CardioNerds Multimodality Cardiovascular Imaging PageCardioNerds Episode PageCardioNerds AcademyCardionerds Healy Honor Roll Pearls Shift in Paradigm: CCTA is no longer just an anatomic test; with some key limitations, it can provide anatomy, physiology (CT-FFR), and plaque biology (AI-CPA) in a single non-invasive scan. The “Power of Zero” vs. Plaque: While a normal CCTA has a >95% negative predictive value, future MIs often arise from non-obstructive plaque that traditional stress tests might miss. CAD-RADS 2.0 Utility: The addition of plaque burden modifiers (P1–P4) is a “game changer,” allowing clinicians to identify high-risk patients who need aggressive lipid-lowering despite having only mild stenosis. CT-FFR as a Virtual Stress Test: CT-FFR uses computational fluid dynamics to simulate blood flow, potentially reducing unnecessary invasive catheterizations by approximately 61% without sacrificing safety. Seeing the Invisible: AI-based quantitative plaque analysis (QCPA) can identify “subvisual” plaque and low-attenuation (lipid-rich) components that are the primary drivers of acute coronary syndromes. Show Notes How has the role of CCTA changed compared to traditional functional testing? Historically, stress testing answered “is there ischemia today?”, which often reflects late-stage disease. CCTA identifies disease across the entire spectrum, asking “is there atherosclerosis and how much plaque is present?”. Landmark evidence: SCOT-HEART showed a 41% relative risk reduction in MI at 5 years attributed to intensified preventive therapies, and PROMISE showed CCTA was better at selecting patients who truly needed invasive angiography. Diagnostic CCTA imaging depends on the protocol, contrast timing, heart rate, heart rhythm, breathholding, scanner quality, and several patient factors (obesity, prior stents, heavy calcification, complex bypass anatomy, and motion artifact all may limit imaging). “CCTA is exceptional for the right patient, with the right scanner, and the right team.” What are the key modifiers introduced in CAD-RADS 2.0, and why do they matter? CAD-RADS 2.0 moved beyond stenosis severity to include plaque burden (P0 to P4), high-risk plaque (HRP) features, and the presence of ischemia based on CT-FFR. It serves as a clinical decision support tool: a patient with mild (25-49%) stenosis but “extensive” (P4) plaque burden is considered high risk and warrants aggressive risk factor modification. How is CT-FFR calculated, and when is it most useful in clinical practice? CT-FFR uses resting CCTA data and computational fluid dynamics to create a 3D model of coronary flow during simulated maximal hyperemia. It is often used for intermediate lesions (40–90% stenosis) to predict if they are ischemia-producing, guiding the decision whether to proceed with invasive angiography. The assumptions necessary for this computational modeling may not apply well to patients with microvascular dysfunction, significant myocardial scar or prior infarction, or ventricular hypertrophy. Still, data indicate that CT-FFR performs similarly to PET in predicting hemodynamically significant lesions. CT-FFR performs well at the extremes (either clearly normal or clearly abnormal). Accuracy dips, however, in the intermediate range (~0.75-0.80), where decision-making is most critical. In this grey zone, additional factors can help guide the approach, including the amount of myocardium supplied, translesional gradient, and plaque features. CT-FFR has not been validated in distal segments, stented segments, heavily calcified coronary arteries, or in patients with severe aortic stenosis. Caution with CT-FFR should be utilized in very calcified coronary segments. What is AI-based quantitative plaque analysis (QCPA), and what metrics are ready for clinical use? This is potentially a paradigm shift, moving away from stenosis-centric thinking to a more disease burden and plaque biology focus. QCPA uses deep learning