Prothrombin complex concentrates (PCCs) are frequently used off label for the management of factor Xa inhibitor-associated major bleeding. In 2018, accelerated approval was granted for andexanet alfa, a specific factor Xa inhibitor reversal agent, for reversal of apixaban and rivaroxaban in the setting of life-threatening or uncontrolled bleeding. Following accelerated approval, some clinical practice guidelines were updated to include recommendations for andexanet alfa preferentially over PCCs for reversal of life-threatening or uncontrolled bleeding due to rivaroxaban or apixaban. Other guidelines stated no preference of andexanet alfa over PCC. In 2020, Vizient convened an expert panel to critically appraise the literature and provide consensus-based, expert opinions on the utilization of pharmacological reversal agents for factor Xa-related major bleeding. Since then, the body of literature evaluating these agents has expanded to include a randomized controlled trial, ANNEXa-I, the results of which were submitted to the US Food and Drug Administration to convert the approval of andexanet alfa from accelerated to full approval. During high-profile shortages, institutions must rapidly establish command centers, engage stakeholders, and implement plans for frontline staff. The 1984 Hatch-Waxman Act codified the 505(b)(2) drug approval pathway and allowed the use of studies from a previously approved drug. To be clear, medications approved via this pathway are not generic molecules of the reference listed drug. They may have similar clinical effects and roles in therapy, but they also bring challenges. Sepsis has long been a challenge for healthcare organizations. It is a leading cause of morbidity, readmissions and mortality. As a reaction to an infection, sepsis can spread quickly, progress to septic shock and eventually shut down organ systems. Catching it early is a priority and the basis for much of a hospital's approach to the disease. RSV infections are not new and have devastating impacts. The recent advent of new prevention strategies has renewed the focus on the disease with a hopeful outlook. For the last 25 years, the only FDA approved product preventing RSV, lower respiratory tract disease, was palivizumab or Synagis, a monoclonal antibody indicated for use in high risk infants children and administered intramuscularly monthly throughout the RSV season. In May, the first ever RSV vaccines were approved, Arexvy and Abrysvo. These two new RSV vaccines were approved for use in adults greater than or equal to 60 years of age. In August this year, one of the RSV vaccines, Abrysvo, also received approval for expanded use for maternal vaccination to prevent RSV lower respiratory tract disease in infants. Nirsevimab or Beyfortus, a new long-acting monoclonal antibody, was approved in July for prevention of RSV lower respiratory tract disease in infants and children. Compared with palivizumab, nirsevimab is indicated for a much broader patient population including recommended use in all infants less than eight months of age, born during or entering their first RSV season. The rollout of nirsevimab has proved challenging due to supply shortages, among other factors.
