able to bring his deep understanding of Whitechapel's sound into the production process, ensuring maximum impact with every track. Savage also discusses how the band approached writing in a structured, weekday-only schedule, which allowed them to stay focused while avoiding burnout. Whitechapel's Hymns In Dissonance North American Tour With the album set to drop on March 7, Whitechapel is gearing up for a massive North American tour in support of Hymns In Dissonance. The month-long trek kicks off on March 18 in Raleigh, NC, and wraps up on April 19 in Atlanta, GA. For Mark Lewis, MD, cancer has defined his entire life. Growing up, he witnessed his father's valiant struggle with cancer before it eventually ended his life. While still in medical training, he not only developed pancreatic cancer but also discovered the culprit. Multiple endocrine neoplasia type 1, an inherited syndrome that drastically increases one's risk of cancers, runs in his family. So now, as a father, he guides his son in making sense of a life burdened with that risk. What's more, as an oncologist, Dr. Lewis has also dedicated his professional life to understanding and treating cancers of the gastrointestinal system.