algorithms to automatically segment the vessel wall and quantify plaque volume in mm³. Ready for “prime time” metrics include: Total Plaque Volume (TPV), non-calcified plaque volume, and Low-Attenuation Plaque (LAP) burden. Can serial CCTA be used to monitor the effectiveness of medical therapies like statins? While not yet a routine guideline-driven practice, trials like PARADIGM and EVAPORATE show that therapies can stabilize plaque; notably, CCTA is better for monitoring than CAC scores, which can be misleading as statins often increase plaque calcification as part of the stabilization process. There are no randomized trials that serial CCTAs improve outcomes. Cost and radiation exposure will be notable limitations. Serial scan timing, scan acquisition and interpretation standardization would be key. Dr. Gallagher notes that we are moving toward a world in which plaque burden may become a “treatment biomarker,” similar to tumor burden in oncology. References 1. Detox ist heute in aller Munde. Doch was bedeutet Entgiftung eigentlich wirklich? Alexander Djordjevic erzählt, dass er früher selbst vieles ausprobiert hat unter anderem extreme Säftekuren über mehrere Tage. Heute sieht er das differenzierter. Gerade wenn es um Muskelaufbau, Energie und Testosteron geht, seien reine Säftekuren oft nicht optimal. Spannend ist seine Sicht auf sogenannte Binder, die den Körper dabei unterstützen sollen, belastende Stoffe wie Schwermetalle zu reduzieren. Nicht komplett entfernen, aber zumindest minimieren. Das könne sich auch positiv auf den Testosteronwert auswirken. Ein großes Thema im Interview: Nahrungsergänzungsmittel. Hier gibt es laut Alexander drei Lager: Die einen sagen: „Brauche ich nicht." Die anderen nehmen täglich unzählige Kapseln. Dann gibt es diejenigen, die Nahrungsergänzung komplett ablehnen. Sein Ansatz ist klar: Messen statt raten. Viele Produkte seien qualitativ sehr unterschiedlich. Manche bringen wenig bis gar nichts. Deshalb empfiehlt er, zuerst die eigenen Blutwerte prüfen zu lassen, Vitamine, Mineralstoffe und wichtige Marker wie Testosteron. Besonders interessant: Ein sogenanntes „großes Blutbild" enthält oft gar nicht die Werte, die wirklich entscheidend sind. Testosteron beispielsweise ist dort meist nicht enthalten. Alexander empfiehlt deshalb, Blutwerte regelmäßig zu dokumentieren. So erkennt man Entwicklungen über Jahre hinweg. Denn häufig heißt es nur: „Die Werte sind noch okay." Aber „okay" bedeutet nicht automatisch optimal. Sehr spannend fand ich auch seine Aussage zu den Referenzwerten. Diese verändern sich ständig. Werte, die früher als normal galten, werden heute teilweise nach unten korrigiert. Auch das Thema Libido wurde offen angesprochen. Laut Alexander spielt Testosteron dabei eine wichtige Rolle. Nicht als einziger Faktor, aber als wichtiger Bestandteil. Die Symptome entstehen oft schleichend: weniger Energie schwierigerer Muskelaufbau Fettabbau funktioniert schlechter sinkende Libido später sogar Burnout oder depressive Verstimmungen Viele Menschen halten das einfach für normales Älterwerden. Alexander begleitet seine Kunden genau bei diesen Themen. Viele sprechen erst nach Monaten offen darüber, weil es natürlich ein sehr persönliches Thema ist. Außerdem sprachen wir über seine Webinare. Laurie Marker has spent nearly half a century with cheetahs. Her career began when she moved to Oregon in the early 1970s to open the third winery in the state. She began working at Wildlife Safari in Winston, OR to help support her business. This move would start a decades long career working with cheetahs. She eventually found the Cheetah Conservation Fund and moved to Namibia to create a dedicated wildlife reserve and research center for these large felines. Today, cheetahs are considered to have a vulnerable status, with less than 7,000 in the wild. Marker joins us to share more on her life and work with the fastest mammal on the planet that is racing to extinction.

