Pain is complex and multifactorial, making diagnosis challenging. The traditional impingement model is being challenged in favor of a broader understanding of shoulder pain. Imaging often does not influence management decisions for rotator cuff issues. Corticosteroid injections provide only short-term relief and can have negative effects on tendon quality. Exercise-based management is crucial for effective rehabilitation of shoulder pain. The rotator cuff tendons are positional and strain less than energy storage tendons. Rotator cuff tears are common and can exist without pain or dysfunction. Age is the biggest risk factor for developing rotator cuff pathology. Understanding the tipping point to pain is essential for effective treatment. Rotator cuff tears are often associated with poor vascular supply and degeneration over time. Metabolic factors like diabetes and smoking can increase the risk of rotator cuff tears. Exercise can be as effective as surgery for massive rotator cuff tears. Expectations of recovery significantly influence rehabilitation outcomes. Education about the commonality of tendon tears can help reduce patient anxiety. Pain during exercise can be tolerated up to a certain level without adverse effects. Sleeping positions can impact shoulder pain and should be modified accordingly. Adjunct treatments like shockwave therapy and corticosteroids have limited long-term benefits. Tendon stiffness is important for efficient force transfer, but its role in pain management is still being studied. Understanding the psychological aspects of pain can enhance recovery from tendon injuries. In this episode I interviewed Jared Powell who is a Physiotherapist with an interested in the shoulder. He has a website with some great resources on the shoulder, does plenty of webinars and talks on the shoulder, and is currently completing his PhD. In this episode, we focus on different shoulder injuries that commonly occur in athletes and ways to identify/treat them. Some topics discussed… Start with more non-traumatic type injuries. Commonly thought of as “chronic” or “overuse.” What questions and aspects Jared thinks are important to focus on when someone comes in with this type of shoulder pain. Does he think it possible to relate the cause of pain to a certain structure? Does it matter if you can or can’t? His thoughts on “shoulder impingement” and why he doesn’t like or agree with the term. How he views a grouping of injuries as rotator cuff related shoulder pain and goes about managing it. Specific exercises he likes to incorporate. Pain vs performance. Does thoracic/shoulder mobility matter to him? Outcome measures for return to sport and performance. Jared Powell is a Gold Coast based Physiotherapist who is currently completing his PhD in shoulder pain at Bond University, where he is also a visiting lecturer and tutor. Shoulder pain is an enormous burn in society with half of the population experiencing shoulder pain in any given calendar year. It's the third most common complaint that general practitioners hear from their patients and for those who have suffered from shoulder pain you know what an impact it can have on your daily function and your overall well-being.