The 400 is known in track & field as the “race of truth” because it sequentially exhausts the various high-intensity systems of the body—testing all of your attributes: speed, strength, power, metabolic fitness, and your ability to hold it together when your body is under maximum stress. No hiding, no faking it. While your aerobic competency is also a great longevity indicator—things like your mile run time (per Cooper Institute and UT Southwestern Medical School research) and your daily step count are great ways to track longevity potential, the 400 reveals your competency with the anaerobic energy systems and muscle fibers that are often ignored—even by devoted fitness enthusiasts. In this show, I break down the fascinating physiology behind this brutal event—how it uniquely blends anaerobic power with aerobic support—and why it reveals far more about real-world fitness than jogging miles or grinding through long workouts. Here’s the problem: modern fitness culture is heavily biased toward endurance—steady-state cardio, group classes, even so-called broad based or hybrid fitness formats like CrossFit and Hyrox that, in the big picture, are still dominated by the aerobic energy systems. Meanwhile, the qualities that decline fastest with age—and matter most for longevity—are strength, power, explosiveness, and the ability to produce force quickly. Those are the attributes that protect you from the number one cause of injury and death after age 65: falling. And they’re exactly what the 400-meter run demands. By the end of this episode, you’ll understand why the 400 might be the ultimate longevity benchmark, why most fitness programming has it backwards, and how to start tracking your own performance—whether it’s a full lap or a scaled effort—to keep score as you age. Time to hit the track, fire up the stopwatch, and find out where you really stand! TIMESTAMPS: The 400 meters is called the race of truth because it exposes everything about your fitness. [01:13] When we overemphasize endurance with our fitness pursuits, it comes at the expense of the qualities that are probably the most critical for aging gracefully. [01:42] Much of the fitness information out there is not based on real science or exercise physiology. [03:32] People who are fast, explosive and powerful, and have good competency in the 400 meters have, by necessity, tremendous aerobic endurance. [05:49] Walking forms the foundation of tremendous aerobic endurance for all other fitness attributes that you pursue. [07:22] What Masters Track and Field has observed across many events is a round a 6 percent decline per decade in the peak performance standard. [09:54] You can factor in a 6% decline per decade to compare yourself to the other challengers in your event. The records, the milestones need to be adjusted for age. [11:00] What is the single best exercise to promote longevity? The answer is....the thing that you enjoy doing the most. [15:19] Doing what you love to do is the number one best longevity exercise, but we need to integrate challenges that include functional muscle strength, cardiovascular enduraance, and mobility flexibility. [18:47] You have to load the skeleton with resistance exercise on a regular basis to prevent sarcopenia and dynapenia. [24:12] The 400 meter competency might be more valuable and more illustrative of your fitness and longevity potential, than simply your ability to throw around heavy weight. [27:33] Your body is not capable of delivering pure maximum effort for longer than seven seconds. [31:46] The 400 meters is only one lap around the track and in most cases, for fit people, it is about 66% contribution of the anaerobic system and 33% of the aerobic system. [35:56] If you are training for a race that lasts for one hour, you are depending on fat as your fuel source. Depending on your sport, your body composition needs differ. [37:07] One's time in the mile run at age 50 is strongly predictive of the potential to live to age 80 in good health. [39:56] It is extremely important that you learn to rest during your workouts. Even if you are at the low fitness level, hit it hard for 10 seconds and rest for one minute. [44:06] Why is anaerobic exercise so important for everyone? [51:27] The true sprint from exercise physiology is only up to seven seconds. [55:44] Preserving your anaerobic function, muscle power, and explosiveness is what's going to keep you safe from what the Centers for Disease Control calls the number one cause of demise and death in seniors over age 65, and that is falling and adverse related health consequences. [58:28] A 400 meter runner has a balanced metabolic engine of high glycolytic capacity, strong acid buffering capacity, and a meaningful anaerobic contribution. This is part 3 of a conversation with Chirag Patel. Chirag is based in London where consults on the ethical and science-based practice of behavior management and training for animals housed in domestic, zoo, and laboratory environments. Chirag earned his BSc (Hons) in Veterinary Sciences from the Royal Veterinary College in London and a Postgraduate Certificate in Clinical Animal Behavior from the University of Lincoln, UK. He is also a certified parrot behavior consultant (CPBC) with the International Association of Animal Behavior Consultants (IAABC). Currently, Chirag is studying for a MSc in Applied Behavior Analysis. In Part 1 talked about the up-coming March 2026 Clicker Expo. For several years now Chirag and several other faculty members have done a presentation that they call The Backstage Pass. Basically each trainer picks a dog/handler team from the audience and works with that team on stage towards a stated goal behavior. Each team works with the trainer for a few minutes at a time, then there's a discussion of the why's and wherefores of the methods used. It's an interesting presentation. You get to see different styles of training as each trainer gets to know the dog and the handler they are working with. This year they decided to do something a little different. Instead of inviting dog specialists to participate as the trainers, they decided to ask faculty members who don't typically work with dogs. When they asked me, for some very bizarre reason, I said yes. Chirag will be the ring master for the backstage pass. When I've watched him in previous years I've seen that he is a very creative trainer who comes up with some inventive solutions for helping dogs and handlers to be at ease. His training style is very different from mine which for me is of interest to me. We ended Part 1 with a question about marker signals. Often in those backstage pass demos Chirag trains with food, but he doesn't yet use a marker signal. He described his thinking behind this strategy. In Part 2 we began with my reasons for the choices I make related to the use of marker signals. The click in clicker training has been referred to in many ways. It is a bridge, a marker signal, a snap shot. It is very much a cue. I add yet another metaphor for the role the click plays. It is a gatekeeper. I explained what I mean by that in Part 2. In Part 3 we add the use of chains and the role that cues play in linking behaviors together to the discussion of marker signals.
